1.Writing Model of Inpatient Medication Records

OBJECTIVE:To discuss an useful and convenient medication record writing mode to promote rational drug use.METHODS:Based on SOAP model,inpatient medication histories were established by combining medication data with clinical data collected from patients' clinical histories and pharmacists.RESULTS & CONCLUSIONS:The inpatient medication records that suited to the needs of clinical pharmaceutical care have been established and in which the clinical course of diseases and drug treatment course are recorded simply yet in detail,which can facilitate the evaluation on the safety,validity and rationality of medication.
2.Effect of methylcobalamin,gliclazide and combined application of both on sciatic nerve morphology and nerve growth factor contents in streptozotocin-induced diabetic rats

Aim To investigate the curative effect and the mechanism of methylcobalamin,gliclazide and combined application of both on diabetic peripheral neuropathy(DPN)in streptozotocin-induced diabetic(STZ-D)rats.Methods STZ-D rats were administered with drugs orally for 8 weeks after streptozotocin treatment.Morphologies of sciatic nerve were observed with histological method.Nerve growth factor(NGF)contents in plasma or in sciatic nerve tissues were determined by ELISA kit or immunohistochemistry.Results Morphological degeneration in sciatic nerve,decrease of NGF concertration in plasma,as well as in nerve axis cylinder,in diabetic rats was all ameliorated by methylcobalamin and/or gliclazide.However,no significant differences in morphology and NGF contents were found between single drug treated groups and combined application group.Conclusions The observations suggest that methylcobalamin,gliclazide and combined application are effective in treating DPN in STZ-D rats.No significantly enhanced effect is found in combined application group as compared with methylcobalamin or gliclazide group.
3.Case study of pharmaceutical direct supply at Wuhan Pu Ai Hospital

Wuhan Pu Ai Hospital implemented the pharmaceutical direct supply mode since October 2006 as an experiment.The pilot proved the mode as successful in reducing drug costs proportion,promoting rational use of drugs,and raising business volume of the hospital.Authors also recommend to further efforts in drug supply price supervision mechanism,government subsidy mechanism,direct supply catalogue and drug supply mode among others.Other recommendations include innovation in directly supply mode,hospital internal reform,packaged performance appraisal and incentive mechanism for medical workers,and building a ceiling mechanism for total medical expense control.
4.The effect of complete left bundle branch block on left ventricular radial dyssynchrony and rotation and twist

ObjectiveTo investigate left ventricular (LV) radial dyssynchrony and LV rotational change of isolated complete left bundle branch block (CLBBB) and CLBBB with heart failure (HF).Methods Twenty-six isolated CLBBB patients,twenty-four CLBBB patients with HF and twenty healthy controls were studied.Two-dimensional speckle tracking imaging (2D-STI) was applied to short-axis views to calculate LV radial dyssynchrony and LV rotational and twist degree and time to peak rotational and twist degree.ResultsLV radial dyssynchrony and LV rotation and twist degree were intermediately damaged in isolated CLBBB patients and was significantly damaged in CLBBB patients with HF,compared with healthy controls.ConclusionsFor isolated CLBBB patients,even if LV ejection fraction was in the normal range,another parameters of cardiac mechanic had changed and 2DSTI could be used to evaluate these alterations.
5.Medication Analysis and Pharmaceutical Care for One Patient with Crohn’ s Disease

Objective:To analyze the drug use in a patient with Crohn’ s disease and provide relevant pharmaceutical service by clinical pharmacists. Methods:Clinical pharmacists participated in the treatment of the patient. The medication was analyzed according to the check results and diagnosis and treatment process, and relevant pharmaceutical service was provided for the patient and nurses. Results: Through the participation in the patient treatment and health education for the patient and nurses performed by clinical pharmacists, the patient was recovered and discharged. Conclusion: Clinical pharmacist participating in pharmaceutical service can promote rational drug use in clinics.
6.Effect of Tongjiangning Capsule on reproductive organ of female immature mice and its anti-inflammatory analgesia

AIM: To observe the effect of Tongjingning Capsule (Radix Angelicae Sinensis, Rhizoma Chuanxiong, Rhizoma Cyperi, etc.) on reproductive organs of female immature mice and anti inflammatry analgesia. METHODS: Mouse uterus and ovaries were weighted. Mouse twisting numbers induced by acetic acid and mouse pain thresholds after heat stimulation were recorded. Swelling of mouse external ears, swelling of rat toes and rat granuloma were used for experiment models. RESULTS: Tongjingning Capsule significantly increased the weight of mouse uterus and ovaries. It could reduce mouse twisting numbers and prolong mouse pain thresholds after heat stimulation. It had inhibitory effects on swelling of mice's external ears resulted by dimethylbenzene, rat granuloma and swelling of rat toes resulted by egg white. CONCLUSION: Tongjingning Capsule can promote the growth of reproductive organs of female immature mice and ease pain and diminish inflammation.
7.Medication Analysis for One Patient with Severe Ulcerative Colitis

Objective:To analyze the medication of one patient with ulcerative colitis to provide pharmaceutical care and support for rational drug use in patients with ulcerative colitis. Methods:During the treatment of the patient with severe ulcerative colitis, clin-ical pharmacists analyzed the drugs used by the patient and provided pharmaceutical care for doctors and the patient according to the ex-amination and diagnosis of the patient. Results:The compliance, therapeutic effect and medication safety of the patient were all im-proved by giving clinical drug rationalization suggestions and targeted medication monitoring and education, which fully embodied the necessity of work of clinical pharmacists in the medication of patients. Conclusion:Through case analysis, clinical thinking of clinical pharmacists can be developed to promote rational drug use, avoid adverse drug reactions and achieve optimal effect of drug treatment.
8.Antitumor and immunological regulation effects of ardipusilloside-Ⅰ

Aim To study the antitumor and immunological regulation effects of ardipusilloside-Ⅰ(ADS-Ⅰ) on tumor-bearing mice of S180, H22, EAC and L1210. Methods The tumor-bearing mouse model was made through injecting cancer cells in the right limb oxter(or in abdominal cavity), and the mice were administrated orally with ADS-Ⅰfor 8 days. Then we detached the tumor and counted the inhibition rate of tumor growth. Incorporation of ()~3H-TdR was used to assess the proliferation of T spleen lymphocyte cells; phagocytosis of abdominal cavity macrophage was measured with the method of chicken red blood cells. Results ADS-Ⅰ was proved to be effective on the tumor-bearing mice of S180, H22 and EAC, and the maximal inhibition rate of the dose of 100 mg?kg~(-1) was 38.7%, 48.6% and 50.0%, respectively;ADS-Ⅰ promoted the proliferation of T spleen lymphocyte cells,and strengthened the phagocytosis of macrophage in the tumor-bearing mice significantly. Conclusion ADS-Ⅰ can inhibit the growth of transplanted tumor,and improve the immunological function of tumor-bearing mice.
9.Clinical analysis of combined antegrade and retrograde technique of laparoscopic cholecystectomy in prevention of bile duct injury

Objective To discuss the clinical value of combined antegrade and retrograde technique of laparoscopic cholecystectomy in prevention of bile duct injury.Methods Clinical data of 613 patients who underwent laparoscopic cholecystectomy with combined antegrade and retrograde technique from March 1991 to June 2006 were analyzed retrospectively.Results Cholecystolithiasis with atrophy of gallbladder was found in 121 cases,acute and subacute cholecystitis in 432 cases,cholecystolithiasis with chronic cholecystitis in 42 cases and polyps of cholecyst in 18 cases.Nine cases were converted to laparotomy(5 cases of cholecystolithiasis with atrophy of gallbladder and 4 cases of subacute cholecystitis).The average operation time was 43.5 minutes.Bile leakage occured in only 1 case after operation and fully recovered after 3 days of drainage,and all the other cases had smooth recovery with no bile duct injuries or serious complications such as haemorrhage,infection or death.The mean hospitalization time after operation was 5.5 days.Conclusions The application of combined antegrade and retrograde technique can increase the success rate of laparoscopic cholecystectomy,and reduce the incidence of injuries of bile duct,especially in those patients with inadequate exposure or anatomical variation of Calot′s triangle.
10.Study of left ventricular systolic volume and synchrony in patients with premature ventricular complexes from the right ventricular outflow tract by instantaneous full-volume imaging

Objective To assess alternations in left ventricular volume and systolic synchrony in patients with frequent premature ventricular complexes(PVCs) from the right ventricular outflow tract(RVOT).Methods Twenty-nine patients with frequent isolated PVCs from RVOT were included and 30 healthy subjects as control.Instantaneous full-volume imaging(IFI) was performed to evaluate left ventricle volumetric parameters,including end-systolic volume (ESV),end-diastolic volume (EDV),stroke volume (SV),ejection fraction (EF),and systolic synchrony parameters,including systolic dyssynchrony index (SDI),dispersion end-systole (DISPES),mean end-systolic time (MES),pre-contraction time volume (PreContr) and post-contraction time volume (PostContr).Contraction front mapping was performed to visualize volumetric contraction sequence.All values of patients with PVCs were recorded during sinus beats (PVC-S) and premature ventricular beats (PVC-V) respectively.Results Significant differences were observed in left ventricular systolic volumetric and synchrony parameters between PVC-V and control subjects (P<0.01),as well as in MES and PreContr between PVC-S and control subjects (P<0.01).Conclusions Left ventricular systolic dysynchrony was demonstrated in patients with PVCs from RVOT.IFI was a novel tool to analyze left ventricular global and regional volumetric alternations.
