Objective Extracting high content of DHA, EPA from rude fish oil. Methods Extracting fish oil ethyl ester with silver nitrate-water method. Results With silver nitrate-water method extracting three kinds of products from rude fish oil were gained(1) The total content of EPA and DHA mixture was higher than 95%. (2) The content of DHA was higher than 95%. (3) The content of EPA was higher than DHA. Conclusion The researching technology may reduce the production cost of high content of EPA and DHA. The method researched can increase the stabilities of DHA, EPA, and decrease the side effects of the products.
2.Predicting genetic modification targets based on metabolic network analysis--a review.

Construction of artificial cell factory to produce specific compounds of interest needs wild strain to be genetically engineered. In recent years, with the reconstruction of many genome-scale metabolic networks, a number of methods have been proposed based on metabolic network analysis for predicting genetic modification targets that lead to overproduction of compounds of interest. These approaches use constraints of stoichiometry and reaction reversibility in genome-scale models of metabolism and adopt different mathematical algorithms to predict modification targets, and thus can discover new targets that are difficult to find through traditional intuitive methods. In this review, we introduce the principle, merit, demerit and application of various strain optimization methods in detail. The main problems in existing methods and perspectives on this emerging research field are also discussed, aiming to provide guidance to choose the appropriate methods according to different types of products and the reliability of the predicted results.

3.Results of different postoperative adjuvant therapies for stage Ⅰ b-Ⅱ a cervical carcinoma with risk factors

Objective To investigate the effects of postoperative adjuvant chemotherapy (CT) and chemoradiotherapy (CRT) or radiotherapy(RT) for Ⅰ b-Ⅱ a cervical cancer with risk factors.Methods From March 1995 to June 2010,there were 137 patients underwent radical hysterectomy and systematic pelvic lymphadenectomy for stage Ⅰ b-Ⅱ a cervical cancer admitted at Peking University First Hospital.These patients had risk factors,intermediate risk factors including bulky tumor (>4 cm),lymph vascular space invasion,deep stromal invasion; high risk factors including positive surgical margin,parametrial invasion,lymph node involvement.Of the all patients,79 cases of them were treated with CT,58 of them were treated with RT or CRT.The 5-year survival and prognosis factors were analyzed retrospectively,the prognosis was compared between two adjuvant therapy groups.Results The univariate analysis shown that types of pathology,different grade of risk factors,stroma invasion and lymph node involvement were prognostic factors of 5-year overall survival Patients with squamous cell carcinoma,intermediate risk factors,no parametrial invasion,and no lymph node involvement had better prognosis (P < 0.05).Whether patients with high-risk factors or intermediate-risk factors,the 5-year overall survival and 3-year disease-free survival had no difference between CT and RCT or RT groups respectively.Cox regression multivariate analysis of survival indicated that clinical stages,types of histology,different grade of risk factors were independent prognostic indicator.Patients with early stage,squamous cell carcinoma,intermediate risk factors had better prognosis.Univariate and multivariate analysis indicated that different postoperative adjuvant therapies had no effects on the prognosis.The 5-year overall survival was 88.6% in patients treated with CT,and 89.7% in patients treated with RT or CRT (P =0.455).Conclusion There are equivalent therapeutic results between CT and RT or CRT for patients with risk factors after radical surgery,CT may be as one choice of postoperative adjuvant therapy for stage Ⅰ b-Ⅱ a cervical carcinoma with risk factors.
4.Research Progress of TCM in Improving Ulcerative Colitis Based on PI3K/AKT Signaling Pathway

Ulcerative colitis(UC)is a common disease of the digestive system.Phosphatidylinositol-3-kinase(PI3K)/synuclein/threonine kinase(AKT)is closely related to cell survival,apoptosis,inflammation and other biological processes,and the expression levels of PI3K and AKT significantly increase during the course of UC,with accelerated apoptosis,improved inflammation,and damaged intestinal mucosal barrier function.In recent years,a large number of basic and clinical trials have been conducted on PI3K/AKT signaling pathway in TCM,and the results indicate that PI3K/AKT signaling pathway is expected to be an important potential target for UC treatment.This article analyzed the mechanism of the regulation of PI3K/AKT signaling pathway in TCM from monomer,extract,compound and acupuncture,and suggested that the regulation of this signaling pathway is of great significance for the prevention and treatment of UC,and provide reference for drug development.
5.Interventional Bronchoscopies for the Treatment of 112 Cases with Complex Malignant Central Airway Diseases Involved in Bifurcation

Background and objective It is very diffcult to deal with the comprehensive central airway diseases, especially the malignant airway obstruction. The aim of this study is to explore the clinical effects of multiple interventional bronchoscopies for the treatment of 112 cases with comprehensive malignant central airway diseases so as to evaluate the clini-cal applications of thermoablation and cryosurgery.Methods The 112 cases with malignant airway diseases retrospectively reviewed for bronchoscopic interventions performed in more than two lesions of carina and/or bifurcation. hTe age was from 22 to 90 years, which including 55 cases with squamous carcinoma (SQ), 16 adenocarcinoma (AD), 15 adenocystic carcinoma (ACC), 10 metastasis tumor (MT), 8 small cell lung carcinoma (SCLC) and 8 mixed carcinoma (MC). All bronchoscopic interventions such as argon plasma coagulation (APC), cryosurgery, electronic snare and stenting were performed under rigid bronchoscopy combined with electric bronchoscopy in general anesthesia.Results hTe 460 bronchoscopic procedures were successfully performed in 112 cases. Cryosurgery and APC were the most used in every group. Stent was more common in patients with MT, balloon dilation was more common in patients with ACC and AD. Electric snare and radioactive seeds im-plantation were frequently used in cases with SCLC. Trachea stricture is more severe in MT than that in others, while bronchus stricture is more severe than that of trachea in SQ and ACC group. Karnofsky performance score (KPS) is lower and shortness of breath score (SBS) is higher in MC, MT and SCLC group. Trachea and bronchus stricture is improved atfer comprehensive treatment in 5 groups except of mixed group. KPS increased and SS decreased signiifcantly in 5 groups except of adenocarci-noma. In follow-up, the overall survival time (OS) is 15 months and median survival time is 10 months, especially OS is 28.4 months in ACC and 21.7 months in AD, 8.9 months in SCLC and 7.4 months in mixed group.Conclusion APC combination with Cryosurgery and other bronchoscopic interventions are indicated for the treatment of complex or diffcult airway diseases whether they are located in trachea or bronchus. It is a safe and rapid during procedure.
6.Clinical efficacy of human papillomavirus (HPV) genotyping in screening for cervical disorders

Objective To evaluate the clinical value of HPV genotyping in diagnosis of cervical lesions.Methods Totally 1715 patients seen in Department of Obstetrics and Gynecology,Peking University First Hospital from January 2010 to December 2012 were chosen to be evaluated.All the patients experienced cervical Liquid-based cytology test,HPV genotyping and multiple punch biopsy during colposcopy and they were confirmed of cervical intraepithelial neoplasia (CIN) through histopathological examination.The clinical efficacy of HPV genotyping was evaluated.Results HPV 16,58,52,33 and 31 were the first five types detected in all of the 1715 patients.The first five types detected in the patients with CIN2 + (including CIN2/CIN3/adenocarcinoma in situ/cervical cancer) were HPV 16,58,33,52 and 31.Logistic regression analysis showed that in the CIN2 + patients who were negative for intraepithelial lesion or malignancy (NILM),those with positive HPV 16,33 and 18 had higher risk of CIN2 +,with the regression coefficient OR 5.031 (P =0.000),2.375 (P =0.000) and 1.598 (P =0.027).The cervical liquid-based cytology showed that there was a higher risk of CIN2 + for atypical squamous cells of undetermined significance (ASCUS) with positive HPV 16 and 18,and the regression coefficients OR were 5.139 (P =0.000) and 2.096 (P =0.025),respectively ; and the low-grade squamous intraepithelial lesion (LSIL) with positive HPV 16,33 and 52 had higher risk of CIN2 +,the regression coefficients OR were 5.774(P =0.000),3.368(P =0.000) and 1.747(P =0.034).Conclusion HPV genotyping is significantly useful for cervical screening.It is an important parameter for directing the clinical treatment in ASCUS and LSIL patients with negative cytology results,especially HPV 16 positive patients need to be further evaluated.
7.Analysis of plague monitoring results from 2018 to 2020 in Guizhou Province

Objective:To analyze the monitoring results of plague foci in Guizhou Province, to master the epidemic dynamics of the plague and to formulate the prevention and control measures.Methods:The plague monitoring data from 2018 to 2020 in Guizhou Province were collected from China Disease Prevention and Control Information System Plague Prevention and Control Information Management System to analyze the plague epidemic situation. The murine animals capture methods used were cage clip method, 5 m cage clip method and day-by-day rodent capture method. Bacterial culture and hemagglutination were used in laboratory.Results:A total of 2 273 murine animals were captured indoors with cage clip method, and the Rattus norvegicus was the dominant species, accounting for 47.29% (1 075/2 273), Rattus flavipectus and Mus musculus were common species, accounting for 43.55% (990/2 273) and 5.10% (116/2 273). A total of 1 460 murine animals were captured outdoors with 5 m cage clip method, and the Rattus rattus sladoni was the dominant species, accounting for 35.48% (518/1 460), Rattus flavipectus and Eothenomys miletus were common species, accounting for 16.03% (234/1 460) and 8.70% (127/1 460). A total of 5 742 murine animals (including 3 733 animals captured with cage clip method and 5 m cage clip method, 2 009 animals captured with day-by-day rodent capture method) were examined and 1 176 with flea, the flea infestation rate of murine animals was 20.48%, Xenopsylla cheopi was the dominant flea species [56.63% (3 020/5 333)] and the index was 0.53. Laboratory test results were all negative. Conclusions:The plague foci of Guizhou Province is at a quiescent stage, but the reignition cannot be ruled out. The monitoring of plague should be strengthened, the quality of the monitoring should be improved, and health education and publicity should be carried out to prevent the occurrence of human plague outbreaks.
8.Analysis of risk factors for drug resistant mutations in the reverse transcriptase region in chronic hepatitis B virus-infected patients

Objective:To analyze the risk factors that may affect the mutations in the reverse transcriptase region in chronic hepatitis B virus-infected patients.Methods:678 hospitalized cases with chronic HBV infection who underwent HBV RT testing at Tianjin Second People's Hospital from January 1, 2016 to December 31, 2016 were collected retrospectively. Among them, 417 cases were diagnosed with chronic hepatitis B, 219 cases with liver cirrhosis and 42 cases with primary liver cancer. There were 268 cases of non-use of any antiviral therapy, 138 cases of discontinuation of antiviral drugs for 6 months or more, and 272 cases of continuous antiviral therapy. HBV genotyping and RT region mutation sites were detected by direct sequencing. The risk factors that may affect the drug resistant mutation in the HBV RT mutation, including age, genotype, antiviral drug selection and medication time, hepatitis B virus infection, and biochemical markers were analyzed by univariate analysis to screen out independent risk factors.Results:Among 678 HBV-infected cases, 290 cases (42.8%) were detected with RT-region mutation. Among them, the pre-existing drug resistant rate was 6.72%, and the drug resistant mutation rate was 23.19% in treated patients. The drug resistant mutation rate of patients with continuous antiviral therapy was 66.18%. Gene mutations highest rate for 1 ~ 5 years was 27.14% in chronic HBV patients treated with antiviral therapy. Logistic regression analysis of the factors that had led to HBV mutation showed that old age, the selection of nucleoside drugs at the beginning of treatment and medication time were the main factors affecting HBV RT mutations.Conclusion:Abnormal ALT level, HBV genotype, HBV DNA quantitative level are the main factors influencing non-drug resistant mutations. Age over 60 years old, and long-term use of low-barrier nucleoside drugs are high-risk groups for HBV resistant. Therefore, HBV resistant monitoring should be strengthened.

Novel coronavirus Omicron variant infection can cause severe illness and even death in certain populations. Omicron variant infection may lead to systemic inflammatory response, coagulation disorder, multi-organ dysfunction and other pathophysiological changes, which are different from other Novel coronavirus variants to a certain extent, so therapeutic strategies should not be the same. The National Medical Center for Major Public Health Events invited experts in fields of infectious diseases, respiratory medicine, intensive care, pediatrics and fever clinic to develop this quick guideline based on the current best evidence and extensive clinical practices. This quick guideline aims to standardize the diagnosis and treatment of novel coronavirus Omicron infection, and to improve the disease management abilities of clinicians.
