OBJECTIVE:To observe clinical efficacy of acitretin combined with TGP in the treatment of psoriasis vulgaris. METHODS:81 patients were randomized into control group(n=40)and treatment group(n=41). Control group were treated with 0.5 mg?kg-1?d-1acitretin then the dose of acitretin was adjusted to 0.1~0.2 mg?kg-1?d-1after clinical symptom improved.Treat-treatment course. Blood and urine routine,liver and kidney function and the level of blood lipid of 2 groups were detected before and after 8 weeks treatment as well as ADR. 3.Evaluation of tissue doppler echocardiography on detecting early myocardial relaxation abnormality in adriamycin-induced cardiomyopathy rabbits

AIM: To evaluate myocardial relaxation function changes in an adriamycin-induced cardiomyopathy model using the transmitral flow velocity to mitral annular velocity ratio (E/E'), a strong positive relationship with left ventricular filling pressure and a good indicator for evaluating left ventricular diastolic relaxation abnormality. METHODS: Twenty-eight Japanese rabbits were divided into two groups. Adriamycin was administered at cardiomyopathy model. 8 rabbits served as controls receiving the same amount of saline once a week for a total of 8 weeks. Conventional and tissue Doppler echocardiography (TDE) were performed at baseline, 4th, 6th,8th, 10th and 12th week. RESULTS: In the adriamycin-treated group, LV chamber diameter significantly increased, while ejection fraction and fraction shortening significantly decreased in 10th and 12th week (P <0.05). The significant changes were firstly found in 10th week. Mitral annulus systolic peak velocity (S') by TDE significantly decreased in 8th, 10th and 12th week ( P <0.05). The significant changes were firstly found in 8th week. The ratio of E/E' significantly increased in 6th,8th, 10th and 12th week (P < 0.05). The significant changes were firstly found in 6th week. In the control group, no significant changes were found in all parameter by tissue Doppler conventional echocardiography (P >0.05). CONCLUSION: Myocardial function is reduced in adriamycin-induced rabbit model of dilated cardiomyopathy. The relaxation parameter (E/E') by TDE changes is earlier than contraction indices S' by TDE and conventional echocardiography in adriamycin-induced cardiomyopathy rabbits, which provides a new sensitive and reliable method to evaluate LV relaxation function.

5.Relationship between serum matrix metalloproteinase-9 level and coronary heart disease in type 2 diabetic patients

Twenty-seven cases of simple type 2 diabetes mellitus,30 cases of coronary heart disease,and 32 cases of type 2 diabetes with coronary heart disease were enrolled in this study according to the results of coronary angiography.Meanwhile,32 healthy subjects were taken as a control group.The serum matrix metalloproteinase-9 (MMP-9) and other clinical and laboratory parameters were determined.The results showed that serum MMP-9 may play a minor role in the progression of coronary heart disease in type 2 diabetic patients.
6.Evaluating myocardial function of a cardiomyopathy rabbit model following bone marrow stromal cell transplantation by tissue Doppler echocardiography

BACKGROUND: Tissue Doppler echocardiography (TDE) has been proved to evaluate general and local function of heart but less reported on adriamycin-induced cardiomyopathy following bone marrow stromal cell (BMS) transplantation.OBJECTIVE: To evaluate myocardial function of an adriamycin-induced cardiomyopathy rabbit model following BMS transplantation using TDE.DESIGN, TIME AND SETTING: A randomized animal control study was performed at Laboratory of Ultrasound, Union Hospital Affiliated to Tongji Medical College, Huazhong University of Science and Technology from September 2002 to December 2003.MATERIALS: A total of 28 male adult Japanese rabbits weighing (2.0+0.2) kg were used in this study. Adriamycin was used to induce cardiomyopathy model in 20 rabbits.METHODS: Twenty-eight male Japanese rabbits were randomly divided into three groups: cell transplantation group (n=10),PBS group (n=10), and sham operation group (n=8). BMSs were isolated from cell transplantation group at the 8th day. On the 12th week, cells were labeled with 4, 6-diamidino-2-phenylindole (DAPI), and then epicardial directly injected into the myocardium of the same rabbits in thoracotomy surgery. Non-cell only culture fluid PBS was injected in PBS group. Sham operation group underwent thoracotomy surgery with the same volume of saline injection.MAIN OUTCOME MEASURES: Left ventricular function was assessed by conventional and tissue Doppler echocardiography before and 4 weeks after surgery. Histological examination including apotisis study and DAPI fluorescent were assessed after sacrificed.from (4.0+1.1) cm/s to (5.3+1.2) cm/s (P < 0.05) around the inject site, but the improvement of global myocardial function was not found by conventional echocardiography. In PBS and sham operation group there were no differences in global and myocardium at 4 weeks after transplantation. Histological findings showed that the injury of the myocardium around the injection site was relieved with less apoptosis.
7.Protective effect of hyperoxic fluid on rat liver transplantation

BACKGROUND: Organ for transplantation is insufficient, and primary transplant of nonfunction caused by perfusion cryopreservation occasionally occurs. It is clinically significant to reduce organ damage caused by perfusion preservation. OBJECTIVE: To explore the protective effect of hyperoxic perfusion fluid on liver transplantation in rats. METHODS: A total of 40 Wistar rats were randomly divided two groups (n = 20) and respectively poured with Ringer lactate solution or hyperoxic ringer lactate solution. Each group comprised equal number of donors and recipients to prepare liver, kidney, and pancreas transplantation models. Hyaluronic acid (HA), alanine aminotransferase (ALT) and CD8+CD28- T cells were compared between two groups at the end of perfusion, and 1st and 3rd days after liver transplantation. The acute rejection score of liver tissues were also compared after operation. RESULTS AND CONCLUSION: The HA, ALT and CD8+CD28-T cells were no significantly different between two groups before operation (P> 0.05). The HA and ALT of hyperoxic ringer lactate solution group was significantly Ringer lactate solution group after liver transplant (P < 0.05), but the CD8+CD28-T cells were greater (P < 0.05). The acute rejection scores for liver in hyperoxia liquid group were significantly less than the common liquid group (P< 0.05). Results show that hyperoxic solution can attenuate ischemia/reperfusion injury and protect rats undergoing liver transplantation.

9.Significance of the neutrophil-lymphocyte ratio in elderly type 2 diabetic patients with coronary heart disease

Objective To evaluate the value of the neutrophil-lymphocyte ratio (NLR) in elderly type 2 diabetic patients (T2DM) with coronary heart disease (CHD).Methods We performed a retrospective observational study on 228 patients undergoing coronary angiography in Guizhou Provincial People's Hospital from April 2014 to July 2015.Patients were divided into three groups:the simple T2DM group (n=77),simple CHD group (n=72),and T2DM complicated with CHD group (n=79).Meanwhile,70 healthy elderly subjects served as the control group.The white blood cell count,high-sensitivity C-reactive protein (hs-CRP) and other clinical and laboratory parameters were collected,and NLR was calculated.Risk factors for CHD in T2DM patients were determined by logistic regression analysis.Multiple stepwise regression analysis was adopted to identify factors influencing NLR.Results The white blood cell count,neutrophil count,NLR and hs-CRP level in the simple T2DM,CHD,and T2DM+CHD groups were higher than in the control group [(7.48 1.81) 109/L,(7.72± 1.89) 109/L,(7.98±2.12) 109/L vs.(6.22± 1.61) 109/L;(4.49±1.38) 109/L,(4.88±1.56) 109/L,(5.35±1.40) 109/L vs.(3.52±0.84) 109/L;(2.84± 0.77),(3.07±0.79),(3.34±0.83) vs.(1.58±0.42);(2.92±0.65) mg/L,(3.20±0.86) mg/ L,(4.98±1.10) mg/L vs.(1.105±0.23) mg/L;respectively,P<0.05 or P<0.01].The lymphocyte count in the simple T2DM,CHD,and T2DM+CHD groups were lower than in the control group [(1.57±0.41) × 109/L,(1.58±0.40) × 109/L,(1.61±0.48) × 109/L vs.(2.22± 0.51) × 109/L,P<0.05].NLR and hs-CRP levels in the T2DM+CHD group were higher than in the former two groups (all P<0.05).Pearson correlation analysis showed that NLR was positively correlated with the Gensini score and hs-CRP level (r=0.7455 and 0.7792,both P<0.01).Logistic regression analysis showed that NLR,hs-CRP levels and glycosylated hemoglobin A1c (HbA1c) were the risk factors for CHD in T2DM patients (OR=4.331,3.997 and 2.928,all P<0.05).Multiple stepwise regression analysis showed that NLR was positively correlated with fasting plasma glucose,HbA1 c levels and systolic blood pressure (β' =0.3133,0.4720 and 0.3069,all P<0.05).Conclusions NLR may be a valuable predictive factor for CHD in elderly T2DM patients.
10.Impact of remature clopidogrel cessation and intra-operative tranexamic acid on bleeding and transfusion outcomes in on-pump CABG and their interaction

Objective To evaluate premature clopidogrel cessation,intraoperative tranexamic acid and their interaction on bleeding and transfusion outcomes in on-pump CABG patients.Methods The current study is a prospective and randomized trial with 2 × 2 factorial design.The first factor is preoperative clopidogrel with 2 levels,clopidogrel ingestion within 7 days preoperatively (group E) and nave to clopidogrel (group B).The second level is antifibrinolytic therapy with 2 level,tranexamic acid (group T) and placebo (group P).A total of 333 patients receiving selective on-pump CABG were recruited.The tranexamic acid regimen was a bolus of 10 mg · kg-1 followed by a maintenance of 10 mg · kg 1 · h-1 throughout the surgery.Results Baseline characteristics were fairly balanced among the groups.Tranexamic acid significantly reduced postoperative blood loss.major bleeding,the volume of erythrocyte and plasma transfused,the exposure of erythrocyte,plasma and any allogeneic products (ET vs EP,P < 0.01 ; BT vs BP,P < 0.01).Clopidogrel within 7 days preoperatively significantly increased blood loss (EP vs BP,P<0.05),major bleeding,the volume of erythrocyte (EP vs BP,P<0.01) and the exposure of erythrocyte and plasma (EP vs BP,P < 0.05) and any allogeneic products (EP vs BP,P < 0.01).Under the protection of tranexamic acid,the bleeding and transfusion outcomes were comparable between the patients with premature clopidogrel cessation and those nave to clopidogrel (ET vs BP,P >0.05).Perioperative mortality,morbidity and the incidence of adverse events were comparable among the groups except for IABP.Conclusion Comparing with nave to clopidogrel,premature cessation within 7 days preoperatively deteriorated bleeding and transfusion outcomes in on-pump CABG patients.Intraoperative tianexamie acid could reduce the risk.
