Neonatal acute heart failure is a common critical illness,and also one of the material cause of perinatal death. It is difficult for early diagnosis due to the different characteristics and clinical manifestation between neonate and older children, which leads to the difficulty of diagnosis timely and affect the condition adversely. This article introduced the common etiology, characteristics of clinical manifestation, diagnosis and treatment of neonatal acute heart failure.
2.Effects of Shenqi Compound Recipe on the expression of adiponectin in white adipose tissue in GK rats

AIM: To explore the anti-low-grade-inflammation mechanisms of Shenqi Compound Recipe(SQCR) in GK rats. METHODS: Specefic pathogen free(SPF) GK rats were divided randomly according to blood glucose level into four groups:Model,Ramipril,SQCR low dosage,SQCR high dosage group and normal Wistar rats(normal control group).GK rats were injected N-?-nitrol-L-arginine methyl ester(L-NAME) intra-peritoneally and took high-fat diet and Wistar rats were injected saline intra-peritoneally and took common diet freely,respectively.In the experiment periods,each group was administrated correspondent substance respectively for 32 days.Serum concentrations of C reactive protein(CRP) and adiponectin were determined by ELISA.Adiponectin mRNA expressions in white adiposed tissue were measured by real time reverse transcriptase PCR. Objective To determine chest ultrasound locating treatment of paraelectronic effusion by medical thoracoscopy.Methods We chose 40 Xinxiang center hospital patients in respiratory wards from June 2012 to June 2014,who was randomly divided into two teams,one was electron-assisted Thoracoscopic surgery,the other was thoracic cavity closed drainage combination of intrapleural injection of urokinase.It was 3rd reviewed by chest ultrasound,we evaluated curative effection ac cording to the drainage effect,we statisticsed two draining days and total inpatient days,and total hospital costs.The results two teams were compared between gender,age,nutritional status,effusion volume (length x width x depth),pleural effusion conventional,biochemical,lactate dehydrogenase,adenosine deaminase,carcinoembryonic antigen,the difference was not statistically significant (P > 0.05).Pleural fluid culture results were negative.Medical thoracoscopy treatment group hospitalization days,drainage time was significantly shorter than the thoracic drainage combined with pleural urokinase group,The same was the cost of hospitalization.Medical thoracoscopy poor drainage treatment group was significantly lower than the thoracic drainage combined with pleural urokinase group (P < 0.05).Conclusion To the middle-large number encapsulated paraelectronic effusion,chest ultrasound locating treatment by medical thoracoscopy early can reduce the formation of empyema,reduce the formation of pleural fiberboard.It is a safe,effective and worthy of promotion treatment.
4.Effect of pigment epithelium-derived factor on p38MAPK-CREB pathway and fibronectin in high glucose cultured human mesangial cells

Objective To investigate the effect of pigment epithelium- derived factor (PEDF) on p38MAPK-CREB pathway and the expression of fibronectin (FN) in human mesangial cells (HMCs) cultured with high glucose. Methods HMCs were treated with different concentrations of glucose and the osmotic control respectively in the presence or absence of PEDF for 24 h:normal glucose (5.6 mmol/L),24.4 mmol/L mannitol,high glucose (30 mmol/L),high glucose+PEDF(30 mmol/L glucose with 10 nmol/L PEDF,40 nmol/L PEDF or 100 nmol/L PEDF).After samples were collected,the expression of phospho-p38MAPK (p-p38) and p-CREB was assessed by Western blotting,while FN mRNA and protein expression was assessed with RT-PCR and ELISA respectively. Results In contrast to normal glucose and mannitol treatments,the expression of p-p38MAPK,p-CREB and FN increased significantly in high glucose group (all P< 0.01).However,PEDF abolished the up-regulation of p-p38MAPK,p-CREB and FN induced by high glucose (all P<0.05). Conclusion PEDF may inhibit fibrosis through P38MAPK-CREB pathway in diabetic nephropathy.
5.A study on pathway of apoptosis in murine thymocytesinduced by candida albicans

Objective:To investigate the pathway of C.albicans-induced murine thymocyte apoptosis.Methods:The chages of TNF-? level in murine plasma and caspase-3 activity were measured by ELISA and flurorescence spectrophoto meter respectively;FCM measurement was adopted for the expression levels of related genes.Results:The level of TNF rise,the caspase-3 activity serengthen significantly and bax,p53 gene expression are upregulated.Conclusion:It is possible that C.albicans elicits apoptosis in murine thymocytes by upregulating bax and p53 gene expression as well as raising the serum TNF-? level to strenghen the activity of caspase-3.
6.The effect of fluoride on the secretory ameloblast apoptosis

Objective To elucidate the relationship between f luoride and cell apoptosis of ameloblasts and to further study the mechanism of dental fluorosis. Methods By establishing an experimental m odel of fluoride intoxication in pregnant C57BL/6N, we observed apoptosis in the fetal secretory ameloblasts in mice by means of transmission electron microscop e and TUNEL. Results High fluoride exposure in pregnant mic e could cause subameloblastic cysts in first lower molar of fetal mice. Programm ed cell death (PCD) occurred in the disturbed ameloblasts above cysts. Conclusion Intoxicat ion mechanism of fluoride on ameloblasts may occur by means of PCD.
7.Effects of heme oxygenase-1 preconditioning on oxidative damage in alveolar epithelial type Ⅱ cells in rats

Objective To investigate the effects of heme oxygenase-1 (HO-1) preconditioning on oxidative damage in alveolar epithelial type Ⅱ (AE- Ⅱ) cells in rats. Methods The primarily cultured AE- Ⅱ cells isolated from male SD rats were randomly assigned to one of 6 groups (n = 8 each): control group (group C),H2O2 group and 4 different concentrations of HO-1 preconditioning group (group H1-4). The cells were continuously incubated for 5 h in group C. H2O2 0.5 mmol/L was added and the cells were incubated for3 h in group H2O2.In group H1-4, HO-1 0.01, 0.10, 1.00 and 10.00 μmol/L were added and the cells were incubated for2 h, then H2O2 0.5 mmol/L was added and the cells were incubated for 3 h. After the end of incubation, the cell morphology was observed under inverted phase contrast microscope, the AE- Ⅱ cells were counted, and the cell viability was determincd. Results The most AE- Ⅱ cells were adherent and round, had homogeneous cytoplasm, and the cytoplasm contains granular materials in group C and H2-4, while the vacuoles appeared in AE- Ⅱ cells and the cell debris appearecd in the supernatant in group H2 O2 and H1 . Compared with group C, the cell count and cell viability were significantly decreased in group H2 O2 and H1 (P < 0.05). Compared with group H2 O2 and H1, the cell count and cell viability were significantly increased in group H2-4 (P < 0.05). There was no significant difference in the cell count and cell viability between group H2O2 and H1, and among group H2~4(P > 0.05) .Conclusion Preconditioning with 0.10-10.00 μmol/L HO-1 can reduce oxidative damage in rat AE- Ⅱ cells.
8.Influence of radiofrequency ablation on spleen dendritic cells in normal rats

Objective To study the change of spleen Dendritic cells in normal rats treated by radio-frequency ablation(RFA). Methods Eighteen healthy SD rats were separated into group 1 week after RFA with 6 rats,group 2 week after RFA with 6 rats and control group with 6 rats. Spleen tissue were taken out respectively before RFA, 1 week after RFA and 2 weeks after RFA. The number and the phenotype of Dendritic cells in spleen were analyzed with flowcytometry. Results Pathologyical examination after RFA showed the characteristic that coagulation necrosis and cellular degeneration and granulation tissue forming appeared from target center to peripheral of the target. (10. 36±3. 21) % of normal rat mononuclear cells in spleen express OX-62, the ratio became (18. 03±5. 7) % 1 week after RFA and (12. 63±8. 0) % 2 weeks after RFA, the difference between group 1 week after RFA and control group was marked. (76. 33±7. 86) % of normal rat mononuclear cells in spleen express OX-6,the ratio became (78.33±7.25)% 1 week after RFA and (86. 04±7. 25) % 2 weeks after RFA, the difference between group 2 weeks after RFA and control group was marked. (63. 06±8. 77) % of normal rat mononuclear cells in spleen express CD86,the ratio was (55. 74±14. 49)% 1 week after RFA and (63.49±11.81)% 2 weeks after RFA,the difference between groups 1 week or 2 weeks after RFA and control group was not marked. Conclusions RFA can increasing the number of precursor Dendritic cells migrating from peripheral blood to spleen, and those cells may furtherly differentiate or maturate, which may be contributed to improve the ability delivery of body to antigen to a certain extent.
9.Study of influential factors of radiofrequency ablation on differentiation of dendritic cells of rats with liver tumor

Objective To discuss the change of serum TGF-β1 and IL-10 in peripheral blood of rats with liver tumor treated by RFA.Methods Thirty experimental liver tumor model of SD rats were prepared by implantation of tumor particles.These rats were randomly divided equally into three groups including 1 week after RFA,2 week after RFA and control group.Peripheral blood of control group,group 1 week after RFA and group 2 week after RFA was taken out respectively without RFA,1 week and 2 week after RFA.The mononuclear cells of peripheral blood were separated by Ficoll density gradient centrifugation.The expression level of IL-10 in peripheral blood was analyzed with flowcytometry.Serum TGF-β1 were dectected by ELISA.Results The serum expression level of TGF-β1 of control group was(6.61±0.12)μg/L,and that of group 1 week and 2 week after RFA were respectively(5.63±0.46)μg/L and(5.53±0.56)μg/L.There was statistical significance for the difference between control group and group 1 week or group 2 week after RFA.The serum expression level of IL-10 of control gorup was 95.92±2.31,and that of group 1 week and 2 week after RFA were respectively 89.71±5.44 and 87.67±11.11.There was statistical significance for the difference between control group and group 1 week after RFA.Conclusions RFA can destroy the tumor tissues in situ and relief immune suppression by IL-10,TGF-β1 secreted by tumor tissue.RFA can improve differentiation and maturation of dendritic cell in local area of tumor and promate ability of antigen-presentation of body.
10.Evaluation of fractionated stereotactic radiotherapy for residual lesion after the first course of radiotherapy for nasopharyngeal carcinoma

Objective To analyze the long-term results of fractionated stereotactie radiotherapy(FSRT)for the local residual lesion after the first course of radiotherapy for nasopharyngeal carcinoma.Methods From July 1997 to July 2002,46 patients were treated with FSRT.According to the 1992 Fuzhou staging system,the number of patients was 1,6,30 and 9 with stage Ⅰ,Ⅱ,Ⅲ and Ⅳ disease,respectirely;3,11,27 and 5 with T1,T2,T3 and T4 tumor,respectively;14,16,12 and 4 with N0,N1,N2and N3 disease.Radiotherapy was delivered to tumors with the total of dose 68-70 Gy in 7-8w.Chemotherapy(2 cycles of PVF or POF)was given to the patients with stage Ⅲ and Ⅳ a disease.FSRT was given to the residual disease with the total dose of 18-24 Gy in 3 fractions with an interval of 3-7 days.The reference dose line was 70%-90%.Resuits CR and PR rates in this group were 61%and 39%,respectively.The overall survival rates of each year from 1- to 5-year were 100%,87%,83%,78%and 76%.The 1-,3- and 5-year disease-free survival rates were 100%,93%and 89%;The distant metastasis-free survival rates were 100%,85%and 79%;The local-regional control survival rates were 100%,94%and 91%.Seventeen patients who died during the follow-up period were 1 for local cervical lymph node recurrence,2 for fatal nasopharyngeal hemorrhage,4 for local nasopharynx recurrence,and 10 for distance metastases. Conclusions Fraetionated stereotactic radiotherapy is safe and effective for the patients with residual lesion of nasopharyngeal carcinoma at the primary site after radiotherapy.The optimized fractionation and total dose requires the further investigation.
