2.Treatment for pancreatic pseudocysts, report of 53 cases

Objective To evaluate the treatment and clinical outcome of pancreatic pseudocysts.Methods Clinical data of 53 pancreatic pseudocyst patients treated from Jan 2008 to Oct 2014 was retrospectively analyzed.Results 9 cases underwent CT-guided percutaneous catheter drainage,among them 3 cases underwent reoperation.8 cases underwent endoscopic ultrasonography-guided transmural drainage,including transgastric approach in 7,transduodenal approach in 1,Procedures in all these 8 cases were successful and pseudocyst disappeared completely without complication.Surgical procedures were performed in 36 cases,3 by external drainage,1 of the 3 underwent reoperation;11 cases underwent cystgastrostomy,with 2 suffering from postopertative gastrointestinal bleeding;3 cases underwent cystduodenostomy;19 cases underwent Roux-en-Y cystojejunostomy,with 1 suffering from postopertative gastrointestinal bleeding.Follow-up ranged from 4 months to 6 years,there was no cyst recurrence.Conclusion The treatment of pancreatic pseudocysts is diversified and should be individualized.Internal drainage is the mainstay of treatment,preferably through endoscopic ultrasonography-guided transmural drainage,leaving difficult cases to open cystojejunostomy.
3.Characteristics of multiple evanescent white dot syndrome with multimodal imaging

Objective To observe the characteristics of multiple evanescent white dot syndrome (MEWDS) with modern multimodal imaging modalities.Methods This was a retrospective case study.Eleven patients (11 eyes) diagnosed with MEWDS were enrolled.There were 10 females and 1 male,mean age was 27.6 years (range 15-41 years).The period between disease onset and visiting to the hospital was between 2 to 13 days,the average time was 4.7 days.All the patients underwent examinations of best corrected visual acuity,slit-lamp biomicroscope,indirect ophthalmoscope,fundus color photography,fundus autofluorescence (FAF),fundus fluorescein angiography (FFA),indocyanine green angiography (ICGA) and spectral domain optical coherence tomography (SD-OCT).The mean follow up duration was 3.2 months.The imaging characteristics were compared.Results Fundus color photography showed foveal orange-red granularity in all eyes.FAF showed strong autofluorescence with a vague boundary.FFA showed a variable number of highly fluorescent fine needle-like dots arranged in a ring in the early stage,and fluorescence remained in the late stage.ICGA showed advanced lesions of vague boundary merged into a large plaque of deep retinal hypofluorescence.SD-OCT showed the hyperreflectant material deposit over the retinal pigment epithelium and extending anteriorly through the interdigitation zone,ellipsoid layer,and toward the external limiting membrane.At the site of extrafoveal lesions,SD-OCT revealed the presence of discontinuities or disruptions centered on the ellipsoid zone to include the interdigitation.Conclusions In MEWDS patients,fundus photography showed foveal orange-red granularity;FFA showed early fluorescent dots distributed in a ring pattern;ICGA showed hypofluorescent lesions in the later stage;SD-OCT showed disruption of the interdigitation zone and ellipsoid zone and accumulations of hyperreflective material that was of variable size and shape;FAF showed strong autofluoresce areas that correlated to spots observed with FFA and ICGA.
4.Effect of Metformin Combined with Insulin Aspart on Serum Cholesterol, Total Bilirubin, Uric Acid, Urinary Micro Protein Levels, Matemal and Infant Outcomes of Gravida with Gestational Diabetes Mellitus

Objective:To study the effect ofmetformin combined with insulin aspart on the serum cholesterol(TC),total Bilirubin (TBil),uric Acid(UA),urinary Micro Protein(mAlb) levels and Maternal and Infant Outcomes of gravida with Gestational Diabetes Mellitus.Methods:84 patients ofgestational diabetes mellitus who received therapy from June 2014 to June 2016 in our hospital were selected.According to random number table,those patients were divided into the observation group (n=42) and the control group (n=42),on the basis of routine treatment,The control group was treated with insulin aspart,while the observation group was combined with metformin hydrochloride.The blood glucose index and the levels of TC,TBil,UA,mAlb and maternal and infant outcomes were compared.Results:After treatment,the levels of fasting blood glucose (FBG),postprandial 2h blood glucose (2hPG),glycosylated hemoglobin (HbA1c),TC,TBil,UA and mAlb in the observation group were significantly lower than the control group,the levels of TBil was significantly higher than the control group (P<0.05);the incidence ofgestational hypertension,hydramnios,premature birth,cesarean section,giant child and neonatal jaundice were significantly lower than the control group (P <0.05).Conclusion:Metformin combined with insulin aspart was well for gestational diabetes mellitus,which could effectively improve the blood glucose indicators and TC,TBil,UA,mAlb levels,maternal and infant outcomes.
5.Rapid automated analysis method of flow cytometry data

Objective A major component of flow cytometry data analysis involves gating , which is the process of identifying homogeneous groups of cells .As manual gating is error-prone, non-reproducible, nonstandardized, and time-consuming , we propose a time-efficient and accurate approach to automated analysis of flow cytometry data .Methods Unlike manual analysis that successively gates the data projected onto a two-dimensional filed, this approach, using the K-means clustering results , directly analyzed multidimensional flow cytometry data via a similar subpopulations-merged algorithm.In order to apply the K-means to analysis of flow cytometric data , kernel density estimation for selecting the initial number of clustering and k-d tree for optimizing efficiency were proposed .After K-means clustering , results closest to the true populations could be achieved via a two-segment line regression algorithm .Results The misclassification rate (MR) was 0.0736 and time was 2 s in Experiment One, but was 0.0805 and 1 s respectively in Experiment Two. Conclusion The approach we proposed is capable of a rapid and direct analysis of the multidimensional flow cytometry data with a lower misclassification rate compared to both nonprobabilistic and probabilistic clustering methods .
6.An algorithm of real-time peak detection for pulse signals based on FPGA

Objective To present a new algorithm of real-time peak detection for pulse signals based on FPGA , which is known as the dynamic threshold with half peak detection method ( DT-HP ) .Methods With Gaussian-shaped pulse signals as the target , the method was improved from conventional methods .The FPGA detection process of the algorithm required no more than three detection parameters: the starting point , the maximum value and the pulse width .Results The algorithm solved the floating baseline and repeating detection that occur in traditional methods .Compared with the results of polynomial fitting method and flow cytometry , the difference was only 3.2% and 9.3%.Conclusion The algorithm takes less time ,RAM, and cache while allowing floating baseline detection , which can be used as an effective method for rapid detection in FPGA .
7. Progress of Burkitt lymphoma treatment

Burkitt lymphoma (BL) is an aggressive B-cell non-Hodgkin lymphoma which often occurs in children. The cure rate of BL is significantly increased with the wide application of short-duration and high-intensity immunochemotherapy, however, chemotherapy-related adverse reactions are still a big problem in adult patients and human immunodeficiency virus (HIV) positive patients. Meanwhile, the absence of effective immune targeting new drugs in patients with relapsed and refractory BL needs to be solved clinically. How to optimize the therapeutic regimen to reduce the chemotherapy-related adverse reactions and develop effective immune targeting new drugs is the hot spot in current research. This paper reviews the treatment progress of BL according to the 59th American Society of Hematology (ASH) Annual Meeting.
8.Combined lumbar pelvic fixator for treatment of unstable vertical fracture and dislocation of old posterior pelvic ring

Objective:To investigate the clinical effect of using combined lumbar pelvis fixation device in the treatment of old vertical unstable fracture dislocation of posterior pelvic ring.Methods:Data of 7 patients with old vertical unstable fracture dislocation of posterior pelvic ring admitted and followed up from January 2017 to April 2020 were retrospectively analyzed, including 4 males and 3 females with an average age of 42.4 years old (range, 22-73 years old). There were 3 cases of traffic injury, 3 cases of falling injury and 1 case of tumble injury. According to Tile classification for pelvic fractures, there were 5 cases of type C1, 1 case of type C2 and 1 case of type C3. The average time from fracture to surgery was 5.4 weeks (range, 3-10 weeks). Among the 7 patients, 4 patients' posterior ring fractures were fixed by combined lumbar pelvic triangle fixation, and 3 patients' posterior ring fracture were fixed by combined lumbar pelvic fixation. 4 patients' anterior ring injury were not treated, 2 patients' anterior ring injury were treated by closed cannulated screw and internal fixation, and 1 patient's anterior ring injury was fixed by INFIX and cannulated screw. Every patient's operating time, intraoperative blood loss, length of incision and times of X-ray fluoroscopy were recorded. Pelvic X-ray and CT scan were taken postoperatively to observe the condition of reduction and screw position. Postoperative fracture reduction quality was assessed by Matta radiological criteria and Majeed criteria was used at the final follow-ups to evaluate the degree of functional recovery after pelvic fracture.Results:The average operating time of 7 patients was 143 min (range, 96-205 min); the intraoperative average blood loss was 579 ml (range, 300-1 650 ml); the average length of incisions was 12.9 cm (range, 9-15 cm) and the average time of X-ray fluoroscopy was 27 times (range, 15-52 times). Postoperative X-ray and CT scan showed that the displacements of the posterior rings were reset well and all the hollow screws were located accurately and firmly. Postoperative radiation quality was evaluated according to Matta radiological criteria, and there were 4 cases of excellent, 2 cases of good and 1 case of fair, with an excellent and good rate of 85.7% (6/7). Seven patients had good fracture union. The average followed up time for all 7 patients was 12 months (range, 6-16 months). At the last follow-up, imaging examination showed good reduction of the sacroiliac joint, and the reduction of anterior and posterior rings were not lost. The healing time was 14.2 weeks (range, 12-20 weeks). Majeed score: postoperative 4.90±6.64 points (range, 48-58 points), postoperative 3 months 71.40±7.32 points (range, 67-75 points), postoperative 6 months 84.90±8.14 points (range, 68-96 points), the difference was statistically significant ( F=0.614, P=0.004). Majeed score 6 months after operation showed that 5 cases were excellent, 1 case was good and 1 case was fair, and the excellent and good rate was 85.7% (6/7). Conclusion:Using combined lumbar pelvis fixation device in the treatment of old vertical unstable fracture dislocation of posterior pelvic ring has good reduction quality, high fixed strength and good postoperative effect.
9.Analysis of clinical features in pulmonary lymphoma

Objective To investigate the clinical features of pulmonary lymphoma and to get a better understanding of this disease. Methods Clinical data of 253 lymphoma patients in the Department of Lymphoma and Hematology in Jilin Cancer Hospital from October 2014 to March 2017 were retrospectively analyzed. The patients were divided into 30 cases of pulmonary lymphoma (lung lymphoma group) and 223 cases of non-pulmonary lymphoma (the control group). Rate assay and latex turbidimetry was used to detect lactic dehydrogenase (LDH) and β2macroglobulin (β2-MG) respectively. The expressions of programmed death 1 (PD-1), programmed death ligand 1 (PD-L1) and cytotoxic T lymphocyte-associated antigen 4 (CTLA-4) in peripheral blood CD4 +CD8 +T lymphocytes were detected by using flow cytometry. The count and measurement data of both groups were compared by using χ 2test and t test respectively. Results The patients in pulmonary lymphoma group showed secondary lesions. The proportion of smoking people in pulmonary lymphoma group was higher than that in the control group [43.3 % (13/30) vs. 24.2 % (54/223), χ 2= 4.964, P= 0.026]. The proportion of the patients in Ⅲ-Ⅳ stage in pulmonary lymphoma group was higher than that in the control group [93.3 % (28/30) vs. 57.0 % (127/223), χ2= 14.750, P < 0.001]. The proportion of the patients with higher international prognostic index (IPI) score in pulmonary lymphoma group was higher than that in the control group (χ2= 21.888, P < 0.001). The proportion of the patients with increased expression of β2-MG in pulmonary lymphoma group was higher compared with the control group [66.7 % (20/30) vs. 50.2 % (112/223), χ2=6.682, P =0.091]. The proportion of the patients with the increased LDH was higher compared with the control group [63.3 % (19/30) vs. 41.5 % (86/223)], and the difference was statistically significant (χ2= 6.682, P = 0.010). Diffuse large B-cell lymphoma (DLBCL) was the common pathological type in pulmonary lymphoma group (15 cases), followed by Hodgkin lymphoma (7 cases); imaging showed single mass or nodular type, multiple masses or nodular type, bilateral pulmonary infiltration, pleural effusion were 36.7 % (11/30), 30.0 % (9/30), 63.3 % (19/30) and 36.7 % (11/30), respectively. There were no statistical differences in the protein expression of immune check points such as PD-1, PD-L1 and CTLA-4 in both groups (all P > 0.05). Conclusions Pulmonary DLBCL should be considered a secondary disease, but not a primary lesion. Smoking history is a risk factor for lymphoma patients with pulmonary involvement. Pulmonary lymphoma is similar to other extra-nodal lymphoma with high IPI scores, advanced stage and elevated LDH.
10.Significance of fibroblast growth factor 23 in metabolic bone disease of prematurity

Objective To investigate the significance of fibroblast growth factor 23 in metabolic bone disease of prematuriy.Methods 60 patients who had been treated in our hospital from March 2016 to March 2017 were included in this study.Blood biochemistry was examined two weeks after birth,and values of blood phosphorus, serum calcium and alkaline phosphatase were recorded. Serum levels of 25 hydroxyvitamin D3,parathyroid hormone and fibroblast growth factor 23 were detected two weeks after birth. 20 premature infants with metabolic bone disease were selected as a study group. 40 infants without metabolic bone disease were treated as a control group. Two weeks after treatment,the above indicators were measured and compared in the study group. Results Serum levels of 25 hydroxyvitamin D3,parathyroid hormone and fibroblast growth factor 23 were compared between the two groups 2 weeks after birth,the difference was statistically significant(P<0.05).Levels of serum parathyroid hormone and fibroblast growth factor 23 in the study group were not statistically significant after treat-ment(P > 0.05). Levels of 25 hydroxy vitamin D3 in the study group had statistically significant after treatment (P<0.05).Conclusions Early detection of fibroblast growth factor 23 can reflect metabolic bone disease in pre-term infants.It suggests that vitamin D should be adequately supplemented in early.
