Objective: To explore the influence of lipid emulsion of early intravenous nutrition on gain in weight and observe the side effect in very low birth weight(VLBW) infants.Methods: The infants with birth weight below 1 500 g were randomly divided into the treatment group and the control group.The treatment group received intravenous nutrition with fat emulsion within 24 hours after birth,and the control group received intravenous nutrition without fat emulsion.The weight,biochemical parameters and complication were observed after 10 days.Results:The daily weight gain of treatment group was significantly higher than the cotrol group.The risks of azotemia,high bilirubin and cholestasis were not increased in treatment group.The incidences of complication between the two groups showed no significant difference(P>0.05).Conclusion: The use of fat emulsion in early intravenous nutrition can shorten the average hospital stay,promote the weight gain,and is safe.

OBJECTIVE:To observe therapeutic efficacy and safety of Xiaoer jiegan granules with Ribavirin in the treatment of acute upper respiratory tract infection. METHODS:98 patients with upper respiratory tract infection were randomly divided into obser-vation group and control group with 49 cases in each group. Control group was given Ribavirin injection for routine upper respiratory tract antiviral therapy,10-15 mg/kg,added into 5% Glucose injection 250 ml,ivgtt,qd. Observation group was given Xiaoer jiegan granules,1g/time for 1-3 year-old children,2g/time for 4-6 year-old children and 3 g/time for 7-9 year-old,tid. A treatment course of 2 groups lasted for 5 days,both received 2 courses of treatment. Clinical efficacy,cough and other symptoms score,ADR were ob-served in 2 groups. RESULTS:The effective rate of observation group (97.96%) was higher than that of control group (87.76%), with statistical significance(P<0.05). Before treatment,there was no significant difference in the scores of fever and cough and other symptoms between 2 groups,with statistical significance(P>0.05);after treatment, the scores of fever and cough and other symp-toms in observation group were lower than in control group,with statistical significance(P<0.05). The incidence of ADR in the ob-servation group(6.12%)was significantly lower than in control group(22.45%),with statistical significance(P<0.05). CONCLU-SIONS:Xiaoer jiegan granule with Ribavirin is effective in the treatment of acute upper respiratory tract virus infection,and can im-prove fever,cough and other clinical symptoms,so as to promote better conditions and reduce the occurrence of ADR.

It puts forward suggestions on the appropriate size and work load of the nursing force in domestic second-tier hospitals in the hope of providing empirical basis for revising the size of the nursing force in such hospitals.

ObjectiveTo assess the quality of applications for the state funds for natural sciences and the research capabilities of researchers in medicine and other relevant fields in China so as to provide the State Fund Committee for Natural Sciences with scientific basis for systematically planning research funds for the medical field and improving the system of supervising funds for life sciences. MethodsAnalysis was conducted on project applications that were submitted to the Life Sciences Department from 1984 through 2001 and underwent quantitative peer review to explore trends of changes in project applications, the scores and the rate of fund-winning projects. ResultsBoth the quantity and quality of project applications in medicine and other relevant fields showed the trend of going up year by year whereas the rate of fund-winning projects presented an opposite trend. ConclusionThe State Fund Committee for Natural Sciences ought to broaden the scope of subsidization for medicine and other relevant fields and readjust existing funds for life sciences so as to shunt and alleviate the application pressure in medicine and other relevant fields.

Objective To comparative analysze of the applications value of breast-conserving surgery and improved surgical in breast cancer patients.Methods One hundred and sixty-eight female breast cancer patients from February 2005 to October 2010 in the People's Hospital of Rizhaoour hospital were selected.The patients were divided into breast-conserving surgery group (n =80) and improved surgical group (n =88).The patients in two groups were followed-up and investigated for three years.Operative time,blood loss,postoperative drainage,postoperative drainage time and hospital stay,incidence of complications,local recurrence,distant metastasis and survival after 3 year,satisfaction ratio of breast cosmetology,psychological anxiety scores and quality of life scores after 3 years of the patients in two groups were compared and analyzed.Results The complication rate of breast-conserving surgery group was 12.50％ (10/80),lower than the improved surgical group,and the difference was significant statistically (29.55％ (26/88),x2 =7.23,P＜0.05).The operative time ((72.68±5.28) min),blood loss ((39.21 ± 8.59) ml),postoperative drainage ((162.25 ± 28.56) ml),postoperative drainage time((8.16±2.58) d) and the length of hospital stay((11.43±1.98) d) of the patients in breast-conserving surgery group were lower than those of the improved surgical group,and the differences of two groups were significant statistically ((92.48 ± 6.01) min,(68.26 ± 10.23) ml,(343.29 ± 62.18) ml,(16.59±4.96) d,(18.47±2.94) d;t =22.72,19.99,24.61,13.99,18.35;P＜0.05).The satisfaction rate after 3 years of breast-conserving surgery group was 92.50％(74/80),higher than that of the improved surgical group,and the difference was significant statistically (81.82％ (72/88),x2 =4.20,P＜ 0.05).The psychological anxiety scores after 2 and 3 years of breast-conserving surgery group were (40.02 ± 2.63) points and (34.26 ± 3.59) points,lower than those of improved surgical group((42.58±2.37) points and (39.85±2.10) points),the quality of life scores after 3 years of breast-conserving surgery group was (67.58± 11.26)points,higher than that of improved surgical group ((50.85 ± 10.42) points),and the differences were significant statistically (t =6.60,12.16,9.96;P ＜ 0.05).Conclusion Breast-conserving surgery could reduce the incidence of complications and psychological anxiety situations,improve clinically relevant indicators and satisfaction ratio of breast cosmetology and quality of life for the breast cancer patients.

Electronic case report forms (eCRFs) instead of the traditional paper case report forms (pCRFs) are increasingly used by investigators and sponsors of clinical research. We include a total of 14 phase III studies (8 pCRF, 6 eCRF) to compare paper and electronic data documentation both quantitatively and qualitatively in clinical studies. The result suggests that adaptions of electronic data capture (EDC) in clinical trials have the advantages in optimization of data capture process, improvement of data quality and earlier clinical decision compared to paper-based methods. Furthermore, the successful implementation of EDC requires accouplements with corresponding data management processes and reallocation of resources.

Objective To observe the positive rate of serum anti-cardiac troponin Ⅰ autoantibodies (anti-cTnI) in the patients with coronary atherosclerotic heart disease(CHD) and its clinical significance.Methods A total of 200 CHD patients and 200 healthy controls from the Department of Cardiology,Drum Tower Hospital of Nanjing University Medical School during September 2015 and May 2016 were collected in the case-control study.Serum anti-cTnI was detected with an indirect ELISA method.Meanwhile,the general data,chnical biochemical parameters and coronary vascular lesion from the two groups were analyzed.Results The positive rate of anti-cTnI in CHD patients(21/200,10.5％) was significantly higher than that in healthy controls(4/200,2.0％,P ＜ 0.01).According to the detection results of serum anti-cTnI,the participants were divided into three groups,including anti-cTnI positive group with CHD,anti-cTnd negative group with CHD and anti-cTnI positive group in healthy controls.The median age and serum Glu level in the anti-cTnI positive group with CHD were significantly higher than those in the anti-cTnI positive group in healthy controls (P ＜ 0.05),while the levels of serum HDL-C and apoA I were inverse (P ＜ 0.01).The median age and Gensini score in the anti-cTnⅠ positive group with CHD were significantly higher than those in the anti-cTnI negative group with CHD,while the level of serum HDL-C was inverse (P ＜ 0.05).Conclusion Anti-cTnI may be more likely to occur in old CHD patients with severe coronary artery lesions,and the low levels of serum HDL-C and apoA Ⅰ may correlate to the production of anti-cTnⅠ.

Objective: To observe the effect of coxsackie virus B3 on airway tract and lung morphology, and to study the relation between CVB infection and asthma. Methods: We established CVB3 infective model: 5 d neonatal rats inhaled CVB3 by ultrasonic brume. CVB3-IgM was examined 10 d after inoculating of CVB3, and LW/BW, airway tract and lung pathological change 10 d and 30 d after inoculation of CVB3 were observed. Results: Rats from the virus group had higher D of CVB3-IgM than control's (+2s ) and had higher LW/BW 10 d after inoculation of CVB3 than control (P<0.01). Neonatal rats had acute inflammatory changes 10 d after inoculation of CVB3 and persistent changes in morphology and cytology. Conclusion: Neonatal rats virus model is established. Respiratory infection by CVB3 in neonatal rats has persistent changes in airway tract inflammatory and morphology.

Objective To evaluate the postoperative prognosis of the modification of diet in renal disease formula (MDRD) in coronary artery bypass graft surgery (CABG) in hospital or 4 years after hospitalization. Methods Two hundred and seventy-two CABG patients were divided into 3 groups according to the levels of estimated glomerular filtration rate (eGFR) including 35 cases in eGFR < 60 ml/min group, 119 cases in 60 ≤ eGFR < 90 mL/min group and 118 cases in eGFR ≥90 ml/min group. The prognostic factors of CABG patients were analyzed by COX proportional hazards models. Kaplan-Meier survival analysis was used to compare survival curves among the three groups stratified by eGFR levels. The Log-rank statistic was used for comparing between groups. Results By multivariate COX regression adjustment for body mass index, smoking, hypertension, hyperlipaemia, diabetes mellitus, previous MI, perioperative PCI and etc. , the relative risk (RR) of the increasing age for cardiac events was 1.077(95% CI 1.002-1.158,P =0.044). RR of left ventricular ejection fraction (LVEF) was 0.005(95% CI 0.000-0.456,P =0.022). RR of eGFR was 0.968(95% CI 0.948-0.988,P =0.002). The survival rate in the first, second, third and fourth year were same in every group. The survival rate of group with eGFR < 60 ml/min, 60 ≤ eGFR < 90 ml/min and eGFR≥ 90 mL/min was 76.4%, 93.1%, and 96.6%. The survival rates among three groups were statistically significant. In the survival curve of 4 year follow-up after CABG, the survival rate of group with eGFR < 60 mL/min was lower than that of 60 ≤ eGFR < 90 ml/min group and eGFR ≥ 90 ml/min group. Conclusions The preoperative eGFR is an independent risk factor in evaluating cardiac events in hospital and after hospitalization. It has a higher prognosis value in patients undergoing CABG.
