1.Application of a reinforced laryngeal mask airway with drain tube in elderly patient undergoing endoscopic sinus surgery

Objective To investigate the safety and efficiency of a reinforced laryngeal mask airway(LMA) with drain tube in elderly patient undergoing endoscopic sinus surgery.Methods Sixty elderly patients were selected and randomly divided into two groups:a reinforced LMA group (group R,n=30)and a reinforced endotracheal tube group(group T,n=30).Observation items include the difficulty degree of device insertion,the time for successful insertion,the number of attempts required for successful insertion,fiberoptic view of glottis,the difficulty degree of gastric tube insertion,oropharyngeal leak pressure(OLP),ventilatory indicators at a certain time,haemodynamics during insertion,the blood stain on the mask were compared between the two group.The postoperative complications 1,6,24 hours after operation were recorded.Results There were no significant differences in general conditions,the successful insertion rate at first attempt,the total success insertion rate,airway quality and the anesthetic drug dosage between the two groups.The insertion time was shorter in group R than in group T [(34.1± 13.0 s)vs (45.4± 15.6 s),t =5.274,P=0.001].The incidence of cough during extubation was less in group R than in group T [0% (0/30) vs.53.3%(16/30),x2 =21.222,P=0.000].Conclusions The reinforced LMA with drain tube is safe and effective for elderly patients during endoscopic sinus surgery.
2.Evaluation of Dexamethasone in Treating Acute Cerebral Stroke

Objective: To evaluate the effect of glucocorticoid on the treatment of acute cerebral stroke. Methods: Seventy-six patients with acute cerebral infarction were randomly divided into two treatment groups which included the group of intravenous infusion with dexamethasone 10 mg, Xuesaitong 0.4 g, and sodium citicoline 0.5 g Qd,and the group without dexamethasone treatment. Results: The infarction area enlarged on CT scan and the deficits of neural function accelerated in dexamethasone group. There was no change on CT scan and no neural function deterioration in patients without dexamethasone treatment. Conclusion:Side effect is obtained with the early use of dexamethasone for the treatment of acute ischemic cerebral stroke.
3.Genetic structure analysis of cultivated Scrophularia ningpoensis in Zhejiang Province

Objective To observe the genetic structure of cultivated Scrophularia ningpoensis in Zhejiang Province.Methods The genetic structures of six typical S.ningpoensis populations were analyzed by fluorescence AFLP marker.Results Bands(12 552) were generated by seven pairs of AFLP primer combinations,of which 8 808 were polymorphic,and the polymorphic rate was 70.17%.The variety ranges of PPB among different populations were 41.67%—55.56%,and 47.30% in average.I was between 0.190 8—0.238 3,and 0.221 8 in average.Ne was between 1.201 4—1.280 6,and 1.236 9 in average.Gst was 0.127 1,Nm was 3.432 4.UPGMA Cluster analysis showed that the six populations can be divided into two clusters,as that of Tiantai,Jinyun,and Jingning were one sub-cluster,and Dongyang,Pan′an,and Xianju were another one sub-cluster.Conclusion There is a relative high genetic diversity level in cultured S.ningpoensis of Zhejiang Province.Genetic differentiation exists among populations,but it exists in population mostly.There is a relative high genetic intercommunion among populations.The genetic distance is not related to the geographic environment.
4.Efficacy and safety of levofloxacin and rifapentine for elderly initial treatment pulmonary tuberculosis

Objective To investigate the efficacy and safety of levofloxacin and rifapentine for elderly initial treatment pulmonary tuberculosis.Methods 120 cases of elderly initial treatment pulmonary tuberculosis patients January 2015 to January 2016 in our hospital were randomly divided into A,B,C three groups,with 40 cases in each group.Treatment regimen of group A: 2HRZE/4HR; treatment regimen of group B: 2Rft(rifapentine capsules)HZE/4HRft; treatment regimen of group C: 2Rft(rifapentine capsules)HELfx/4HRft.The clinical symptoms,signs and adverse drug reactions were compared between the three groups of patients,observation the changes of blood,urine,liver and kidney function,perform imaging and bacteriological examination to determine the treatment effect.Results Five cases in group A and one case in group B abandoned treatment because of liver damage and severe gastrointestinal reaction,but no one in the 40 cases of group C abandoned treatment.There was no statistically significant difference among three groups in sputum negative conversion rate and focus absorption.The number of cases presenting gastrointestinal reaction and liver damage in group C was significantly smaller than that in group A and group B(P<0.05).Rifampicin was replaced by levofloxacin in two cases of group A as a result of allergy,and no allergy occurred in the other two groups.Conclusion Levofloxacin and rifapentine have good efficacy and few side effects for elderly initial treatment pulmonary tuberculosis,initial treatment regimen for elderly pulmonary tuberculosis patients should be individualized,drugs with few side effects can be used to guarantee smooth proceeding of treatment.
5.China Health and Retirement Longitudinal Study: depressive symptoms of middle-aged and elderly disabled people and its related factors

Objective:To explore the status of depression symptoms and related factors of depressive symptoms in middle-aged and elderly disabled people in China and provide references on the preventive cares of depression.Methods:Research data were obtained from the 2018 follow-up data of China Health and Retirement Longitudinal Study(CHARLS).Disabled people were selected as the research object. The scores of center for epidemiological studies depression scale(CES-D) were used to evaluate depressive symptoms. Fourteen variables derived from the 3 dimensions of demographic characteristics, health status, and health related behaviors were utilized in this study.SAS 9.4 were used for descriptive analysis, univariate analysis and Logistic regression analysis.Results:A total of 1 708 middle-aged and elderly disabled people were included, among whom 932(54.5%)were diagnosed with depressive symptoms, and the score of CES-D was (11.45±7.19).Self-rated poor health, self-rated poor memory, life dissatisfaction, physical pain were the risk factors of depressive symptoms ( OR=1.87, 4.22, 1.83, 5.30, 1.75). Male, educational level (middle-high school, high school or above), sleeping duration (6-9 h) were the protective factors of depressive symptoms( OR=0.74, 0.72, 0.10, 0.56). Conclusion:Middle-aged and elderly disabled people in China have a high detection rate of depressive symptoms. Female, lower education background, inadequate amount of sleep, self-rated unsatisfactory healthy, self-rated poor memory, dissatisfaction towards life and suffering from physical pain may be more likely to have depressive symptoms in middle-aged and elderly people with disabilities.
6.Prevalence of nontuberculous mycobacteria isolated from pulmonary specimens in Wenzhou during 2014 to 2016

Objective To investigate the isolation rate, distribution and trend of nontuberculous mycobacteria (NTM) in Wenzhou during 2014 to 2016.Methods Sputum or alveolar lavage specimens of patients with suspected pulmonary tuberculosis were collected for mycobacteria culture from January 2014 to December 2016.Mycobacterium culture positive strains were further identified by gene chip, 16S rRNA and hsp65 gene sequencing.SPSS 19.0 software was used to analyze the data.Results After excluding repetitive strains, 3 295 mycobacteria strains (MTB) were isolated from respiratory specimens, included 3 032 mycobacterium tuberculosis complex strains, 238 NTM strains, 20 Gordon genera strains, 3 Nocardia genera strains and 2 Tsukamurella genera strains.The proportion of NTM among confirmed mycobacteria was 8.5% (86/1 006), 6.7% (72/1 079) and 6.8% (80/1 185) in 2014, 2015 and 2016, respectively (x2 =2.459,P > 0.05).The overall prevalence of NTM was 7.3 % (238/3 270).There were 15 species of NTM, and the most common NTM strain was Mycobacterium intracellulare (52.5%,125/238), followed by Mycobacterium abscessus (22.7%, 54/238) and Mycobacterium avium (10.1%, 24/238), other species were only accounted for 14.7% (35/238).The ranking of Mycobacterium avium went up rapidly from the fifth in 2014 to the second in 2016 (x2 =18.259, P < 0.01), while proportion of Mycobacterium abscess, dropped from 34.9% (30/86) in 2014 to 17.5% (14/80) in 2016 (x2 =7.335, P<0.01).Among patients from whom the NTM strains were isolated, 56.7% (135/238) were male and most of them were aged 45 years or above (79.8%, 190/238).Conclusions In the past three years, the trend of NTM isolation rate in Wenzhou is steady.The most prevalent NTM species is Mycobacterium intracellulare, followed by Mycobacterium abscessus and Mycobacterium avium.Mycobacterium avium shows a continuously upward trend, while the separation of Mycobacterium abscessus shows a downward trend.
7.Transcriptomic Features of Echinococcus granulosus Protoscolex during the Encystation Process

Cystic echinococcosis (CE) is a zoonotic infection caused by Echinococcus granulosus larvae. It seriously affects the development of animal husbandry and endangers human health. Due to a poor understanding of the cystic fluid formation pathway, there is currently a lack of innovative methods for the prevention and treatment of CE. In this study, the protoscoleces (PSCs) in the encystation process were analyzed by high-throughput RNA sequencing. A total of 32,401 transcripts and 14,903 cDNAs revealed numbers of new genes and transcripts, stage-specific genes, and differently expressed genes. Genes encoding proteins involved in signaling pathways, such as putative G-protein coupled receptor, tyrosine kinases, and serine/threonine protein kinase, were predominantly up-regulated during the encystation process. Antioxidant enzymes included cytochrome c oxidase, thioredoxin glutathione, and glutathione peroxidase were a high expression level. Intriguingly, KEGG enrichment suggested that differentially up-regulated genes involved in the vasopressin-regulated water reabsorption metabolic pathway may play important roles in the transport of proteins, carbohydrates, and other substances. These results provide valuable information on the mechanism of cystic fluid production during the encystation process, and provide a basis for further studies on the molecular mechanisms of growth and development of PSCs.

9.Progress of the application of peripheral nerve blocks for older hip fractures in older adults

Older patients with hip fractures often experience moderate to severe pain, which can increase the risk of complications during surgery and hinder early postoperative mobility and rehabilitation.To address this, peripheral nerve blocks have been suggested as a method for perioperative pain management in these patients.This article aims to provide an overview of the neural innervation of the hip joint, the techniques used for peripheral nerve blocks in hip fracture surgeries, and their impact on postoperative complications and recovery in older patients.Current evidence suggests that peripheral nerve blocks may have a positive effect on the prognosis of older patients with hip fractures.However, further high-quality clinical studies are required to validate these findings.
10.Clinical and mutation analysis of a case of Noonan-like syndrome with loose anagen hair

To report a case of Noonan-like syndrome with loose anagen hair, and to detect gene mutations in the family. A 3-year-old female patient presented with a special facial appearance, short stature, deep palmar and plantar creases, and dark skin. The scalp hair was sparse and thin, dermoscopy showed black spot sign, single-hair follicular units, hair shafts varying in the diameter, upright hair and new vellus hair, and constriction of hair shafts and nodular hair were occasionally observed. Peripheral blood samples were collected from the proband and her parents, and the genomic DNA was extracted for whole-exome sequencing. A heterozygous missense mutation c.4A>G was identified in exon 2 of the SHOC2 gene in the proband, resulting in the substitution of serine by glycine at amino acid position 2 (p.S2G) . The mutation was not identified in either of her parents. Combined with the clinical phenotype and gene mutation, the diagnosis of Noonan-like syndrome with loose anagen hair was confirmed.
