1.Correlation between plasma NGAL and ET-1 levels and cognitive dysfunction in patients with chronic obstructive pulmonary disease

Objective To investigate the relationship between the plasma neutrophil gelatinase-associated lipocalin (NGAL)and endothelin-1 (ET-1) levels and cognitive dysfunction in chronic obstructive pulmonary disease (COPD) patients.Methods A case-control study was performed,consisting of 128 patients with COPD(68 patients without cognitive dysfunction and 60 patients with cognitive dysfunction) and 70 normal controls.All patients with COPD were diagnosed by pulmonary function tests and plasma levels of NGAL and ET-1 were determined by enzyme immunoassay.The cognitive function was evaluated by the MMSE and MoCA.Results ①Compared with normal control,the levels of plasma NGAL and ET-1 were increased(NGAL:(2.20±0.60) μg/L vs (1.69±0.73) μg/L,P<0.05;ET-1:(26.19± 10.55)pg/ml vs (13.05±2.37) pg/ml,P<0.05) in COPD patients without cognitive dysfunction and in COPD patients with cognitive dysfunction(NGAL:(3.80±2.75) μg/L vs (1.69±0.73) μg/L,P<0.01;ET-1:(37.82±0.29) pg/ml vs (13.05±2.37) pg/ml,P<0.01).Compared with the COPD patients without cognitive dysfunction,the levels of plasma NGAL and ET-1 were also increased in COPD patients with cognitive dysfunction (all P<0.05).②The plasma NGAL levels were correlated negatively with MMSE scores(r=-0.524,P<0.05),and negatively correlated with MoCA scores (r=-0.527,P<0.05).The plasma ET-1 levels were negatively correlated with MMSE scores(r=-0.549,P<0.05),and negatively correlated with MoCA scores(r=-0.558,P<0.05).The levels of NGAL and ET-1 were positively correlated(r=0.564,P<0.05).Conclusion NGAL and ET-1 may be involved in the pathophysiological process of cognitive dysfunction in patients with COPD,which provides a certain clinical value for the assessment of cognitive dysfunction in patients with COPD.
2.Effects of early fluid management on prognosis of elderly patients with septic shock

Objective To investigate the risk factors affecting prognosis in elderly patients with septic shock and explore the effective treatment. Methods A retrospective study of 147 patients with septic shock from the Affiliated Hospital of Hebei United University and Affiliated People's Hospital from Jan. 2011to Jan. 2013 was conducted. Early fluid loading was defined as the administration with an initial fluid bolus within 6 h in ICU. Conservative fluid management was defined as negative fluid balance measured in at least 1 d during the first 3 d as the case group,while the daily fluid balance and cumulative daily fluid balance of the first 3 d was as the control group. The effects of early fluid management on prognosis of elderly patients septic with shock combining the living index within 7 d in ICU were recorded. Results In the case group with 7 d ICU,21. 4%(15 / 70)patients were in progressed and 37. 1%(26 / 70)patients were dead,while 25. 9%(20 / 77)patients were in progressed and 59. 7%(46 / 77)patients were dead in control group(χ2 = 13. 17,P = 0. 00). Univariate analysis showed that APACHEⅡ scores,SIRS score,organ failure,patients with chronic diseases related were associated with 7 d survival rate in elderly patients with septic shock. Multivariate Logistic regression analysis showed that higher APACHEⅡ scores and SIRS score,organ failure,patients with chronic diseases related (have)were the risk factors(OR = 1. 88,5. 34,1. 96 and 4. 76;P < 0. 05). The restrictive fluid management strategy have related with the septic shock patients in elderly that the OR of death rate of fluid management in elderly patients with septic shock was 0. 657(P = 0. 032). Conclusion The prognosis in elderly patients septic with shock is associated with APACHE Ⅱ scores,SIRS score,organ failure,patients with chronic diseases related. Early fluid management can improve to the prognosis of elderly patients with septic shock.
3.Clone of a novel liver cancer associated gene and analysis of the secondary structures of the predicted protein.

OBJECTIVETo clone a novel liver cancer associated gene, and to explore the molecular basis of liver cancer genesis.
METHODSUsing mRNA differential display polymerase chain reaction (DDPCR) and screening the human placenta cDNA library, we got a full-length cDNA of the gene. We prepared and purified the GST fusion protein and the special polyclonal antibody, engaged in the Western blot and immunohistochemical staining analysis, and analyzed the second structures and predicted the function of the protein by the computer soft.
RESULTSWe have got a full-length cDNA of the liver cancer associated gene and identified that the full-length cDNA of the gene could be expressed in 293 eukaryocytes by Western blot assay. We localized the target protein in cytoplasm using the immunohistochemical staining methods, and found two SH3 binding domains and several protein kinase phosphorylation sites by analyzing the second structures.
CONCLUSIONSWe have got a novel full-length cDNA of human liver cancer associated gene.

Objective To observe the influence of phentolamine on N terminal B-type natriuretic peptide precursor(NT-proBNP),blood gas analysis,hypersensitive c-reactive protein and plasma D-dimer in patients with chronic cor pulmonale Methods One hundred and one cases patients with chronic cor pulmonale were randomly divided into two groups,51 patients in the treatment group,50 patients in the control group.All patients were treated with normal anti-infection,eliminating phlegm to smooth wheezing antithrombotic for one week,as the same time patients in the treatment group were treated with phentolamine for one week.Through observed the treatment effect of phentolamine to chronic cor pulmonale,the level of NT-proBNP,blood gas analysis,hypersensitive c-reactive protein and plasma D-dimer in patients with chronic cor pulmonale before and after the treatment were analyzed.Results Compared with before treatment,the levels of NT-proBNP,PCO2,hypersensitive c-reactive protein,plasma D dimer were lower than after one week in two groups,while the level of PO2 was higher.Treatment group:NT-proBNP (1 712.76±572.32) ng/L vs.(271.59±163.05) ng/L,t=20.42,P<0.05,PCO2 (66.34±5.81) mmHg vs.(52.58±5.82) mmHg,t=16.46,P<0.05,PO2 (59.28±6.13) mmHg vs.(73.64±6.10) mmHg,t=23.02,P<0.05,hypersensitive c-reactive protein 86.0(28.0) mg/L vs.23.0(12.0) mg/L,Z=-6.22,P<0.05 mg/L,plasma D-dimer (4 953.37±1 654.09) μg/L vs.(1 847.90±838.66) μg/L,t=17.11,P<0.05.Control group:NT-proBNP (1 527.24±658.70) ng/L vs.(612.58±357.59) ng/L,t=14.52,P<0.05,PCO2 (65.41±5.23) mmHg vs.(56.46±5.65) mmHg,t=13.04,P<0.05,PO2(60.57±5.84) mmHg vs.(67.21±5.19) mmHg,t=-10.06,P<0.05,hypersensitive c-reactive protein 79.0(29.0) mg/L vs.43.0(20.0) mg/L,Z=-6.16,,P<0.05,plasma D-dimer (4 408.02±1 682.83) μg/L vs.(2 598.28±1 242.73) μg/L,t=12.15,P<0.05.But the levels of NT-proBNP,PCO2,hypersensitive c-reactive protein and plasma D-dimer reduced significantly,the level of PO2 increased more significantly in treatment group(t(z)=-6.19,-3.39,-7.16,-3.56,5.70,all P<0.05).Conclusion Phentolamine can reduce the level of NT-proBNP,PCO2,hypersensitive c-reactive protein and plasma D-dimer and increased the level of PO2 in patients with chronic cor pulmonale.Phentolamine combined with routine treatment can improve the clinical efficacy of patients with chronic cor pulmonale.
5.Qualitative and quantitative analysis of flavonoids in Chinese hawthorn fruit.

OBJECTIVETo develop an LC-MS method for identification of flavonoids in Chinese hawthorn fruits and an HPLC method for quantification of 5 flavonoids.
METHODAccurate molecular weights of the target flavonoids were acquired by time-of-flight mass spectrometer (TOF-MS) in both positive and negative ion modes. The flavoniods were then further confirmed by standards. An RP-HPLC method was established to quantitatively analysis five major flavonoids (chlorogenic acid, procyanidin B2, epicatechin, hyperoside and isoquercitrin) in hawthorn fruits from ten different regions.
RESULTSThe molecular formulae of 8 components were determined by TOF-MS. In the quantitative method validation, the method is linear over the studied range of 1.8-178.5, 2.2-284.8, 2.7-270. 6,1.5-150.0, 1.5-150.0 mg x L(-1) for chlorogenic acid, procyanidin B2, epicatechin, hyperoside and isoquercitrin, respectively. The correlation coefficient for each analyze was greater than 0.999. The inter-day precision of the analysis was lower than 4%, The RSDs of precision and stability were lower than 5%. The recovery rate was from 98.5% to 102.5%, and RSDs were less than 3.5%.
CONCLUSIONThe method is simple, specific and reliable, and is suitable for quality control of Chinese hawthorn.

9.Evaluation of the effect of two kinds of drainage materials in surgical cavity wound

Objective:To compare the effects of non-woven cotton of surgical dressing and medical gauze in surgical cavity wounds.Methods:A total of 118 patients with surgical cavity wounds in Run Run Shaw Hospital Affiliated to Medical College of Zhejiang University from March to August in 2017 were recruited and divided into two groups by random digits table method, 58 in the observation group and 60 in the control group. The wounds were filled with non-woven cotton of surgical dressing in the observation group and medical gauze was received in the control group. The wound healing, drainage, surrounding skin maceration, the amount of fluid absorption, the pieces of dressings used, residues of yarn, the time of dressing change and the costs of treatment were compared between the two groups.Results:There was no significant difference in the healing rate, surrounding skin macerations and the cost between the two groups ( P>0.05). The intervention group was superior to the control group in the following indicators: the none-residual rate of yarn in the wound were 100.0% (282/282), the median amount of liquid absorbed were 5.43 g, the median pieces of dressings used were 14.00 pieces, the median time of dressing change were 98.00 s, while those index were 70.7% (208/294), 4.29 g, 19.50 pieces, 152.00 s in the control group, compared to the control group, the observation group showed a higher none-residual rate of yarn, more liquid absorbed, less dressing used, less dressing change time in the observation group, the difference was statistically significant ( χ2 value was 96.968, Z values were 2.199, 2.364, 3.852, P<0.05 or 0.01). Conclusions:The non-woven cotton of surgical dressing has no residual yarn in the wound, more liquid absorption, less pieces of gauze, and shorten dressing change time than the medical gauze. It can be used as a kind of superior surgical drainage gauze.
10.Effectiveness of exercise on sleep quality in patients with Parkinson's disease:a Meta-analysis

Objective To evaluate the effectiveness of exercise for people with Parkinson's disease on sleep quality.Methods Computerized retrieval of PubMed,Web of Science,Embase,the Cochrane Library,CINAHL,CN-KI,WanFang Data,VIP,CBM was conducted to collect randomized controlled trials about the effect of exercise on people with Parkinson's disease from inception to December,2022.There were 2 researchers who independently screened the literature,extracted the data and evaluated the risks of bias in the included studies.Meta-analysis was performed using RevMan 5.4 software.Results A total of 13 studies were included,with 874 patients.The result of meta-analysis show that the overall effect size of exercise intervention on sleep quality for people with Parkin-son's disease is significant(SMD=-0.54,95%CI=[-0.90,-0.19],P<0.01).Subgroup analysis show that the maxi-mum effect size of intervention frequency is 4-5 times/week(SMD=-0.75);the maximum effect size of exercise intensity is light intensity(SMD=-2.19);the maximum effect size of a single intervention time is 40-55 minutes(SMD=-0.69);the maximum effect size of exercise type is traditional Chinese exercise(SMD=-0.63);the maximum ef-fect size of intervention cycle is 12 weeks(SMD=-0.66).Conclusion Exercise intervention has significantly ef-fects to improve sleep quality on Parkinson's disease patients.It is a more effective way to improve sleep quality by exercising 4-5 times per week,while each exercise lasts about 40-55 min for 12 weeks in Traditional Chinese Medicine exercise with light intensity.
