Objective To summarize the correlation between clinical manifestations and imaging characteristics of schizencephaly in order to provide a basis for the diagnosis and prognosis. Methods Thirty-five outpatients with schizencephaly diagnosed at Department of Pediatric Neurology of Henan Provincial People's Hospital from January 2009 to May 2015 were retrospectively selected, and they were divided into different groups (patients with unilateral lesions or bilateral lesions) according to their cranial magnetic resonance imaging (MRI) features. The clinical manifestations were compared between different groups. Results Of 35 patients,19 (54.3%) patients were male and 16 (45.7%) patients were female.Twenty-four patients were found with unilateral lesions (68.6%),including 16 cases (66.7%) with hemiparesis and 8 cases(33.3%) with no motor impairment.Mental retardation was observed in 8 patients (33.3%) and 6 patients (25.0%) showed speech impairment.Eleven patients were found the bilateral lesions (31.4%),including 5 patients (45.4%) with tetraparesis,4 patients (36.4%) with hemiparesis, and 2 paticnts (18.2%) with no motor impairment.Mental retardation was observed in 9 patients (81.8%) and 9 patients (81.8%) showed speech impairment. There were significant differences in motor impairment, mental retardation and speech impairment between the unilateral lesion group and bilateral lesion group (Z =-2.40,P =0.002;x2 =7.09,P =0.012;x2 =9.94;P =0.003).Epileptic seizure occurred in 18 patients (51.4%).Binary Logistic regression analysis indicated that cortical dysplasia beyond the cleft and open-lip lesions were the major risk factors for seizures (OR =4.44,2.73;P =0.005,0.029).Imaging characteristics:there were closed-lip lesions in 10 patients (28.6%),open-lip lesions in 21 patients (60.0%) and open/closed-lip lesions in 4 patients (11.4%).Anatomic localization of all clefts was found in the frontal lobes in 19 patients (54.3%),in frontoparietal lobes in 11 patients (31.4%),in parietal lobes in 2 patients (5.7%),and in occipital lobes in 3 patients (8.6%).After 1-6-year follow-up,12 patients treated with antiepileptic drug were seizure-free (all with unilateral lesions),and 6 patients had refractory epilepsy (3 patients with bilateral lesions). Conclusions Schizencephaly is a rare structural disorder of cerebral cortical development. Those with bilateral lesions are usually manifested with severe motor, speech impairment and mental retardation and their prognosis is poor.Schizencephaly patients complicated with cortical dysplasia beyond the cleft or open-lip lesions are more easily attacked by seizures.MRI plays an important role in the diagnosis and prognosis judgment of schizencephaly.
2.Induced by low dose dexmedetomidine to prevent restless after laparo-scopic surgery
China Modern Doctor 2014;(25):119-121
Objective To research the efficacy of using low dose dexmedetomidine in short time to prevent restless after laparoscopic surgery. Methods Forty patients ASA grade ⅠorII,age 20~70 yeas old, scheduled for selective laparo-scopic cholecystectomy or ectopic pregnancy or oophorocystectomy etc, operation time about 30min,were randomly as-signed to low groupsgroup D and N. Group DIntravenous dexmedetomidine(0.5~0.8)μg/(kg·h) when we inducing, for 10~15 min,no more than 50 μg; Group Nno dexmedetomidine. Observed extubation tiwe after surgery, riker sedation and restless score(SAS),restless incidence rate. Results There was no obvious difference in the time of extract the tra-cheal tube (P>0.05). The SAS in group D was significantly lower than group N (P<0.05),incidence of restlessness of group D was significantly lower than N. Conclusion Intravenous low dose dexmedetomidine while inducing for short time can effectively prevent restlessness after Laparoscopic Surgery.

Objective To investigate the status and needs of training medical staff in county-level medical institutions in Guangxi to provide preferences for developing training programs. Methods Self-designed questionnaires were used to investigate a random sample of 477 medical staffs in 9 county-level medical institutions including People's Hospi-tals, Chinese Medical Hospitals, Maternity and Child Care Hospitals in Wuming, Liujiang, Rongxian counties. Re-sults 13% of the medical staffs had never participated in training; 28.1% of them were trained once a month; 50.7% of them were trained for less than a week. 39.4% of them expected to receive training once a year , 21.6% expected a week to half a month and 21.8% expected three months to six months. 60.4% of them expected to receive new knowl-edge and technique training. 73.6%of them expected to go to provincial hospitals for training and 57% of them expect-ed to receive extension study. Conclusion There are differences between medical staff training needs and status. Exist-ing questions are incomplete training coverage, too dense training frequency ,too short training period, medical staffs urgently need the training of continuing professional development, new technology and professional knowledge, and training effect is not obvious. We can establish medical staff in-service training incentives ,increase medical staffing and reform training methods to encourage and attract more medical staff to attend training.

Objective:To explore the effect of sports training guidance based on Internet platform on the limb function and quality of life in home rehabilitation of stroke hemiplegic patients. Methods:Eighty patients with stroke and hemiplegia admitted to Beijing Rehabilitation Hospital Affiliated to Capital Medical University from June 2018 to December 2019 were selected and divided into observation group and control group with 40 cases in each group by random digits table method. The control group was given routine rehabilitation training. In addition to the regular rehabilitation training, the observation group was supplemented with home rehabilitation based on the WeChat platform for modified constraint-induced movement therapy. The limb motor function Fugl-Meyer Assessment (FMA) score, Upper Extremity Function Test (UEFT) score, Stroke-Specific Quality of Life scale (SS-QOL) score before and after intervention, and patients′ satisfaction and cares′ satisfaction score were compared between the two groups. Results:There was no significant difference in the score of FMA, UEFT, SS-QOL before intervention between the two groups( P>0.05). After discharge, the FMA score, UEFT score and physical function, physiological function, mental health, social function of SS-QOL score were (68.87±6.64),(72.38±7.33), (35.28±4.13), (36.55±4.23), (37.22±3.84), (38.23±3.44) points in the observation group and (55.25±6.20), (61.24±6.47), (28.72±4.56), (30.85±5.30), (31.28±4.63), (31.02±5.16) points in the control group, and the differences were statistically significant between the two groups( t values were 5.316-9.482, P<0.01). During the nursing period, the patients′ satisfaction and cares′ satisfaction score were 92.50%(37/40), 87.50%(35/40) in the observation group and 72.50%(29/40), 67.50%(27/40) in the control group, and the differences were statistically significant between the two groups( χ 2 values were 5.541, 4.588, P<0.05). Conclusions:The home rehabilitation based on the Internet platform for modified constraint-induced movement therapy helps to promote the recovery of limb motor function in stroke hemiplegic patients, improve the nursing satisfaction of patients and their families, and has a positive effect on improving the quality of life of patients.

The widespread application of next generation sequencing (NGS) in clinical settings has enabled testing, diagnosis, treatment and prevention of genetic diseases. However, many issues have arisen in the meanwhile. One of the most pressing issues is the lack of standards for reporting genetic test results across different service providers. The First Forum on Standards and Specifications for Clinical Genetic Testing was held to address the issue in Shenzhen, China, on October 28, 2017. Participants, including geneticists, clinicians, and representatives of genetic testing service providers, discussed problems of clinical genetic testing services across in China and shared opinions on principles, challenges, and standards for reporting clinical genetic test results. Here we summarize expert opinions presented at the seminar and report the consensus, which will serve as a basis for the development of standards and guidelines for reporting of clinical genetic testing results, in order to promote the standardization and regulation of genetic testing services in China.
