1.Procalcitonin in guiding antibiotic use in patients with AECOPD *

Objective To investigate the value of procalcitonin in guiding the antibiotic use in acute exacerbation of chronic ob-structive pulmonary disease(AECOPD) .Methods A total of 184 patients with AECOPD in our hospital from March 2014 to Sep-tember 2015 were selected and divide into two groups by the random number table method .Ninety-two cases in the control group received the antibiotic therapy according to clinical experience .Ninety-two cases in the observation group was implemented the anti-biotic therapy according to serum procalcitonin level .Results Compared with before treatment ,the procalcitonin level after treat-ment in the observation group was decreased significantly (P<0 .05) .The procalcitonin level after treatment ,occurrence rate of a-cute exacerbation within six months and re-hospitalization rate within six months in the observation group were lower than those in the control group(P<0 .05) .The antibiotic use time ,antibiotic expenses ,hospitalization time and hospitalization cost in the obser-vation group were less than those in the control group(P<0 .05) .The total effective rate and hospitalization mortality rate had no statistical difference between the two groups (P>0 .05) .Conclusion Serum procalcitonin is significantly increased in the patients with AECOPD .Procalcitonin can be used for the optimized use of antibiotics .
2.Gene transfer system mediated by PEI-cholesterol lipopolymer with lipid microbubbles

The properties of polyethyleneimine-cholesterol cationic lipopolymer (PEI-Chol) as gene carries and its gene transfer efficiency in vitro with lipid microbubbles were presented in this paper. PEI-Chol lipopolymer was synthesized by linking cholesterol chloroformate to the amino groups of branched poly(ethyleneimine) (PEI) of 1 800. The structure and molecular weight of PEI-Chol were confirmed by IR, 1H NMR and MADI-TOF-MS (matrix-assisted laser desorption/ionization time-of-flight tandem mass spectrometry), respectively. The average molecular weight of PEI-Chol was approximately 2 000. The gene delivery system of bubble/PEI-Chol/DNA was constructed by mixed PEI-Chol/pDNA (N/P 10:1) complexes with lipid microbubbles (2-8 microm) which were prepared by DPPC, DSPE-PEG2000 and perfluoropropane with the reverse phase evaporation technique. pEGFP-Cl (enhanced green fluorescent protein) was used as report gene to investigate the DNA condensing ability of PEI-Chol lipopolymer by agarose gel electrophoresis. And their cytotoxicity and in vitro transfer efficiency of different complexes were compared with each other in A549 and MCF-7. The results indicated PEI-Chol lipopolymer can condense plasmid DNA when N/P ratio upto 4, PEI-Chol complexes and bubble/PEI-Chol/DNA complexes were nontoxic to A549 and MCF-7 when formulated at the N/P ratio of 10/1 as determined by MTT assay. This bubble/PEI-Chol/DNA delivery system provided good transfer efficiency with other desirable characteristics such as against-precipitation of plasma proteins. In conclusion, bubble/PEI-Chol/DNA complex is a novel non-viral gene delivery system.
3.Nursing for Acute Epididymitis during Intermittent Catheterization

Objective To summarize the nursing for acute epididymitis within intermittent catheterization. Methods From August, 2013 to February, 2016, twelve patients suffering acute epididymitis within intermittent catheterization after spinal cord injury were reviewed. Re-sults Epididymitis was cured in all the patients after retention catheterization, psychological nuring, diet and drinking guide, reasonable drug use, etc. Conclusion Acute epididymitis within intermittent catheterization can be controlled by appropriate medication and nursing.
4.Application of PDCA Cycle in Management of Teaching Quality of Rehabilitation Nurse

Objective To explore the effectiveness of plan-do-check-action (PDCA) cycle in the management of teaching quality of reha-bilitation nurse. Methods From March, 2014 to March, 2015, 108 nurses were recruited in three terms. PDCA cycle was applied in the man-agement. Problems existing in their nursing work and nursing continuing education were investigated and analyzed, then improved in the next term. Results The teaching satisfactions were 86.12%, 90.13%, and 93.32%, respectively. The scores of theoretical examination, opera-tion examination and Power Point report gradually increased in nurses of three terms. Conclusion The application of PDCA cycle is effec-tive in the management of teaching quality of rehabilitation nurse.
5.Determination of Residual Organic Solvents in Erlotinib Hydrochloride by Headspace Gas Chromatography

Objective:To establish a headspace capillary gas chromatography method for the determination of residual solvents in erlotinib hydrochloride .Methods:A DB-624 capillary column (30 m ×0.53 mm, 3.0 μm) was used and the carrier gas was nitro-gen.The flow rate was 2.0 ml· min-1 and the inlet temperature was 190℃.The FID detector temperature was 230 ℃.The column temperature program was with the initial temperature of 35℃( maitaining 8 min) , risen to 170℃with the rate of 28℃· min-1 ( main-taining 8 min) , and then risen to 200℃with the rate of 32℃· min-1 ( maintaining 7 min) .The headspace vial temperature was 100℃and the time was 30 min.Results:Ethanol, isopropanol, methylene chloride and n-butanol had a good linear relationship within the range of 0.68-409.14 μg· ml-1 (r=0.999 8),0.67-404.88 μg· ml-1 (r=0.999 8),1.71-51.31μg· ml-1 (r=0.999 7) and 0.72-431.12 μg· ml-1(r=0.999 8), respectively.The average recovery was 99.0% (RSD=0.41%, n=9), 100.2%(RSD=0.52%, n=9),97.1%(RSD=1.75%, n =9) and 102.5% (RSD=1.08%, n=9), respectively.Conclusion: The method is simple and accurate , which can be used for the determination of four residual organic solvents in erlotinib hydrochloride .
6.Effect of Depression on the Incidence of Osteoporosis in Patients with Maintenance Peritoneal Dialysis

Objective To investigate the effect of depression on the incidence of osteoporosis in patients with maintenance peritoneal dialysis (MPD).Methods We enrolled 80 MPD patients,who underwent peritoneal dialysis in our dialysis center.The clinical data and biochemical parameters of all patients were collected,bone mineral density was detected and the Self-rating depression scale was used to evaluate depression state.Results Among the 80 enrolled MPD patients,34 patients had osteoporosis (42.5%),48 patients had the presence of depression (60%).Logistic regression analysis showed that age,dialysis age,gender,diabetes history and depression state were the risk factors for osteoporosis in MPD patients,the depression state was negatively correlated with BMD of lumbar and femoral neck (r=-0.347,r=0.426,P<0.05).Conclusion Depression state may be a risk factor for osteoporosis in clinic,it is of great significance to focus on the psychological state of MPD patients in the prevention and treatment of osteoporosis.
7.Study on Stability of Volatile Oil Extracted fromYinqiaosan Decoction

Objective To investigate the factors affecting the stability of the volatile oil extracted fromYinqiaosan Decoction.Methods The main chemical compositions and the extraction repetitiveness of the compound volatile oil were determined by GC-MS, and the stability of multiple extracted volatile oil was studied. Absorbance of the compound volatile oil was used as the evaluation index, and the factors affecting the stability of the of the compound volatile oil were investigated, such as illumination, temperatures and pH values of volatile oil solution and metal ions.Results The results of the GC-MS chromatograph indicated that the main chemical compositions of the compound volatile oil extracted fromYinqiaosan Decoction twice were the same. The results of the stability of the volatile oil showed that the preservation temperature and illumination affected the stability of the volatile oil to a certain extent. The absorbance values of the compound volatile oil changed slowly when it was stored at a relatively low temperature (4℃) and shielded from light, and it was less stable when stored in normal temperature and under illumination. Meanwhile, the absorbance of the compound volatile oil changed quickly in acid or alkaline solutions and was in instability. The metal ions, such as Cu2+ and Fe3+, have chemical reactions with the compositions of the compound volatile oil and there was a big change in the UV-Vis spectrum of the compound volatile oils.Conclusion The compound volatile oil should be stored at a relatively low temperature (4℃) and shielded from light. At the same time, it should be stored avoiding acids, alkaline and the metal ions, such as Cu2+ and Fe3+, to guarantee its stability. This study provides a reference for the preservation conditions and the preparation conditions of the compound volatile oil extracted fromYinqiaosan Decoction.
8.Protective effects of resveratrol on sepsis and its involved mechanisms

Aim To investigate the effect of resveratrol (Res)on septic mice and LPS-insulted H9c2 cells,as well as its involved mechanisms.Methods By use of a mouse cecal ligation and puncture-induced septic model (CLP),the survival of septic mice was evalua-ted after resveratrol treatments.H9c2 cells were insul-ted by LPS and then treated with resveratrol,the mR-NA expressions of TNF-α,SIRT1 and other class III HDAC members were detected using RT-PCR and real-time PCR,Finally,the protein levels of nuclear p65, an important subunit of NF-κB,were measured in H9c2 cells using Western blot assay,to reveal the effect of resveratrol on LPS-induced nuclear transloca-tion of NF-κB.Results Compared with the control septic animals,intraperitoneal injection of resveratrol (1 or 5 mg·kg -1 ·d -1 )significantly increased the survival of septic mice.Furthermore,resveratrol signif-icantly increased mRNA expressions of SIRT1,SIRT2, SIRT6 and SIRT7 in LPS-insulted H9c2 cells.Res-veratrol also remarkably inhibited LPS-induced nuclear translocation of NF-κB.Conclusion An appropriate dose of resveratrol protects septic mouse hearts from the injury induced by LPS through the activation of SIRT family members and the inhibition of NF-κB pathway.
9.Clinical Observations on Acupuncture Treatment of Post-Stroke Dysphagia

Objective: To seek a treatment that can improve the effect on dysphagia following post-stroke pseudobulbar palsy. Methods: Sixty patients were randomly allocated into observation and control groups, 30 cases each. The observation group was treated with Western drugs plus acupuncture at Tiantu (CV 22) and other three acupoints around Lianquan (CV 23) and the control group with simple Western drugs. Results: The effective rate was 86.7% in the observation group and 30.0% in the control group. Conclusion: Acupuncture can improve the curative effect of Western drugs on dysphagia following post-stroke pseudobulbar palsy.
10. Effect of platelet-rich plasma on pressure ulcers after spinal cord injury

BACKGROUND: Studies have reported that platelet-rich plasma (PRP) can promote accelerated healing of pressure injury. OBJECTIVE: To explore the effect of PRP on pressure injury of patients with spinal cord injury. METHODS: From July 2017 to December 2019, 40 patients with spinal cord injury complicated by stage 3 and stage 4 pressure ulcers were randomly divided into a PRP group (n=20) and a control group (n=20) using a random number table. Patients in the control group were given routine wound care and standardized rehabilitation treatment, whereas those in the PRP group received PRP treatment based on routine wound care and standardized rehabilitation treatment once a week. The surface area, wound type and exudate volume were assessed by pressure ulcer scale for healing (PUSH) before treatment and 2, 4, 6, 8 weeks after treatment. The total PUSH score, score of each item in the PUSH scale, and clinical efficacy were compared between the two groups. An ethical approval was obtained from the Biomedical Ethics Committee of West China Hospital of Sichuan University, with an approval No. 2019(622). RESULTS AND CONCLUSION: The total PUSH score and scores in terms of wound area, type, and exudate volume were significantly reduced after treatment in the PRP group (P < 0.01), while those scores in the control group were also significantly reduced at 6 and 8 weeks after treatment (P < 0.01). Compared with the control group, the total PUSH score and scores on exudate volume were significantly lower in the PRP group at 4, 6, and 8 weeks after treatment, and the scores on wound type were significantly lower in the PRP group at 6 and 8 weeks after treatment. The healing rate in the PRP group was 100% after treatment, which was significantly higher than that in the control group (35%; P < 0.001). These findings indicate that PRP treatment contributes to the recovery of pressure injury in patients with spinal cord injury.
