1.Risk factors for mechanical ventilation in infants and young children undergoing cardiac surgery with pulmonary hypertension

Objective To evaluate factors associated with mechanical ventilation in infants and young children undergoing cardiac surgery with pulmonary hypertension.Methods 161 Children who were ≤36 months of age, had congenital heart disease with pulmonary hypertension, and had been treated at our hospital between October 2011 and April 2013 were included in this retrospective study.Stepwise linear regression analysis was used to analyse the independent risk factors associated with mechanical ventilation (MV).Results The average of MV time was 40.9 h, the independent risk factors for MV were cardiopulmonary bypass(CPB) time (P =0.020), risk adjustment for surgery for congenital heart disease (RACHS-1) (P =0.014) ,low cardiac output syndrome(LCOS) (P <0.001), ventilator-associated pneumonia(VAP) (P =0.001) and postoperative pulmonary artery systolic pressure (sPAP) (P < 0.001).Conclusion The present results suggest that CPB time,RACHS-1 score, LCOS, VAP and postoperative sPAP can significantly affect the MV time in infants and young children with pulmonary hypertension after cardiac surgery.
2.Immunosuppressive Agents for Renal Transplantation Patients in Our Hospital in 2004:Drug Use Analysis

OBJECTIVE:To investigate the recent use of immunosuppressive agents for renal transplantation patients during our hospital.METHODS:Case history of124renal transplantation patients in our hospital in2004was reviewed ret-rospectively,and the categories and the associated costs of immunosuppressive agents were statistically analyzed,which were compared with that of2000.RESULTS:In2004,patients’mean hospitalization cost was74462.77yuan and mean immuno-suppressive agents cost was25809.93yuan,increased by75.12%and175.35%,respectively as compared to2000.Of the total drug costs,the immunosuppressive agents accounted for61.37%in2004and46.4%in2000.Dominated the first3places in the list of DDDs of immunosuppressive agents in2004were Methylprednisolone,ciclosporin A and dehydrocortisone,respec-tively.CONCLUSIONS:As compared with2000,the application categories and costs of neotype immunosuppressive agents for renal transplantation patients in our hospital increased in2004,thus to find a more effective immunosuppressive remedy that of less adverse drug reaction and lower cost is of great significance.
3.Drug-induced pulmonary injury: CT findings in hemopathic patients

Objective To investigate the spiral CT findings in hemopathic patients with druginduced pulmonary injury.Methods CT images obtained in 11patients with drug-induced pulmonary injury were retrospectively analyzed.Six patients had antineoplastic agent-induced pulmonary injury and 5 patients had non-neoplastic agent-induced pulmonary injury (immunosuppressor in 2 patients,antifungal in 2 patients,antineoplastic immunomodulators in 1 patient).CT findings were reviewed by a chest radiologist.Results All 11patients had parenchymal abnormalities on MSCT scans,including ground-glass opacities( n =8 ),consolidation( n =5 ),interlobular septal thickening( n =3 ) and focal fibrosis ( n =2 ).The abnormalities were bilateral and asymmetric in all patients.They were mainly in the peripheral lung regions in 6 patients,in the central lung regions in four,and irregularly located in one.The abnormalities involved mainly the lower lung zones in six patients,the upper lung zones in two,and all lung zones homogeneously in three.One patient had fluid in bilateral pleural cavities.Three patients were given the same agent once more after the imaging turned to normal,and they presented with same clinical symptoms and similar but more serious imaging findings.Conclusions Drug-induced pulmonary injury usually manifests as areas of ground-glass opacity and consolidation,which most commonly involves the peripheral lungs and lower lung zones.Drug-induced pulmonary injury shows reproducible but more serious lesions when the patient is given the same agent once more.
4.THE USE OF 6-AMINOCAPROIC ACID (EACA) TO REDUCE POSTOPERATIVE BLEEDING FOLLOWING IN MITRAL VALVE REPLACEMENT

Objective To determine the effect of 6-aminocaproic acid (EACA) to reduce postoperation bleeding following in mitral valve replacement.Method 50 adult patients uddergoing mitral valve replacement was adopted a double-bisind randomized trial to be divided into two groups.Each group is 25 patients.The patients in control groups received NS,the ones in study groups received NS as same as control groups but added EACE 10 g by intravation.The bleeding amount in cavitas thoracis were recorded on the sixth,twenty-fourth postoperative hours and ACT was recorded as well.Results The bleeding amount in study groups on the sixth postoperative hours were (290.0±41.3) ml.On twenty-fourth postoperative hours were (336.3±81.3) ml.The bleeding amount in control groups on the sixth postoperative hours were (393.4±73.6) ml.On twenty-fourth postoperative hours were (450.0±79.6) ml.The bleeding amount in study groups were decreased significantly as compared with in control groups (P<0.05).Conclusion:6-aminocaproic acid can reduce postoperation bleeding following in mitral valve replacement.
5.Clinical Evaluation of Lowering the Morbidity of Cerebrovascular Disease and the Plasma Cysteine Levels by B Group Vitamins Based on Meta-analysis

Objective:To assess the effect of B group vitamins on cerebrovascular diseases, and also to evaluate the relationship between B group vitamins and homocysteine(Hcy). Methods:Using the words homocysteine,folic acid,folate,cerebrovascular dis-ease,B group vitamins and randomized controlled trial as the keywords,PubMed,the Cochrane library,WanFang Database,VIP and CNKI were searched. The randomized controlled trials about preventive effect of folic acid,vitamin B12or vitamin B6on cerebrovascular disease occurring published between 1998 and July 2016 were searched by manually retrieving relevant journals. Two reviewers inde-pendently screened the trials,extracted the data and evaluated the quality of included trials. The Cochrane Collaboration's RevMan 5.3 software was used for the statistical analysis. Results:Twelve trials involving 36 053 patients were included. The results of meta-analysis showed that B group vitamins could lower the risk of stroke(RR=0.90,95% CI:0.82-0.99,P=0.02). The sensitivity a-nalysis conducted after the exclusion of low-quality trials did not change the overall results, and B group vitamins could lower plasma Hcy concentration (MD= -3.40,95% CI:-4.21--2.59, P<0.000 01). Conclusion: B group vitamins can lower plasma Hcy concentration,and lower the risk of stroke. B group vitamins supplementation should be recommended for the secondary prevention of stroke. The results of this study still need more reasonable design combined with strict implementation of larger samples of randomized double-blind controlled trials.

With the rapid development and wide application of traditional Chinese medicine injection (TCMI), a number of adverse events of some TCMIs have incessantly been reported and have drawn broad attention in recent years. Establishing effective and practical analytical methods for safety evaluation and quality control of TCMI can help to improve the safety of TCMIs in clinical applications. In this study, a sensitive and rapid high-performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS) method has been developed and validated for the quantitative determination of potentially harmful substance 5,5'-oxydimethylenebis (2-furfural, OMBF) in TCMI samples. Chromatographic separation was performed on a C18 reversed-phase column (150 mm × 2.1 mm, 5 µm) by gradient elution, using methanol-water containing 0.1% formic acid as mobile phase at the flow rate of 0.3 mL/min. MS/MS detection was performed on a triple quadrupole mass spectrometer with positive electrospray ionization in the multiple reaction-monitoring mode. The method was sensitive with a limit of quantification of 0.3 ng/mL and linear over the range of 0.3-30 ng/mL (=0.9998). Intra- and inter-day precision for analyte was <9.52% RSD with recoveries in the range 88.0-109.67% at three concentration levels. The validated method was successfully applied to quantitatively determine the compound OMBF in TCMIs and glucose injections. Our study indicates that this method is simple, sensitive, practicable and reliable, and could be applied for safety evaluation and quality control of TCMIs and glucose injections.
7.Mid-term effectiveness of hip preservation in the reconstruction of ultrashort bone segments in the proximal femur with three-dimensional printed customized cementless intercalary endoprosthesis with an intra-neck curved stem.

OBJECTIVE:
To explore the design points of a three-dimensional (3D) printed customized cementless intercalary endoprosthesis with an intra-neck curved stem and to evaluate the key points and mid-term effectiveness of its application in the reconstruction of ultrashort bone segments in the proximal femur.
METHODS:
Between October 2015 and January 2021, 17 patients underwent reconstruction with a 3D printed-customized cementless intercalary endoprosthesis with an intra-neck curved stem. There were 11 males and 6 females, the age ranged from 10 to 76 years, with an average of 30.1 years. There were 9 cases of osteosarcoma, 4 cases of Ewing sarcoma, 2 cases of chondrosarcoma, 1 case of liposarcoma, and 1 case of myofibroblastoma. The disease duration was 5-14 months, with an average of 9.5 months. Enneking staging included 16 cases of stage ⅡB and 1 case of stage ⅢB. The distances from the center of the femoral head to the body midline and the acetabular apex were measured preoperatively on X-ray images. Additionally, the distances from the tip of the intra-neck curved stem to the body midline and the acetabular apex were measured at immediate postoperatively and last follow-up. The neck-shaft angle was also measured preoperatively, at immediate postoperatively, and at last follow-up. The status of osseointegration at the bone-prosthesis interface and bone growth into the prosthesis surface were assessed by X-ray films, CT, and Tomosynthesis-Shimadzu metal artefact reduction technology (T-SMART). The survival status of the patients, presence of local recurrence or distant metastasis, and occurrence of postoperative complications were assessed. The recovery of lower limb function was evaluated pre- and post-operatively using the Musculoskeletal Tumor Society (MSTS) scoring system, and pain relief was evaluated using the visual analogue scale (VAS) scores.
RESULTS:
The patient's femoral resection length was (163.1±57.5) mm, the remaining proximal femoral length was (69.6±9.3) mm, and the percentage of femoral resection length/total femoral length was 38.7%±14.6%. All 17 patients were followed up 25-86 months with an average of 58.1 months. During the follow-up, 1 patient died of lung metastasis at 46 months postoperatively, and the remaining 16 patients survived tumor-free. There was no complication such as periprosthetic infection, delayed incision healing, aseptic loosening, prosthesis fracture, or periprosthetic fracture. No evidence of micromotion or wear around the implanted stem of the prosthesis was detected in X-ray and T-SMART evaluations. There was no significant radiolucent lines, and radiographic evidence of bone ingrowth into the bone-prosthesis interface was observed in all stems. There was no significant difference in the distance from the tip of the curved stem to the body midline and the apex of the acetabulum at immediate postoperatively and last follow-up compared with the distance from the center of the femoral head to the body midline and the apex of the acetabulum before operation, respectively (P>0.05), and there was no significant difference in the above indexes between immediate postoperatively and last follow-up (P>0.05). The differences in the neck-shaft angle at various time points before and after operation were also not significant (P>0.05). At last follow-up, the MSTS score was 26.1±1.2 and the VAS score was 0.1±0.5, which were significantly improved when compared with those before operation [19.4±2.1 and 5.7±1.0, respectively] (t=14.735, P<0.001; t=21.301, P<0.001). At last follow-up, none of the patients walked with the aid of crutches or other walkers.
CONCLUSION
The 3D printed customized cementless intercalary endoprosthesis with an intra-neck curved stem is an effective method for reconstructing ultrashort bone segments in the proximal femur following malignant tumor resection. The operation is reliable, the postoperative lower limb function is satisfactory, and the incidence of complications is low.
