Juvenile systemic sclerosis (SSc) is a chronic connective tissue disease characterized by symmetrical thickening and hardening of the skin,associated with fibrous changes in internal organs.In the last years,several therapies have showed promising beneficial effects in the treatment of SSc.We summarized the recent advances in the anti-inflammation and immunosuppressive therapy,treatment of SSc related vascular diseases,anti-fibrosis therapy as well as prospective treatments.
2.Effects of endovascular cooling in different degrees on severe traumatic brain injury in dogs

Objective To investigate the effects of endovascular cooling in different degrees on severe traumatic brain injury in dogs.Methods Eighteen dogs of both sexes,weighing 12-15 kg,were randomly divided into 3 groups ( n =6 each):hypothermia at 31 ℃ group (group A),hypothermia at 35 ℃ group (group B) and normothermia group (group C).The model of severe traumatic brain injury was established according to the improved Feeney's free-fall method.Six hours of endovascular cooling was performed using an endovascular cooling system in A and B groups.The cerebrospinal fluid (CSF) was collected before traumatic brain injury (T0),and at 24,48 and 72 h after traumatic brain injury (T1-3) for measurement of the concentrations of neuron-specific enolase (NSE),S-100β,myelin basic protein (MBP),aspartate (Asp),glutamie acid (Glu),glycine ( G ly) and gammaaminobutyric acid (GABA).Results The concentrations of NSE,S-100β,MBP,Asp,Glu and Gly in CSF were significantly lower at T1-3,while the concentration of GABA in CSF was significantly higher at T1-3 in groups A and B than in group C (P <0.05 or 0.01).The concentrations of NSE,Asp,Glu and Gly in CSF were significantly lower at T1-3,while the concentration of GABA in CSF was significantly higher at T1-3 in group B than in group A ( P <0.05 or 0.01).Conclusion Endovascular cooling can reduce severe traumatic brain injury in dogs,and the efficacy is better when the temperature is reduced to 35 ℃ than that when reduced to 31 ℃.
3.Clinical analysis of 9 cases of neonatal septicemia caused by Lester bacteria

Objective To explore the clinical features of neonatal septicemia caused by Lester. Methods The clinical features of septicemia caused by Lester bacteria in 9 neonates confirmed by blood culture during from January 2011 to June 2016 were retrospectively analyzed. Results All of the 9 cases were premature and cesarean delivery. The main clinical manifestations were cyanosis (7 cases), fever (5 cases), anhelation (4 cases), hypomyotonia (4 cases), and respiratory distress (3 cases). Blood cultures in 9 cases were detected Lester bacteria with 3 cases found in sputum and 1 case in both sputum and cerebrospinal fluid. Drug sensitivity test showed that Lester bacteria were sensitive to ampicillin, penicillin, and sulfamethoxazole. All of the 9 cases adjusted anti-infective medications after the diagnosis of Lester septicemia by blood culture, and all of them were cured or improved, and discharged. Conclusion Neonatal Lester infection is a serious infectious disease. Fever in pregnant women should alert clinicians to Lester infection. Early detection of pathogens and targeted treatment can help improve prognosis.
4.Association of bacterial translocation with cachexia and its influence on the outcome of gastric cancer patients

ObjectiveTo investigate the association of bacterial translocation (BT) with cachexia and its impact on the outcome of gastric cancer patients.MethodsSixty cachectic gastric cancer patients,50 age- and sex-matched non-cachectic gastric cancer patients,and 55 healthy controls were enrolled in this study between January 2008 and July 2009.Polymerase chain reaction was performed to detect bacterial DNA in the peripheral blood of cancer patients and healthy controls,Cytokine levels were tested by enzyme-linked immunosorbent assay.Flow cytometry was used to detect immune indicators.All the enrolled patients were followed up for two years,and the two-year survival rate was calculated.ResultsThe BT ratio was significantly higher in cachectic patients than in non-cachectic patients (25.0% vs.8.0%,P =0.019) and healthy controls (25.0% vs.0.0%,P =0.000).BT-positive cachectic patients had significantly higher levels of IL-1α,IL-6,TNF-α,and IFN-γ compared with BT-negative cachectic patients ( P =0.012,0.003,0.036,and 0.017,respectively ) and BT-positive non-cachectic patients ( P =0.011,0.034,0.000,and 0.022,respectively).The two-year survival rate in BT-positive cachectic patients was significantly lower than in BT-negative cachectic patients (P =0.023 ).The levels of CD3 +T,CD4+ T,natural killer cells,and CD4 + T/CD8 + T in gastric cancer patients were significantly lower than in healthy controls ( P =0.023,0.031,0.016,0.041,respectively),whereas CD8 + T level was significantly higher in gastric cancer patients than in healthy controls (P =0.038).ConclusionBT may contribute to the development of cancer cachexia and influence the long-term survival of locally advanced gastric cancer patients.
5.A new technique of periorbital rhytidectomy with musculus dormitator suspension

Objective To study one simple, safe and effective method of periorbital rhytidectomy. Methods With turgescent anesthesia, 1 cm -extent was dissected laterally deep under musculus orbicularis oculi at the approach of eyebrow area. The musculus orbicularis oculi was fixed to the deep temporal fascia and frontal periosteum. The superciliary corrugator muscle and depressor muscle were exposured and cut. Then the eyelid blepharoalasty was performed through the skin incision, meanwhile the lateral pars orbitalis muscle was liberated to form an orbitalis muscle flap under the orbitalis muscle and the fat pad, which was fixed to the os orbitale periost by lifting tight to lateral and upper side. For patients with generous pars buccalis and obvious nose lip ditch, the cheekbone fat cushion was also hanged and fixed. Results 65 middle age female patients were followed up for 3~16 months. The scar was no obvious, glabellar wrinkle and canthal wrinkle disappeared at static state, relaxed upper eyelid and eyebrow prolapse reached to complete correct, nasolabial groove wrinkle obvious lessened. 95 % patients were satisfacted with the appearance. Conjunctival edema was found in 4 cases, and disappeared within 1 months. There were no facial nerve injury and other complications. Conclusions It is a new incisional approach that avoids the facial nerves and lessens fossa orbitalis retrogradation. The operative procedure is simple and safe, and its incision is occult. The wound is small and restores rapidly, but the effect is reliable and lasting. It proves to be an ideal approach for face rejuvenation.
6.Intervention effect of proteasome inhibitor MG132 in rats with collagen-induced arthritis

Objective:To explore the intervention effect of proteasome inhibitor MG132 in rats with collagen-induced arthritis(CIA),which resembles human rheumatoid arthritis(RA).Methods:Forty-eight female SD rats were randomly divided into three groups,including blank control group,CIA model group and MG132-treated group.There were sixteen rats in each group.Rats in CIA model group and MG132-treated model group were injected with type Ⅱ collagen to established CIA rats.21 days after the initial immunization,the rats in the MG132-treated model group were injected subcutaneously with 1 mg/kg MG132 once daily for 2 weeks.42 days after the initial immunization,the change of paw-swelling and the arthritis scores were determined.The synovial pathology examination was performed with HE staining.The 20S proteasome activity in synovial tissue was measured by fluorescence substrate assay.The expression of NF-κB/p65,IκBα in synovial tissue were analyzed by Western blot.Results:Proteasome inhibitor MG132 significantly attenuated the severity of arthritis and histopathological changes in CIA rats.Compared with the blank control group,the 20S proteasome activity was increased significantly in the CIA model group(P<0.05),and decreased after injection of MG132.Compared with CIA rats,the expression of NF-κB/p65 significantly decreased in rats treated with MG132(P<0.01).Compared with the blank control group,the expression of IκBα protein decreased in CIA model group.After injected with MG132,the protein was significantly increased(P<0.01).Conclusion:The proteasome inhibitor MG132 may attenuates the severity of arthritis and histopathological changes in CIA rats.These effects may be mediated through the inhibition of NF-κB activity.
7.Analysis of the group A rotavirus infection in children with diarrhea in Shenzhen

Objective To investigate the epidemiological characteristics of the group A rotavirus (RV) among the children with diarrhea in Shenzhen ,in order to provide reference for clinical diagnosis and treatment .Methods Stool samples were collected for RV detection from children with diarrhea .The results were analyzed .Results A total of 3 509 cases of children with RV infection were checked out from 14 511 cases of children with diarrhea ,with the infection rate accounting for 24 .18% .RV infection occurred all year around ,and the infection peak was November ,December and January .RV infected children mainly distributed in the age group of 6 months to 2 years .Conclusion RV infection in children is a universal problem in Shenzhen .Clinic should pay attention to it .

Objective:To study the inhibitory effect of allogeneic natural killer(NK) cells on subcutaneously transplanted human multi-drug resistant nasopharyngeal carcinoma cells(CNE2/DDP) in BALB/c nude mice.Methods:Human leucocyte antigen(HLA) genotypes of CNE2/DDP cells and the genotypes of inhibitory killer cell immunoglobulin-like receptor(KIR) in NK cells(isolated from 3 healthy persons by immuno-magnetic microbead technique) were analyzed by PCR-SSP.Twelve BALB/c nude mice were evenly divided into 2 groups:the control group and the treatment group.Mice in the treatment group were injected subcutaneously with 1?106 CNE2/DDP cells together with 3?107 NK cells via the tail veins;mice in the control group were injected with 1?106 CNE2/DDP cells subcutaneously.The tumor formation time,tumor formation rate and changes of tumor size were observed.Three weeks after tumor formation,all the mice were killed and human NK cells in peripheral blood were analyzed by flow cytometry;the tumors were weighed and the tumor inhibitory rates were calculated.Results:The HLA genotypes of CNE2/DDPcells were A2,24,B18,35,Cw4,and 7;the KIR genotypes of the 3 healthy persons were KIR2DL1,KIR2DL3,KIR3DL1,and KIR3DL2.There were mismatches between the KIRs expressed in NK cells and HLA class Ⅰ molecules expressed in the CNE2/DDP cells.NK cells obviously inhibited the growth of CNE2/DDP xenograft in nude mice.The tumor formation periods of control group and NK cell group were(17.17?1.17) d and(24.83?1.47) d,respectively(P

Objective To investigate the effect of remote ischemic preconditioning (RIPC) combined with dexmedetomidine on lung injury during perioperative period and postoperative pulmonary complications in elderly patients with thoracotomy and pulmonary dysfunction.Methods Sixty ASA Ⅱ or Ⅲ patients aged 65-76 years [mean (70.4±6.3) years],weighing 50-75 kg,with moderate and severe pulmonary dysfunction,who were scheduled for elective radical operation for esophageal cancer,were randomly divided into 3 groups (n=20,each) by using a random number table:the control group (group C),RIPC group (group OR) and RIPC plus dexmedetomidine group (group ORD).At 10 min after endotracheal intubation,group ORD was induced by three cycles of 5 min of lower extremity ischemia followed by 5 min of reperfusion,at the same time a loading dose of dexmedetomidine 0.5 μig / kg was infused iv over 15 min,and then was infused at a rate of 0.5 μg · kg-1 · h-1 until the end of operation.Group OR was induced by three cycles of 5 min of lower extremity ischemia followed by 5min of reperfusion without dexmedetomidine.Group C received only the equal volume of normal saline.Blood samples were obtained from radial artery immediately before anesthesia induction (T0),before one lung ventilation (T1),at 1 h after one lung ventilation (T2),the end of surgery (T3) and 24 h after operation (T4).Blood gas analysis was done at T1,T2,and T3.Plasma superoxide dismutase (SOD) activity and concentration of malonyldialdehyde (MDA),matrix metallo-proteinase-9 (MMP-9),interleukin-6(IL-6) and white blood cell (WBC) and neutrophil granulocyte (PMN) counts were measured at T0,T2,T3 and T4.The complications including pulmonary infection and atelectasis were recorded at 1,3 and 7 days after operation.Results At T2-3,PaO2 was higher in groups of OR and ORD than in group C Group ORD had higher PaO2 than did group OR [(265±15) mmHg,(305±23) mmHg vs.(231±17) mmHg,(273±21)mmHg,(312±24) mmHg vs.(242±18) mmHg,F=34.791 and 31.813,P<0.01].At T2-3,RI was lower in groups of OR and ORD than in group C,and group ORD had lower RI than did group OR [(1.48±0.16),(1.14 ±0.14) vs.(1.86±0.18);(1.35±0.13),(0.96±0.09) vs.(1.73±0.15),F=119.260 and 164.855,P<0.01].At T3-4,SOD activity was higher in group OR and ORD than in group C,and group ORD had higher SOD activity than did group OR [(83.6 ± 7.8) U/mg prot,(97.6± 9.5) U/mgprot vs.(70.5±7.4) U/mgprot;(73.5 ± 6.3) U/mgprot,(87.7 ± 8.9) U/mgprot vs.(61.6 ± 5.4) U/ mgprot,F=94.540 and 90.839,P<0.01].At T3-4.plasma concentration of MDA,MMP-9,WBC and PMN counts were lower in group OR and ORD than in group C,and the above indices were lower in group ORD than in group OR [(7.5 ± 1.4) nmol/mgprot,(5.8 ± 1.0) nmol/mgprot vs.(9.5 ±1.5) nmol/mgprot;(8.2± 1.5) nmol/mgprot,(6.5 ± 1.0) nmol/mgprot vs.(10.1 ±1.6) nmol/mgprot;(205±23) μg/L,(173±21) μg/L vs.(237±26) μg/L,(179±16) μg/L,(158±12) μg/L vs.(203± 20) μg/L;(8.0±0.5) ×109/L,(7.2±0.6) × 109/L vs.(9.2±0.8)×109/L;(9.4±0.7) ×109/L,(8.2±0.6)×109/Lvs.(11.2±0.8) ×109/L;(7.4±0.7) ×109/L,(6.5±0.5) ×109/Lvs.(8.3 ±0.8) ×109/L,(7.8±0.8) ×109/L,(6.7±0.6) ×109/L vs.(9.2±0.9) ×109/L,F=98.872,52.723;198.307,47.622,20.319,36.935,18.197,35.036,respectively,all P<0.01].At T2-4,IL-6 level was lower in groups of OR and ORD than in group C,and group ORD had lower IL-6 level than did group OR [(105±14) ng/L,(86±12) ng/L vs.(127±18) ng/L;(125±19) ng/L,(101±16) ng/ L vs.(156±22) ng/L;(110±16) ng/L,(89±12) ng/L vs.(132±20) ng/L,F=85.449,139.848,124.129,respectively,P<0.01].The incidences of postoperative pulmonary infection and atelectasis were lower in group OR and ORD than group C,and group ORD had lower incidences of postoperative pulmonary infection and atelectasis than did group OR (x2 =6.303 and 14.545,P < 0.05).Conclusions RIPC combined with dexmedetomidine can improve pulmonary function in elderly patients with thoracotomy and pulmonary dysfunction,may relieve the inflammatory reaction and oxidation reaction during perioperative period and finally help to improve the prognosis of patients.
