1.Concentration Determination of Catecholamine in Human Urine by HPLC-Electrochemistry

OBJECTIVE:To establish a method for the concentration determination of catecholamines in human urine. METH-ODS:After extracted by aluminum oxide,using 3,4-dihydroxybenzylamine as internal standard,HPLC-electrochemistry was per-formed on the column of Shim-Pack C18 with mixed solution [every 500 ml mixed solution containing ion-pairing reagents(octane sulfonate) 0.5 mmol,methanol 10 ml,0.06 mmol/L Di-n-butylamine 50 μl,sodium acetate anhydrous 2.0 g,citric acid 5.5 g, EDTA-Na2 60 mg and sodium chloride 584.4 mg,pH=3.5] at a flow rate of 0.8 ml/min,column temperature was 30 ℃,detection voltage was 750 mV,and injection volume was 40 μl. RESULTS:The linear ranges were 3.125-100 ng/ml for both norepinephrine (r=0.998 5)and epinephrine(r=0.999 2),lower limits of quantitation were 3.125 ng/ml;RSDs of within days and daytime were lower than 10%,method recoveries were 99.2%-101.6% and 98.9%-100.5%,extraction recoveries were 63.8%-66.8% and 60.5%-62.6%. CONCLUSIONS:The method shows strong specificity,high sensitivity and low detection cost,and can be used for the concentration determination of catecholamines in human urine.
2.Effect of Salvia miltiorrhiza extracts on morphine physical dependence in mice

Objective To investigate the effect of Salvia miltiorrhiza extracts on morphine physical dependence in mice. Methods The physical dependence model was induced by repeated sc morphine daily in mice and then withdrawal symptom was induced by ip naloxone. Different doses of S. miltiorrhiza extracts were administrated by ip during or after the induction phase of morphine dependence, and effects of S. miltiorrhiza extracts both by preventive and acute administration on naloxone-precipiated withdrawal symptom in mice were investigated. 3.Study on indications for extubation of percutaneous transhepatic gallbladder drainage

Objective To study the indications for extubation of percutaneous transhepatic gallbladder drainage(PTGD).Methods Between January 2009 and December 2013,PTGD were performed on 104 patients with acute cholecystitis.The clinical data were analyzed retrospectively.Results PTGD were performed on all 104 patients successfully.In all the patients,after drainage the abdominal pain was relieved within 0.5 to 8 hours and mean time was(2.3 ± 1.9) h.The temperature decreased to normal in 1 days after drainage.The time of drainage was from 7 to 49 days,(22.4 ± 10.3) days on average.Thirteen patients'tubes were accidentally off during 3 to 12 days,5 patients were performed PTGD again,and 8 patients were given anti-infection,fluid infusion and other symptomatic treatments and all these patients were back to normal in time.Re-intubation rate was 4.8%.In 78 patients laparoscopic cholecystectomy (LC) were performed 1 to 2 months after PTGD.Operation time was between 35 to 95 min and mean time was(52.7 ± 15.0) min.There were 6 patients done open operation and the conservation rate was 7.7%.A mount of blood during LC was 30 to 150 mL,(61.7 ± 31.0) mL on average.No operative deaths occurred during therapeutic session.Conclusions Patients who formed PTGD obey the indications can extubate in time and reduce retrograde infection and skin irritation,also shorten the time during hospital's,which will have a positive impact about LC.
4.Correlation between waist circumference and organ damage in patients with cardiovascular disease after cardiac rehabilitation program.

OBJECTIVETo clarify the role of waist circumference (WC) in estimating organ damage and assessing the prognosis in young and middle-aged patients with cardiovascular disease undergoing cardiac rehabilitation program (CRP).
METHODSWe recruited 147 young and middle-aged patients with cardiovascular disease to participate in the CRP for 4 or 5 weeks. The baseline clinical data and results of routine fasting blood test and ultrasonic cardiography were evaluated for these patients.
RESULTSWC had a linear correlation with the indices reflecting cardiac structure alteration (LVSd and LVPWd) of the patients after CRP but showed a negative correlation with their HDL-C levels. The fasting blood glucose (FBG) and serum uric acid (UA) levels showed a positive correlation with WC before CRP but not after CRP.
CONCLUSIONWC monitoring allows accurate assessment of organ damage in patients undergoing CRP after cardiovascular disease and has special value in evaluating the changes in cardiac structure and function.

Objective To study the dietary nourishment of adult patients with leukemia and compare acute leukemic patients with chronic leukemic patients. Methods Adopting dietary review of 24 hours and seven consecutive days of dietary records method to obtain the food category and quantity of 122 patients with acute leukemia and 10 patients with chronic leukemia. Using statistic software SPSS11.0 to calculate the patients'intake of various kinds of nutfiments. and the difiences between acute and chronic leukemic patients were analyzed. Results The rate of most ontrients of patients'intake reaches RNI/AI is lower,especially vitamin A,vitamin C and caleium.There's a tendency that intake diet,energy and nourishments of acute leukemic patients is lower than that of those chronic leukemic patients. Conclusion There is a tendency of unbalanced dietary intakes in leukemic patients.including the low intakes.There is the tendency that nutritional status of acute leukemic patients iS poorer than that of chronic leukemic patients.
6.Analysing the risky factors of central lymph node metastasis in papillary thyroid microcarcinoma

Objective To evaluate the relationship between clinicopathological features and central lymph nodes metastasis in patients with papillary thyroid microcarcinoma (PTMC),and aim to provide an appropriate operation in clinic.Methods The clinical data of 536 patients undergoing surgical treatment in Department of Thyroid Surgery,the First Affiliated Hospital of Zhengzhou University from January 2015 to December 2017,who had pathologically confirmed popillary thyroid microcarcinoma (PTMC) were retrospectively analyzed.Thyroidectomy and isthmectomy with central compartment neck dissection were performed in all patients.The patients were divided into two groups according to whether there existed central lymph node metastasis,and compared the differences of gender,age,number of cancer lesions,tumor breakthrough envelope,BRAFV600E gene mutation,and tumor maximum diameter in the central lymph node metastasis,respectively,by t-test x2-test.Univariate logistic regression analysis and multivariate binary logistic regression analysis were conducted to find risky factors.Results There existed difference between two groups by completing the gender,age,the BRAFV600E gene mutation and maximal tumor diameter(P < 0.05).Univariate logistic regression analysis indicated that gender (P =0.046),age (P < 0.01),maximal tumor diameter(P <0.01) and the BRAFV600E gene mutation(P =0.016) were significant predictors for central lymph nodes metastasis.And multivariate binary logistic regression analysis revealed that the rate of lymph node metastasis significantly increased in cases of larger tumor diameter(P <0.01),BRAFV600E gene mutation(P =0.035) and ageing below 45 years old (P < 0.01).Conclusions The treatment for central lymph node metastasis of PTMC should be different considering elements including BRAF600E gene mutation prophylactic,ageing below 45 years old and larger tumor diameter.Therefore central lymphadenectomy should be performed when the primary lesion was resected.
7.Formulation of Criterion for Clinical Application of Shuxuetong Injection by Clinical Pharmacists in Our Hospital Based on Evidence Quality Evaluation Method

OBJECTIVE: To provide reference for strengthening clinical application of key monitoring drugs and promoting rational drug use in clinic. METHODS: Based on evidence-based medicine, taking key monitoring drugs Shuxuetong injection as example, clinical evidence of domestic and foreign clinical studies were collected. The included literatures were graded according to the quality of GRADE evidence and recommended strength system. Evidence-based medicine evidence for the indications of Shuxuetong injection were evaluated, and criterion for clinical use of Shuxuetong injection was formulated in Huaihua First People’s Hospital (our hospital). RESULTS: The main content of criterion for clinical application of Shuxuetong injection formulated by our hospital was that there was A-level evidence support for acute ischemic cerebral infarction, but it was weakly recommended and only used for adjuvant therapy; there was B-level evidence support for anticoagulation (for preventing DVT), diabetic peripheral nerve lesion, but it was weakly recommended; there was only C-level or D-level evidence support for other indications, it was strongly recommendation against use. CONCLUSIONS: Clinical pharmacists formulate the criterion for clinical application of Shuxuetong injection by evidence quality evaluation method, provide reference for clinical application management of key monitoring drug and play an important effect on rational drug use in clinic.

Polymyxin B, which is a last-line antibiotic for extensively drug-resistant Gram-negative bacterial infections, became available in China in Dec. 2017. As dose adjustments are based solely on clinical experience of risk toxicity, treatment failure, and emergence of resistance, there is an urgent clinical need to perform therapeutic drug monitoring (TDM) to optimize the use of polymyxin B. It is thus necessary to standardize operating procedures to ensure the accuracy of TDM and provide evidence for their rational use. We report a consensus on TDM guidelines for polymyxin B, as endorsed by the Infection and Chemotherapy Committee of the Shanghai Medical Association and the Therapeutic Drug Monitoring Committee of the Chinese Pharmacological Society. The consensus panel was composed of clinicians, pharmacists, and microbiologists from different provinces in China and Australia who made recommendations regarding target concentrations, sample collection, reporting, and explanation of TDM results. The guidelines provide the first-ever consensus on conducting TDM of polymyxin B, and are intended to guide optimal clinical use.
