1.Percutaneous balloon mitral valvuloplasty of the patients with mitral stenosis caused by active rheumatism

Objective To evaluate the effects of percutaneous balloon mitral valvuloplasty (PBMV) on repeated rheumatism attacks after continual anti rheumatic treatment in vain over a long period of time.Methods 23 cases received treatment by PBMV and follow up visits were performed.Results After operation,patients' cardiac function and their physique were improved obviously and the activity of rheumatism ceased after routine anti rheumatic treatment and prevention of streptococcal infection.Follow up visits for 36~81 months showed that mitral re stenosis occurred to only 2 patients.Conclusion Patients with mitral stenosis whose rheumatism attacks repeatedly in spite of longterm anti rheumatic treatment may be treated with PBMV first and then given anti rheumatism.In this way,a better curative effect can be obtained.
2.Influence of psychological intervention on mental state in patients with rheumatic heart disease after ;valve replacement surgery

Objective:To explore influence of psychological intervention on mental state in patients With rheumatic heart dis-ease (RHD)after valve replacement surgery.Methods:A total of 60 RHD patients after valve replacement surgery Were randomly and equally divided into routine nursing group and psychological intervention group according to number table,the latter received additional psychological intervention besides routine nursing intervention.Symptom checklist 90 (SCL-90) Was used to assess mental state of tWo groups 1 d before and one Week after surgery.Results:There Were no significant difference in SCL-90 scores betWeen tWo groups before psychological intervention (P>0.05).Compared With routine nursing group one Week after operation,there Were significant reduction in SCL-90 scores of somatization [(1.31±0.47) scores vs.(0.55±0.68)scores],obsessive-compulsive symptom [(2.11±0.35)scores vs.(0.78±0.35)scores],inter-personal relationship sensitivity [(1.57±0.53)scores vs.(1.22±0.46)scores],depression [(1.63±0.29)scores vs. (1.00±0.41)scores],anxiety [(0.98±0.17)scores vs.(0.76±0.13)scores],hostility [(1.11±0.31)scores vs.(0.80 ±0.37)scores]and phobia anxiety [(1.39±0.26)scores vs.(0.53±0.19)scores]in psychological intervention group,P<0.01 or P<0.05. Conclusion:Psychological intervention can alleviate the adverse psychological state and improve quali-ty of life in patients With rheumatic heart disease.
3.Comparison of pathogenicity between Ureaplasma urealyticum serotype 1 and 3 in genital tract of female BALB/c mice

Objective To investigate the pathogenicity of the same concentration of Ureaplasma urealyticum serotype1 (Uu1) and 3 (Uu3),alone or in combination,in genital tract of female BALB/c mice.Methods A total of 144 estradiol-pretreated adult female BALB/c mice were randomly and equally divided into 3 groups to be inoculated intravaginally with the same concentration (106 copies/g) of Uu1 and Uu3 alone or in combination.Twelve estradiol-pretreated female BALB/c mice were inoculated with sterile culture solution and served as the control group.At 1,3,7,14,21,35 days after the inoculation,8 mice in each Uu-inoculated group and 2 mice in the control group were sacrificed.Tissue specimens were obtained from the cervix,endometria and fallopian tube and subjected to hematoxylin-eosin staining followed by light microscopy.Scanning electron microscopy was used to observe the tissue specimens collected at 14 days from cervical mucosa of mice infected by Uu.Chi-square test was performed for statistical evaluation.Results The total infection rate of Uu within 3-35 days after the inoculation was 35.0% (14/40),47.5% (19/40) and 62.5% (25/40) in the Uu1 group,Uu3 group,and combination group respectively (x2 =6.07,P < 0.05).None of these mice was infected by Uu in the control group.Conclusions At the same inoculation concentration,the combination of Uu1 and Uu3 shows the strongest pathogenicity in genital tract of BALB/c mice,followed sequentially by Uu3 and Uu1 alone.
4.Relevant factors in predicting the relapse of Graves disease after treatment with antithyroid drugs

Objective To evaluate the prognostic factors of predicting the outcome of Graves disease (GD) after treatment with antithyroid drugs.MethodsA retrospective audit was performed for 306 consecutive patients with newly diagnosed GD.They were divided into successful and failure groups (including recurrent and non-stop subgroups )according to the treatment outcomes.Different prognostic factors after treatment with antithyroid drugs were compared and the state of thyrotropin suppression was observed as the euthyroid state at Months 3,6 and 12 respectively.ResultsAmong them,141patients (46.1% ) were cured and 165 patients (53.9%) had treatment failures.Age at the time of diagnosis was (46 ±10) years in the successful group and (36 ±9) years in the failure group(t =3.152,P =0.002).Free T3 ( FT3 ) was (25.2 ±8.9) and ( 18.7 ±9.4) pmol/L in the failure and successful groups respectively (t =3.326,P =0.001).The FT3 to FT4 ratio and thyrotrophin receptor antibody (TRAb) levels were higher in the failure group ( t =3.331,3.389,P =0.001).Logistic regression analysis showed that thyroid size,FT3 to FT4 ratio and TRAb at the time of diagnosis were associated with failure outcomes.The ratio of continuing thyrotropin suppression in the recurrent subgroup was more than that in the successful group ( X2 =77.515,114.441,136.232,all P < 0.01).ConclusionsThe GD patients with a large thyroid size and high pre-mediation levels of TRAb and FT3 to FT4 ratio are more prone to respond unfavorably to antithyroid drug treatment.And those with a large thyroid size and post-medication ophthalmopathy and continuing thyrotropin suppression have a high rate of recurrence.
5.Diagnostic efficiency of postprandial plasma glucose and oral glucose tolerance test in patients with metabolic disorders

A total of 1342 individuals underwent physical examinations according to the criteria of metabolic syndrome of International Diabetes Federation (IDF) in 2005.And 314 patients with metabolic disorders were screened for diabetes by standard meal and methods oral glucose tolerance test (OGTT).Newly diagnosed diabetics was 12 (4.1% ) vs.17 (5.8%) respectively.No significant difference existed between two methods (P =0.332).Kendall's (τ)b =0.313,Kendall's (τ)c =0.208 and Gamma coefficient =0.580 (P =0.000).The mixed meal method was correlated with OGTT,Kappa =0.258 (P =0.000) and two methods were consistent.Diabetic screening should be stressed in the subjects with metabolic abnormalities.And the detecting efficiency of postprandial plasma glucose is similar to OGTT.
6.Prevalence of the metabolic syndrome in middle-aged and elderly individuals

Totally 1136 individuals aged over 40 underwent health check up in March to May 2009. Fasting blood glucose, 2-h post-challenge blood glucose, glycated hemoglobin A1c ( HbA1c), total cholesterol, triglyceride, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol and serum uric acid were measured. The diagnosis of metabolic syndrome was based on the International Diabetes Federation Criteria. The results shows that total body fat, region body fat indices, blood pressure, blood glucose and serum uric acid levels increased with the age ( P < 0.01) . The prevalence rate of metabolic syndrome in this group of individuals were 15. 1% (171/1136) , and increased with age (P<0.01). The most common combination of metabolic syndrome was central obesity-hypertension-dyslipidemia (40. 9% , 70/171). Unconditional logistic regression revealed that waist-hip-ratio, body mass index and uric acid were the risk factors for metabolic syndrome.
7.Relationship between serum matrix metalloproteinase-9 level and coronary heart disease in type 2 diabetic patients

Twenty-seven cases of simple type 2 diabetes mellitus,30 cases of coronary heart disease,and 32 cases of type 2 diabetes with coronary heart disease were enrolled in this study according to the results of coronary angiography.Meanwhile,32 healthy subjects were taken as a control group.The serum matrix metalloproteinase-9 (MMP-9) and other clinical and laboratory parameters were determined.The results showed that serum MMP-9 may play a minor role in the progression of coronary heart disease in type 2 diabetic patients.
8.Predictive value on chemotherapeutic sensitivity of the changes of cell cycle and proliferation on locally advanced cervical cancer in the pre-neoadjuvant chemotherapy and post-neoadjuvant chemotherapy

ObjectiveTo investigate the effect of neoadjuvant chemotherapy in locally advanced cervical cancer on cell cycle,proliferation,and evaluate the feasibility of proportion of cell in different phase and proliferating cell nuclear antigen(PCNA) as sensitive indices to assess chemotherapeutical sensitivity and therapeutical effect.MethodsForty-nine cases of locally advanced cervical cancer were divided into response group and no-response group according to the clinical efficacy of neoadjuvant chemotherapy.Compared the proportion of cell in different phase and proliferation index of PCNA between two groups.The clinical therapeutic effect was evaluated after 4 weeks in the second neoadjuvant chemotherapy regimen.Results In 49 patients with locally advanced cervical cancer,clinical effective of 39 cases (response group),no effective of 10 cases(no-response group).The S-phase proportion of cell in the pre-neoadjuvant chemotherapy and post-neoadjuvant chemotherapy of response group[ (21.47 ± 5.21 )% and(18.32 ±5.07)%] were higher than those of no-response group [ (9.63 ± 2.58)% and ( 10.14 ± 2.32)% ] (P < 0.05 ).The proliferation index of PCNA of cervical cancer in the pre-neoadjuvant chemotherapy of response group [ ( 81.67 ± 7.14)% ] was higher than that of no-response group [ (66.99 ± 2.29 )% ] (P < 0.05 ).Conclusion The S-phase proportion of cell and proliferation index of PCNA in the pre-neoadjuvant chemotherapy are important indexes to assess chemotherapeutical sensitivity and therapeutical effect.
9.The influence factor of 18F-FDG and 18F-FLT PET/CT for diagnosing pulmonary nodules

Objective To evaluate the influence factor of 'SF-FDG and 18F-FLT PET/CT for diagnosing pulmonary nodules.Methods 55 patients with pulmonary nodules were chosen as study target,include 33 male cases and 22 female cases,the age range from 17 to 82 years old.In all cases,28 cases had one pulmonary nodule,others had 2 or 3 pulmonary nodules,and the nodule size was 0.6-11.0 cm.18F-FDG and 18F-FLT PET/CT were performed on 55 patients.The relationship of SUV of 18F-FDG and 18F-FLT with sex,age,size and pathology of all pulmonary nodule patients were analyzed.Results The SUV of 18F-FDG and 18F-FLT had no statistics difference on sex,age and size(P>0.05),the SUV of 18F-FDG had no statistics difference on pathology of pulmonary nodule(P>0.05).Otherwise,the SUV of 18F-FLT had statistics difference on pathology of pulmonary nodule(P<0.05).Conclusion The pathology of pulmonary nodule was an important influence factor for SUV of 18F-FLT,and the ~SF-FLT PET/CT has the high diagnostic effectiveness for pulmonary nodules and this method will has great influence on the clinical management of pulmonary nodule patients.
10.Treatment of discoid meniscus injury by arthroscopic surgery

Objective To investigate the bettertherapeutic method and clinical efficacy of discoid meniscus injury under arthroscope.Methods Forty-one patients with discoid meniscus injury were selected as our subjects from Nov.2011 to Feb.2013,who were treated by arthroscopic surgery and performed with meniscus plasty.Thirt-eight patients were received ultra Fast-fix meniscal repair system.Joint function exercises were performed on bed within 1 month after operation.Incomplete weight bearing were performed after 1 month postoperation.After 3 month postoperation,they were permitted for full weight bearing.Results All patients were treated successfully with surgery and no postoperative complications occurred.The patients were followed up from 3.0 to 18.0 months and average was 10.5 months.The preoperative Lysholm scores were(68.28 ±8.04)) lower than that at the postoperation (88.17 ± 7.49)) and the difference were significant (t =11.59,P < 0.05).Furthermore,according to Ikeuchi criterion,the outcomes were 28 cases with excellent,10 cases with good and 3 cases with general rank.Conclusion Discoid meniscus plasty and suture under arthroscope is proved to be a better approach with less trauma,recovery quickly and fewer complications.It should be a preferred method for discoid meniscus injury.
