1.Effects of different doses of dexmedetomidine administered intranasally on median effective concentration of sevoflurane inhibiting responses to laryngeal mask airway insertion in pediatric patients

Objective To evaluate the effects of different doses of dexmedetomidine administered intranasally on the median effective concentration (EC50) of sevoflurane inhibiting responses to laryngeal mask airway (LMA) insertion in the pediatric patients.Methods American Society of Anesthesiologists physical status Ⅰ or Ⅱ pediatric patients of both sexes, aged 1-3 yr, with body mass index of 20-25 kg/m2, scheduled for elective surgery under general anesthesia, were randomly divided into 3 groups: control group (group C) , dexmedetomidine 1 μg/kg group (group D1) , and dexmedetomidine 2 μg/kg group (group D2).In D1 and D2 groups, dexmedetomidine 1 and 2 μg/kg (in 1 ml of normal saline) were administered intranasally at 1 h before induction of anesthesia, respectively.Group C received the equal volume of normal saline administered intranasally.The children were separated from their parents at 45-60 min after intranasal administration and admitted to the operating room.The children inhaled 8% sevoflurane until they lost consciousness.After loss of consciousness, the end-tidal concentration of sevoflurane was determined using the modified Dixon's up-and-down method.The initial end-tidal concentration of sevoflurane was set at 2%.The concentration was increased/decreased by 0.2% in the next patient according to the response to LMA insertion.The patient's sedation status and with LMA acceptance were evaluated, and the patient's satisfaction with sedation and with LMA acceptance was recorded.Probit analysis was used to calculate the EC50 and 95% confidence interval of sevoflurane inhibiting responses to LMA insertion.Results The patient's satisfaction with sedation and with LMA acceptance was significantly higher in D1 and D2 groups than in group C, and in group D2 than in group D1 (P<0.05).The EC50 (95% confidence interval) of sevoflurane inhibiting responses to LMA insertion was 2.03% (1.91%-2.15%), 1.76% (1.65%-1.87%) and 1.63% (1.53%-1.73%) in C, D1 and D2 groups, respectively.The EC50 was significantly lower in D1 and D2 groups than in group C, and in group D2 than in group D1 (P<0.05).Conclusion Dexmedetomidine 1 and 2 μg/kg administered intranasally can decrease the EC50 of sevoflurane inhibiting responses to LMA insertion in the pediatric patients, and 2 μg/kg produces better efficacy.
2.Sedative and hypnotic interaction between propofol and remifentanil by target-controlled infusion during induction of anesthesia

Objective To investigate the sedative and hypnotic interaction between remifentanil and propofol by target-controlled infusion (TCI) during induction of anesthesia.Methods Third-two ASA Ⅰ or Ⅱpatients,aged 22-63 yr,body mass index 18-25 kg/m2,scheduled for elective surgery under general anesthesia,were randomly divided into 4 groups(n=8 each).Group Ⅰ only received TCI pmpofol.GroupⅡ,Ⅲ,and Ⅳreceived a target concentration of 2,4 or 6 ng/ml remifentanil respectively.While the blood-effect site concentrations of remifentanil were equilibrated,patients received TCI of propefol,with an initial target concentration of 0.5μg/ml.After the blood-effect site concentrations of propofol were equilibrated then with 0.5μg/ml increments until the loss consciousness was achieved.The eyelash reflex and state of consciousness were assessed and radial arterial blood sample 6 ml was taken every 3 min to determine the remifentanil and propofol concentrations in blood.Propofol and remifentanil concentrations in blood were measured by reversed-phase high-performance liquid chromatography and high-performance liquid chromatography with ultraviolet detection respectively.The sedative and hypnotic interaction between propofol and remifentanil was determined with a pharmacodynamie interaction model by regression analysis and determined using the isobolographic method.Results Propofol concentrations in blood were lower in group Ⅱ,Ⅲ and Ⅳ than group Ⅰ(P<0.05).The propofol concentratopms in blood were significantly decreased in trun with the increase in the remifentanil concentrations in blood in group Ⅱ-Ⅳ(P<0.05).At loss of eyelash reflex and loss of consciousness of patients,the pharmacodynamic interaction model by curve fitting was superior to linear regression (P<0.05).At loss of eyelash reflex of patients,the curve fitting result showed EC50,prop=2.77μg/ml and EC50,rem=26.67 ng/ml,and the isobolographic method equation is ECprop/2.77+ECrem/26.67=0.69.At loss of consciousness of patients,the curve fitting result showed EC50,prop==3.76μg/ml and EC50,rem=31.56ng/ml,and the isobolographic method equation is Ecprop/3.76+Ecrem/31.56=0.65.Conclusion Remifentanil (Cp 2-6 ng/ml) and propofol by TCI shows a synergistic type of pharmacodynamic interaction on the sedative and hypnotic during induction of anesthesia.

To clarify the effect of the sympathetic nerves on the body's physiopathological changes and acupuncture effect by the alphal-AR mediation in the past 30 years.The paper has referred to the database of CNKI and Pubmed,and systematically reviewed the publications in the past 30 years about the research of the alphal-AR mediation of the sympathetic nerves to the body's physiological andpathological changes and acupuncture effect.Alphal-AR not only mediated the sympathetic nerves on the inotropic change of the heart,and the contraction of the vascular smooth muscles,bladder sphincter,and uterine smooth muscles and other physiological effect,but also mediated cardiac arrhythmia,myocardial hypertrophy and other pathological process.In addition,Alphal-AR also mediated the acupuncture signal transmission,and acupuncture was able to adjust the sympathetic nervous tension.The body's physiopathological changes are closely related to alpha1-AR and subtype alteration.The in-depth study of alphal-AR helps to explain physiopathological mechanism of the body,as well as provide theoretical basis and the corresponding pharmacological models for better selective drugs.Studying Alpha1-AR on meridians contributes to the discovery of meridian essence and the substantial basis of acupuncture effects.
4.Study of the Post Effect of Electroacupuncture on Blood Flow in the Skins of Related Meridian Points in Rats with Acute Ischemic Myocardial Injury

Objective To observe over-time changes in rat blood flow in the skins of related meridian points during physiological status, the pathological state of ischemic myocardial injury and low or high frequency electroacupuncture intervention and explore the post effect of different frequency electroacupuncture on related meridian points after treating ischemic myocardial injury. Method Fifty male Wister rats were randomized into five groups: blank control, sham operation, model, low frequency electroacupuncture (meridian point A) and high frequency electroacupuncture (meridian point B), 10 rats each. Blood flow in the skins of bilateral points Neiguan (PC6), Ximen (PC4) and Tianquan (PC2), and non-meridian and non-acupoint control points was measured by laser speckle contrast imaging in every group immediately and at 30 and 60 min after the end of three treatments. Statistical analysis was made. Result Blood flow in the skins of bilateral points Neiguan, Ximen and Tianquan was significantly lower in the blank control group than in the model group (P<0.01,P<0.05). Blood flow in the skin of every acupoint increased in varing degrees after low or high frequency electroacupuncture treatment. Blood flow in bilateral points Neiguan, Ximen and Tianquan regions in meridian point group A was closer to that in the blank control group immediately after treatment. Blood flow in three left-side and three right-side acupoint regions in meridian point group B was closer to that in the blank control group at 30 and 60 min, respectively, after treatment. Conclusion The immediate effect of low frequency electroacupuncture on blood flow in acupoint regions is better than high frequency electroacupuncture during intervention in ischemic myocardial injury. The post effect of high frequency electroacupuncture on blood flow in acupoint regions is better than low frequency electroacupuncture during intervention in ischemic myocardial injury.
5.Alpha-lipoic acid alleviates the 6-OHDA-induced SH-SY5Y cell damage by inhibiting autophagy via AMPK/mTOR pathway

Objective To investigate the effects of α-lipoic acid(ALA)on 6-hydroxydopamine(6-OHDA)-induced autophagy in human neuroblastoma(SH-SYSY)cells and its possible mechanisms.Methods SH-SYSY cells were divided into 5 groups:blank control group (group A),ALA group (group B),6-OHDA group(group C),ALA+6-OHDA group(group D),and rapamycin(RAPA)group (group E).The cell viability,cell apoptosis,and oxidative stress were assayed and analyzed in A-D group.The expression of autophagy-related proteins LC3-Ⅱ,AMP-activated protein kinase(AMP-K),phosphorylated AMPK (p-AMPK),the mammalian target of rapamycin (mTOR) and p-mTOR were detected by Western blot in A-E group.Results Compared with the blank control group,the 6-OHDA group significantly reduced the cell viability(P < 0.01) and p-mTOR protein expression (P <0.05),and increased the cellular apoptosis rate(P<0.01),oxidative stress level(P <0.01),LC3-Ⅱ protein expression(P<0.05,with the highest level at 6 h after treatment),and p-AMPK protein expression(P<<0.05).There was no significant difference in these indices between ALA group and the blank control group.Compared with 6-OHDA group,ALA+ 6-OHDA group showed that the cell viability(P < 0.01) and p-mTOR protein expression (P < 0.05) were increased,while the cellular apoptosis rate(P<0.01),oxidative stress level(P<0.01),LC3-Ⅱ protein expression(P <0.05),and p-AMPK protein expression (P < 0.05)were decreased.Conclusions The 6-OHDA can induce oxidative stress and autophagy in SH-SY5Y cells and decrease the cell viability.ALA can alleviate the 6-OHDA-induced cell injury possibly by inhibiting autophagy via AMPK/mTOR pathway.
6.Influence of electro-acupuncture of different frequency and stimulation times on skin blood perfusion around Neiguan (PC6)and sham acupoint in rats with myocardial ischemia

Objective To compare the skin blood perfusion around Neiguan (PC 6)and sham acupoint needled by the different frequency and stimulation times of electro-acupuncture (EA),in healthy rats and myocardial ischemic rats.Methods According to random number table,altogether 100 male Wistar rats were divided into once treatment group and three times treatment group,which were both divided into control group,sham-operation group,model group,EA low-frequency group and EA high-frequency group (each n =10).After successful modeling,rats of EA groups were acupunctured at left Neiguan (PC 6)acupoint.60 minutes after last treatment of acupuncture,skin blood perfusion around bilateral Neiguan(PC 6)acapoints and sham acupoint were tested by laser speckle contrast imaging technology (LSCI) and analyzed,and the myocardial morphological changes were observed by HE staining. Results Compared with control group,the skin blood perfusion around PC 6 in model groups decreased significantly after three time treatment (P <0.01 ),and that around PC 6 or sham acupoint in model group of three times treatment group was much less than that of once treatment group (P <0.01).Com-pared with the model group,the skin blood perfusion around PC 6 both significantly increased after low-frequency EA or high-frequency EA after three times treatment (P <0.01).Conclusion The variation of skin blood perfusion around PC 6 should be useful for specific reflection of normal condition or ische-mic condition of rats myocardium,but that around sham acupoint didn’t.The effect of three-times-treat-ment against declination of skin blood perfusion due to myocardial ischemia was superior to that of once-treatment regardless of high frequency or low frequency,and EA of high frequency and three-times stimu-lation showed the significant effect.However,the mechanism remains to be further studied.
7.Influence of high frequency electroacupuncture on skin temperature around meridian acupoints, and non-menridian sham acupoints in rats with myocardial ischemia

Objective To compare the skin temperature around relevant and irrelevant meridian acu-points,or non-meridian sham acupoints in rats with myocardial ischemia by high frequency electro-acu-puncture(EA)at Neiguan(PC6)point or sham acupoint,in healthy rats and myocardial ischemic rats. Methods Fifty male Wister rats were randomly divided into five groups:blank control group,sham-op-eration group,model group,high frequency EA at Neiguan group(PC6)and high frequency electro-acu-puncture at sham acupoint groups.Rats in high frequency EA at and at sham acupoints group received EA intervention after their models were successfully established.Temperature around the skin of Neiguan (PC6),Yanglingquan(GB34)and sham acupoints were measured and recorded by infrared thermogra-phy in every group at 30 minutes 60 minutes after three-day treatment for statistical analysis.Results Compared with the blank control group,skin temperature around PC6 in model groups was significantly lower(P<0.01);Compared with the model group,skin temperature around PC6 in high frequency EA at PC6 group was significantly increased(P <0.01);Skin temperature around PC6, GB34 and sham acupoint in high frequency EA at sham acupoint groups presented with no statistical significance; skin temperature around PC6 in high frequency EA at PC6 group was similar to the skin temperature of blank control group,which was measured 30 minutes after the treatment.Conclusion The relative specificity of skin temperature around PC6 can be useful for the reflection of different conditions of rats' myocardi-um.High-frequency electro-acupuncture at PC6 can prevent the reduction of skin temperature caused by myocardial ischemia injury,of which the mechanism needs further study.
8."Medicinal Part-Habitat-Nature, Taste, and Effect" Correlations of Pteridophyte-derived Chinese Medicinal Materials

ObjectiveTo explore the correlations between botanical characteristics, biological characteristics, growth environment, and medicinal properties of common pteridophyte-derived Chinese medicinal materials, thus providing evidence for the theory of quality evaluation through morphological identification and giving insights into the extensive and reasonable application of pteridophytes in traditional Chinese medicine (TCM). MethodThe medicine parts, habitats, natures, tastes, and effects of the commonly used pteridophyte-derived Chinese medicinal materials were summarized. The commonly used pteridophyte-derived Chinese medicinal materials were retrieved from the Pharmacopoeia of China, Dictionary of Chinese Materia Medica, and related literature. Excel 2016, ChiPlot, Cytoscape 3.7.1, SPSS 21.0, and weiciyun software were used for statistical analysis. ResultThe frequency of the habitats followed the trend of streamside wetland>tree trunk and rock crevices>sunslope>water surface. The frequency of medicinal parts presented the trend of whole plant>rhizome>leaf>dried aboveground part>spore. The frequency of natures was in the order of cool>cold>plain>warm>hot, and that of tastes was in an order of bitter>pungent>sweet>bland>salty. The frequency of meridian tropism followed the trend of liver meridian>stomach meridian>lung meridian>kidney meridian>bladder meridian>heart meridian>large intestine meridian>spleen meridian>small intestine meridian. The effects of the pteridophyte-derived Chinese medicinal materials followed a frequency trend of clearing heat and detoxifying>promoting urination and relieving stranguria>cooling blood and stopping bleeding>activating blood and resolving stasis>dispelling wind and eliminating dampness. ConclusionThe pteridophyte-derived Chinese medicinal materials mainly have a cool nature, a bitter taste, and tropism to the liver meridian. Whole plants and roots are mainly used for medicinal purposes, and most of these plants grow in the wetlands near rivers, under trees, and in tree trunk and rock crevices. The main effects of these medicinal materials are clearing heat and detoxifying, dispelling wind and removing dampness, cooling blood and stopping bleeding, activating blood and resolving stasis, and soothing meridians and dredging collaterals. There are certain correlations between the structures, habitats, medicinal parts, and effects of pteridophyte-derived Chinese medicinal materials, which provide reference for the development and utilization of pteridophyte-derived Chinese medicinal material resources.
