2.The changes of intestinal flora and the effect of the antibiotics and micro-ecological agent in the elderly diabetes

Objective:To investigate the changes of intestinal flora and the effects of antibiotics and micro-ecological agent on the intestinal flora in the elderly diabetes.Methods:Intestinal flora detection by Stool cultures,compared 55 diabetes with 184 non-diabetes.Those who need antibiotics were randomly divided into two groups:one group used micro-ecological agent(bacillus licheniformis capsules) and the other not.Their fecal were collected to detect intestinal flora before antibiotic used and at the 5th、10th and 15th day of antibiotic used.Results:Compared with non-diabetes,the quantity of Escherichia coli significantly increased(9.53?1.76 vs 9.14?1.29,P
3.Factors affecting the development of genetic testing service by using quantitative SWOT analysis

Objective To analyze the factors affecting the development of genome technology-based genetic testing services. Methods Purposive sampling was used to select experts in the field of genome or genetic testing. Following interview with 11 experts,data was obtained from 25 individuals and analyzed by using quantitative SWOT analysis, including the concept of SWOT method, Analytic Hierarchy Process,Internal Factor Evaluation ( IFE ) Matrix, and External Factor Evaluation ( EFE ) Matrix. Consistent ratio (CR) was chosen to test the logic. Results Of the 15 critical affecting factors, the weighting coefficient of deficiency of laws and market regulations, state supporting policies, and lack of intellectual property protection was 0. 099,0. 091 and 0. 087, respectively. The development of genetic testing services was found in the third quadrant of SWOT analysis. Conclusion These findings suggest that defensive strategy should be taken to improve genetic testing services and to reduce the risk of genetic testing.
4.Social collaboration network analysis of animal-derived regenerative implantable medical devices:an overview based on Chinese literature from databases of CNKI, WANFANG DATA and VIP

BACKGROUND:Tissue engineering involves many disciplines such as biology, material science, medicine and engineering, so, the col aboration among different research departments is becoming an important factor to enhancing research outputs with the rapid development of related sciences and technologies. OBJECTIVE:To draw the visualizing map of col aboration network of animal-derived regenerative implantable medical devices based on tissue engineering technology and describe its evolving process and current situation. METHODS:2 518 Chinese literatures about the animal-derived regenerative implantable medical devices based on tissue engineering technology published before 31 December 2014 were searched in CNKI, WANFANG DATA and VIP. Subsequently, social network analysis was conducted on those literatures by utilizing UCINET software and SASI developed by Peking University. RESULTS AND CONCLUSION:The col aboration network of the animal-derived regenerative implantable medical devices has evolved from scattered to single-core dominated, and then to a core-edge one, characterized by increasing and extensive col aborations as wel as decreasing network density and centralization. The core units from 2010 to 2014 include Tsinghua University, General Hospital of Chinese PLA and Affiliated Nanfang Hospital of South Medical University. Also, plenty of edge institutes exist. In conclusion, edge institutes should expand their scope of cooperation, while core institutes should improve their cooperation sustainability. Furthermore, cooperation among enterprises, research institutes and clinical hospitals should be strengthened to promote the industrialization of tissue engineering technology.
5.Effect evaluation of high quality nursing service in patients with artificial joint replacement

Objective To explore the effects of high quality nursing service mode on nursing of patients with artificial joint replacement.Methods 82 patients from January to December 2010 were set as the control group,another 82 patients from January to November 2011 were named as the observation group.The observation group was given high quality nursing service mode,while the control group received conventional nursing.The satisfaction degree of patients to nursing,average hospitalization time and cost and incidence of complications were compared between two groups.Results The satisfaction degree of the observation group was 98.78%,significantly higher than 93.90% in the control group.The average hospitalization time was(14.68±5.88)days in the observation group,shorter than(19.22±6.78)days of the control group.The average hospitalization expense in the observation group was (25 665±528)yuan,less than (30 866± 732)yuan of the control group.The complication rate in the observation group was 21.95%,significantly lower than 46.34% of the control group.Conclusions Targeted high quality nursing service mode can effectively improve the quality of nursing,decrease the incidence of complications,reduce the patient's hospitalization time and cost,as well as improve the patients' satisfaction degree.

7. Attaching importance to health of elderly population and promoting national healthy ageing actively in China

China has entered an aging society since 1999. By the end of 2017, the number of persons aged ≥60 years had increased to 240.90 million. At present, the average life expectancy of China’s population is 76.4 years, and the healthy life expectancy is around 68.7 years, which means that the elderly will be accompanied by disease for about 8 years. The main health problems of the elderly in China include increased prevalence of chronic diseases, increased prevalence of disability, insufficient attention to older persons’ mental health and area specific differences in the health level of the elderly. The disease status of the elderly cannot be ignored. Strengthening the medical services in western and rural areas, emphasizing the psychological and mental concern for the elderly, formulating unified health evaluation criteria, promoting a new model of medical and nursing care combination, establishing an elderly security system with equal responsibilities and rights, jointly managing under the cooperation of whole society and multi-dimensions to protect the health of the elderly are essential to the construction of an active ageing society in China and the realization of the strategic goal of healthy China.
8.Histiocytic necrotizing lymphadenitis characterized by fever and joint pain:a case report

A 27-year-old woman was admitted to a hospital due to intermittent fever for 20+ days and swelling pain in knee joint for 7 days.Patients was with persistent fever,the highest temperature to 40.1°C,the double knee joint swelling pain,a large number of scattered red needle-like rash were visible on bilateral hands and legs,lymph-adenectasis,splenectasis,bilateral hands interphalangeal joint and knee joint swelling pain,lymph nodes were con-fined to the bilateral axillary with soft texture,local tenderness,and good activity.Pathological results of left axil-lary lymph node showed the following finding:fragmental lymph node tissue with incomplete structure,lymphoid follicles existed,there were diffuse histocytes,immunoblasts,and a few small lymphocytes proliferation in the re-duced or disappeared area of follicular,caryokinesis could be easily seen,there were patches of necrosis foci with varied size,nucleus disintegrated,fibrinoid necrosis blood vessel with bleeding could be seen.Immunohistochemical detection showed the following results:CD3 (diffuse +);CD20 (follicular +);CD79α(follicular +);CD68 (+);EMA (-);ALK (-);CD15 (a small amount of cells +);CD30 (+),CD4 (-),CD5 (+);CD10 (-);bcl-2 (+);CD21 (+);telomerase B (-);TIA-1 (-);EBV(-)Ki-67 (+).Through a combination of clinical and immunohistochemical detection results,the diagnosis of histiocytic necrotizing lymphadenitis was made.Patient discontinued antimicrobial drugs,after she was treated with 5% glucose 100 mL + hydrocortisone sodium succinate 200 mg intravenous drip for 3 days,patients had no fever,axillary lymph node gradually dwindled.Then patients was treated with methylprednisolone 8 mg/d,twice a day,reduced 1 tablet every two weeks,and stopped eventual-ly.Patients was followed up for two years,repeated examination of blood routine,liver and kidney function were in the normal range,bilateral knee didn’t swell,could walk freely,there appeared no enlargement of lymph node.
9.The relationship between plasma homocysteine levels and diabetic peripheral neuropathy

Objective To explore the relationship between plasma homocysteine levels and diabetic peripheral neuropathy (DPNP). Methods A crossectional analysis was conducted on 227 patients with type 2 diabetes. Peripheral neuropathy was confirmed using electromyography (EMG). The risk factors possibly associated with diabetic neuropathy or plasma homocysteine levels were analyzed in relation to likelihood of occurrence of DPNP. Results Eighty patients with neuropathy and 147 patients without neuropathy were included. Plasma homocysteine levels were significantly higher in patients with diabetic neuropathy [( 12. 6 ± 3.6 ) μmol/ L] than without diabetic neuropathy [( 8. 2 ± 0. 9 ) μmol/L] ( P <0. 001 ), and the relationship remained significant after adjusting for duration of diabetes, glycosylated hemoglobin A1c (HbA1c), age, renal status, serum folate acid and vitamin B12, and metformin [OR 1.15( 1.02-1.28 ) ,P < 0. 05]. In addition, per increase of 4. 0 μmol/L plasma homocysteine was closely related to the occurrence of neuropathy after controlling for per unit increase of other confounding factors [OR 1.17(0. 94-1.33), P < 0. 05]. Conclusions Hyperhomocysteinemia was an independent risk factor for the occourence of diabetic peripheral neuropathy.
10.Study on the risk factors of chronic obstructive pulmonary disease in acute exacerbation complicated with pulmonary embolism

Objective To analyze the risk factors of chronic obstructive pulmonary disease (COPD) in acute exacerbation (AE-COPD)complicated with pulmonary embolism ,and to provide reference for the pathogenesis and treatment .Methods A total of 73 patients with COPD suspected PE admitted to our hospital from May 2015 to April 2016 were enrolled in this study .All patients were examined including WBC ,Neu% ,CRP ,IL-8 ,ESR ,PCT ,ET-1 ,D-dimer ,fibrinogen ,NT-proBNP ,myocardial enzyme ,arterial blood gas ,lactic acid ,CT pulmonary angiography(CTPA)within 48 h of admission .The risk factors of AECOPD with PE or with-out PE were analyzed .Results There were 15 cases with PE ,58 cases without PE in all objects .Neu% ,PCT ,NT-proBNP ,D-di-mer ,LDH ,cTnI ,CRP ,IL-8 ,ET-1 in patients with PE were significant higher than those in patients without PE (P<0 .05) .In the PE group ,the correlation coefficient between CRP and IL-8 was 0 .457(P=0 .087) ,the correlation coefficient between CRP and ET-1 was 0 .598(P=0 .019) ,the correlation coefficient between IL-8 and ET-1 was 0 .695(P=0 .004) .Conclusion Acute exacer-bation of COPD combined with PE is associated with the severity of inflammation in the body ,the more serious the inflammatory re-action ,the corresponding increase in myocardial injury ,the higher the risk of PE .
