1.Determination of inflammatory factor levels and inflammatory cell percentages in cough variant asthma patients unresponsive to bronchodilator treatment and their significances

Objective:To determine the inflammatory factor levels and inflammatory cell percentages in the patients with cough variant asthma (CVA ), and to clarify their potential role in the pathogenesis of CVA unresponsive to bronchodilator treatment.Methods:60 patients with CVA were randomly selected and divided into CVA unresponsive to bronchodilator treatment group (n=30)and CVA responsive to bronchodilator treatment group (n=30).As the same time 30 cases of healthy persons were used as normal control group.The levels of interluekin-8 (IL-8)and esoinophil cationic protein (ECP)in their induced sputum were detected,the classification of inflammatory cells in their induced sputum were observed, and their scores of cough symptom were recorded. Results:The IL-8 level in the induced sputum of the patients in CVA unresponsive to bronchodilator treatment group was higher than that in CVA responsive to bronchodilator treatment group and normal control group (P<0.05).The ECP level in the induced sputum of the patients in CVA unresponsive to bronchodilator treatment group was lower than that in CVA responsive to bronchodilator treatment group (P<0.05),but it was similar to the level in normal control group (P>0.05).The neutrophil percentages in the induced sputum of the patients in CVA unresponsive to bronchodilator treatment group were higher than those in CVA responsive to bronchodilator treatment group and normal control group (P<0.05).The scores of cough symptom of the patients in CVA unresponsive to bronchodilator treatment group was positively correlated with IL-8 level (r=0.764,P<0.01), and the scores of cough symptom of the patients in CVA unresponsive to bronchodilator treatment group was positively correlated with the neutrophil percentage in induced sputum (r=0.889,P<0.01).Conclusion:IL-8 and neutrophil may be associated with the incidence of CVA unresponsive to bronchodilator treatment. They can aggravate the inflammation and hypersensitivity of airway and cough symptom. The determination of IL-8 and neutrophil can be used as an accessory method in the diagnosis and j udgement of severity degree and curative effect of CVA in clinic.
2.The mechanism of heated humidified high flow nasal cannula oxygen therapy and its application in pediatrics

Heated humidified high flow nasal cannula oxygen therapy (HFNC) has the characteristics of improving the work of breathing,creating the effect of positive end-expiratory pressure,being setted up easily,well tolerated by patients and high safety.At present,it has been widely used in neonatal respiratory support,but lack of its extensive use and research in infants and children.HFNC is suitable for use in infants with viral bronchiolitis,pneumonia,asthma,ARDS,and postextubation respiratory support from the aspect of the mechanism of action of HFNC and the pediatric physiology.HFNC should be the preferred mode of conventional oxygen therapy in pediatrics.
3.Clinical Analysis of 53 Cases of Postmenopausal Vaginal Bleeding Caused by Hormone Replacement Therapy

Objective To investigate the effects of HRT on endometrium of postmenopausal women.Methods 53 cases of postmenopausal vaginal bleeding(PMB),from Jan,1995 to Dec.1999 treated by HRT were selected to make a comparison with the other 60 cases of PMB without HRT during the corresponding period.Both groups had integrated ultrasonic and pathological data and getting information from succedent inspection.Results The rates of the endometrial carcinorma incidences that happen in the HRT group were non-HRT group were 5 66% and 8 33% respectively,which had little significant.While the occurring rates of inflammation were 1 89% and 16 67% respectively,a significant difference between the both existed here.Conclusions With HRT,the various symptoms of post menopause women can be efficiently improved.It is safe and effective in short term.But a close monitoring is absolutely necessary for its long term effect(5 years or more).
4.Clinical Oservation on Transdermal Acupoint Electric Stimulatlion Treat Intractable Hiccups of Blood Diseases Patients

Objective To observe the clinical effect of the transdermal acupoint stimulation treatment on intractable hiccups of patient with blood disease. [Methods]The 57 inpatients with intractable hiccups induced by chemotherapy or corticosteroids were randomly divided into two groups, percutaneous acupoint electric stimulation group and control group with Ritalin injecting in zusani, from January 2010 to December 2013. Clincal effect and adverse reaction were compared and analyzed after treatment. [Result]There was no significant difference on the clinical effect between two groups,while the adverse reaction in percutaneous acupoint electric stimulation therapy group was relatively mild.[Conclusion] Percutaneous acupoint electric stimulation is an effective method for the treatment of intractable hiccups, which should be popularized in clinical practice.
5.The inactivation of duck hepatitis virus B in blood cell suspenion induced by specific photosensitizer

Objective:To study the specific photochemical effects of a newly designed target photosensitizer. Methods Based on the technique of antisense nucleic acid and the principle of photochemical reaction effects,a specific sensitizer,TFO P has been designed and synthesized.When in coordination with long wave ultraviolet ray(UVA) ,this decorated complex (TFO P) was added into the blood cell suspension to inactivate the contaminating virus( duck hepatitis virus B,DHBV).The efficacy of specific binding to DHBV DNA and viral inactivation by TFO P was detected by gel shift blot assay and infection of primary culture of duck hepatocyte.Results The designed TFO P could specifically bind to different DHBV DNA line sample and present different linking level.With a TFO P concentration of 0.1 nmol/ml and UVA intensity of 1800 ?W/cm 2,the DHBV in blood cell suspension could be reduced by 1.90~5.40 logs.Conclcusion The photochemical effects of TFO P could significant inactivate DHBV in blood.

Objective Explore the relationship between translational medicine and research-oriented hospital and to sum up the experiences of developing of research oriented hospital.Methods combined literature research method and experience method.Results In our hospital,under the guidance of the translational medicine,we construct research-oriented hospitals system,train personnel,establish innovative teams,build diversified research platform,and strengthen scientific research management to promote the construction and development of research-oriented hospital.Conclusions Translational medicine research is a concept,and it emphasized the interaction between basic research and clinical practice.The concept of research-oriented hospitals is consistent with translational medicine,and emphasizing the problem comes from the clinical,service to clinical,and it is a model of sustainable development.
8.Rationality Analysis of Prophylactic Application of Antibiotics during Perioperative Period in Patients with Type Ⅱ Incision Operation in Gynecology and Obstetrics before and after Intervention

Objective:To investigate the rationality of prophylactic antibiotics application during perioperative period in the pa-tients with type Ⅱ incision operation in gynecology and obstetrics in our hospital after the intervention. Methods:Totally 274 medical records of obstetrics and gynecology patients with typeⅡincision operation in our hospital from April 2012 to March 2013 were collect-ed as the control group, another 321 medical records of obstetrics and gynecology patients with typeⅡincision operation in our hospital from April 2013 to March 2014 were collected as the intervention group. The rationality of prophylactic antibiotics application before and after the intervention was compared. Results:Compared with those in the control group, the average administration time, the hos-pitalization time and the cost of antimicrobial agents were significantly decreased in the intervention group (P<0. 05). The incidence of adverse drug reactions and operation incision infection between the two groups was not significant difference (P>0. 05). Compared with those in the control group, the incidence of irrational choice of antibacterial drugs, the combined use without indication, irrational medication time,over-time use, inappropriate use and improper choice of solvents in the intervention group was significantly reduced ( P<0. 05). Conclusion:After the intervention, the rationality of prophylactic antibiotics application during perioperative period in the patients with type Ⅱ incision operation in gynecology and obstetrics in our hospital is improved significantly.
9.Review on the pay for performance impact on hospital costs

Objective:To analyze the pay for performance related costs and provide suggestions for further stud-y. Methods:Empirical literatures from inside the country and overseas were collected with a systematic review. Costs were summarized on the basis of three drivers ( performance appraisal, performance improvement and incentive per-formance) . Results:A total of 141 papers, 47 in Chinese and 94 in English were enrolled. Most papers in Chinese were subjected on healthcare facilities while the English ones emphasized on hospitals. Pay for performance may lead to the healthcare service costs, regulation costs and the resources allocation related costs reduction. In addition to these visible costs, a large amount of others hidden from different hospital management levels were also due to pay for performance. Conclusions:(1) Differences in costs could be found from home and overseas experience which per-haps resulted from the pay for performance, the supporting measures and the policy development. (2) More attention should be paid to the quantification researches on the health facilities’ hidden costs. (3) Most available researches fo-cus on pay for performance cost-effectiveness from the society and service purchase but not the providers’ perspective and the hidden costs were also ignored.
