1.The effect of sevoflurane pretreatment or aftertreatment on intrapulmonary shunt for patients with one lung ventilation during thoracic surgical procedures

Objective To study the effect of sevoflurane pretreatment or aftertreatment on intrapulmonary shunt for patients with one lung ventilation(OLV)during thoracic surgical procedures .Methods 80 ASA I or II patients underwent thoracic surgery with OLV were randomly divided into four groups(n= 20 for each):(1)Propofol group(group P);(2)Sevoflurane pretreatment group(group S1);(3)Sevoflurane aftertreatment group(group S2);(3)Sevoflurane whole-treatment group(group S3) .Blood sam-ples were taken from radial artery and right atrial blood before induction of anesthesia(baseline T .0) ,before OLV(sevoflurane pre-treatment 30 min ,T1) ,after OLV 30 ,60 min and before the end of OLV(T2-T4)and after the end of OLV(T5)for measurement of blood gases and calculation of Qs/Qt .The indexs of hemodynamics and relative clinical data were recorded .Results Arterial oxy-gen pressure of the four groups for OLV significantly reduced ,when compared with double lung ventilation(P<0 .05) ,but there was no significant difference among the four groups(P>0 .05) .Intrapulmonary shunt of the four groups for OLV significantly in-creased ,when compared with double lung ventilation(P<0 .05) ,but intrapulmonary shunt of S1 ,S2 and S3 groups significantly de-creased ,when compared with P group ,and there was no significant difference for S1 ,S2 and S3 groups(P> 0 .05) .Conclusion Sevoflurane can decrease intrapulmonary shunt for patients during OLV ,but there is no significant difference for pretreatment or af-tertreatment or whole treatment and do not increase arterial oxygen pressure .
2.Expression of cadherin genes in chronic myeloid leukemia and its significance

Objective To explore the expression of cadherin (Cad) genes in patients with chronic myeloid leukemia (CML),and to elucidate the significance of Cad genes in the development of CML.Methods E-Cad and N-Cad gene expression levels in bone marrow mononuclear cells (BM-MNC) from 48 CML patients (29 in chronic phase,19 in progressive phase) were detected by real time quantitative polymerase chain reaction (qRT-PCR).Results Gene expression of E-Cad and N-Cad was detectable in CML BM-MNC.E-Cad gene expression level was lower in progressive CML than that in chronic CML (0.20 ± 0.35 vs 1.19 ± 0.87,P < 0.01),while N-Cad was highly expressed in progressive CML than in chronic CML (0.89 ± 0.45 vs 0.57 ± 0.47,P < 0.05).E-Cad gene expression level was negatively corrclatcd with thc pcrccntagc of peripheral blood progenitor cells (r =-0.705,P < 0.01).Conclusion E-Cad and N-Cad gene expression correlates with the progression of CML,and might be used as an evaluation index for disease development.
3.Experimental Study of Effect of Kidney-Invigorating & Blood-Activating Herbs on Endometriosis

[Objective] To study the influence on humoral immunity and ectopic endometrium by infusion of Bushen Tiaojing Decoction (BTD) with the Huoxue Sanjie Suppository (HSS) in anus. [Methods] One hundred and thirty SD rats were allocated to ten groups: normal control (A), model (B), normal dose of HSS (C), normal dose of BTD with HSS (D), 3 doses of BTD (E), 3 doses of BTD with HSS (F), 5 doses of BTD (G), 5 doses of HSS (H), 5 doses of BTD with HSS (I), and Danazol (J) groups. The endometriosis (EM) rats were modeled by endometrium implantation in all groups except those in Group A. Four weeks after the treatment, the contents of immunoglobulin (IgG、IgM、IgA), C_3, C_4 and ALT were tested, and histomorphomological analysis of the thymus, ovary, uterus, EM implants, liver and kidney were performed. [Results] Four weeks after EM implantation, the ectopic endometria were growing well, IgG and C_3 increased, but IgM decreased, and the uterus and ovary significantly enlarged. With the treatment of BTD and HSS or Danazol, the ectopic endometria were atrophied. In Groups F and G, IgG decreased; in Groups H and I, C_3 decreased, and uterus and ovary shrunk in Group I. Danazol can help to prolong the sexual cycle, while BTD and HSS can not. In all the medication groups, no obvious difference was found in ALT of the rats. [Conclusion] Oral administration of BTD with HSS can cause endometrial atrophy and improve humoral immunity in model rats.

Objective To examine the relationship between CCT 5 ,γδ T cell and autoimmune diseases .Methods Recombinant CCT5 protein was cloned , expressed and purified in E.coli.Three peptides of CCT5 protein were used to prepare for anti-CCT5 monoclonal antibodies .Purified CCT5 protein was used to expand γδT cells from pe-ripheral blood mononuclear cells (PBMC).Plasma level of CCT5 in healthy donors, patients with rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE) were detected by ELISA assays .The correlation analysis between plasma CCT5 concentration and the percentage of different subtypes of γδT cells measured by flow cytometry was made . Results The CCT5 gene was amplified by PCR and the length of the target fragment was 1 750 bp.The expressed 65 ku CCT5 protein was purified and validated by SDS-PAGE.Two paired monoclonal anti-CCT5 antibodies were screened to detect CCT5 protein in plasma.Immobilized recombinant CCT5 protein was able to induce specific sig-nificant amplification of peripheral γδT cells.Correlation analysis of 10 healthy donors indicated significant corre-lation between the plasma CCT 5 concentration and the proportion of Vγ9 and Vδ2 γδ T cells.The plasma CCT5 concentration significantly decreased in autoimmune diseases patients , including RA and SLE .Conclusions These data suggest that CCT 5 could be a novel Vγ9δ2 γδT cell-related factor in autoimmune diseases , which deepen the understanding of Vγ9δ2 γδT cell function in autoimmune diseases .

AIM: To investigate the effect of diltiazem on mean pulmonary arterial pressure(mPAP) and nitric oxide synthase(NOS) in arterioles in chronic hypoxic hypercapnic rats. METHODS: Twenty-four rats were randomly divided into three groups: control group(A),hypoxic hypercapnic group(B), hypoxic hypercapnia+ diltiazem group (C), constitutive endothelial NOS(ceNOS) were observed in arterioles of rats using the technique of immunohistochemistry,ceNOS mRNA were observed by the technique of in situ hybridization . RESULTS: (1)mPAP was significantly higher in rats of B group than that of A and C group( P 0 05),but mCAP was lower in rats of C group than that in B group.(2)Light microscopy showed WA/TA (vessel wall area/total area) was significantly lower in rats of C group than that of B group ( P

Purpose To explore the value of Doppler ultrasound in the diagnosis of congenital hypertrophic pyloric stenosis (CHPS) by observing pylorus. Materials and Methods Sixyt-five children with CHPS diagnosed by ultrasound in Neonatology Department of Guangzhou First People's Hospital from 2006 to 2015 were retrospectively analyzed. CHPS group and 50 infants in control group were examined by Doppler ultrasound to observe the features of blood flow distribution and blood flow grading, and Doppler spectrum analysis was performed to observe contrast agent passing through pyloric canal. Results The difference of the thickness of muscular layer and mucous layer, and that of the long diameter and inner diameter of pyloric canal in CHPS group and control group all suggested marked statistical significance (P<0.01); the blood flow grading in muscular layer and mucous layer between the two groups showed statistical significance (t=13.33 and 18.77, all P<0.01). The blood flow velocity in muscular layer of CHPS group was (16.96±0.91) cm/s, resistance index (RI) 0.68±0.33, inner diameter of pyloric canal (1.98±0.33) mm. Conclusion Doppler ultrasound enables us to grasp the distribution and grading of blood flow of pylorus as well as the condition of contrast agent passing through pyloric canal, in which way the degree of pyloric stenosis can be evaluated by combining indexes such as Vmax and RI, and an objective basis for clinical choice of treatment can be provided, possessing high application value.
7.Clinical features of liver function and coagulation function in patients with Alongshan virus infection

ObjectiveTo investigate the clinical features of liver function and coagulation function in patients with Alongshan virus (ALSV) infection. MethodsClinical data were collected from 27 patients with ALSV infection who were admitted to Inner Mongolia General Forestry Hospital from May 2018 to September 2019, among whom there were 18 male patients and 9 female patients. Related data were extracted, and a database of relevant case reports was established. The descriptive epidemiological method was used to analyze the clinical features of liver function and coagulation markers, and the features of liver injury caused by ALSV infection were analyzed. ResultsFor the 27 patients, the abnormal rates of alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), gamma-glutamyl transpeptidase (GGT), cholinesterase, and total bile acid were 25.9%, 33.3%, 25.9%, 40.7%, 8%, and 8%, respectively; among the 27 patients, 4 (14.8%) had an ALT level of >2×upper limit of normal (ULN), 3 (11.1%) had an AST level of >2×ULN, 1 (3.7%) had an ALP level of >2×ULN, and 5 (18.5%) had a GGT level of >2×ULN. Among the 27 patients, 25 (17 male patients and 8 female patients) had the results of bilirubin test, among whom 1 had a reduction in total bilirubin (TBil) (3.30 μmol/L) and 3 had an increase in TBil (23.7 μmol/L, 26.2 μmol/L, and 32 μmol/L, respectively). The abnormal rates of the coagulation markers international normalized ratio, activated partial thromboplastin time, and fibrinogen were 3.7%, 11.1%, and 22.2%, respectively. ConclusionThere is a certain degree of liver injury in patients with ALSV infection, generally with mild symptoms.

The epidemic of COVID-19 has become the topmost public health threat worldwide. The authors suggested that in addition to strengthening the organization and leadership of the abovementioned work, greater attention be paid to establishing and improving the prevention and control mechanism.Furthermore, special efforts should be given to the safety of the medical workers, by strengthening their infection monitoring and outbreak management. Medical workers in different work areas and positions should be placed under careful protection with due cleaning and disinfection measures. The protection during specimen collection, transportation and medical waste management should also be prioritized. Such special issues in the management of pediatric patients, as allocation of nurses, the screening and management of caregivers, the problem of breastfeeding, and the disinfection of children′s toys and diapers were discussed. This paper also put forward management suggestions for the outpatient department, isolation ward and other key departments, which aim to guide the prevention and control of COVID-19 nosocomial infection in the pediatric outpatient and ward.
9.Differential expression of BKCa channels in atrial fibroblasts in patients with sinus rhythm and atrial fibrillation

Objective Through analyzing BKCa channel expression in atrial fibroblasts in patients with sinus rhythm and atrial fibrillation (AF), to explore the mechanism of myocardial fibrosis and provide new therapeutic strategies for the treatment and reversal of AF structure reconstruction. Methods We selected 10 patients of rheumatic heart valvular disease who underwent valve replacement surgery. They were 5 patients with sinus rhythm (a sinus rhythm group, 2 males and 3 females with an average age of 49.1±8.3 years) and 5 with AF (an AF group, 3 males and 2 females with an average age of 50.3±5.8 years). About 100 mg tissue was obtained from the right auricula dextra, and the atrial fibroblasts were cultured by tissue block adherence method, and the expression of BKCa channel genes and proteins in cultured fibroblasts was detected by quantitative real-time polymerase chain reaction (qRT-PCR) and western blotting methods. Results (1) The general data of 10 patients between the AF group and the sinus rhythm group were compared. There was no significant difference between the two groups in age (t=1.21, P=0.67) and sex (t=2.56, P=0.75). There was statistical difference in the left atrial diameter and the right atrium diameter between the two groups (t=19.45, P=0.01; t=23.52, P=0.06); (2) the mRNA expression of BKCa subunit was detected by qRT-PCR method, and there was no significant difference in the mRNA expression of BKCa α and BKCa β1 between the two groups (t=3.14, P=0.79; t=2.88, P=0.69); (3) the expression of BKCa protein was detected by western blotting method, and there was no significant difference in the protein expression of BKCa α and BKCa β1 between the two groups (t=0.55, P=0.31; t=0.73, P=0.46). Conclusion BKCa pathway may not be involved in the pathogenesis and maintenance of AF, but it may play an important role in the process of myocardial fibrosis.

This threat is posing a severe challenge on the diagnosis and treatment of 2019-nCoV infection, the prevention and control of hospital cross infection of medical staff. It is suggested that in addition to strengthening the organization and leadership of the abovementioned work, establishing and improving the prevention and control mechanism deserve greater attention. Furthermore, special attention should be given to the safety of the medical staff, strengthening their infection monitoring and outbreak management. Medical staff in different work areas and positions should be placed under careful protection, cleaning and disinfection measures.
