1.Clinical analysis of multiple myeloma in patients over 80 years old:A report of 11 cases
Chinese Journal of Clinical Oncology 2014;(13):849-852
This study aimed to explore clinical features of multiple myeloma in patients over 80 years old. Methods: We retrospectively analyzed 11 cases of multiple myeloma over 80 years old, which were diagnosed from 2000 to 2013 in our hospital. Results:The mean age was 83.5 years. All patients had more than two complicated diseases, and the performance status was poor in the majority of the patients. Ten cases received individualized treatments. The results showed that three patients had progressed, one patient achieved complete remission, three patients achieved partial remission, and four patients achieved minor remission. Median survival time was 28 months. The causes of death included six progressed myelomas, three pneumonias, and two acute myocardial infarctions. Conclusion: Patients aged 80 years old with myeloma are often at a late stage. Treatment should be individualized based on patient status. Survival time was evidently prolonged in very elderly patients with MM after individualized treatments, particularly in patients in stagesⅠandⅡ.

Objective:We clarify the relationship between dysfunction of gastric motility and the plasma motilin,in order to study the pathogenesis, diagnosis and therapy of the hepatocirrhosis with dyspepsia. Methods:The liquid gastric emptying (GE),electrogastrograpgy(EGG) and plasma motilin were examined in 51 patients with hepatocirrhosis. Results:The dysfunction of gastric motility is associated with the liver function, gastric rhythm and upper gastrointestinal symptom. The plasma motilin concentration increased significantly in patients with hepatocirrhosis as compared with that of controls (P<0.01).The sorbefacient prokinetic drug (prepulsid) can improve the function of gastric power, but the lever of motilin no change. Conclusion:The patients with hepatocirrhosis have the dysfunction of gastric motility, and gastric emptying delayed, and there is a relation between GE and the liver function. We evaluate that this disorder is associated with the low sensitivity about stomach to the plasma molitin in this case.We can judge the degree of the liver damage, gastric emptying delayed.gastric rhythm by testing the gastric motilin.Drug can deal it.

Objective To analyze the superiority of implanting operation with autoskull stored at ultrahypothermia over low temperature's and frozen skull bone of oneself. Methods 42 case were observed and followed up after operation. Results Incision is healing by firct intention.there were many advantages, such as good biological activity, beautiful appearance,no repellent response,no immunoreaction and hard to infection, etc. After storing 1,3,6 and 12 months, the histological structure of frozen skull observed by electronscope was similar to those of fresh skull.Pestroyed skull bone cells were not found. All cases had no complication during the period of 3 to 12 months(mean 6.5 months) follow-up after operation. After 12 months,X ray and CT scan showed that skulls were healed. Conclusions Implanting operation with autoskull stored at ultra-hypothermia is one of the most effective technique for repairing skull defects.

The CD19-targeted chimeric antigen receptor (CAR) T-cell treatment has achieved clinical success in treating B-cell lineage hematopoietic malignancies. This accomplishment involved the precise and efficient elimination of tumor cells by tumor-associated an-tigen-redirected immune cells. As a result, the reinitiation of several CAR T-cell (CART) based clinical trials in solid tumors was promot-ed. However, developing a feasible and efficient CAR T based therapeutic modality for solid tumors is urgently needed given the differ-ential properties between liquid and solid tumors. In this review, we discuss the advances in the management of cytotherapeutic mo-dality for solid tumors. The aspects considered include toxicity management, target selection, and primer or conditioning treatment.

Aim To evaluate the effect of 15-hydroxy eicosatetraenoic acid(15-HETE) on the tension of isolated internal carotid artery(ICA) rings in normoxic or hypoxic rats in order to find out the pathophysiological effect of 15-HETE in ischemic/hypoxic brain tissues. Methods Sixteen healthy Wistar rats were randomly divided into two groups(n=8): group A with normal oxygen supply(FiO_2 21%) and group B with hypoxia(FiO_2 10%).9 days later, the rats were killed and the ICA rings were prepared.15-HETE in different concentration were added accordingly, and the after washing up,blockers of different kinds of K~+ channels such as 2 mmol?L~(-1) 4-AP,10~(-2) mol?L~(-1) TEA and 10~(-6) mol?L~(-1) GLYB were added to the KH solution separately, followed by the administration of 10~(-6) mol?L~(-1) 15-HETE.The isometric tension was recorded using a four-channel force-displacement transducer. Results The ICA tension was significantly increased both in groups A and B by 15-HETE in concentration-dependent manner(P0.05,n=8).Furthermore,10~(-2) mol?L~(-1) TEA,10~(-6) mol?L~(-1) GLYB had little effects on the tension of rings.Application of 10~(-6) mol?L~(1) 15-HETE after administration of TEA or GLYB induced nearly the same increase in ICA tone as it happened in the group administered with 10~(-6)mol?L~(-1) 15-HETE alone. Conclusion 15-HETE can induce brain vasoconstriction especially under the hypoxic condition.Voltage-dependent potassium channels play an important role in 15-HETE induced constriction.

To investigate the effect of pancreatic elastase on diabetic nephropathy,104 cases of type Ⅱ diabetic patients were selected as control group and group treated with elastase separately for a course of 6 months. The results showed that pancreatic elastase reduced microproteinuria significantly, especially mi- croalbuminuria and microtransfer rrinuria which reflect the glomerular filtration rate.It was concluded that elastase could improve early diabetic nephropathy and might have some protective effect as well.At the same time, elastase had some good effect on lipid and lipoprotein metabolism.

BACKGROUND: The formation mechanism of biliary cast syndrome following liver transplantation has not been thoroughly illuminated, and it is unclear that whether some proteins correlated to the formation mechanism of biliary cast or prewarning to the formation of biliary cast.OBJECTIVE: To investigate the holoprotein expression in biliary cast syndrome patients following liver transplantation. METHODS: Four patients underwent liver transplantation at Liver Transplantation Institute, General Hospital of Chinese People's Armed Police Force. Three months later, 10 g biliary cast was harvested. Four kinds of biliary cast specimens at different colors and textures were preserved at deep hypothermia, followed by protein abstraction and restriction enzyme digestion, the total protein abstraction solution of biliary cast were analyzed by high definition mass spectrometry and query on MASCOT database. All protein name of biliary cast were list, the conjunct protein was found by comparing 4 specimens. RESULTS AND CONCLUSION: There were totally 208 proteins in 4 biliary cast specimens, 82, 44, 56 and 65, respectively. By comparison, 5 proteins were found to overlay in 2 biliary cast specimens, 7 proteins in 3 specimens and 13 proteins in 4 specimens. Among the latter 13 proteins, 5 unnamed-proteins, as well as 8 named-proteins (termed alpha-fibrinogen precursor, beta-fibrinogen precursor, fibrinogen gamma chain, proapolipoprotein, Chain A of Human Cathepsin G, S100 calcium-binding protein A9, lactoferrin) were included. The proteins exists in biliary cast, the common proteins of 4 biliary cast specimens imply a correlation between the formation of biliary cast and the exudative inflammation following the damage of biliary tract epithelium; Some proteins might be considered as a marker of prewarning the presence of biliary cast syndrome, judging the inflammation severity following the damage of biliary tract epithelium and the prognosis of biliary cast syndrome.
