1.Observation of intra-articular Hydromorphone with Ropivacaine for analgesia after knee arthroscopy

ObjectiveTo assess the effects of intra-articular Hydromorphone with Ropivacaine for postoperative analgesia after arthroscopic knee surgery.Methods 90 patients undergoing arthroscopic knee surgery were randomly divided into 3 groups. Group R: 0.375% Ropivacaine 20ml; group H1: Hydromorphone 0.3 mg and 0.375 % Ropivacaine 20 ml; group H2: Hydromorphone 0.6 mg and 0.375 % Ropivacaine 20 ml. The visual analogue scale (VAS) at rest were recorded at 6, 12, 18 and 24 h after surgery, Duration of analgesia, number of patients and frequency requiring Parecoxib at 24 h after surgery were observed.Results Compared with group R, VAS of group H1 and group H2 were signiifcantly lower at 12 and 18h after the operation, duration of analgesia was much longer, number of patients and frequency requiring Parecoxib was lower in group H1 and H2 (P < 0.05); Compared with group H1, No signiifcant differences of VAS, duration of analgesia and number of patients and frequency requiring Parecoxib of group H2.Conclusions After knee arthroscopic surgery, intra-articular 0.3 mg Hydromorphone can signiifcantly improve the efifcacy of Ropivacaine for postoperative analgesia; the efifcacy of Hydromorphone can’t increased with the increase of dosage.
2.Effect of hypercholesterolemia on the ionic currents in cardiac ventricular myocytes of rats

AIM: To determine whether chronic hypercholesterolemia affects ionic currents on cardiac ventricular myocytes of rats. METHODS: Whole - cell patch - clamp technique was used to record the ionic currents in single cardiac myocytes isolated from normal cholesterolemia and hypercholesterolemia rats. RESULTS: In the hypercholesterol group (group Ⅱ ), serum total - cholesterol level was significantly higher than that of normal group (group Ⅰ) [ (3. 10 ±tricular myocytes of rats, 50% repolarization of action potential duration (APD50) prolonged from (70. 86 ± 8.12) ms (group Ⅰ) to (116.16±6.90)ms (group Ⅱ) (n=10 in each group, P<0.01); APD90 prolonged from (95.10±7. 27)ms (group Ⅰ ) to (144. 04 ± 7.39)ms (group Ⅱ ) (n = 10 in each group, P < 0. 01 ); at the test potential of - 120 mV, Ik1 increased from ( - 16. 98 ±4. 54) pA/pF(group Ⅰ ) to ( - 19.92 ±4.08) pA/pF (group Ⅱ ) (n = 12 in each group, P < 0. 05 ); at the test potential of 0 mV, ICa- L decreased from ( - 8.56 ± 1.29) pA/pF ( group Ⅰ ) to ( -5. 24 ± 0. 90) pA/pF ( group Ⅱ ) ( n = 10 in each group, P < 0. 01 ); at the test potential of + 60 mV, Ito decreased from (13.20±1.97) pA/pF (group Ⅰ) to (10.30±1.97) pA/pF (group Ⅱ) (n=8 in each group, P<0. 05). CON-CLUSION: Hypercholesterolemia affects the ionic currents on cardiomyocytes of rats greatly, which may be the ionic mechanism of cardiac toxicity induced by hypercholesterolemia.
3.Inhibition of matrine on potassium currents in guinea pig ventricular myocytes

AIM To elucidate the possible antiarrhythmic mechanism of matrine. METHODS Whole-cell patch-clamp technique was used to record ionic currents in ventricular myocytes. RESULTS In guinea pig ventricular myocytes, matrine 100 μmol·L-1 prolonged 90% action potential duration (APD90) by 40% at a stimulation of 0.1 Hz in a frequency-independent manner, inhibited IK1 by 47% at the test potential of -120 mV, reduced IKr,tail by 50% and had no effect on IKs,tail. CONCLUSION Matrine prolonged APD through blockade of multiple potassium currents, which may relate to its antiarrhythmic efficacy.
4.The HRCT Scan of Normal Middle-inner Ear

Objective To study the scan technique that can completely demonstrate the structures of middle-inner ear.Methods Ninety volunteers underwent 30° axial and 105° coronal HRCT scan.The manifestations of the ministructures of middle-inner ear were observed and their demonstration rate was calculated.The demonstration of different structures of middle-inner ear was evaluated and compared on HRCT.Results (1)The scan planes of 30° axial and 105° coronal were correspondent to some structures of middle-inner ear and could completely demonstrate them.(2)30° axial and 105° coronal HRCT scan had their own advantages respectively in demonstrating the structures of middle-inner ear.Conclusion The technique of 30° axial and 105° coronal HRCT scan of middle-inner ear is useful in demonstrating the structures,including the nerve,joint,ligament etc and the relationship among them.
5.Study on breastfeeding evaluation indicator system for preterm infants and its strategy of clinical application

Objective To establish the breastfeeding evaluation indicator system for preterm infants and to provide theoretical basis for breastfeeding implementation and evaluation.Methods Based on literature review,clinical on-spot investigation and experts consultation,the evaluation system frame was initially formed.With specialists meeting method and Delphi study,the evaluation indicator system for preterm infants was established.Results The breastfeeding evaluation indicator system for preterm infants comprised 3 levels,with 3 indicators in level 1,7 indicators in level 2 and 18 indicators in level 3.Conclusions It is emphasized that the purpose for breastfeeding evaluation of preterm infants is to continuously improve the breastfeeding quality for preterm infants.
6.The effect of atorvastatin and rosuvastatin on endothelial function in diabetics with coronary heart disease

Objective To investigate the effect of atorvastatin and rosuvastatin on endothelial function in diabetics with coronary heart disease. Methods A total of 73 consecutive diabetics with coronary heart disease who were not receiving statins were randomized to receive atorvastatin 20 mg/d or rosuvastatin 10 mg/d. The levels of lipids, high-sensitivity C-reactive protein(hs-CRP), monoxide nitrogen(NO), endothelin-1 (ET-1) and endothclium-dependent relaxing function(EDF) were assessed before and after 3 months of the treatment. Results The treatment with statins significantly improved endothelial function in diabetics with coronary heart disease. For both rosuvastatin group and atorvastatin group in pro-treatment versus post-treatment, EDF was higher[(5.2±2.4)% vs. (7.9±3.1)%, P=0.008; (5.0±2.8)% vs. (7.6±3.6)%, P=0.024, respectively], NO was also higher((46.6±14.5) μmmol/L vs. (73.3±18.5) μmmol/L; (51.7±14.0) μmmol/L vs. (79.8±16.0) μmmol/L,both P<0.001], ET-1 was lower[(108.2±29.6) pg/L vs. (77.5±26.4) pg/L, P =0.005;(117.1±34.5) pg/L vs. (80.7±28.2) pg/L, P= 0.003, respectively],and hs-CRP was lower[3.17(1.33~6.32) mg/Lvs. 1.39(0.81~2.58) mg/L, P=0.006; 3.43(1.51~7.02) mg/L vs. 1.63(0.69~3.11) mg/L, P = 0.001, respectively]. There were no differences in these between rosuvastatin group and atorvastatin group. Significant negative correlation existed between EDF and hs-CRP. Conclusions A 3 months treatment with either atorvastatin 20 mg/d or rosuvastatin 10 mg/d is effective in improving endothelial function in diabetics with coronary heart disease.
7.Shenshao decoction improves myocardial inflammatory injury in diabetic rats by regulation of TLR4/MyD88 pathway

Objective To investigate the improving effects of Shenshao decoction on myocardial structure and function in diabetic cardiomyopathy,and its effect on expression of TLR4/MyD88-dependent pathway signal in diabetic cardiomyopathy.Methods Diabetes mellitus was induced in 8-week-old male Wistar rats by a single intraperitoneal injection of streptozotocin.The changes of plasma myocardial enzyme (CK, LDH) and high sensitive C reactive protein (hsCRP) were measured.Cardiac function was measured by left ventricular intubation.Hematoxylin and eosin staining and electron microscopy were used to observe the changes of myocardial morphology and ultrastructure in rats.Expression of Toll-like receptor 4(TLR4), myeloid differentiation protein 88(MyD88), and nuclear factor-κB P65(NF-κB P65) were tested by immunohistochemistry.Results After 6 weeks of treatment, the left ventricular diastolic and systolic functions were obviously improved;the degrees of myocardial fibers and mitochondrial damage were obviously relieved;the content of CK, LDH and hsCRP decreased (P< 0.05), and hsCPR was positively correlated with CK and LDH (r=0.823,r=0.819,P < 0.05).The expressions of TLR4, MyD88, NF-κB P65 were significantly decreased (P< 0.05).There was no significant difference in the above mentioned indicators between the control group and control+Shenshao decoction group (P> 0.05).Conclusions Shenshao decoction can reduce the myocardial injury in diabetic cardiomyopathy and improve cardiac diastolic and systolic functions.The mechanism may be related to attenuated inflammation by TLR4/MyD88 dependent signaling pathway.
8.Study on the senescence of rat glomerular mesangial cells induced by Tert-Butyl hydroperoxide in vitro and the influnce of probucol on its senescence

Objective To investigate the senescence of rat mesangial cells induced by Tert-Butyl hydroperoxide (tBHP) and the protective effect of probucol on senesecence. Methods Human glomerular mesangial cells(hGMC) were cultured in vitro and intervened by tBHP. The cell survival rate was observed by methyl thiazolyl tetrazolium( MTT). β-gal staining and cell cycle analysis were used to identify cell senescent status;transmission eletric microscopy was used to evaluate the ultra-microstructure of hGMC. Senescent-related indexes were detected after treatment with probucol. Results The cell survival rate with 30 μmol/L tBHP was (80. 12 ± 3. 25 ) % , the positive rate of β-gal staining was significantly higher in tBHP-induced cells (about 81% )than that of the control cells( P <0. 01). 86% of the cells was arrested at G0-G1 phase. Invagination of nucleus membrane and chromatin condensation at the nuclear margin in tBHP-induced cells was observed through transmission eletric microscopy. In the probucol intervented cells, the cell survival rate was higher than that of tBHP-induced cells (92. 68 ± 5.03) % vs. ( 80. 12 ± 3. 25) % (P < 0. 05 ). The positive rate of β-gal staining decreased to 45. 2%. The proportion of cell cycle stage was similar to the control cells.The change of morphous and ultrastructure was relieved. Conclusions tBHP can induce hGMC senescence in vitro and probucol may play a role in preventing hGMC senescence.
9.The Value of Spiral CT Examination in Central Bronchial Carcinoma Post-operation

Objective To assess the usefulness of spiral CT examination in detecting the local recurrence and the metastatic lymph nodes of chest in the patients with post-pulmonectomy for central lung cancer.Methods The data of 110 contrast spiral CT of chest in 55 patientswith central bronchial carcinoma post-operation were retrospectively analysed.Results 12 person-time in 9 cases recurred in thebronchial stump or anastomose.The short diameter of lymph nodes in the thorax ≥1 cm was showed in 33 person-time of 26 cases,of them,14 cases was considered as metastasis in combination with the clinical data.Conclusion Spiral CT is the one of the best examination indetecting the post-operation recurrence in the patients with central bronchial carcinoma,but it is still of limitation in evaluating the nature of lymph nodes.
10.Effect of Acupoint Thread-embedding at Neiguan (PC6) and Zusanli (ST36) on Anxiety and Sleep Disorders After Heroin Withdrawal

Objective To observe the effect of acupoint thread-embedding at Neiguan (PC6) and Zusanli (ST36) on anxiety and sleep disorders after heroin withdrawal. Method Sixty-seven patients with heroin addiction were randomized into a Neiguan group (25 cases), a Zusanli group (22 cases) and a Neiguan plus Zusanli group (20 cases), to receive corresponding treatments once every 20 d, for 4 sessions in total. The Self-rating Anxiety Scale (SAS) and Pittsburgh Sleep Quality Index (PSQI) were adopted to observe the anxiety and sleep condition before and after the treatment. Result After 60 d thread-embedding treatment, the total and standard SAS scores, sleep initiation time and total PSQI score were significantly decreased in the 3 groups (P<0.05), and the sleep durations were significantly extended (P<0.05); in comparing the improvement of anxiety, Neiguan plus Zusanli group > Neiguan group >Zusanli group (P<0.05); in comparing the improvement of sleep, Neiguan plus Zusanli group was superior to both Zusanli group and Neiguan group (P<0.05), while there was no significant difference between Zusanli group and Neiguan group (P>0.05). Conclusion It's effective to use thread-embedding at Neiguan and Zusanli independently or together for anxiety and sleep disorders after heroin withdrawal, but the efficacy is more significant when the two points are used in combination, indicating the synergy of acupoints.
