Cemental tear is a rare and indetectable condition unless obvious clinical signs present with the involvement of surrounding periodontal and periapical tissues. Due to its clinical manifestations similar to common dental issues, such as vertical root fracture, primary endodontic diseases, and periodontal diseases, as well as the low awareness of cemental tear for clinicians, misdiagnosis often occurs. The critical principle for cemental tear treatment is to remove torn fragments, and overlooking fragments leads to futile therapy, which could deteriorate the conditions of the affected teeth. Therefore, accurate diagnosis and subsequent appropriate interventions are vital for managing cemental tear. Novel diagnostic tools, including cone-beam computed tomography (CBCT), microscopes, and enamel matrix derivatives, have improved early detection and management, enhancing tooth retention. The implementation of standardized diagnostic criteria and treatment protocols, combined with improved clinical awareness among dental professionals, serves to mitigate risks of diagnostic errors and suboptimal therapeutic interventions. This expert consensus reviewed the epidemiology, pathogenesis, potential predisposing factors, clinical manifestations, diagnosis, differential diagnosis, treatment, and prognosis of cemental tear, aiming to provide a clinical guideline and facilitate clinicians to have a better understanding of cemental tear.

2.Application of multi-disciplinary treatment and internet visualization platform in clinical education of head and neck malignant tumor

Head and neck malignant tumor is one of the most heterogeneous diseases.The multi-disciplinary team(MDT)is an essen-tial component for personal precise diagnosis,treatment and integrated care management of oncologic diseases including head and neck malignant tumor.MDT clinical practice is also an important teaching mode for head and neck malignant tumors,but it is limited by time and space in actual teaching.An internet visualization platform was constructed based on the Internet,hospital HIS/PACS/LIS/EMR system,medical visualization screen,oral endoscope,remote consultation platform and other accessible audio and video terminals,and has been applied in MDT clinical teaching of head and neck malignant tumors,allowing medical students to participate in MDT through a networked visualization platform.Medical students will achieve deep learning for the most heterogeneous malignant tumor.MDT sup-ported by the internet visualization platform provides a new pathway for clinical medical education.
3.Construction of a droplet digital PCR detection system for platelet HPA-3 and HPA-15 genotyping

【Objective】 To establish a highly sensitive detection method of platelet HPA-3 and HPA-15 genotyping by droplet digital PCR (ddPCR), and to explore the feasibility of applying it to the detection of human platelet antigen (HPA) compatibility in maternal peripheral blood fetal free DNA. 【Methods】 For SNP mutation sites of HPA-3 and HPA-15, specific primers and MGB probes were designed, and amplification conditions such as annealing temperature and primer concentration of ddPCR were optimized to establish the optimal reaction system and clarify the test procedures. The methodological performance of the assay was evaluated, including specificity, sensitivity, repeatability and stability. ddPCR was used to detect 67 clinical blood samples, and the allele typing results were compared with the gene sequencing results. The fetal free DNA HPA antigen of 52 maternal peripheral blood samples was detected. 【Results】 The ddPCR method for detecting platelet HPA-3 and HPA-15 showed good specificity of primers and probes. The optimal annealing temperatures for HPA-3 and HPA-15 were 61.6℃ and 60.2℃, respectively. The optimal concentrations of primers were 900 nM and 700 nM respectively. The final concentration of the probe was 250 nM. The quantitative detection range of copy number was 2 to 20 000 copies, with lower limit of detection of 0.1 copies/μL, and the linearity is good. In low copy number samples, the intra - and inter batch coefficient of variation (CV) of actual detection values for HPA-3 and HPA-15 were both lower than 5%. The detection results of HPA-3 and HPA-15 genotypes of 67 blood samples were consistent with the gene sequencing results, and its application in fetomaternal platelet HPA-3, HPA-15 genotype detection met expectations. 【Conclusion】 The HPA-3 and HPA-15 ddPCR detection system constructed in this study has high accuracy, good repeatability, stability and sensitivity, and can be applied to the establishment of platelet HPA-3 and HPA-15 genotype donor pool, gene matching and fetomaternal platelet compatibility detection.
4.UGT1A1 gene mutation spectrum with indirect hyperbilirubinemia in children

Objective:To explore the relevancy between the uridine diphosphate-glucuronylgly-cosyltransferase 1A1 (UGT1A1) gene mutation and the phenotype of indirect hyperbilirubinemia in children.Methods:Sixteen cases with indirect hyperbilirubinemia who visited the Department of Gastroenterology, Children's Hospital of Nanjing Medical University from July 2013 to November 2019 were retrospectively analyzed and were divided into Gilbert syndrome (GS), Crigler-Najjar syndrome type II (CNS-II), and indirect hyperbilirubinemia groups unexplained by UGT1A1 gene mutations. The differences in gene mutation site information and general clinical data were compared. The association between gene mutation spectrum and bilirubin level was explored by t-test analysis.Results:Ten of the sixteen cases with indirect hyperbilirubinemia had GS, three had CNS-II, and three had indirect hyperbilirubinemia unexplained by UGT1A1 gene mutations. A total of six mutation types were detected, of which c.211G?>?A accounted for 37.5% (6/16), c.1456T?>?G accounted for 62.5% (10/16), and TATA accounted for 37.5% (6/16), respectively. Compared with the GS group, the CNS group had early disease onset incidence, high serum total bilirubin ( t ?=?5.539, P ?0.05), and indirect bilirubin ( t ?=?5.312, P ?0.05). However, there was no significant difference in direct bilirubin levels ( t ?=?1.223, P ?>?0.05) and age of onset ( t ?=?0.3611, P ?>?0.05) between the two groups. There was no significant correlation between the number of UGT1A1 gene mutations and serum bilirubin levels. Children with c.1456T?>?G homozygous mutations had the highest serum bilirubin levels. Conclusion:The common pathogenic variants of the UGT1A1 gene sequence are c.1456T?>?G, c.211G?>?A, and TATA, indicating that these site mutations are related to the occurrence of indirect hyperbilirubinemia and have important guiding significance for the etiological analysis of indirect hyperbilirubinemia in children.
5.Study on the application of sound thinking combined with Sandwich teaching method in oncology nursing teaching

Objective:To explore the application of sound thinking combined with Sandwich teaching in oncology nursing practice teaching.Methods:A total of 68 nursing students who were interns in the Department of Oncology, The First Affiliated Hospital of Air Force Medical University from 2020 to 2021 were included in the study, and they were divided into a control group ( n=34) and an observation group ( n=34). The control group took routine teaching for interns, while the observation group took sound thinking combined with Sandwich teaching. The examination results, critical thinking abilities, and the evaluation of nursing teaching effectiveness of the two groups of nursing interns were evaluated. SPSS 22.0 was used for Chi-square test and t-test. Results:The examination scores of nursing students in the observation group were higher than those in the control group ( t=3.44, 2.87, 3.45, P<0.05). Compared with those before training, the scores of critical thinking ability of nursing interns in both groups increased after the training, and the observation group was better than the control group ( t=0.180, 3.64, 0.61, 2.92, 0.31, 2.74, 0.45, 2.65, 0.25, 3.58, 1.16, 2.85, 0.36, 3.20, 0.33, 2.38, P<0.05). The scores of autonomous learning ability, communication and collaboration ability, independent thinking ability, clinical reasoning ability, and problem-analyzing and -solving ability in the observation group were higher than those in the control group ( t=2.82, 3.46, 2.68, 3.29, 2.44, P<0.05). Conclusion:Combining sound thinking with Sandwich teaching in nursing clinical practice teaching in department of oncology can improve the examination scores of nursing students, improve their critical thinking abilities, and enable them to give a high evaluation of nursing teaching effectiveness.
6.Study on the tissue distribution and pharmacokinetics of PELGE-crebanine nanoparticles in rats

OBJECTIVE To study the tissue distribution and pharmacokinetic characteristics of polyethylene glycol-(polylactic acid-hydroxyacetic acid) -polyethylene glycol triblock copolymer (PELGE) -crebanine nanoparticles (PELGE-Cre-NPS) in rats. METHODS The SD rats were divided into 9 groups (1 group at each time point), with 6 rats in each group,half male and half female. After PELGE-Cre-NPs(5 mg/kg) was injected into tail vein of rats, appropriate amounts of heart, liver, spleen, lung, kidney and brain tissues were taken at 5, 15, 30, 60, 90, 120, 180, 240 and 300 min, respectively. With verapamil hydrochloride as internal standard, the content of crebanine (Cre) in each tissue was determined by HPLC, and the main pharmacokinetic parameters such as area under the drug-time curve (AUC0-)t and mean retention time (MRT0-)t were calculated.RESULTS At 5-90 min after medication, the content of Cre in each tissue of rats from large to small was lung, kidney, spleen, liver, brain and heart; at 120-300 min after medication, the sequence was lung, spleen, kidney, liver, brain and heart. AUC0-t of Cre in heart, liver, spleen, lung, kidney and brain were (18.86±1.66), (43.36±4.99), (51.36±5.34), (81.86±12.34), (53.31±3.19) and (27.73±4.76) mg·h/L, respectively. MRT0-t of Cre were (1.94±0.12), (1.97±1.02), (1.98±1.23), (1.89±0.21), (1.88± 0.06), (1.85±0.19) h, respectively. CONCLUSIONS PELGE-Cre-NPs mainly distribute in lung tissue, but less in heart tissue, and the elimination of PELGE-Cre-NPs in heart, lung and liver tissue is slow.
7.Discussion on economic value of pharmaceutical care in medical institutions based on cost-benefit analysis

OBJECTIVE To evalu ate relat ed researches about the cost- benefit of pharmaceutical care in medical institutions with cost- benefit analysis,in order to provide evidence-based basis for related policy decisions and provide methodological reference for the cost- benefit evaluation of pharmaceutical care in the future. METHODS Retrieved from PubMed ,Embase,the Cochrane Library ,CBM,Wanfang database ,VIP and CNKI ,cost-benefit analysis was used to evaluate the researches about the cost-benefit of pharmaceutical care in medical institutions. Two researchers independently screened the research and extracted data according to the “Consolidated Health Economic Evaluation Reporting Standards Checklist ”. The quality of included studies was scored and evaluated systematically. RESULTS A total of 46 studies from 17 countries were included. Most of them came from the United States (21.74%),China(19.57%)and France (8.70%). Average score of 46 literature was 14.30,of which 1 literature was excellent ,5 literature were good ,25 literature were qualified and 14 literature were unqualified. There were 25 research protocols of prospective study type ;the type of pharmaceutical care involved mostly was pharmaceutical monitoring (60.87%), followed by prescription review (23.91%),medication reconciliation (8.70%)and outpatient pharmacy (6.52%)were less. The median cost-benefit ratio of pharmaceutical care was 5.05 (3.08,11.28). CONCLUSIONS Pharmaceutical care shows good economic value ,and pharmacists have played an important role in saving medical resources ,but the design level and report quality of the existing studies need to be improved.
8.Content Determination of Lupanone and Stigmasterol in Different Parts of Musa basjoo by UPLC

OBJECTIVE:To esta blish a method for determining the contents of lupenone and stigmasterol in the rhizome ,stem and leaf of Mosa basjoo from the same plant ,and to provide reference for the substitute resource for the effective components of M. basjoo . METHODS :UPLC method was adopted. The determination was performed on Zorbax Rrhd Eclipse Plus C 18 column (100 mm×2.1 mm,1.8 μm)with mobile phase consisted of acetonitrile-methanol (78.5∶21.5,V/V). The detection wavelength was set at 210 nm;the flow rate was 0.15 mL/min;the column temperature was 30 ℃ and the sample size was 1 μL. The results of content determination of lupinone and stigmasterol in the rhizome ,stem and leaf of 9 batches of M. basjoo from the same plant were analyzed by the methods of comparative analysis between groups ,principal component analysis and cluster analysis. RESULTS:The mass concentration of lupenone and stigmasterol had a good linear relationship with the corresponding peak area within the range of 11.16-357.10 and 8.83-160.40 g/mL(R2 were 0.999 2 and 0.999 1,respectively). RSDs of precision , repeatability and stability tests were all less than 3%. The average recovery rates of lupenone and stigmasterol were 101.44% and 98.32%,and the RSDs were 1.77% and 1.81%(n=6),respectively. The average contents of lupenone and stigmasterol in stems of M. Basjoo were significantly higher than those of rhizome and leaves of M. basjoo (P<0.05). There was no statistical significance in the contents of lupenone and stigmasterol between stem and leaf of M. basjoo from same plant (P>0.05). Results of principal component analysis showed that the contents of lupanone and stigmasterol were different in rhizome ,stem and leaf of M. basjoo from the same plant. Rhizome ,stem and leaf of M. basjoo were divided into three types through cluster analysis ,among which the rhizome had significant difference with the other two parts. CONCLUSIONS :The method is simple ,rapid,specific, reproducible and accurate. It can be used for the content determination of lupenone and stigmasterol in different parts of M. basjoo . The stem of M. basjoo can replace the rhizome of M. basjoo as the source of lupinone and stigmasterol.
9.Biosynthesis of spherical selenium nanoparticles with halophilic Bacillus subtilis subspecies stercoris strain XP for inhibition of strawberry pathogens.

Biosynthesis of nanomaterials has attracted much attention for its excellent characteristics such as low energy consumption, high safety, and environmental friendliness. As we all know, the toxic selenite can be transformed into higher-value nanomaterials by using bacteria. In this study, nano-selenium was synthesized by halophilic Bacillus subtilis subspecies stercoris strain XP in LB medium supplemented with selenite (electron acceptor). The physicochemical characteristics of nano-selenium were analyzed by scanning electron microscope (SEM), X-ray energy dispersive spectral analysis (EDAX), X-ray diffraction (XRD), and fourier transform infrared spectroscopy (FTIR). Meanwhile, the antifungal activity of nano-selenium to strawberry pathogens (fusarium wilt, erythema, and purple spot fungi) was determined. The products from reduction of selenite by strain XP was amorphous spherical selenium nanoparticles (SeNPs) with a diameter range of 135-165 nm. The production of SeNPs was positively correlated with time (0-48 h) and no changes were observed on cell morphology. Selenium was dominant in the surface of SeNPs where the organic elements (C, O, N, and S) existed at the same time. SeNPs were coated with biomolecules containing functional groups (such as -OH, C=O, N-H, and C-H) which were associated with the stability and bioactivity of particles. Although the highest concentration of SeNPs had significant (P<0.05) inhibitory effects on three strains of strawberry pathogens, antifungal activity to erythema and fusarium wilt pathogenic fungi was higher than that to purple spot pathogenic fungi from strawberry. In conclusion, strain XP not only has strong tolerance to high salt stress, but can be also used to synthesize biological SeNPs with good stability and biological activity. Thus, the strain XP has bright perspectives and great potential advantage in pathogens control and green selenium-rich strawberry planting as well as other fields.

10.Development of the Chronic Disease Management Ability Self-assessment Questionnaire for Community Nurses and its reliability and validity

Objective:To develop the Chronic Disease Management Ability Self-assessment Questionnaire for Community Nurses, and test its reliability and validity, so as to provide an evaluation tool for assessing community nurses ' chronic disease management ability. Methods:Guided by the competency theory, the first draft of the Chronic Disease Management Ability Self-assessment Questionnaire for Community Nurses was formed through literature review, semi-structured interviews, Delphi expert consultation and pre-experiment. From July to August 2019, 30 community nurses from a community health service center in Zhengzhou were selected by convenience sampling method for pre-survey and the project analysis. From September to November 2019, the convenience sampling method was used to select 415 community nurses from 60 community health service centers in Zhengzhou were selected for a formal survey, and the reliability and validity of the questionnaire were evaluated.Results:Exploratory factor analysis extracted 4 common factors, and the cumulative variance contribution rate was 61.040%. The content validity of each item of the questionnaire was from 0.800 to 1.000, and the overall level content validity of the questionnaire was 0.980. The Cronbach 's α coefficient was 0.964, and the Cronbach 's α coefficient of each dimension was from 0.800 to 0.958. The test-retest reliability of the questionnaire was 0.864, and the test-retest reliability of each dimension was from 0.908 to 0.986. The final version of the questionnaire contained 4 dimensions of professional knowledge, professional skills, professional abilities, and personal characteristics, with a total of 44 items. Conclusions:The Chronic Disease Management Ability Self-assessment Questionnaire for Community Nurses has good reliability and validity, and can be used as an assessment tool for community nurses ' chronic disease management ability.
