2.Imaging Diagnosis of Bronchogenic Cyst

Objective To probe imaging manifestations of bronchogenic cysts,as that to improve the knowledge of this disease.Methods Imaging features of congenital bronchial cyst in 32 cases confirmed by operation and pathology were analyzed.Results 4 cases of cyst localized in mediastinum ,all were liquid cyst.28 cases of cyst localized in lungs,aqmong them,22 cases were air-liquid cyst and 6 cases were air cyst,the diameter of lesions was 1~10 cm,there were 2 cases with pyothorax,4 cases with pulmonary maldevelopment,one case with brochio-pleural fistula.The cysts appeared as round or similar round,the walls of cyst was thin.The wall of cyst was obscure and there was infiltrating shadow around cysts when in company with infection.Conclusion X-ray and CT are very important in diagnosing congenital bronchial cysts,CT scan is of very help in diagnosing the compley cysts.
3.Assessment of the presence of esophageal varices bleeding in patients with hepatitis B virus-related cirrhosis by a noninvasive score system

Objective To retrospectively analyze the diagnostic value of a noninvasive score system based on transient elastography (TE),serological test and imaging examination on esophageal variceal bleeding (EVB) in patients with hepatitis B virus (HBV)-related cirrhosis.Methods Between April 2011 and December 2012,172 patients with HBV-related cirrhosis including 120 males and 52 females who visited clinic or hospitalized at the Department of Hepatology,Tianjin Third Central Hospital,were retrospectively enrolled.The mean age was (52.9 ± 10.6) years.Patients underwent upper gastrointestinal endoscopy to evaluate esophageal varices (EV) and were further categorized into three stages of mild,moderate and severe according to the morphology of EV and the risk of bleeding.Liver stiffness and spleen stiffness measurement were performed using Fibroscan.Portal vein width,splenic width and spleen thickness were measured using color Doppler ultrasound.All the patients were tested for white blood cell counts and platelet counts.With endoscopy as the gold standard,receiver operating characteristic (ROC) curves and the areas under curves (AUC) were used to assess the performance of the noninvasive score system in predicting EV by liver stiffness,spleen stiffness,portal vein width,spleen thickness and platelet counts.Student's t-test was performed to determine differences between continuous variables.Pearson's correlation was used to evaluate the association between EVB and these parameters.Results All these 172 patients underwent endoscopy.Among them,41 were EVB patients and 131 with no bleeding of EV.Among 172 EV patients,39 without EV,30 were mild EV,47 were moderate EV and 56 were severe EV.EVB was all positively correlated with liver and spleen stiffness,portal vein width,spleen thickness,splenic vein width (r=0.224,0.771,0.214,0.425 and 0.364,respectively; all P<0.05).EVB was negatively correlated with platelet counts (r=-0.408,P=0.000).Liver stiffness,spleen stiffness,portal vein width,spleen thickness and splenic vein width in EVB patients were significantly higher than those in EV patients (P<0.05).In contrast,platelet counts level was lower in EVB patients with difference of statistical significance (P<0.05).AUC of non-invasive score system for EV and EVB were 0.953 and 0.882,respectively (P<0.05).The optimal cut-off level of noninvasive score system for prediction of EV and EBV were 7 (sensitivity:96 %,specificity:85 %) in EV patients and 10 (sensitivity:78%,specificity:89 %) in EVB patients.Conclusion Non-invasive score system based on liver stiffness,spleen stiffness,spleen thickness,width of splenic and portal vein and platelet counts is of clinical importance in assessing the presence of EV in patients with HBV-related cirrhosis,which is higher clinically valuable in the diagnosis for EV.
4.Noninvasive assessment of esophageal-gastric varices by spleen stiffness in liver cirrhosis patients

Objective To investigate the value of spleen stiffness measured by transient elastography (FibroScan,FS) for diagnosing esophageal-gastric varices in liver cirrhosis patients.Methods A total of 259 cirrhotic patients in Tianjin Third Central Hospital from Apr 2011 to Apr 2012,and 30 healthy controls were enrolled.All the patients and controls were evaluated for spleen and liver stiffness by FS and 201 cirrhotic patients also underwent gastroscopy for the diagnosis of esophageal-gastric varices.By using gastroscopy as the gold standard,the receiver operating characteristic (ROC) curves of three parameters including spleen stiffness,liver stiffness and platelet/thickness of spleen were delineated for different disease stage.The areas under curves (AUC) were used to evaluate the value of these parameters in the diagnosis of esophageal-gastric varices.Results The spleen and liver stiffness values in cirrhotic patients were (44.64 ± 22.27) kPa and (24.27 ±18.89) kPa,respectively,while those in healthy controls were (20.94± 14.78) kPa and (6.12±5.77) kPa,respectively,which were both lower than cirrhotic patients (P<0.05).The stiffness values of liver and spleen both increased with higher Child-Pugh scores.And the liver stiffness values were different among groups (F=0.068,P =0.000),while the spleen stiffness values in patients with Child-Pugh A and B were different from that in patients with Child-Pugh C (P<0.05).In patients with moderate or serious esophageal-gastric varices,the spleen and liver stiffness values were significantly higher.The ROC curve analysis showed that the AUC of spleen stiffness,liver stiffness and platelet/thickness of spleen in the patients with moderate to serious esophageal-gastric varices were 0.918,0.749 and 0.743,respectively.The corresponding optimal cut-off values were 50.7 kPa,20.1 kPa and 1.65.The AUC,sensitivity and specificity of spleen stiffness were all higher than liver stiffness and platelet/thickness of spleen.Conclusion Spleen stiffness measured by transient elastography is a valuable parameter for non-invasive diagnosis of esophageal-gastric varices in cirrhotic patients.
5.Complementary value of 11C-choline PET/CT to 18F-FDG PET/CT for diagnosing nasopharyngeal and hepatocellular carcinoma

Objective To evaluate the complementary value of 11C-choline (CHO) PET/CT to 18F-FDG PET/CT in the staging of locally advanced nasopharyngeal carcinoma (NPC) and diagnosis of HCC.Methods From December 2007 to January 2010,15 patients with locally advanced NPC and 76 patients with HCC were prospectively enrolled into this study.The research was approved by the ethics committee,and all patients signed informed consents.Whole body 18 F-FDG PET/CT scans were performed on all patients and regional 11C-CHO PET/CT was conducted in 43 patients (15 with NPC,28 with HCC).A lesion with increased uptake of either 11 C-CHO or 18F-FDG was considered positive.SUVmax,tumor/brain (T/B) ratio and tumor/liver (T/L) ratio were calculated for semi-quantitative analysis.Two-sample t test,x2 test,Fisher exact test and linear correlation analysis were used for statistical analysis.Results (1) The lesion SUVmax of 18 F-FDG was higher compared to 11C-CHO (12.81 ± 5.00 vs 6.84 ± 2.76 ; t =6.416,P <0.01) in NPC patients.However,11C-CHO PET/CT had a much higher T/B ratio than 18F-FDG (18.62 ±7.95 vs 1.38 ±0.59 ; t =8.801,P < 0.01).Significant correlation was found between the 2 tracers with regard to NPC lesion uptake (r =0.712,P <0.01).Compared with 18F-FDG PET/CT,11C-CHO PET/CT had better delineation of intracranial invasion in 50.0% of patients (12/12 vs 6/12; x2 =8.000,P <0.05),skull base invasion in 4/14 patients and orbital invasion in 3/3 patients.(2) 18F-FDG PET/CT showed positive findings in 63.1% (48/76) of HCC patients.In 28 HCC patients with negative findings on 18F-FDG PET/CT,11 C-CHO PET/CT was positive in 71.4% (20/28) of patients.The dual-tracer PET/CT improved the diagnostic sensitivity (89.5%,68/76) of HCC compared with 18F-FDG PET/CT (63.1%,48/76) alone (x2 =14.559,P <0.01).11C-CHO PET/CT was more sensitive than 18F-FDG PET/CT for the detection of well differentiated HCC (6/9 vs 35.7% (5/14) ; P =0.214).For the detection of moderately differentiated HCC,the sensitivity of 11C-CHO and 18F-FDG PET/CT was similar to each other (6/7vs 72.0% (18/25),P =0.648).11C-CHO PET/CT was more sensitive than 18F-FDG for the detection of HCC lesions <5.0 cm (72.7% (16/22) vs 42.1% (16/38) ; x2 =5.249,P <0.05),especially for lesions < 2.0 cm (5/7 vs 0/7; P =0.021).Conclusions 11 C-CHO PET/CT could improve the accuracy in T staging of NPC.It might also play a complementary role for 18 F-FDG PET/CT in the detection of HCC.
6.Value of the dual phase 18F-FDG PET/CT in the diagnosis of recurrent and metastatic bladder cancer after surgery

Objective To investigate the clinical value of dual phase 18F-FDG PET/CT in the diagnosis of recurrent and metastatic bladder cancer after surgery. Methods The imaging data from 84 patients underwent the dual phase 18F-FDG PET/CT after surgery with known histories of bladder cancer were analyzed. Among the 84 patients, 16 had symptoms of recurrence, 24 had symptoms of metastasis and 44 didn′t have any symptom. The median interval time between the primary tumor resection and the PET/CT scan was 11.5 months (0.5 ~ 240 months). According to the PET/CT imaging procedures, all patients underwent whole body PET/CT scan at 60 minutes after IV injection of 18F-FDG. Additional delayed pelvic PET/CT images were acquired after forced diuresis by using oral 40 mg furosemide. The 18F-FDG PET/CT findings were compared with histopathologic examination results and (or) the clinical follow-up. All patients were followed up for more than six months. Results Results of detecting recurrence and metastasis of bladder cancer showed that the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of the dual phase FDG PET/CT imaging protocol were 91.7%(22/24), 95.0%(57/60), 94.0%(79/84), 88.0%(22/25), 96.6%(57/59) and 90.0%(27/30), 96.3%(52/54), 94.0%(79/84), 93.1%(27/29), 94.5%(52/55), respectively. Conclusion Dual phase FDG PET/CT can be used to detect the recurrence and metastasis with high accuracy, contributing to the restaging and follow-up in bladder cancer after surgery.
7.The Diagnostic Value of Cystography with Low-concentration Contrast Medium in Diagnosing Small Tumor of Urinary Bladder

Objective To evaluate the value of cystography with low-concentration contrast medium in diagnosing small tumor of urinary bladder.Methods Cystography in 187 cases with bladder disease from 1992 to 2001 was performed using 6%~8% Meglumine Diatrizoate 100~120 ml under TV monitored.Radiogrames of A-P position and bilateral oblique position were taken when the focus was found.The radiogram on patient’s position at head low was adopted when necessary.Results Of 187 cases,106 cases of bladder tumors were detected,including 8 cases of small bladder tumor(≤1.0 cm in diameter),3 cases were misdiagnosed,the detectable rate was 73.0%.All cases were confirmed by operation and pathologiy.In these 8 cases,1 case was adenocarcinoma,5 cases were transitional epithelia cell carcinoma,2 cases were papilloma.The X-ray appearances were nipple-like or cauliflower shape with filling defect,a narrow pedicel could be occasionally found in papilloma or transitional epithelia cell carcinoma and it had somewhat movement when changed patient’s position.Conclusion Cystography with low-concentration contrast medium is a non-injury procedure in diagnosis of small tumor of urinary bladder.
8.Effects of pulmonary function training on the lung function and surgical tolerance of lung cancer patients with obstructive ventilatory defects

Objective To study the effects of pulmonary function training on the lung function and surgical tolerance of lung cancer patients with different degrees of obstructive ventilatory defect.Methods Pulnonary function training was performed with 103 lung cancer patients with obstructive ventilatory defects (26 mild cases,53 moderate cases,24 severe cases).The duration of the intensive training was 5-7 days.Vital capacity (VC),vital capacity percentage (VC%),forced vital capacity (FVC),forced vital capacity percentage (FVC%),forced expiratory volume in one second (FEV1),percentage of the forced expiratory volume in one second (FEV1%),maximum voluntary ventilation percentage (MVV%),peak expiratory flow percentage (PEF%),fraction of the forced expiratory volume in one second (FEV1/FVC),and maximum mid-expiratory flow percentage (MMEF%) were measured before and after the training.Any postoperative complications were recorded.Results The average MMEF% improved significantly among the moderate patients after their training.Eventually 19/26 patients (73.08%) in the mild group,34/53 patients (64.15%) in the moderate group,and 8/24 patients (33.33%) in the severe group met the requirements to undergo lung operations.These were significant differences among the groups.Among those operated on,27 (44.26%) suffered from postoperative complications,a significantly higher percentage than among the normal controls (20.00%).Conclusion Lung function exercises can improve the operation tolerance of lung cancer patients with obstructive ventilation dysfunction,but the risk of operating is still much higher than for patients with normal ventilation function.
9.Clinical Study on Combined Tuina with Qi Zheng Qing Peng Paste for Herniation of the Lumbar Intervertebral Disc

Objective: To observe the clinical efficacy of combined tuina manipulations with Qi Zheng Qing Peng paste for herniation of the lumbar intervertebral disc. Method: All 568 cases were randomly allocated into an observation group (189 cases), control Ⅰ group (187 cases) and control Ⅱ group (192 cases). Combined tuina with Qi Zheng Qing Peng paste, combined tuina with diclofenac diethylamine emulsions and tuina alone were adopted in the above three groups respectively. The efficacies in the three groups were compared after 4 weeks of treatment. Results: The total effective rates in the observation group, control Ⅰ group and control Ⅱ group were 89.9%, 86.6% and 84.9% respectively, showing a significant difference (P <0.05) . Conclusion: Combined tuina manipulations with Qi Zheng Qing Peng paste works well for herniation of the lumbar intervertebral disc.
10.Comparison of the effect of glucosamine on the cartilage oligomeric matrix protein secretion in vitro chondrocytes and synoviocytes

Objective To compare the effect of glucosamine (Virtral-s) on the cartilage oligomeric matrix protein (COMP) secretion of chondrocytes and synoviocytes in vitro.Methods Chondrocytes and synoviocytes isolated from knee cartilage of osteoarthritic patients were cultured by phased enzymatic digestion.Sera containing Virtral-s of the experimental animals were obtained after orally administrated Virtral-s at the dosages that equal to human.Cells were cultured in the medium with Virtral-s containing sera.Super-natant COMP level was tested by enzyme-linked immunoabsorbent assays (ELISA).Results COMP conceu-tration of synoviocytes cultured in vitro was significantly higher than that of chondrocytes (P<0.05).Virtral-s could significantly increase COMP secretion in cultured chondrocytes in vitro (P<0.05),however,it had a weaker role on synoviocytes,ie,it could only mildly reduce COMP secretion of synoviocytes.Conclusion Glucosamine (Virtral-s)-containing serum can promote COMP secretion of chondrocytes in vitro,and it has no significant effect on synoviocytes in vitro.
