1.Escitalopram combined with aripiprazole in the treatment of 46 patients with obsessive compulsive disorder

ObjectiveTo observe and explore the clinical efficacy and safety of escitalopram combined with aripiprazole in the treatment of obsessive-compulsive disorder.Methods97 patients with obsessive-compulsive disorder were randomly divided into two groups,control group of 51 patients were given oral administration of escitalopram treatment,46 patients in the experimental group received escitalopram and oral aripiprazole treatment.At the end of treatment,the clinical efficacy and adverse events were evaluated.ResultsIn control group and experimental group patients overall effective rate was 68.6%,89.1% ;The experimental group the total effective rate was higher than the control group,the difference between two groups was statistically significant( x2 =12.32,P <0.05 ),The control group and experimental group after treatment Y-BOCS scores were (23.68 ± 2.74 ) points,( 21.82 ± 2.88 ) points ; The experimental group after treatment Y-BOCS score was significantly better than the control group,between the two groups the difference was statistically significant ( t =2.64,P < 0.05 ) ; While after treatment the TESS score and incidence of adverse reactions between the two groups were not statistically different ( t =0.61 ; x2 =0.03,all P > 0.05 ).ConclusionEscitalopram combined with aripiprazole in the treatment of obsessive compulsive disorder obtain excellent effect,had few adverse reactions,and was worthy of clinical application.
2.The efficiency analysis of tandospirone citrate in the adjuvant therapy of functional dyspepsia

ObjectiveTo observe the therapeutic effect of tandospirone citrate in the adjuvant therapy of functional dyspepsia (FD).MethodsEighty-nine FD patients from January 2009 to January 2012 were selected and divided by random digits table method into treatment group (47 cases) and control group (42 cases).The patients in control group were treated with conventional therapy to promote stomach bowel power and inhibit gastric acid,while those in treatment group were added tandospirone citrate on the basis of conventional therapy.The overall therapeutic effect,anxiety and depression scale scores of two groups before and after treatment were compared.ResultsThe overall effective rate was 91.5% (43/47) in treatment group and 76.2%(32/42) in control group,which had statistical significance between two groups (P<0.01).The anxiety scale score in treatment group after treatment was (51.1 ± 16.2) scores and depression scale score was(54.4 ± 14.7) scores,while the scores in control groups were (62.6 ± 15.3)(67.5 ± 11.3) scores,respectively.There was significant difference between two groups (P<0.05).ConclusionsTandospirone citrate in treating FD patients can improve the clinical symptoms with safety and effect.Therefore,it is worthy of application.
3.Experimental cerebral vasospasm induced by injecting autologous blood into cisterna magna twice after puncture of cisterna magna with a fine needle in dogs

Objective To investigate the feasibility and effectiveness of inducing cerebral vasuspasm by injecting autologous blood into cisterna magna (CM) twice after puncture of CM with a fine needle in dogs.Methods Six healthy aduh mongrel dogs of both sexes weighing 14-18 kg were involved in this study. The animals were anesthetized with intraperituneal 3% pentobarbital sodium 30-40 mg/kg. Spontaneous breathing was kept. Nonheparinized autologous arterial blood 0.5 ml/kg was injected into CM twice at the rate of 0.5 ml/s at 48 h interval after puncture of CM with a fine needle. Neurological function was assessed and scored ( 1 = normal, 5 =dead) and CT angiography was performed to determine the diameter of cerebral basilar artery before the first CM injection (T1 , baseline) and at days 7, 14 and 21 after the second CM injection (T2.4 ). Results The injection of autologous blood into CM twice significantly increased the success rate of the puncture and intraoporative survival rote of the animal*. The neurological function scores were significantly increased and diameter of cerebral basilar artery shortened at T2 and T3 compared with the baseline value. Conclusion The model of cerebral vasospasm after subarachnoid hemorrhage can be successfully established by injecting autologous blood into CM twice following puncture of CM with a fine needle in dogs, with higher postoperative survival rate and less injury to the animals.
4.Study on the Quality Standard for Chaige Ganmao Mixture

OBJECTIVE:To establish the quality standard for Chaige ganmao mixture. METHODS:TLC was conducted to iden-tify the Scutellaria baicalensis and Paeonia lactiflora;HPLC was conducted to determine the content of puerarin. The column was Zorbax Eclipse XDB-C18 with mobile phase of methanol-water(25:75,V/V)at flow rate of 1.0 ml/min,detection wavelength was 250 nm,and column temperature was 25℃,and the injection volume was 5μl. RESULTS:TLC showed clear spots and good sepa-ration. The linear range of puerarin was 18-198 μg/ml(r=0.999 9);RSDs of precision,stability and reproducibility tests were low-er than 2%,recovery was 99.04%-99.75%(RSD=0.28%,n=6). CONCLUSIONS:The standard can be used for the quality con-trol of Chaige ganmao mixture.
5.An transcranial Doppler ultrasonography and X-ray study of cervical vertigo patients treated by manipulation in supine position

OBJECTIVE: To observe the transcranial Doppler ultrasonography and X-ray changes of cervical vertigo patients caused by cervical instability before and after the manipulation treatment in supine position. METHODS: Sixteen cervical vertigo patients who underwent supine manipulation treatment were reviewed retrospectively. After the treatment, the changes of transcranial Doppler ultrasonography of cervical vertebral artery and X-ray of cervical spine were observed. RESULTS: Before the treatment, the mean blood flow velocity of vertebral artery was (48.6+/-3.1) cm/s, much higher than that of the normal group (P<0.05), the means of the horizontal displacement of C3,4 and C4,5 were (3.18+/-0.42) mm and (3.46+/-0.26) mm respectively. After the treatment, the blood flow velocity and the C3,4 horizontal displacement decreased significantly (P<0.05). The effective rate of the manipulation treatment was 100%. CONCLUSION: The manipulation treatment is an effective method for cervical vertigo patients caused by cervical instability, and the effect is correlated with the improvement of the blood flow of cervical artery and the restoration of cervical stability.

7.Expression of FSH receptor in ovary tissue of rats with letrozole-induced polycystic ovary syndrome

Objective To investigate the expressions of FSH receptor mRNA and protein in ovary tissue in rats with letrozole-induced polycystic ovary syndrome(PCOS),and to provide experimental data for the model application. Methods Forty rats were randomly divided into two groups(n=20),in PCOS model group letrozole was administered once daily during 21 d,and in control group without any treatment.The gonadal hormone concentrations in serum were determined by radioimmunoassay,the histologic changes in ovaries were observed by HE staining,the expression of FSH receptor gene in ovary tissue was detected by realtime -PCR,Western blotting and immunohistochemistry.Results Compared with control group,estradiol(E2) and progesterone in model group showed a considerable reduction(P0.05).Compared with control group,the ovaries from model group showed high incidence of subcapsular ovarian cyst and capsular thickening and decreased number of corpora lutea.The expressions of FSH receptor mRNA and protein were significantly higher in model group than those in control group(P
8.Advances in the diagnosis and treatment of occult inguinal hernia

With the advent of laparoscopic inguinal hernia repair, occult inguinal hernias not reported by physical examination are frequently found during surgery. Occult inguinal hernia is an early inguinal hernia, which is easy to be ignored because its clinical symptoms are not obvious. Currently, the best diagnostic method for occult inguinal hernia is laparoscopy, Noninvasive tests such as ultrasound and MRI should be performed in patients with first symptoms. With the increase of age, abdominal wall strength decrease, and the proportion of occult inguinal hernia requiring treatment will also increase, doctors need to choose a reasonable treatment plan according to the actual situation of patients. The key to the diagnosis and treatment of occult inguinal hernia lies in making clear its definition and establishing uniform diagnostic criteria. Through reasonable and accurate preoperative examination combined with laparoscopic techniques and the adjustment of surgical approaches, occultic defects can be found and repaired. Individualized treatment for different populations can avoid future symptoms and reduce additional surgical risks.
9.Effect of whole-process nursing intervention on comfort of patients undergoing radial artery puncture

Objective To study the effect of whole-process nursing intervention on comfort of patients undergoing radial artery puncture. Methods 100 patients with radial artery puncture from July, 2007 to June, 2008 were divided into the control group and the experimental group with 50 cases in each group accord-ing to time sequence. Routine whole nursing mode was used in the control group, the whole-process nursing intervention mode based upon routine nursing mode under the instruction of evidence-based method was used in the experimental group. Pain, psychological tensity, body numbness, success rate of radial artery puncture, patients' satisfaction degree were compared between the two groups. Results Every indexes of the experi-mental group were better than those of the control group. Conclusions The whole-process nursing interven-tion mode can promote the comfort degree of patients undergoing radial artery puncture.
10.Effect of endovascular cooling on perioperative brain injury in patients undergoing intracranial aneurysm resection

Objective To investigate the effect of endovascular cooling on perioperative brain injury in patients undergoing intracranial aneurysm resection.Methods Sixteen Hunt-Hess Ⅱ -Ⅳ patients of both sexes aged 18-64 yr undergoing intracranial aneurysm resection were randomly divided into 2 groups(n = 8 each): mild hypothermia group(group MHT)and nonnothermia group(group NT).A CL-2295AE catheter was placed in the femoral vein after anesthesia induction to perform endovascular cooling.Bladder temperature was reduced to 34 ℃ and maintained for 24 h.The hemodynamic parameters were recorded during and after operation.Coagulantion function and electrolyte levels were determined at 24 h before operation and at 12 and 24 h after operation.The serum neuron-specific enolase(NSE)and S100B concentrations were determined at 1 d before operation and at 1,3and 7 d after operation by ELISA.Neurological function was assessed with GOS grade at 1 and 3 months after operation.Results There was no significant difference in hemodynamic parameters,electrolyte levels(Na+ ,K+ ,Ca2+)and coagulantion function(PT,APTT,Plt)between the two groups(P > 0.05).The GOS grade was significantly higher,while serum NSE and S100B concentrations were significantly lower after operation in group MHT than in group NT(P < 0.05).Conclusion Endovascular cooling(34℃,24 h)can reduce the brain injury safely and effectively during the perioperative period in patients undergoing intracranial aneurysm resection and improve the prognosis.
