1.CURATIVE EFFECT OF NB-UVB COMBINED WITH PREDNISONE ON NEURALGIA FOL-LOWING HERPES ZOSTER

Objective To observe the clinical curative effect of NB -UVB combined with prednisone on neuralgia following herpes zoster .Methods 84 patients with neuralgia following herpes zoster were randomly divided into treatment group and control group , with 42 in each.The treatment group were given NB -UVB 3 times weekly, and oral prednisone tablet 10mg three times daily for 14 days.The control group was treated with prednisone tablet 10mg three times and indomethacin 25 mg two times daily for 14 days.The visual analogue scale (VAS) was used to evaluate the therapeutic effects in the two groups .Results The effective rate of the treatment group was 88.1%, and that of the control group was 69.05%.The therapeutic effect in the treatment group was statistically significant higher than that of the control group (χ2 =5.974, p=0.015).No marked side effects were observed in both groups .Con-clusion NB-UVB combined with prednisone is effect in the treatment of neuralgia following herpes zostert .
2.Clinical application value of the measurement of thyroid function during early pregnancy

[Objective]To study the clinical application value of the measurement of thyroid function during early pregnancy.[Methods] Six hundred and thirteen women in early pregnancy(observation group)accepted thyroid function lest.Patients with abnormal thyroid function received therapeutic intervention.The results were compared with 586 pregnant women(control group)who didn't get thyroid function screening and treatment.[Results]There were 182 cases(29.69%)out of 613 pregnant women with various thyroid diseases.The incidences of fetal distress,premature rupture of membrane,gestational hypertension and neonatal deformity in observation group were significantly lower than those in control group[3.92%(24/613)vs.7.34%(43/596),5.06%(31/613)vs.12.97%(76/586),2.28%(14/613)vs.5.46%(32/586),1.63%(10/613)vs.5.63%(33/586),P<0.01].[Conclusion] Thyroid function detection and therapeutic intervention during early pregnancy has important clinical value.
3.Comparison between anterior and posterior surgical treatments of thoracolumbar burst fractures

Objective To discuss the results and characteristics of surgical treatments through anterior or posterior approach for thoracolumbar burst fractures. Methods The retrospective review included 43 cases with an average age of 30.4 years from September, 1999 to November, 2004. The surgical approach was chosen according to conditions of the injury. 17 cases received an anterior approach operation. The Frankel scale was used for assessment of nerve function. 3 cases were rated as Grade A, 12 as Grade B, 19 as Grade C, and 9 as Grade D before surgery. Results The mean follow-up period was 36.7 months. There were no severe postoperative complications, such as deterioration of nerve function. All the cases showed notable improvement. Except in 3 cases of complete paraplegia, the improvement was 1.6 and 1.3 Frankel grades respectively for the anterior and posterior approach operations. Conclusions The anterior and posterior approaches are different in advantages and characteristics. The surgery through anterior approach provides more decompression and better maintenance of sagittal plane alignment.
4.Diagnosis and treatment of lumbosacral nerve roots anomalies

[Objective]To investigate the diagnosis and treatment of lumbosacral nerve roots anomalies. [Method]Etiological factors,typing,diagnosis and treatment of lumbosacral nerve roots anomalies confirmed by operation were analyzed.Operative treatment included wide laminectomy(n=8),hemilaminectomy(n=8) and enlargement fenestration(n=9).[Result]The presenting symptoms of lumbosacral nerve roots anomalies often resulted from lumbar disc herniation or spinal canal stenosis.The typs of lumbosacral nerve roots anomalies included conjoined nerve roots(n=10),closely adjacent roots(n=8),thickening of nerve roots(n=2),caudal origin roots(n=2),division of nerve roots(n=1),double nerve roots(n=1),and anastomosis of nerve roots(n=1).The levels of nerve roots anomalies were L4 in 1,L5 in 14,and S1 in 10.Only 5 cases were diagnosed preoperatively by myelography,CT or MRI.The others were found at operation.All patients were followed up for more than 2 years.Postoperatively,the results were rated as excellent in 12,good in 8,and fair in 4.One patient had residual radiating pain in the lower lims and weakness in dorsiflexion of the foot. [Conclusion] Radicular symptoms of lumbosacral nerve roots anomalies are changeable.The preoperative diagnosis is difficult to establish by myelography,CT or MRI(axial and sagittal plane).It is necessary to heighten the sensitivity of diagnostic modalities.It may improve the surgical outcomes to expose sufficiently,to explore the nerve roots carefully and to decompress thoroughly.
5.Treatment of avascular necrosis of femoral head by implanting a composite of autogenous bone marrow and noncelluar tissue engineered bone allograft with BMP

Objective To investigate the effect of treatment of avascular necrosis of femoral head in adolescence by implanting a composite of autogenous bone marrow, bone morphogenetic protein(BMP), and noncelluar tissue engineered bone allograft. Methods The BMP was partially purified from bovine cortical bone (bBMP) by Urist method. The bone allograft chips were mixed with bBMP and bone marrow. The composite was implanted into the necrotic area of the femoral head after core decompression, then partial deminerallized allograft fibula segments were inserted in the core decompression holes to support the necrotic area in preventing the collapse in 64 adolescent patients (78 hips). Results 55 cases (67 hips) were followed up for 3 months to 6 years(mean 44 months). 28 case(36 hips) in FicatⅠ,Ⅱ were followed up over 3 years, of whom there were no obvious pain and dysfunction in 18 cases(22 hips), high density new bone was shown in core decompression area in CT scanning, and no evidence of progressive necrosis. There was no worsening of the symptoms in 4 cases(6 hips) in Ficat Ⅰand Ⅱ, but the lesion progressed. In 6 cases (8 hips) in Ficat Ⅲ, there were no obvious pain and dysfunction in 2 cases, but 4 cases underwent total hip replacement because of persistent pain and progressive lesion. Conclusion The partial demineralized allograft fibula can provide direct mechanical support to prevent the necrotic femoral head from progress and collapse, autogenous bone marrow and BMP is able to promote new bone formation. The method can be used as an alternative for the treatment of osteonecrosis of femoral head at stage Ⅰ,Ⅱ.
6.Pharmacodynamic Study of Kanggan Granules

OBJECTIVE:To study the pharmacodynamic action of Kanggan granules(KG).METHODS:After KG was administered i.g.to experiment animals,the antifebrile effect by using pyretic rats induced by fresh beer-yeast,anti-in?flammation effect by using auricle swelling in mice caused by bimethylbenzene and antitussive effect by using cough-model in mice caused by ammonia were observed and the quantity of phenolred secreted from the mouse's trachea were determined. RESULTS:KG could evidently decrease the temperature of febrile rats,lighten the auricle swelling in mice,delay the cough incubation period and reduce the cough frequency in mice,and markedly promote the secretion of phenolred from the mouse's trachea.CONCLUSION:KG has evident effects of antifebrile,anti-inflammation,antitussive and expectorant.
7.The comparison of curative effect of minimally invasive percutaneous nephrolithotomy in two lithotripsy

Objective To compare the efficacy of minimally invasive pereutaneous nephrosomy for complicated renal calculi between holimium laser hthotfipsy and ultrasonic lithotripsy.Methods 120 patients with renal calculi according to the different treatment methods,were divided into holimium laser lithotfipsy group 60 cases(group A)and tdtrasonic lithotripsy group 60 cases(group B).The index of operative time、hemoglobin drop blood transfusion rate、postoperative hospital stay、stone clearance rate were observed.Results Operative time(60.8±16.4)min,postoperative hospital stay(7.5±2.0)d in group A were shorter than[(80.5±18.5)min,(9.5±2.5)d]in group B (t=2.328,2.240,all P<O.05);Hemoglobin decline(5.0%)in group A was lower than the(9.0%)in group B (x2=3.89,P<0.05);Calculi clearance rate(81.7%)in the B group was highter than that(61.6%)in group B (x2=3.98,P<0.05);Complication incidence of postoperative 10.O%in group A compared with group B 13.3%had no significant difference(x2=1.56,P>0.05).Conclusion Percutaneous nephrostomy with ultrasonic lithotripter for complicated renal calculi had the advantages of mini-invasion,less operative time,less bleeding,and faster clearance calculi efficiency compared with laser lithotripsy.

9.Study on adsorption specificity of HB-H-6 resin

Objective The aim of this article was to deeply study the effects of different molecular weight of proteins and different structures of chemical substances on the adsorption specificity of HB-H-6 resin.Methods HB-H-6 resin was adopted to adsorb 5 different molecular weight proteins and different structural chemical substances including proteins,saccharides,human serum albumin (HAS),dextran and lipid,and then underwent static adsorption experiments in vitro.The adsorption rates of different structural chemical substances were analyzed from two experiments and the results were compared.Results The experiment results of HB-H-6 resin adsorption showed that the average adsorption rates of 5 different molecular weight proteins,myoglobin (Myo,16 700),ovalbumin (OVA,44 000),HAS (66 200),β-gal (130 000) and IgG (150 000),were significantly different:(0.00±0.33)%,(8.02± 1.23)%,(43.19±2.31)%,(34.25±1.07)% and (0.00±0.69)%.In the studies on adsorption of different structural chemical substances,the average adsorption rates of different structural chemical substances proteins,saccharides,lipid were significantly different:the absorption rates of plasma total protein,albumin,globulin,glucose,triglyceride and cholesterol groups were:(11.18±0.72)%,(10.74±0.66)%,(11.74± 1.22)%,(7.17±0.12)%,(1.06± 1.04)%,(3.05± 0.65)%.The absorption rates of HAS and dextran groups were:(43.19±2.31)% and (5.44±1.46)%.Conclusion In conclusion,the proteins' molecular weight of best adsorption condition is from 66 Ku to 130 Ku.The average adsorption rates of different structural chemical substances proteins,saccharides,lipid are significantly different.The average adsorption rates of proteins are higher than that of saccharides and lipid.It shows that HB-H-6 resin has adsorption specificity on different molecular weight proteins and different structural chemical substances.
10.Public-interest-oriented indicator system for performance assessment in public hospitals

Objective: To develop an indicator system for assessing public hospital performance. Methods:Based on literature research and expert consultation, we formed an indicator system framework and indicators for as-sessing public hospital performance. Using Delphi expert consultation, focus group interviews, focus group discussion and individual in-depth interview qualitative research methods, we constructed a public hospital performance evalua-tion indicators. Results:There are four dimensions, 11 secondary indicators, 30 tertiary indicators in the indicator system, depending on the level of hospital, the indicators and weights are adjusted. Conclusion:The indicator system is suitable for public-interest-oriented performance assessment of public hospitals, and provides basis for the perform-ance assessment system.
