1.Effects of Irbesartan on Expression of MMP-2,TIMP-2 and Ⅳ-C in Kidney of Diabetic Rats

OBJECTIVE:To investigate the expression of matrix metallopro teinase-2(MMP-2),tissue inhibitor of metalloproteinase-2(TIMP-2) and collagen Ⅳ(Ⅳ-C) in kidney of diabetic rats and the intervention effect of irbesartan.METHODS:30 male SD rats were randomly divided into three groups:normal control group(NC group),diabetes mellitus group(DM group) and diabetes mellitus+50 mg?kg-1 irbesartan group(DI group).DM and DI group were injected with streptozocin intraperitoneally to reduce diabetic model.After 8 weeks the expression of MMP-2,TIMP-2 and Ⅳ-C in kidney tissure were determined by immunohistochemistry method(SP) and RT-PCR.RESULTS:Compared with NC group,DM group showed the expression of protein and mRNA of MMP-2 significantly reduced while that of TIMP-2 and Ⅳ-C increased(P
2.The effects of dexmedetomidine and ketamine on breast lumpectomy in women patients

Objective To observe the effects of dexmedetomidine and ketamine on breast lumpectomy in women patients .Meth‐ods Thirty women who underwent breast lumpectomy were randomly allocated into two groups (n=15) .Group A received mid‐azolam 2 .0 mg ,fentanyl 0 .1 mg and ketamine 2 .0 mg/kg ,everyone required supplementary propofol 5 .0 mg?kg -1 ?h-1 .In group B ,dexmedetomidine 0 .5 μg/kg was administered as a loading dose during 10 minutes followed by a continuous infusion of 0 .2μg?kg -1 ?h-1 ,Ketamine 2 .0 mg/kg was administered along with the dexmedetomidine loading dose .The recovery time ,the VAS and Ramsay at the end of the first hour of postoperation were recorded along with side effects .Meanwhile ,hemodynamic parameters , like heart rate(HR) ,mean blood pressure (MAP) and SPO2 were also monitored .Results Compared with group A ,the HR and MAP were significantly lower(P<0 .05) ,and no significant changes in SPO2 were noted (P>0 .05) in group B;there was no sig‐nificant difference in VAS and Ramsay scores among two groups ,the recovery time was shorter in group B(P<0 .05) .However ,the incidence of respiratory arrest and glossocoma were greatly higher in group A(P<0 .05) .Conclusion Dexmedetomidine and ket‐amine can offer an effective sedoanalgesia and lower hemodynamic discrepancy without causing any significant side effect on breast lumpectomy in women patients .
3.Clinical study on treatment of early diabetic nephropathy by extract of gingko biloba

objective To investigate the protective effect and possible mechanisms of extract of Gingko biloba(EGB)on early diabetic nephropathy(DN).Methods Fifty-one patients with early DN were randomly divided into control group(n=25)treated by irbesartan alone and treated group(n=26)treated by EGB combined with irbesartan.Serum high sensitive C-reactive protein(hs-CRP)and monocyte chemoattractant protein-1(MCP-1)were determined with ELISA before and after treatment,and urinary albumin excretion rate(UAER),fasting blood glucose (FBG),serum creatinine(SCr),blood urea nitrogen(BUN)and lipid profiles were examined as well.Results Compared with pretreatment,at the end of 6 months'treatment,the values of hs-CRP decreased from(194.37±34.02)and(191.51±27.15)mg/L to(164.13±32.86)and(134.65±20.47)mg/L in control group and treatment group,respectively.MCP-1 was downregulated from(304.23±38.56)and(299.66±44.07)ng/L to (235.43±28.66)and(165.53±21.96)ng/L in control group and treated group(all P<0.05).After treatment,the decrement of hs-CRP and MCP-1 level in the treated group was more significant than that in control group(P<0.05).Conclusion EGB could retard the development of early DN,through down-regulating the expresssion of seram MCP-1,decreasing serum hs-CRP concentration and inhibiting inflammatory reaction.
4.Effect of irradiation-induced wt-p53 positive circuit on cell life cycle,apoptosis and irradiation sensitivity of lung adenocarcinoma cells

Objective To study the effects of irradiation-induced wt-p53 positive feedback circuit on cell life cycle,apoptosis rate and irradiation sensitivity of adenocarcinoma of lung in vitro.Methods The therapeutic group pE6R4-p53-EGFP/H1299 cells and the other 3 control groups H1299(p53-/-) ,pE6-p53EGFP/H1299 and pR4-p53-EGFP/H1299 cells were irradiated with 8-Mev electron beam generated by a linear accelerator at a dose rate of 400 cGy/min and a source-skin distance(SSD) of 100 cm,12 h exposure to irradiation.The cell life cycle and the rate of apoptosis were analyzed by FCM.Mean lethal dose(Do) and sensitive enhancement ratio(SER) index were calculated from the irradiation dose-survival curve.Results In 12 h post 4-Gy 8MV-X-ray irradiation,FCM analysis showed that most cells in therapeutic group were arrested at G0/G1 stage(75.13?1.42) %,compared with the 3 control groups(38.47?0.87) %,(62.57? 0.76) %and(51.23?2.41) %,respectively.After irradiation,the cell apoptotic rate in each group was higher than that in the cells without irradiation.The apoptotic rate in the therapeutic group was(23.73? 0.21) %,which was 5.69,1.51 and 2.57 folds respectively higher in the 3 control groups(4.17?0.12) %,(15.67?0.32) %and(9.23?0.15) %.Do values were 0.91,1.073 and 1.413 Gy respectively in pE6R4-p53-EGFP/H1299,pE6-p53-EGFP/H1299 and H1299 cells.The SER,derived from Do values,was 2.63 and 1.34,respectively.Conclusion Irradiation up-regulates wt-p53 gene expression,regulates cell cycle and induces cell apoptosis.Thus this positive feedback circuit increases the sensitivity of lung adenocarcinoma to irradiation.
5.Treatment of acute cerebral infarction by intranasal administration of nerve growth factor into central nervous system

Objective:To study the reliability and feasibility of intranasal(IN)pathway bypassing the blood-brain barrier (BBB) and observe the potential neuroprotective effects of intranasal NGF on acute cerebral ischemia. Methods:A blinded, vehicle-controlled study of IN NGF and IV NGF administration was performed by intraluminal middle cerebral artery occlusion (MCAO) model. Experiment 1: Rats were randomly divided into IN NGF, IV NGF and untreated group (n=4). The concentration of NGF in different brain regions was measured by ELISA. Experiment 2: Rats were randomly assigned into 4 groups: IN vehicle, IN NGF, IV vehicle, IV NGF (n=8 each). Treatments was initiated 30 min after the onset of MCAO and then again 24 h later. Three neurologic behavioral tests were assessed at 24 h and 48h. Corrected infarct volumes were determined 48 h after the onset of MCAO. Results:Olfactory bulb in IN NGF group obtained the highest concentration among all tissues, arriving at 3 252 pg/g, followed by hippocampus. The NGF concentrations of olfactory bulb and hippocampus in IN NGF group were significantly higher than that in IV NGF and control group. The infarct volume in IN NGF group was reduced significantly by 38.8% as compared with IN vehicle. The vestibulomotor function of IN NGF improved significantly at 24 and 48 h (P=0.02 and P=0.04, respectively). Conclusion:Intranasal NGF could bypass BBB, reach the CNS, reduce infarct volume and improve neurological function in rats following MCAO. Intranasal delivery of NGF holds a promising treatment for stroke and other CNS disorders.

7.Introduction of a modified PICC blind puncture catheterization technique

Objective:To investigate the application and feasibility of the night-vision puncture technique in performing peripherally inserted central catheter(PICC).Methods:Seventy patients were randomly divided into ultrasound guided puncture group or modified blind puncture group,35 cases in each group.The puncture success rate,the achievement ratio of catheterization,the puncture site,arm circumference,catheterization time and complications were recorded in two groups.Results:Two groups had no significant difference in the puncture success rate,the achievement ratio of catheterization,the puncture site,arm circumference,phlebitis incidence,subcutaneous congestion (bleeding) (P > 0.05).The catheterization time in modified blind puncture group was shorter than the ultrasound guided puncture group (P < 0.001).In modified blind puncture group,a negative correlation (correlation coefficient:-0.475,P =0.004)between arm circumference and the puncture success rate was found.Conclusion:The puncture success rate of the modified blind puncture technique in performing peripherally inserted central catheter (PICC) is high and close to the puncture success rate of the PICC under ultrasound.Additionally,the modified blind puncture technique does not increase the incidence of complications and delay the catheter time.

Objective To explore the effect of dexmedetomidine for the dosage of drug of pain pump of the patients postoperative.Methods From April 2013 to July 2015,86 patients with intestinal obstructionwhich come to our hospital for surgical treatment were divided into observation group and control group according to the lottery method,each had 43 cases.The patients of control group were given pain pump for pain treatment ; and the patients of observation group were given dexmedetomidine on the base of control group for treatment.The heart rate,systolic arterial pressure(SAP),arterial oxygen pressure(PaO2),analgesia satisfaction,visual analogue scale/score(VAS)pain score,sleep quality score,sufentanil dosage,the number of self-administration,adverse reactions,postoperative anal exhaust time.Results 36h and 72h,SAP,PaO2 in the observation group were significantly better than those in the control group after treatment(P<0.05); The total satisfaction of analgesia was significantly higher than that of control group(P<0.05); VAS pain score was significantly lower than the control group(P<0.05); The sleep quality score was significantly higher than that of the control group(P<0.05); The dosage of sufentanil and the times of administration were significantly lower than those of the control group(P<0.05); Anal exhaust time was significantly shorter than the control group(P<0.05); The incidence of adverse reactions was significantly lower than that of the control group(P<0.05).Conclusion By using dexmedetomidine on the base of pain pump can improve the analgesic effect postoperative,reduce the dosage of pain pump medication and the adverse reactions.
9.Primary cilium and its role in tumorigenesis.

The primary cilium, a sensory organelle that protrudes from the surface of most eukaryotic cells, receives and transduces various critical signals that are essential for normal development and homeostasis. Structural or functional disruption of primary cilia causes a number of human diseases, including cancer. Primary cilia has cross talks with cell cycle and it may act as a cell cycle checkpoint to suppress cancer development. Moreover, primary cilia has cross-regulation with autophagy, which may affect tumor progression. We then discuss the association of the primary cilia with several oncogenic signaling pathways, including Shh, Wnt, Notch and platelet-derived growth factor receptor (PDGFR). Since these signaling pathways are often over-activated in many types of human cancers, primary cilia are likely to play a role in the tumorigenesis by modulating these pathways. Finally, we summarize current progress on the role of cilia during tumorigenesis and the challenges that the cilia-cancer field faces.

10.The regulative effect of costimulatory molecules B7-H1 on immunosuppressive function of myeloid-derived suppressor cells

Objective To explore the expression of negative costimulatory molecule B7-H1 of myeloid derived suppressor cells(MDSCs) and its roles in age-related immunosuppressive functions.Methods 36 C57BL/6 mice were divided into 2 groups:the elderly group(12-month-old,n=18) and the young group(4-8-week-old,n=18).The expressive level of negative costimulatory molecules of MDSCs in the two groups was measured by flow cytometry(FCM).Besides,MDSCs were sorted by using MDSC isolation kit and T cells were obtained by grinding lymph nodes.To explore the influence of B7-H1 on the MDSCs' immunosuppressive functions,T cell proliferation and intervention assay was conducted using the five following groups:T cells group,T cells + CFSE group [T cells with carboxyfluorescein diacetate succinimidyl ester(CFSE)],T cells +-CFSE cocultured with MDSCs from the young group,T cells + CFSE cocultured with MDSCs from the elderly group,and T cells + CFSE cocultured with MDSCs from elderly group and anti-B7-H1 blocking antibody.The effect of B7-H1 on T cell proliferation in MDSCs were observed.Results The expression level of B7-H1 of the MDSCs was significantly increased in the elderly group as compared with the young group(t=3.27,P<0.05).T cell proliferation assay revealed that T cells were remarkably suppressed by MDSCs in elderly group as compared to the youth group(t=5.42,P<0.05).The suppression of T cells by MDSCs was dramatically reversed by anti-B7-H1 blocking antibody in elderly group(t=8.28,P<0.05).Conclusions B7-H1,a molecule expressed on the surface of MDSCs,plays a key role in the age-related immunosuppressive function of MDSCs.
