1.Treatment and prevention of serious complications after arterial perfusion chemotherapy of esophageal cancer

Objective To investigate the cause of severe complications after arterial perfusion for esophageal cancer and the methods of prevention. Methods 368 cases of esophageal cancer were treated with arterial perfusion of drugs for chemotherapy. The treatment numbers were 909 including 215 males and 153 females with the age ranging from 39 to 86. These patients were verified as esophageal cancers histopathologically. Selective angiography of the relevant esophageal segments and drugs for perfusion chemotherapy were undertaken. Results The complications included one case of paralysis due to spinal cord injury, two cases with esophageal perforation and three cases of necrotic esophagitis. The case of paralysis died of original disease one month after the treatment. Of the cases of esophageal perforation, one formed the esophgus-trachea fistula and survived for eight months after being esophageal stent implantation and the other formed esophagus-mediastinum fistula and died of massive hemorrhage after six weeks. Three cases of necrotic esophagitis occurred at the normal segments of the esophagus and formed esophgeal perforation. Of these three cases, one formed esophago-broncheal fistula and survived up to now after creating drainage stoma of stomach. Two cases of the esophgus-mediastinum and esophgus-bronchius fistula died of severe infection. Conclusions Severe complications of esophageal arterial catheterization with drugs for chemotherapy are rare. Less harmful, non-ionization contrast medium, low cellular toxicity drugs for chemotherapy with proper doses and concentrations should be selected together with optimal speed of infusion. Esophageal internal stent placement drainage stoma creation of stomach should be the useful adjunct for severe complications.
2.Application of tissue-cultured method on preservation of articular cartilage

[Objective]To study the effects of different storage method on the viability of cheodroeytes so as to find out the idem method for preservation of articular cartilage.[Method]Under the aseptic condition,acquiring 240 pieces of osteoehondral plugs from both condyles of femur and tibial plateau.The control group was not treatment using any preserved means.The tissue-cultured group was to used F12-DMEM medium long-term preserved osteochondral plugs under the conditions of 37 ℃.At preserve 8 weeks,takes the osteochondral section to carry on examination observation ohondrocytes Viability activeness change separately.[Result]The control group chnadrocyte survival rate was 94.4%.At preserves 8 weeks,the cell survival rate of the tissue-cultured group was 76%,the grading-cooling cryopreservation group was 66%,the constant-cooling cryopreservation group only was 41.80%.[Conclusion]Under the condition of 37 ℃,tissue culture method can long term preservation of vital articular cartilage in vitro.The chondrocyte survival rate of the preservation of vital articular cartilage in vitro.The chondrocyte survival rate of the tissue-cultured method obvious higher than constant-cooling cryopreservation method.
3.Mid-term outcome of coronary artery bypass graft surgery in patients aged≤40 years with coronary heart disease

Objective To investigate the clinical characteristics,surgical experience and mid-term outcome of coronary artery bypass graft surgery in patients aged ≤ 40 years.Methods From Jan.,2009 to Dec.,2015,12 patients with median age of (37.9 ± 2.5) years (range 32-40 years,10 males) were diagnosed with coronary artery disease and underwent coronary artery bypass graft surgery at Zhongshan Hospital,Fudan University.The clinical characteristics included five cases with hypertension,three cases with diabetes mellitus,four cases with hyperlipemia,eight cases with smoking history,seven cases with myocardial infarction.Six cases had at least one branch totally occluded.Two cases once underwent percutaneous coronary intervention surgery and implanted with five and three stents respectively.The perioperative data and follow-up results were retrospectively analysed.Results There was no in-hospital death and no death during follow-up period.Surgical techniques included bilateral internal mammary arteries combined with radial artery were operated in six cases,bilateral internal mammary arteries combined with great saphenous vein in two cases,left internal mammary arteries combined with great saphenous vein in four cases,off-pump in nine cases and on-pump in three cases.The mean follow-up time was (47.8 ± 24.3) months.During the follow-up one case suffered with saphenous vein graft restenosis 5 years after the surgery and underwent percutaneous coronary intervention.Other cases were all alive and assessed New York Heart Association's function class Ⅰ-Ⅱ.Conclusions Younger patients usually suffer with severe coronary artery disease when diagnosed because they often have excellent compensative capacity and the pathogenesis is concealed.The mid-term results of coronary artery bypass graft surgery in patients aged ≤40 years are satisfactory.
4.Effect of Taohong Siwu decoction combined with tranexamic acid in the treatment of chloasma and its influence on the level of sexual hormone and the oxidative stress parameters

Objective:To explore the therapeutic effect of Taohong Siwu decoction combined with tranexamic acid in the treatment of chloasma, and its influence on the level of sex hormone and oxidative stress parameters.Methods:From January 2017 to December 2019, 96 cases with chloasma in Yiwu Hospital of Traditional Chinese Medicine were selected and divided into observation group (48 cases) and control group (48 cases) according to the random digital table method.The control group was given tranexamic acid.The observation group was given Taohong Siwu decoction on the basis of the control group.The course of treatment in both two groups was 12 weeks.The therapeutic effect, melasma area and severity index (MASI) score, sex hormone and oxidative stress level before and after treatment, and the adverse reactions were compared between the two groups.Results:The total effective rate of the observation group (91.67%) was higher than that of the control group (68.75%) (χ 2=7.940, P<0.05). The MASI score of the observation group[(8.64±1.27)points] was lower than that of the control group[(17.93±2.53)points] ( t=22.736, P<0.05). The serum levels of E 2 [(19.83±4.21)ng/L], LH [(3.79±0.68)mU/L] and FSH [(5.23±1.76)mU/L] in the observation group were lower than those in the control group [(43.79±4.69)ng/L, (5.81±1.07)mU/L and (10.64±2.60)mU/L] ( t=26.339, 11.039, 11.938, all P<0.05). The MDA of the observation group [(3.67±0.40)μmol/L] was lower than that of the control group [(4.52±0.32)μmol/L], while the SOD of the observation group[(78.84±5.67)IU/L] was higher than that of the control group[(60.65±5.29)IU/L], the differences were statistically significant( t=11.496, 16.252, all P<0.05). Conclusion:Taohong Siwu decoction combined with tranexamic acid has good effect on chloasma patients.It can improve the level of sexual hormone and oxidative stress, and has no obvious adverse reactions.
5. Dual roles of immune cells and cytokines in hepatic ischemia reperfusion injury

Ischemia-reperfusion injury is one of the main causes of complications related to liver transplantation, hepatectomy, trauma and hemorrhagic shock. The cells of ischemia and hypoxic injury release of injury-related molecular patterns, lead to the activation of immune cells and cytokine, which further aggravates the inflammatory response and enlarges the injury. It’s indicated that injury-related molecular patterns, liver resident immune cells and cytokines play a key role in promoting inflammation and liver ischemia-reperfusion injury. However, recent studies suggested that the ischemia cells and cytokines played acomplex role in this process. Relevant progresses were reviewed in this article.
6.Survey on the intake of dietary nutrients for 132 aduit patients with acute and chronic leukemia

Objective To study the dietary nourishment of adult patients with leukemia and compare acute leukemic patients with chronic leukemic patients. Methods Adopting dietary review of 24 hours and seven consecutive days of dietary records method to obtain the food category and quantity of 122 patients with acute leukemia and 10 patients with chronic leukemia. Using statistic software SPSS11.0 to calculate the patients'intake of various kinds of nutfiments. and the difiences between acute and chronic leukemic patients were analyzed. Results The rate of most ontrients of patients'intake reaches RNI/AI is lower,especially vitamin A,vitamin C and caleium.There's a tendency that intake diet,energy and nourishments of acute leukemic patients is lower than that of those chronic leukemic patients. Conclusion There is a tendency of unbalanced dietary intakes in leukemic patients.including the low intakes.There is the tendency that nutritional status of acute leukemic patients iS poorer than that of chronic leukemic patients.
