2.Risk factors for the occurrence of diabetes after hematopoietic stem cell transplantation for treating malignant hematopathy

AIM: Hematopoietic stem cell transplantation is the only method to cure hematologic disease. This article analyzes risky factors as to suffering and development of diabetes after hematopoietic stem cell transplantation for malignant hemopathy. METHODS: ①Sixteen patients who received hematopoietic stem cell transplantation were enrolled at the Wuhan Central Hospital from January 2003 to February 2007. They were divided into diabetic (n=6) and non-diabetic (n=11) groups with an average age of 45.2 years. ②The interrelationship between occurrences of diabetes after hematopoietic stem cell transplantation and patients' age, symptom before operations (times of chemotherapy, times of large amount of dosage for intensifying treatment, status of blood pressure, sugar tolerance), immunosuppressant, dosage and time of cortex incretion as well as effect of diabetes on patients. RESULTS: Sixteen patients were included in the final analysis ①5 cases of diabetes occurred after surgery; patients with abnormal sugar tolerance before surgery possessed a higher rates of occurrence of diabetes than those with normal sugar tolerance; ②Those patients of older than 40 years and who had taken in sugar-containing cortex incretion during chemotherapy also possessed a comparatively higher rates of occurrence of diabetes; Patients whose intake of cortex incretion have being cancelled after half a year of transplantation possessed an obviously lower rates of occurrence of diabetes than those intake of cortex incretion had being cancelled within half a year of transplanting operation. ③ Application of immunosuppressant and plasma drug concentration had no affect on occurrence of diabetes in both groups. ④No evident effect of infection rate of cytomegalovirus was detected in the diabetic group. CONCLUSION: Those patients that are older than 40 years and who have taken in sugar-containing cortex incretion during chemotherapy and with abnormal sugar tolerance as well as taking in sugar-containing cortex incretion to prevent occurrence of graft-versus-host-disease (GVHD) possess relatively higher rates of occurrence of diabetes.

4.A comparison of cognitive function of male recruits three months after ascent to the plateau

Objective To explore the effect of 3-month-exposure to high altitude(4400 m) on military cognitive func-tion.Methods Using pre-post paired t-testdesign, thirty-seven male recruits were enrolled , eight cognitive function variables, including reaction time, speed-perception, time-perception, operating-dexterity, memory span, depth-percep-tion, short-term memory and attention span , were analyzed by paired comparison .Results After 3-month-exposure to 4400 m high altitude, movement time (P<0.05) was shortened, speed-perception became more accurate (P<0.05),memory span and short-term memory both declined slightly(P<0.05), and the errors in depth perception deteriorated (P<0.05). Moreover , the depth-perception was more prone to′see close′.Conclusion During the 3-month-exposure to higher altitude environments , many of the cognitive functions of soldiers may be damaged to different extents , and the negative effect on re-action and movement time , depth-perception and memory is more remarkable .
5.Effect of hemoperfusion therapy in children with acute tetramine poisoning

ObjectiveTo analyze the therapeutic experience and value of hemoperfusion in children with acute tetramine poisoning.MethodsAccording to the choice of children's legal guardian,67 cases with acute tetramine poisoning were divided into two groups:the perfusion group (48 cases),and the control group (19 cases).The patients in perfusion group received blood perfusion based on conventional treatment,while the patients in control group received conventional treatment only.Survival outcomes of patients with different serum tetramine concentrations in two groups were compared.ResultsWhen the serum concentration of tetramine was >0.5 mg/dl,patients in both groups died in a short time.When the serum concentration of tetramine was 0.20~0.49 mg/dl,the drop value of serum concentration of tetramine significantly greater in perfusion group [(0.28±0.02) mg/dl] than that in control group [(0.13±0.03) mg/dl] (P<0.05).When the serum concentration of tetramine was <0.2 mg/dl,the drop value of serum concentration of tetramine in two groups showed no significant difference [ (0.12±0.02) mg/dl vs (0.11±0.03) mg/dl] (P>0.05).Conclusion The treatment method for tetramine poisoning should be selected based on the serum concentration of tetramine.
6.The role of adenosine A1 receptor system in the delayed protective effects of ischemic preconditioning in brain ischemia in rabbits

Objective To determine the role of adenosine A1 receptor system in the delayed protective effects of ischemic preconditioning(IPC) in brain ischemia.Methods Thirty-five rabbits weighing 2-2.5kg were randomly divided into 7 groups: control (group Ⅰ), 3-min ischemia (group Ⅱ), 10-min ischemia (group Ⅲ), IPC (group Ⅳ), N6-cyclopentyladenosine (CPA)(group V), 8-cyclopentyl-1,3-dipropylxanthine (DPCPX)+ IPC(group Ⅵ), and DPCPX+3-min ischemia (group Ⅶ).Cerebral ischemia was produced by occlusion of both carotid arteries in combination withdrawing blood to maintain MAP at 35-40 mmHg.After 3-min IPC and 3 days of recovery , cerebral ischemia was induced and lasted 10 min (model of delayed protective effects of IPC). Adenosine A1 receptor agonist CPA or antagonist DPCPX was used instead of IPC to evaluate the role of adenosine A1 receptor in the delayed protective effects of IPC.The expression of heat shock protein 70 (HSP70) was analyzed by immunohistochemistry.Neurons density and expression of HSP70 in the hippocampal CA1 region were examined and measured 3 days later.Results (1) CPA could reduce cerebral ischemic injury, but the effects of CPA were not as good as those of IPC (about 70% of IPC).After adenosine A1 receptors being blocked by DPCPX the delayed protective effects of IPC disappeared.(2) 3-min ischemia alone did not cause neuronal injury but induced obvious expression of HSP70.DPCPX not only could make 3-min ischemia cause injury but also reduce the expression of HSP70.Conclusions (1) The delayed protective effects of IPC is related with activation of adenosine A1 receptor system.(2) One of the mechanisms of the blocking effects of DPCPX may be due to the reduction in expression of HSP70 .

Results: This methods was quick, simple, accurate and reproducible. Conclusion: This method can be used as one of quality control standards for Zhuyun Capsules.

Objective To explore the effect of decoction of four-drug juice plus glutamine on nutritional support in the elderly critically ill patients. Methods One hundred and twenty-four elderly critical ill patients in The First Hospital of Jilin University from May 2010 to May 2012 were randomly divided into treatment group(n = 62)and control group(n = 62). Patients in both two groups were given enteral nutrition(EN)plus parenteral nutrition(PN)after the hemodynamics became stable. In addition,patients in control group were given routine EN + PN,while in treatment group was given decoction o four-drug juice + EN plus glutamine + PN. Prealbumin (PA),serum total protein( TP),transferrin( TF)and serum albumin( ALB)were detected before and after treatment of nutritional support. The informations including recovery of bowel sounds,the occurrence of complications,antibiotic use time and the duration of ICU stay were recorded. Results There were no significant differences between two groups in terms of nutrition indices before treatment(P > 0. 05). After treatment,the contents of pre albumin,transferrin,total serum protein,serum albumin in treatment group were( 145. 24 ± 6. 72)g/ L,(1. 93 ± 0. 98)g/ L,(79. 86 ± 10. 38)g/ L,(54. 91 ± 2. 91)g/ L respectively,better than those in control groups((139. 24 ± 38. 76),(1. 68 ± 0. 33),(74. 73 ± 7. 31),(49. 87 ± 2. 61)g/ L),and the differences were statistically significant( t = 3. 610,2. 929,4. 360,6. 767;P < 0. 05). The incidence of stress ulcer and double infection in treatment group were 42%(26 / 62),37%(23 / 62),significantly lower than the control group(59%(37 / 62),53%(33 / 62)),and the differences were statistically significant( χ2 = 5. 186, 5. 271,P < 0. 05). Meanwhile,patients can transit to full eternal nutrition as soon as possible,then the cost of therapy is reduced and rehabilitation is quickly.
