Cemental tear is a rare and indetectable condition unless obvious clinical signs present with the involvement of surrounding periodontal and periapical tissues. Due to its clinical manifestations similar to common dental issues, such as vertical root fracture, primary endodontic diseases, and periodontal diseases, as well as the low awareness of cemental tear for clinicians, misdiagnosis often occurs. The critical principle for cemental tear treatment is to remove torn fragments, and overlooking fragments leads to futile therapy, which could deteriorate the conditions of the affected teeth. Therefore, accurate diagnosis and subsequent appropriate interventions are vital for managing cemental tear. Novel diagnostic tools, including cone-beam computed tomography (CBCT), microscopes, and enamel matrix derivatives, have improved early detection and management, enhancing tooth retention. The implementation of standardized diagnostic criteria and treatment protocols, combined with improved clinical awareness among dental professionals, serves to mitigate risks of diagnostic errors and suboptimal therapeutic interventions. This expert consensus reviewed the epidemiology, pathogenesis, potential predisposing factors, clinical manifestations, diagnosis, differential diagnosis, treatment, and prognosis of cemental tear, aiming to provide a clinical guideline and facilitate clinicians to have a better understanding of cemental tear.

2.Immune cell-mediated causal relationship between plasma metabolites and knee osteoarthritis:a Mendelian randomization study

Objective To evaluate both the causal relationship between plasma metabolites and the risk of knee osteoarthritis(KOA)and the potential mediating or masking effect of immune cells using Mendelian randomization(MR)systems.Methods The GWAS data on 1400 plasma metabolites,731 immune cell traits and KOA was retrieved from the genome-wide association study(GWAS)database.Two-way MR analysis was used to evaluate the causal relationship between plasma metabolism and KOA.Two-step mediation MR analysis was conducted to evaluate immune cell traits that might have mediating or masking effects.Results After sensitivity analysis and screening,65 plasma metabolites and 35 immune cell traits were found to have causal relationships with KOA(P<0.05).Mediation analysis found that CD45RA+CD28-CD8br%CD8br had a mediating effect in the causal relationship between three metabolites(2-hydroxyhi-ppurate,X-07765,X-23739)and the risk of KOA.2-hydroxyhippurate(salicylic acid)exerted a masking effect,and the effect ratio was 0.0412.Conclusion A variety of plasma metabolites and immune cell traits are causally related to KOA,which should not be regarded as a simple degenerative joint disease.The protective effect of salicylic acid against KOA may be weakened by its role in inducing the differentiation of Treg cells,which is worthy of more studies.
3.Analysis of cases of reinfection of past SARS-CoV-2 patients in Pudong New Area of Shanghai

ObjectiveTo identify the rate, population characteristics, and vaccination history of repeat infections among previously infected people in the current epidemic based on the rate of repeat infection and population characteristics of different mutant strains at different times in Pudong New Area of Shanghai, and to provide reference for the prevention and control strategies of novel coronavirus repeat infections. MethodsA total of 9 250 investigated subjects were randomly selected from the new cases of asymptomatic infection and confirmed cases reported by Pudong New Area from March to May 2022. The investigation mainly focused on demographic characteristics, nucleic acid or antigen test results, and symptoms after infection. The repeat infection rates among different populations were compared, and logistic regression was used to analyze the impact of gender, age, and vaccination status on repeat infections. ResultsThe survey sample of 9 250 people had a response rate of 81.85%. There were 4 043 males (53.40%) and 3 528 females (46.60%), with a median age of 34 years old (P25, P75: 7, 61). The overall vaccine uptake rate was 59.44% (4 500/7 571). In December of 2022, there were 563 cases of repeat infection, with an infection rate of 7.44%. The lowest rate of repeat infection was seen in the 3‒ year-old group (2.86%) and the highest rate in the 30‒ year-old group (12.42%), with significant differences between different age groups. The repeated infection rate for those who had completed their vaccinations was significantly lower (6.57%) compared to those who had not (7.11%). The age groups of 3‒ years, 70‒79 years, as well as individuals who completed full vaccination and received booster shots were protective factors against repeat infections. ConclusionThe overall rate of reinfection among the infected in Shanghai during the spring of 2022 was low in the outbreak of the Omicron variant, and the rate of reinfection in the 3‒ year-old group was significantly lower than in other age groups. Completing the full course of vaccination significantly reduces the risk of reinfection. Although the reinfection rate is high in individuals who received booster shots, it remains a mitigating factor compared to those who do not receive the vaccine. It is recommended to continue monitoring reinfections in key populations and further strengthen immunization efforts.
4.Factors influencing vicarious posttraumatic growth of emergency nurses based on structural equation modeling

Objective:To explore the current status of vicarious posttraumatic growth (VPTG) among emergency nurses, and analyze its influencing factors and pathways of action.Methods:From March to May 2022, convenience sampling was used to select 532 emergency nurses from 20 public general hospitals in Anhui Province as the research subject. A survey was conducted on emergency nurses using the General Information Questionnaire, Posttraumatic Growth Inventory, Chinese Version of the Interpersonal Reactivity Index, Connor-Davidson Resilience Scale, and the Multidimensional Scale of Perceived Social Support. AMOS 24.0 software was used to establish the structural equation model and analyze the path relationships between variables.Results:A total of 532 questionnaires were distributed, and 480 valid questionnaires were collected, with an effective response rate of 90.23% (480/532). The score of the Posttraumatic Growth Inventory for 480 emergency nurses was (49.35±15.82). Empathy, psychological resilience, and perceived social support all had a positive impact on VPTG, with total effect values of 0.503, 0.261, and 0.163, respectively. Empathy directly or indirectly affected VPTG through perceived social support and psychological resilience. Perceived social support indirectly affected VPTG through psychological resilience, and psychological resilience directly affected VPTG.Conclusions:VPTG of emergency nurses is directly and/or indirectly positively influenced by empathy, psychological resilience, and perceived social support. Nursing managers should pay attention to the interaction between factors, take effective measures based on the association between factors, and promote the positive transformation of emergency nurses after trauma.
5.Efficacy and mechanism of Jieyu Fuwei Powder in treating functional dyspepsia based on brain-gut interaction viewpoint

Objective To observe the efficacy of Jieyu Fuwei Powder in the treatment of patients with functional dyspepsia (FD). Methods A total of 66 patients with chronic FD (type of disharmony between liver and stomach) from the outpatient clinic of Hai'an Hospital of Traditional Chinese Medicine were randomly divided into two groups, with 33 cases in each group. Control group was treated with domperidone, while treatment group was treated with Jieyu Fuwei Powder. The total effective rate, scores of traditional Chinese medicine syndromes, levels of serum 5-hydroxytryptamine (5-HT), substance P (SP), gastrin (GAS) and motilin (MTL) as well as the incidence of adverse reactions were compared between the two groups. Volunteers from the two groups after treatment (14 cases in the control group and 13 cases in the treatment group) were recruited to receive outpatient gastroscopy and obtain biopsy tissues for histopathological and immunohistochemical analysis of GAS and MTL. Results The total effective rate in the treatment group was 90.91%, which was significantly higher than 69.70% in the control group (
6.Latent classes and influencing factors of vicarious posttraumatic growth among emergency nurses

Objective:To investigate the latent classes and influencing factors of vicarious posttraumatic growth among emergency nurses.Methods:Convenience sampling method was used to select the 498 emergency nurses of 20 public general hospitals in Anhui province as the survey objects. The general information questionnaire and Posttraumatic Growth Inventory were used to investigate. Latent class analysis was used to classify vicarious posttraumatic growth of emergency nurses, and the influencing factors of latent classes were analyzed by binary logistic regression.Results:There were two latent classes of vicarious posttraumatic growth of emergency nurses including high growth-high fluctuation group (24.1%, 120/498) and low growth group (75.9%, 378/498). Binary logistic regression analysis showed that professional title, average monthly income and self-rated health status were the factors influencing the latent classes (all P<0.05). Conclusions:There are obvious classification characteristics of vicarious posttraumatic growth among emergency nurses. Nursing managers should formulate and implement targeted intervention measures according to the classification characteristics and influencing factors to improve their vicarious posttraumatic growth level.
7.Current Status and Regulatory Considerations of Drug-led Combination Products

Abstract
This paper focuses on the regulatory strategy of drug-led drug device combination products, comparing and analyzing the definition, scope and management mode of drug device combination products in the United States, the European Union, Japan, Canada and China; summarizing and analyzing the types of combination products in the Product-specific Guidances for Generic Drug Development(PSGs) of the US FDA's scientific and technical reports in the past five years, the results show that the top three PSGs newly added to the FDA were transdermal system, nasal sprays, and single-dose prefilled syringes/pens(including auto-injectors).
As of March 1, 2023, the National Medical Products Administration(NMPA) had cumulatively announced the results of 339 combination products applied for attribute definition by enterprises, of which 88 were "drug-led drug device combination products", accounting for 26%; and 78 were "device-led drug device combination products", accounting for 23%; the results of "not belonging to drug device combination products" accounted for more than half (51.0%), which indicated that there was a big difference between the industry and the regulatory understanding of the definition of drug-device combination products, and that the existing guideline and documents were unable to provide clear and predictable positioning of the combination products under research and development for the time being.
This paper also puts forward suggestions for solving the problems in the development of drug-device combination products in China, in order to improve the consistency, predictability and transparency of the classification and definition of drug-device combination products in China, and to provide references for the development and scientific supervision of this kind of products.
8.Application of medical sterile gloves-assisted cutting cloth sample in repairing finger degloving injury

Objective:To investigate the application effect of medical sterile gloves-assisted cutting cloth samples in the repair of finger degloving injury.Methods:The clinical data of patients with finger skin and soft tissue degloving defects treated in Jiangnan Hospital Affiliated with Binjiang College of Zhejiang University of Traditional Chinese Medicine from April 2015 to December 2021 were analyzed retrospectively. Medical sterile gloves were used to make cloth samples of finger defects, and then the dorsal foot flap or toenail flap was cut according to the cloth samples to repair finger defects. The survival of flaps and the appearance of the fingers’ body were observed after surgery, and the patients’ satisfaction was investigated. The finger function was evaluated by the trial standard of upper limb function evaluation of the Hand Surgery Society of Chinese Medical Association.Results:A total of 12 patients with 13 fingers were enrolled. There were 9 males (10 fingers) and 3 females (3 fingers), aged 17-54, average of 37-year-old. There were 5 cases (5 fingers) in the left hand and 7 cases (8 fingers) in the right hand, all with unilateral finger injury. The size of the dorsal flaps and great toenail flaps harvested were appropriate, ranging from 5.5 cm×4.5 cm to 10.5 cm×5.8 cm. After surgery, all patients were regularly followed up for 3-15 months, an average of 7 months. All the dorsal flap and great toenail flap of the foot after surgery survived, and the appearance of the finger body was not bloated. All patients were very satisfied with the appearance and function of the hand, and the finger function was evaluated as excellent in 7 cases and good in 5 cases.Conclusion:The application of sterile gloves-assisted cutting cloth samples for the treatment of finger degloving injury can simplify the procedure, reduce donor damage, make up for the lack as much as needed, and the repaired fingers can obtain better shape and function.

10.Summary of the best evidence for environment management to prevent delirium in ICU patients

Objective:To summarize the relevant evidence of environmental management for preventing delirium in ICU patients, so as to provide references for delirium management in ICU patients.Methods:Evidence-based questions were constructed according to the PIPOST model, and relevant clinical decisions, guidelines, expert consensus, systematic reviews and evidence summaries published by various guideline websites, evidence-based databases and comprehensive databases at home and abroad were systematically searched according to the evidence resource "6S" model. The search period was from the establishment of databases to September 3, 2022. Two researchers independently conducted literature quality evaluation using corresponding literature quality evaluation tools. The Joanna Briggs Institute (JBI) Evidence Pre-grading and Evidence Recommendation Grading System (2014 Edition) was used to grade and recommend included evidence.Results:A total of 30 articles were included, including 5 guidelines, 2 expert consensus, 1 clinical recommended practice, 4 evidence summaries and 18 systematic reviews. A total of 36 best evidences were summarized from 5 aspects, including environment optimization, sleep promotion, cognitive stimulation, positioning and orientation function, and safety protection.Conclusions:Medical staff should manage the ICU environment based on evidence-based evidence to prevent or reduce the occurrence of delirium in patients.
