1.Comparison of the effect and prognosis of intermittent blue light and continuous blue light therapy for neonatal jaundice

Objective To observe the effect of intermittent and continuous blue light therapy for pathological neonatal jaundice,and to evaluate the prognosis of newborns.Methods 214 patients with pathologic neonatal jaundice who treated with blue light irradiation were slected,they were randomly divided into two groups according to the admission number combined with single and double numbers,107 patients in each group.Group A received continuous blue light irradiation treatment.Group B received intermittent blue light irradiation.The clinical efficacy and prognosis of children,blue light exposure time,serum total bilirubin (TBiL) recovery time,the hospitalization time and the adverse reactions during the treatment were observed.The levels of serum TBiLwere measured before and after light exposure 3,5 days.Results The total effective rate of group B was 97.18% (104/107),which was higher than that of group A [90.65 % (97/107)],the difference was statistically significant (x2 =10.30,P < 0.05).The time of blue light irradiation and hospital stay time of group B were (71.59 ± 7.05) d and (11.51 ± 1.23) d,respectively,which were significantly shorter than those of group A [(79.54 ± 6.82) d and (13.64 ± 1.18) d],the differences were statistically significant (u =11.42,9.87,all P < 0.05).The level of TBiL of group B was (96.28 ± 10.25) μmol/L,which was significantly lower than (104.52 ± 10.31) μmol/L of group A (u =8.94,P < 0.05).The incidence rate of adverse reactions in group B was 2.55% (4/157),which was lower than that in group A [7.01% (11/157)],the difference was statistically significant (x2 =9.66,P < 0.05).The prognosis of neonatal in the two groups was good.Conclusion The intermittent and continuous blue light in the treatment of pathological neonatal jaundice has good clinical effect and the neonatal prognosis is good.The clinical efficacy of intermittent blue light irradiation is better,which can significantly shorten the treatment time and has higher safety.
2.Detection and phylogenetic tree analysis of human parainfluenza virus type 3 in children with acute respiratory tract infection from a hospital in Shanghai

Objective To understand the epidemiologic feature of human parainfluenza virus type 3 (HPIV-3) in Shanghai,and to provide scientific evidence for formulating prevention and control measures in the future.Methods A total of 164 nasopharyngeal aspirates samples taken from children with acute respiratory infection (ARI) were collected from Xinhua Hospital Affiliated to Shanghai Jiaotong University and sent to Shanghai Public Health Clinical Center from June 2009 to June 2010.Samples were detected for HPIV-3 by reversed transcription-polymerase chain reaction (RT-PCR).Full-length hemagglutininneuraminidase (HN) gene (1 719 bp) of five positive samples were sequenced for phylogenetic analysis.Comparison between two groups was evaluated by the precise chi-square test (two sided).Results Of 164 samples,70 samples were infected with parainfluenza virus,and HPIV-3 was detected positive in 23 samples with the positive rate of 32.86%.HPIV-3 infections were most common in spring and summer,and most of infections were mainly found in 13-36 month-old infants.Five Shanghai isolates and 36 reference sequences from different countries and areas were divided by HN gene-based phylogenetic tree into three clusters (A,B and C).Five Shanghai isolates and five Beijing isolates belonged to C3a group.The homologies of nucleotide and amino acid sequences between five Shanghai isolates and five Beijing isolates were 99.0%-99.5% and 99.7%-100.0%,respectively.Conclusions HPIV-3 accounts for a high proportion in children with ARI in Shanghai.C3a group may be the main lineage of HPIV-3,which suggests that HPIV-3 may be of regionally correlation.
3.Pathogen detection and clinical characteristics of hand, foot and mouth disease children in Nantong

Objective To analyze the epidemiological characteristics of pathogens of children with hand , foot and mouth disease (HFMD) in Nantong area, in order to provide the basis for the treatment and prevention of HFMD. Methods Multiplex RT-PCR (fluorescence PCR method) was used to detect EV71, CoxA16 and non-infectious cases of EV71, CoxA16 of children with HFMD in Nantong. VP1 gene of strains isolated from 12 cases of EV71 was amplified and underwent phylogenetic tree analysis. 208 cases clinical data of children with HFMD were analyzed and summarized. Results In 208 cases of children with HFMD swab samples, EV71 of 64 cases were positive (30.8%); CoxA16 of 28 cases were positive (13.5%); 40 cases of other enterovirus were positive (19.2%). Phylogenetic tree analysis showed that EV71 epidemic strains are C4 subtypes in Nantong region. The clinical manifestations of enterovirus infections in EV71 , CoxA16 and other enterovirus are caused by rash, fever and other symptoms, the difference of which was not statistically significant (P > 0.05). In laboratory examination, EV71 infection made white blood cell count higher than CoxA16 and other enterovirus, but the difference was not statistically significant (P>0.05). Conclusions EV71, CoxA16 and other enterovirus intestinal virus are main pathogens of HFMD in Nantong. EV71 C4 subtype is the main popular subtype of EV71 in Nantong. The elevation of white blood cell count of HFMD is the main clinical manifestations and characteristics , but can not serve as the identification of children infected with EV71, CoxA16 and other enterovirus.

Objective: To investigate the biological phenotype and cell cycle distribution characteristics of nasal and/or nasopharyngeal epithelia of TgN(p53mt-LMP1)/HT mice.Methods: The biological phenotype changes(mainly the incidence of precancerous lesion) in nasal and nasopharyngeal epithelial tissues of the generation G4 of TgN(p53mt-LMP1)/HT mice aged 5,11,15 and 18 months were determined by H-E staining,and the cell cycle characteristics of nasal and nasopharyngeal epithelia detected by flow cytometry(FCM).Results: The incidences of precancerous lesion in the nasal and nasopharyngeal epithelia were 0,50%,60% and 75% respectively in the 5,11,15 and 18 mos groups of the positively expressed transgenic mice,and 0 in the four age groups of the negatively expressed ones.Compared with the negatively expressed transgenic mice,the number of nasal or nasopharyngeal epithelial cells was markedly decreased in the G0/G1 phase,but obviously increased in the S and G2/M phases,with the proliferation index(PI) significantly enhanced(P
5.Advances of human papillomavirus therapeutic vaccine

Human papillomavirus continuous (HPV) infection is an essential factor to induce cervical intraepithelial neoplasia and cervical cancer.Treating HPV infection is considered as a starting point to develop an effective therapeutic vaccine, which is a new strategy for prevention and treatment of cervical cancer.In recent years, development and trials of therapeutic HPV vaccine have made great progress.Selection of vectors, utilization of adjuvants, synthesis of fusion proteins and chimeric proteins have been widely applied to research to enhance vaccine immunogenicity, to increase vaccination safety, to reduce the side effect and so on.The clinical trial results are encouraging: various types of vaccines can induce a specific immune response with good tolerance.However, numerous studies are still required to obtain further success.In addition, HPV exists in various forms, thus it is also the focus of study to expand the range of action and reduce immune escape.
6.Enterovirus 71 molecular epidemiology research in Nantong,2014

Objective To analyze enterovirus 71 (EV71) molecular epidemiology characteristics in Nantong region of Jiangsu , 2014 .Methods The pharyngeal swab samples were selected from 52 children with hand ,foot and mouth disease .After extracting the virus nucleic acid ,the EV71 VP1 gene of the virus were amplified by RT‐PCR amplification .The phylogenetic tree was con‐structed between isolated and EV71 reference strains from other parts of the country .Results 12 strains EV71 VP1 gene were iso‐lated from 52 cases of clinical specimens .The nucleotide and amino acid homology of 12 strains EV71 VP1 gene was 93 .4% -99 .1% ,the phylogenetic tree analysis showed that 12 strains of VP1 gene all belong to C4 genotype C4a subgenotypes .Conclusion The isolated strains in Nantong in 2014 are all the EV71 C4a subgenotypes of C4 genotype ,the same with the most of isolates in re‐cent years .
7.Preparation and application of cooling bag for heat stroke in wild field

Cooling bag for heat stroke in wild field was invented in order to fulfill rapid cooling for heat stroke in field environment. The cooling bag is composed of the hood, the body cover (which is made up of the anterior portion, the linking, and the posterior portion), the fixed straps, the handles. The length is 200 cm, the regulative width is 60-70 cm, the folding volume is 26 cm× 20 cm× 9 cm, and the weight is 1.4 kg. There are a number of pockets for the cold sources in the hood and the body cover. Fixed straps are set in the upper, middle and lower of the bag. The handles are set in the hood and the body covers. Usage: put the activated cold sources in the pockets, then put off the patient's clothes, stretch his/her arms into the linking, zipper up and wrapped up by the fixed straps; the amount of cold sources can be adjusted or changed according to the temperature. The patient's temperature, blood pressure, intravenous infusion can be monitored during the cooling course. The handles in the hood and the body cover allow the patient to be easily moved and be transferred in rescue. The cooling bag for heat stroke in field has good cooling effect, and are reusable and easy to be carried, operate, monitor and move, with low cost, which make it popularization and application.
8.Determination of Puerarin in Compound Ginkgo Biloba Tablets by RP-HPLC

OBJECTIVE:To determine the content of Puerarin in Compound Ginkgo biloba tablets by RP-HPLC. METHODS:The determination was performed on Kromasil C18 column. The mobile phase consisted of methanol-water (20∶80) with a flow rate of 1mL?min-1. The detection wavelength was 250 nm and the temperature of the column was 35℃. RESULTS:The linear range of Puerarin was 0.8~2.4?g (r=0.999 3, n=5),with the average recovery at 99.98%,RSD=1.31%(n=5).CONCLUSION:The method is simple,sensitive and reproducible,and it can be used for the quality control of Compound Ginkgo biloba tablets.

Objective To assess alternations in left ventricular volume and systolic synchrony in patients with frequent premature ventricular complexes(PVCs) from the right ventricular outflow tract(RVOT).Methods Twenty-nine patients with frequent isolated PVCs from RVOT were included and 30 healthy subjects as control.Instantaneous full-volume imaging(IFI) was performed to evaluate left ventricle volumetric parameters,including end-systolic volume (ESV),end-diastolic volume (EDV),stroke volume (SV),ejection fraction (EF),and systolic synchrony parameters,including systolic dyssynchrony index (SDI),dispersion end-systole (DISPES),mean end-systolic time (MES),pre-contraction time volume (PreContr) and post-contraction time volume (PostContr).Contraction front mapping was performed to visualize volumetric contraction sequence.All values of patients with PVCs were recorded during sinus beats (PVC-S) and premature ventricular beats (PVC-V) respectively.Results Significant differences were observed in left ventricular systolic volumetric and synchrony parameters between PVC-V and control subjects (P<0.01),as well as in MES and PreContr between PVC-S and control subjects (P<0.01).Conclusions Left ventricular systolic dysynchrony was demonstrated in patients with PVCs from RVOT.IFI was a novel tool to analyze left ventricular global and regional volumetric alternations.
