1.Expression of Survivin, RAGE and HMGB1 gene in human breast cancer and its clinical significance

Objective To investigate the expression of Survivin,receptor for advanced glycation endproduct(RAGE) and high mobility group protein B1 (HMGB1) gene in human breast cancer and their clinical significance.Methods The expression of Survivin,RAGE and HMGB1 gene was detected by Real-time PCR technology and fluorescence in situ hybridization (FISH) technology in the following tissue samples:50 early breast cancers,50 advanced breast cancers,and the corresponding adjacent normal mammary tissues.The relationship of their expression and several factors such as differentiation degree,invasion,lymph node metastasis and TNM stage of cancer was explored.Results The positive expression rate of Survivin,RAGE and HMGB1 gene was 62%,73% and 79% detected by Real-time PCR technology,78%,69% and 72% detected by FISH technology in breast cancer tissues.The overexpression of these genes was positively correlated with TNM stage and lymph node metastasis.The expression of Survivin gene was positively correlated with the expression of RAGE and HMGB1.Conclusion Overexpression of Survivin,RAGE and HMGB1 gene is of great significance in early diagnosis and prognosis of human breast cancer.

ObjectiveTo know about the correlation analysis of the physical and mental status of organ transplanted patients' family members during the patients' rehabilitation period and the long term survival of the transplanted organs.MethodsA total of 600 organ transplanted patients' family members were investigated by Zung self-rating anxiety scale (SAS) and simple coping style questionnaire.The results underwent subsequent analysis.ResultsAnxious state of organ transplanted patients' family members was more severe than that of domestic norm (P<0.01).There were some differences in terms of anxious state of family members with different genders,education backgrounds,income and the resource of medical expenditure (P<0.05).Family members most took positive coping styles,whereas fewer adopted negative coping styles (P<0.01).Positive coping styles were negatively correlated with the anxiety of family members (P<0.01) and positively correlated with long term survival of transplanted organ(P<0.05).However,negative coping styles of family members were significantly positively correlated with their anxiety (P<0.05) and were negatively correlated with long term survival of transplanted organ.ConclusionsThe anxiety generally exists in organ transplanted patients' family members.There are differences in terms of anxious state among family members of different genders,different education backgrounds,income or with the resource of medical expenditure.The more they adopt positive coping styles,the lower anxiety level they show and the longer the transplanted organ survive.Conversely,the more they adopt negative coping styles,the higher anxious level they show and the shorter the transplanted organ survive.
4.MSCT manifestations of hepatic fat-poor perivascular epithelioid cell tumor

Objective To investigate the MSCT manifestations of hepatic fat-poor perivascular epithelioid cell tumor (PEComa). Methods CT and pathological findings of 8 patients with hepatic fat-poor PEComa confirmed by surgery were assessed retrospectively.Results 8 cases had solitary lesion,all lesions with round or round-like shape.The largest diameter ranged 20-110 mm.The fat density was not measured by CT scan.6 lesions composed of solid component,and solid part showed obviously enhancement on arterial phase.On portal venous phase and parenchymal phase,the tumors showed equal or low or slightly high density.2 lesions showed cystic necrosis,peripheral enhancement on arterial phase,1 lesion showed continuous enhancement and the enhancement degree increased,and another lesion showed reduced enhancement.All lesions had tortuous vascular in the center of lesions or at the peripheral.Immunohistochemistry examinations showed that HMB45,Melan-A and SMA were positive in all cases,CD31,CD34 and S-100 expressed positive in partial cases. Conclusion The CT findings of hepatic fat-poor PEComa are lack of specificity.When the enhancement and clearance pattern of liver mass is similar to hepatocellular carcinoma or adenoma,the clinical history does not support the diagnosis,may consider the possibility of PEComa when tortuous vascular in the center of lesions or at the peripheral on arterial phase.
5.Safety and feasibility analysis of drinking small amount water early among adult patients with general anesthesia after operation

Objective To explore the safety and feasibility of drinking Small amount water among adult patients with general anesthesia after operation. Methods A total of 200 adult patients with general anesthesia in post-anesthetic care unit ( PACU) were divided into observation group ( PACU routine nursing with giving a small amount of water care early) and control group ( PACU routine nursing care) each 100 cases according to random number table method. We compared the degree of dry mouth changes, recovery satisfaction, incidence of vomiting aspiration and rate of drinking water again. Results The score of dry mouth in the observation group (7. 51 ± 1. 48) significantly higher than that in the control group (1. 39 ± 1. 20) with statistical difference (t=32. 12,P<0. 05). Observation group, the incidence of aspiration was 4% while it was 6% in the control group (χ2 =0. 42,P>0. 05). Observation group for recovery satisfaction score (92. 35 ± 2. 64), significantly higher than the control group in recovery satisfaction score (67. 94 ± 7. 46). The difference was statistically significant (t=30. 85,P<0. 05). 96% of patients having intention to drink again in observation group compared with 91% of people having intention in the control group (χ2 =2. 06,P>0. 05). Conclusions Adult postoperative patients with general anesthesia drink early are safe and feasible.

7.Effects of cyclic stretch on in vitro culture-tensile stimulation models of myoblasts

BACKGROUND:Endoplasmic reticulum stress participates in the occurrence and development of many diseases, such as atherosclerosis, diabetes, and Alzheimer’s disease. GRP78 is a marker of endoplasmic reticulum stress. The expression of GRP78 reflects the degree of endoplasmic reticulum stress.
OBJECTIVE:To investigate the effect of cyclic stretch on GRP78 expression of L6 rat myoblasts, and to identify the relationship between cyclic stretch and endoplasmic reticulum stress.
METHODS:In vitro culture-tensile stimulation models of myoblasts of L6 rats were established successful y. The expression of GRP78 of myoblasts exposed to cyclic stretch was determined by reverse transcription-PCR and western blot assay. Stretch groups were subjected to 15%surface elongation at a frequency of 10 cycles per minute, over a period of 1, 6, 12 and 24 hours. cells were simultaneously seeded on a plate in the control and experimental groups with no stimulation.
RESULTS AND CONCLUSION:The expression of GRP78 mRNA was continuously elevated over time after stretched treatment, and significant differences were detected as compared with the control group (P<0.05). GRP78 protein expression began to increase at 1 hour after stretched treatment, was significantly increased at 6 hours, peaked at 24 hours, and significant differences were visible as compared with the control group (P<0.05). In conclusion, cyclic stretch induced the occurrence of endoplasmic reticulum stress, which was enhanced with prolonged time. However, prolonged stretch caused severe endoplasmic reticulum stress and leaded to apoptosis of myoblasts.
8.Caspase-9 in myoblasts is involved in mechanical signal transdunction under cyclic stretch

BACKGROUND:The adaptive reconstruction of maxil ofacial muscles would happen when functional orthopedic treatment is done to cure micromaxil ary deformity. The myoblast is the main responder in the process of adaptive reconstruction, and cyclic stretch can induce apoptosis of myoblasts. Caspase-9 is an important factor in the mitochondrial apoptosis pathway.
OBJECTIVE:To investigate the expression of Caspase-9 in different cyclic stretch.
METHODS:Based on myoblasts cultured in vitro-mechanical stimulation model, the rat L6 myoblasts were loaded stretch for 1, 6, 12 and 24 hours through multi-channel cellstress loading system, while the control group received no stretch. The morphological change and growth of myoblasts were observed under inverted phase contrast microscope;the expression of the mRNA and protein of Caspase-9 were detected by RT-PCR and western blot analysis, respectively.
RESULTS AND CONCLUSION:Under inverted phase contrast microscope, the rat L6 myoblasts at cyclic stretch maintained a good growth state and biological characteristics;there was no celldegeneration;and the loss rate was extremely low, which could demonstrate that myoblast in vitro-mechanical stimulation model was established successful y. The results of RT-PCR and western blot analysis showed that, the expression of Caspase-9 mRNA and Cleaved Caspase-9 protein was significantly increased as the loading time prolonged, and the expression of Procaspase-9 protein was significantly decreased as the time. We can conclude that Caspase-9 is involved in the mechanical signal transduction of cyclic stretch.
9.Relationships between the pathology classification, Masaoka clinical stage and postoperation myasthenic crisis in patients of myasthenia gravis with thymoma

Objective To investigate the relationships between the pathology classification, Masaoka clinical stage and postoperation myasthenic crisis in patients of myasthenia gravis with thymoma undergoing thymectomy. Methods Clinical records of 56 patients of myasthenia gravis with thymoma from January 2006 to December 2015 who had underwent thymectomy were reviewed retrospectively. The following factors were analyzed to find the relation to the occurrence of myasthenic crisis after thymectomy: WHO pathology classification, Masaoka clinical stage and tumor size. Results Sixteen patients experienced postoperative myasthenic crisis after thymectomy. Statistical analysis revealed that the incidence of postoperative myasthenic crisis in patients with Masaoka Ⅲ stage was significantly higher than that in patients with Masaoka Ⅰ and Ⅱ stage: 39.39% (13/33) vs. 13.04% (3/23), the incidence of postoperative myasthenic crisis in patients with WHO pathology classification B3 and C type was significantly higher than patients with WHO pathology classification B2 type: 50.00% (12/24) vs. 14.29% (4/28), the incidence of postoperative myasthenic crisis in patients with tumor size more than 5 cm was significantly lower than patients with tumor size less than 5 cm: 10/17 vs. 15.38% (6/39), and there were statistical differences (P<0.05 or<0.01). Conclusions WHO pathology classification and Masaoka clinical stage are significantly correlated with the occurrence of myasthenia crisis after thymectomy. The patients with MasaokaⅢstage, WHO pathology classification B3 and C type and tumor size more than 5 cm have the risk of postoperative myasthenic crisis after thymectomy. The comprehensive intervention before and after operation can prevent myasthenia crisis.
10.Electrocardiographic characteristics and their correlation with indicators of disease severity in patients with chronic pulmonary artery stenosis

Objective:To analyze the electrocardiographic characteristics of patients with chronic pulmonary artery stenosis (PAS), and to explore their relationship with disease severity indicators.Methods:The study was a retrospective case-series analysis. Patients with chronic PAS admitted to Gansu Provincial Hospital from January 2018 to July 2021 were enrolled. The clinical data and the results of electrocardiography, transthoracic echocardiography, right cardiac catheterization, N-terminal B-type natriuretic peptide (NT-proBNP) measurement and 6-min walking distance test of patients were analyzed. The linear regression model or logistic regression model was used to analyze the relationship between electrocardiographic characteristics and the disease severity in patients with chronic PAS.Results:Sixty-three patients aged (62.1±9.7) years including 43 females (68.3%) were enrolled in the study. Among them, 62 patients (98.4%) had (R 1+S Ⅲ)-(S Ⅰ+R Ⅲ)<1.5 mV, and no patients had V 5lead R: S ratio to V 1 lead R: S<0.04 and V 6 lead R: S ratio<0.4. There were 55 patients (87.3%), with flat or inverted T-waves in V 1, and 10 patients (15.9%) with flat or inverted T-waves in all precordial leads (V 1-V 6). There were 18 patients (28.6%) with flat or inverted T-waves in inferior leads (Ⅱ, Ⅲ, aVF). Multiple liner regression analysis showed that Max R V1, 2+Max S I, aVL-S V1 combined with the number of flat or inverted T-waves in limb leads was independently correlated with atrial area ( R2=0.290, P=0.002); R V1+S V5 was independently correlated with right ventricular area ( R2=0.257, P=0.001); R peak V 1 combined with the number of flat or inverted T waves in precordial leads was independently correlated with tricuspid annular plane systolic excursion ( R2=0.407, P<0.001); (R 1+S Ⅲ)-(S Ⅰ+R Ⅲ) combined with the number of flat or inverted T waves in precordial leads was independently correlated with NT-proBNP ( R2=0.504, P<0.001); Max R V1, 2+Max S I, aVL-S V1 were independently correlated with right atrial pressure ( R2=0.803, P=0.036); (R 1+S Ⅲ)-(S Ⅰ+R Ⅲ) were independently correlated with mean pulmonary artery pressure ( R2=0.302, P<0.001); R aVRcombined with the number of flat or inverted T-waves in precordial leads was independently correlated with cardiac index ( R2=0.173, P=0.003); (R 1+S Ⅲ)-(S Ⅰ+R Ⅲ) was independently correlated with pulmonary vascular resistance ( R2=0.173, P=0.002); R peak V 1 combined with the number of flat or inverted T-waves in precordial leads was independently correlated with mixed vein oxygen saturation ( R2=0.302, P<0.001). Conclusion:The vast majority of patients with chronic PAS have (R 1+S Ⅲ)-(S Ⅰ+R Ⅲ)<1.5 mV and flat or inverted T-wave in V 1 lead, and some characteristic electrocardiographic manifestations are correlated with indicators of disease severity.
