1.Application development of magnetic nanoparticles in cancer diagnose and therapy

Cancer is one of the three major killers that threatened human health in the twenty-first century,making the early diagnose and high-efficiency therapy crucial.The recent-developed nanotechnology brings new hope for this.Among different nanomaterials,magnetic nanoparticles show positive effects in diagnosis and therapies since they can be located with certain magnetic fields and can vibrate and heat up in the alternating magnetic field.With the ever increasing attention paid to the study,magnetic nanoparticles will play a key role in cancer diagnosis and therapy.This review gives a comprehensive discussion on development of the research and applications of magnetic nanoparticles,including single metallic,double metallic and alloyed magnetic nanoparticles.Moreover,it also prospects the future developing orientations of magnetic nanotechnology.Magnetic nanoparticles are widely used these days,with a speedy research development both in theory and application.However,the investigations into the toxicity of magnetic nanoparticles and the medical and commercial standards have to continue before magnetic nanoparticles are determined as valid and with low toxicity.
2.Prokaryotic soluble expression, purification and function study of LEDGF/p75 protein.

HIV-1 integrase (IN) is a key enzyme for the viral replication. The protein-protein interaction (PPI) between HIV-1 IN and a cellular cofactor lens epithelium-derived growth factor (LEDGF/p75) is a validated target for anti-HIV drug discovery. In order to build the platform for screening inhibitor against PPI between IN and LEDGF/p75, the vector containing the LEDGF/p75 protein cDNA was constructed and expressed in Escherichia coli and the function of the LEDGF/p75 protein was assayed. The LGDGF/p75 encoding gene optimized according to the preference codon usage of E. coli, was synthesized and cloned into the expression vector pGEX-4T-1 to form a recombined plasmid, then transformed into host cell E. coli BL21 (DE3). The recombined clones were identified and confirmed by BamH I/Sal I digestion and sequencing, the successfully recombined plasmid in the host cell was induced by IPTG and the condition of the expression was optimized. The expressed protein was purified by the Ni2+ affinity chromatography column and SDS-PAGE was used to analyze the molecular weight and specificity. In addition, ELISA assay was used to analyze the function of the recombinant protein. The recombinant LGDGF/p75 was soluble, and expressed highly and stably in E. coli. The protein was proved to enhance HIV-1 IN strand transfer activity in vitro by ELISA. It will be helpful to build the platform of screening inhibitors against PPI between IN and LEDGF/p75.
3.Effects of magnetic iron oxide-loaded CA-PLGA nanoparticles on HeLa cells

Objective To investigate the effects of magnetic iron-oxide (Fe3O4) nanoparticles with different skin materials on cytotoxicity against human cervix cancer HeLa cells.Methods The Fe3O4 nanoparticles were prepared by solvent-free dissolving method and then covered with poly(lactic-co-glycolic acid) (PLGA) and cholic acid (CA)-PLGA respectively.After the characterization,the uptake of nanoparticles by HeLa cells was observed by laser scanning confocal microscopy,and thiazolyl blue tetrazolium bromide (MTF) assay was used to assess the cytotoxcity of the Fe3O4-loaded nanoparticles against HeLa cells.Results The size of single Fe3O4 nanoparticle was about 7 nm.The Fe3O4-loaded PLGA and CA-PLGA nanoparticles were spherical in shape under transmission electron microscope,around 200 nm in diameter,and the theoretical drug-loading capacity was 10%.At the same Fe3O4 nanoparticles concentration of 25 μg/ml,the Fe3O4-loaded CA-PLGA nanoparticles showed less toxicity than Fe3O4-loaded PLGA nanoparticles.Conclusions Star-shaped copolymer CA-PLGA is promising in decreasing cytotoxicity of magnetic Fe3O4 nanoparticles and expanding its application in living organisms.
4.A Study on Drug Resistance Mechanism of HIV-1 Integrase Mutants by Molecular Modeling

The drug resistant mutations in human immunodefieiency virus type 1 (HIV-1) are a major impediment to successful highly active antiretrovirai therapy (HAART) and new drug design. In order to understand the drug resistance mechanism of HIV-1 integrase (IN) mutually existed for multiple drug-resistant strains to the most potent IN inhibitors diketo acids (DKAs), three S-1360-resistant HIV-1 strains were selected and molecular docking and molecular dynamics (MD) simulations were performed to obtain the inhibitor binding modes. Based on the binding modes, compelling differences between the wild-type and the 3 mutants for IN have been observed. The results showed that: 1) In the mutants, the inhibitor is close to the funetional loop 3 region but far away from the DNA binding site. Different binding sites lead to the decrease in susceptibility to S-1360 in mutants compared to the wild-type IN. 2) The fluctuations in the region of residues 138~166 are important to the biological function of IN. 2 hydrogen-bonds between S-1360 with residues N155 and K159 restrict the flexibility of the region. Drug resistant mutations result in a lack of the interaction, consequently, the less susceptible to S-1360. 3) In the 3 mutant IN complexes, the benzyl ring of S-1360 is far from the viral DNA binding site, thus, S-1360 can not prevent the end of the viral DNA from exposure to human DNA. 4) After T66I mutation, the long side chain of I occupied the active pocket in the 3 mutants, consequently, the inhibitor could not move into the same binding site or have the same orientation. All the above contribute to drug resistance. These results will be useful for the rational inhibitor modify and design.
5.Color Doppler ultrasonography in assessment of activity of ulcerative colitis

Sixty patients with ulcerative colitis (UC),including 28 males and 32 females were enrolled in the study.Color Doppler ultrasonographic scans were performed and the disease activity was estimated base on the Sourtherland score index (DAI).On the sonography the colon with active inflammatory disease showed bowel wall thickening,alteration of layers structure or even discrimination and increased vascular signals.Using DAI as the gold standard,the sensitivity,specificity,positive predictive value,negative predictive value and accurate rate for ultrasonography in assessment of UC activity were 90.0% (36/40),75.0% ( 15/20),87.8% (36/41),78.9% (15/19),85.0% (51/60),respectively.In conclusion,Color Doppler ultrasonography provides a convenient technique for assessment of bowel involvement and inflammatory activity in ulcerative colitis with high sensitivity and accuracy.
7.Nosocomial Infection Control and Management in Anhui Province:Investigation of Basic Situation and Strategy

OBJECTIVE To know the basic situation of nosocomial infection control and management in all levels of hospitals in Anhui Province,and provide the theory basis for the future work.METHODS To investigate the students participating in the nosocomial infection management training class by using the questionnaire.RESULTS There were altogether 273 people from 217 hospitals accepting the investigation.From 217 hospitals there were 46 1st grade hospitals and 160 2nd grade hospitals and 11 third graed hospitals.In all the people engaged in the nosocomial infection management,83.88% were the females and 67.03% for nursing staff,the age of 30-50 years old was 79.84%,the middle professional rank accounted for 72.53%,and 79.86% people′s record of formal schooling was the technical college and below.CONCLUSIONS The basic situation of nosocomial infection management in majority hospitals of our Province is good,but in some hospitals,the function of the structure has not certainly obtained the full display.In order to adapt the development of the nosocomial infection supervisory work,promote it to be more perfect,personnel structure,resources arrangement,and administrative offices function should give enough reconstruction and construction.
8.Nosocimial Infection and Perioperative Antibiotic Usage in Clean Surgical Sites:An Investigation

OBJECTIVE To investigate the surgical site infection(SSI)in clean surgical sites and the perioperative antibiotic usage.METHODS The incidence of SSI and the antibiotics usages in 842 cases during perioperative were analyzed retrospectively in 32 hospitals.RESULTS One case with nosocomial infection was found(0.12%).The use rate of antibiotics was 98.57%.Among 802 cases using prophylactic antibiotics,the antibiotics were given in 181 cases thirty minutes to two hours before operation.136 cases were given 24 h before operation.And 485 cases were given after operation;195 cases stopped using antibiotics after 24 h,472 cases stopped using antibiotics after 4-6 days and 135 cases stopped to use antibiotics after 7 days.CONCLUSIONS The major problem is excessive use of antibiotic in clean surgical site during perioperation,long duration of antibiotics prophylaxis,more expensive antibiotics and unnecessary combination of antibiotics,suggesting that it be important to strengthen the management of antibiotics.
9.Study of the correlation between different upper limb abduction angles and the occurrence of malposition of PICC into internal jugular vein

Objective To investigate the effects of different upper limb abduction angles on the occurrence of malposition of PICC into internal jugular vein. Methods Totally 210 cases of patients treated with PICCs for unilateral breast cancer of our hospital from August,2015 to January,2017 were randomly assigned to three groups with 70 cases in each group. The abduction angle of the upper limb for placement was set at 45° ,90° and 160° , respectively. We chose the basilic vein of the uninfected arm using modified Seldinger technique under the guidance of ultrasound for PICC,and malposition was confirmed by detecting the tip of PICC in internal jugular vein. Results The incidence rate of internal jugular venous dislocation in 45° group was 7.14%(5 cases) and 8.57% in 90° group (6 cases),and no internal jugular venous dislocation in 160° group. There was no statistically significant difference among 45°,90° and 160° groups(χ2=5.95,P>0.05). Significant differences of group comparisons for 45° v.s. 160° and 160° v.s. 90° were found (P<0.05). There was no statistically significant difference between 45° and 90° groups (P>0.05). Conclusion Abduction angles of 45° ,90° and 160° can all be used for PICC placement. The abduction angle can be selected according to specific situation of patients instead of being limited to the standard abduction angle of 90°.
10.The effect observation of comprehensive intervention on postoperative pain of patients with rectal disease

Objective To explore the effect of comprehensive intervention on postoperative pain of patients with rectal disease. Methods A total of 200 patients with postoperative pain after the treatment of anorectal perianal disease from May 2015 to May 2016 were randomly divided into two groups with 100 cases each. The control group was treated with drugs and usual nursing, the observation group were adopted drugs and comprehensive nursing intervention. The improvement of pain, psychological states and the quality of sleep were compared between two groups. Results The VAS pain scores at 4, 6, 12, 24, 48 h after treatment was (2.1 ± 0.6), (3.3 ± 0.4), (3.5 ± 0.3), (2.3 ± 0.5), (1.9 ± 0.5) points in the observation group, and (3.0 ± 0.5), (5.1 ± 0.6), (6.2 ± 0.6), (5.7 ± 0.8), (5.8 ± 0.5) points in the control group, and the difference was statistically significant (t=8.539-38.806, P < 0.05). The Self-rating Anxiety Scale was (20.32 ± 6.16) points in the observation group, and (35.58 ± 7.43) points in the control group, and the difference was statistically significant (t=41.188, P<0.05). The sleep quality, the amount of sleep , sleep time, sleep efficiency of Pittsburgh Sleep Quality Index Questionnaire scores was (0.91±0.28), (0.86±0.2), (0.83±0.27), (0.59±0.31), (0.62±0.27), (0.58±0.41), (4.39±1.79) points in the observation group, and (1.61± 0.88), (1.32 ± 0.75), (1.59 ± 0.89), (1.34 ± 0.58), (1.36 ± 0.45), (1.29 ± 0.86), (8.51 ± 3.55) points in the control group, and the difference was statistically significant (t=4.557-17.740, all P<0.05). Conclusions The comprehensive intervention on postoperative pain relief in patients with anal disease is significant, it is beneficial to relieve the pain response, improve sleep quality, and achieve physical and psychological comfort, and has a positive effect to clinical.
