OBJECTIVE:To provide reference for development of hospital pharmacy and clinical pharmacy in south Sichuan. METHODS:Questionnaire was used to investigate the general situation of secondary and tertiary hospitals in 4 cities of south Sich-uan in aspects of partition layout,staffing situation and job content. The results were statistically analyzed. RESULTS:A total of 40 questionnaires was sent out,35 were effectively received with the effective rate of 87.50%. The partition layout was unreasonable in most surveyed hospitals,and the rate of reaching the standard about the area of drug dispensing sector and warehouse was below 85%. There were 1042 pharmacists in 35 hospitals,which were mainly female of younger than 45 years old. In secondary and ter-tiary hospitals,the rate of pharmaceutical professional and technical personnel of health technology,full-time undergraduate and vice-high above title accounted for 4.78% and 3.67%,17.37% and 31.27%,3.15% and 10.73%,respectively. The number of full-time clinical pharmacists was 24 and 69 in secondary or tertiary hospitals,mainly primary title,middle title and undergradu-ate,of whom 26(27.96%)passed job training of National Health and Family Planning Commission,4(4.30%)passed teaching qualifications training. The rate of pharmacists mainly engaged in drug dispensing and clinical pharmacy was 6.32% and 11.03% in secondary and tertiary hospitals,respectively. CONCLUSIONS:The research shows irrationality of partition layout,the shortage of highly educated and highly titles personnel,especially the clinical pharmacy professionals. The large differences exist in hospitals of different levels in several aspects,and the main body of hospital pharmacy should turn to clinical pharmacy.

Objective To explore the protective effects and mechanism of shenmaisanjie capsule(SMSJC) combined with methimazole on the L?thyroxine? induced hyperthyroid hepatic damage in rats. Methods so rats were divided into 5 groups randomly as normal group,model group,SMSJC group(0.48 g/kg),methimazole group (12 mg/kg)and combination group. With the exception of the normal group ,all rats were administered L?thyroxine (800 μg/kg ,ig) daily for 6 weeks. Rats were sacrificed. Blood and liver samples were collected for detecting thyroid and liver function , histological analysis and hepatic activity of SOD , GSH?Px and MDA content. Results The levels of T3,T4,FT3,FT4,ALT,AST,ALP,TBIL in serum and hepatic MDA content declined significantly. Hepatic activity of SOD ,GSH?Px increased obviously. Liver morphologic changes improved in methimazole group and combination group,whereas no significant difference of serum FT3,FT4,TSH level and liver activities of SOD and GSH?Px was noticed in SMSJC group in comparison to model group. Compared with SMSJC or methimazole mono?therapy ,the effect of combination therapy was obvious. However ,serum TRAb level was not significantly different in five groups of rats. Conclusion SMSJC combined with methimazole plays a protective role on hyperthyroidism hepatic damage induced by L?thyroxine in rat. It is proposed that the effect is association with inhibiting hepatic oxidative stress.

OBJECTIVE:To study the anti-hepatic fibrosis effects of Yidu tiaogan mixture on rats and its mechanism. METH-ODS:60 SD rats were randomly divided into normal group,model group,colchicine group(positive control,0.2 mg/kg)and Yi-du tiaogan mixture low-dose,medium-dose and high-dose groups(2.7,5.4,10.8 mL/kg). Except for normal group,liver fibrosis model was induced in other groups,and they were given relevant medicine intragastrically during modeling,once a day,for 8 weeks. Serum levels of ALT,AST,HA,PCⅢ,CⅣ,TIMP-1,MMP-1,MMP-2,MMP-13,TGF-β1 and TNF-α and the levels of Hyp,SOD,GSH-Px,CAT,MDA,TGF-β1 and TNF-α in liver tissue were detected in rats. RESULTS:Compared with normal group,the serum levels of ALT,AST,HA,PCⅢ,CⅣ,TIMP-1,MMP-2,TGF-β1 and TNF-α were increased significantly in model group,while the serum levels of MMP-1 and MMP-13 were decreased significantly(P<0.05);the levels of Hyp,TGF-β1, TNF-α and MDA in liver tissue were increased significantly,while the levels of SOD,GSH-Px and CAT were decreased signifi-cantly(P<0.05). Compared with model group,the serum levels of ALT,AST,HA,PCⅢ,CⅣ,TIMP-1 and TNF-α as well as the levels of Hyp,TGF-β1,TNF-α and MDA in liver tissue were all decreased significantly of rats in Yidu tiaogan mixture medi-um-dose,high-dose and colchicine groups(P<0.05);the serum levels of MMP-1 and MMP-13 as well as the level of CAT in liver tissue were increased significantly in Yidu tiaogan mixture high-dose group and colchicine group(P<0.05),while the serum levels of MMP-2 and TGF-β1 were decreased significantly(P<0.05). There was no statistical significance in above indexes in Yidu tiao-gan mixture low-dose group (P>0.05). CONCLUSIONS:Yidu tiaogan mixture exerts protective effects on liver fibrosis of rat through regulating TIMP/MMP balance,suppressing oxidative stress,decreasing the level of TGF-β1 and reducing deposition of ex-tracellular matrix in liver.

BACKGROUND: The conventional X-ray imaging has low absorption contrast for lymphoedema, and the resolution is about mm level, which is not conducive to early diagnosis. Micro-imaging of biological soft tissues can be achieved by phase-contras imaging technique. OBJECTIVE: Using synchrotron radiation in-line phase contrast imaging technique to achieve the lymphoedema model imaging and observe the lesions characteristic of lymphatic vessels. DESIGN, TIME AND SETTING: Observational experiment. The lymphoedema model was prepared at the Capital Medical University from May to June in 2009. And the experiment was completed in Shanghai Synchrotron Radiation Facility (SSRF) BL13W1 in June 2009. MATERIALS: Five New Zealand rabbits were prepared for lower extremity lymphoedema models. METHODS: The lower extremity lymph node eradicates operation was performed on rabbits to result in lower extremity lymphoedema. And the synchrotron radiation in-line phase contrast imaging technology was used to collect the projected image of specimens within the 180°, and to reconstruct three-dimensional image. MAIN OUTCOME MEASURES: Severity of lymphatic vessels and the wall thickness. RESULTS: The lymphatic vessel was swollen, bent and distorted. And the lymphatic vessel generates lateral branches in order to promote lymphatic drainage, which was clearly observed from the image obtained by phase contrast imaging technology.CONCLUSION: The use of synchrotron radiation type coaxial imaging technology can be achieved for lymphedema model for micro-imaging, which has certain significance to the early diagnosis of lymphedema.

Objective　To study the scan technique that can completely demonstrate the structures of middle-inner ear.Methods　Ninety volunteers underwent 30° axial and 105° coronal HRCT scan.The manifestations of the ministructures of middle-inner ear were observed and their demonstration rate was calculated.The demonstration of different structures of middle-inner ear was evaluated and compared on HRCT.Results　(1)The scan planes of 30° axial and 105° coronal were correspondent to some structures of middle-inner ear and could completely demonstrate them.(2)30° axial and 105° coronal HRCT scan had their own advantages respectively in demonstrating the structures of middle-inner ear.Conclusion　The technique of 30° axial and 105° coronal HRCT scan of middle-inner ear is useful in demonstrating the structures,including the nerve,joint,ligament etc and the relationship among them.

Objective To explore the optimal timing of termination of pregnancy,we analyzed the different gestational age in repeat cesarean delivery and maternal and neonatal outcomes.Methods This was a retrospective study.The information of cesarean sections was collected from maternal obstetric records in the electronic medical recording system of the First Affiliated Hospital of Chongqing Medical University from June 1,2011 to June 30,2013,and women with intrauterine viable singleton pregnancies delivered after 37 weeks of gestation without prenatal complications were selected.They were divided into five groups with different gestational weeks.Maternal general information,perioperative outcome and rate of neonatal adverse event were analyzed with one way ANOVA analysis and Chi-square test.Results A total of 579 cases of elective repeat cesarean at term were performed.The ratios of cesarean section prior to 39 and 39-39+6 weeks of gestation were 64.6％ (374/579) and 29.0％ (168/579),respectively.No fetal,neonatal or maternal death occurred.There were no statistically significant differences in the termination of pregnancy at 37-37+6 weeks,38 38+6 weeks,39-39+6 weeks,40 weeks and ≥ 41 weeks between the two time intervals for cesarean section (P＞0.05).There were statistically significant differences in the length of hospitalization [(4.9±3.0),(4.3 ± 1.3),(4.3 ± 1.0),(4.5± 1.2) and (4.0±0.7) d,respectively; F=2.849,P＜0.05].No significant difference was observed in the maternal BMI,placental membrane residue,maternal perioperative bleeding,premature rupture of membrane (PROM),intensive care unit (ICU) admission and uterine resection (P＞0.05).There were statistically significant differences among the five groups in neonatal weight [(3 082.9±479.2),(3 318.1 ±390.8),(3 415.7±431.1),(3 630.5±475.2) and (3 334.0±242.5) g,F=13.798] and length [(48.8± 1.5),(49.3± 1.5),(49.6± 1.5),(50.0± 1.5) and (47.8±3.9) cm,F=7.460; both P＜0.05].One min and 5 min Apgar scores also showed statistically significant differences [1 min:(9.7±0.7),(9.8±0.6),(9.8±0.4),(9.7±0.5) and (8.8±2.7) ; F=4.432; 5 min:(9.9±0.3),(10.0±0.3),(10.0±0.2),(10.0±0.2) and (9.2± 1.8),F=9.625; all P＜0.05].The overall rates of neonatal adverse events,including the admission to neonatal intensive care units (NICU),the rates of cardiopulmonary resuscitation or ventilator therapy,asphyxiation,as well as the length of stay in NICU ≥ 5 d among the five groups also showed statistically significant differences [overall:5.4％ (5/93),1.8％ (5/281),0.6％ (1/168),0.0％ (0/32) and 2/5,x2=16.812;NICU:3.2％ (3/93),1.1％ (3/281),0.0％ (0/168),0.0％ (0/32) and 1/5; x2=1 1.294; cardiopulmonary resuscitation or ventilator therapy:2.2％ (2/93),0.7％ (2/281),0.0％ (0/168),0.0％(0/32) and 1/5,x2=10.584; asphyxiation:1.1％ (1/93),0.7％ (2/281),0.0％ (0/168),0.0％ (0/32) and 1/5,x2=9.637; NICU ≥ 5 d:3.2％ (3/93),1.1％ (3/281),0.0％ (0/168),0.0％ (0/32) and 1/5,x2=1 1.294; P＜0.05].The risks of neonatal adverse outcomes in delivery at 37-38+6 weeks were:OR=1.1(95％CI:1.0-2.1) at 37 37+6 weeks,OR=1.3 (95％CI:0.9-1.9) at 38-38+6 weeks,compared with delivery at 39-39+6 weeks.Conclnsions The percentage of repeat cesarean delivery prior to 39 weeks of gestation is high in our hospital,early termination of pregnancy would not reduce the maternal perioperative adverse outcome,but may increase the risk of neonatal adverse events.Taking into account the maternal benefit,we suggest 39 39+6 weeks of gestation as the best time of elective repeat cesarean in order to reduce the risk of neonatal adverse events.

Objective To detect the expression of BAT2L protein in developmental brain of rats.Methods Expression,distribution and location of BAT2L protein in developmental brain rats on embryonic day 18,postnatal days 1,7 and 15 and in adult at different time points were detected by Western blotting analysis,immunohistochemistry and immunofluorescence,respectively.Results Western blotting showed that the BAT2L protein expression level was higher in brain of rats on embryonic day 18,postnatal days 1 and 7 than on postnatal day 15 and in brain of adult rats.Immunohistochemistry and immunofluorescence showed that the BAT2L protein was distributed and located in cell membrane and cytoplasm of neurons but not in nuclei and extra-cellular space.Positive neurons were widely distributed in cerebral and cerebellar cortex,hippocampus and cerebral ganglion.The morphology of neurons was significantly different in newborn and adult rats.Positive prominences and branches of certain neurons were found in brain tissues of newborn rats but hardly in those of adult rats.Conclusion BAT2L protein is specifically expressed in cell body,membrane and prominence of neurons in brain of rats.

BACKGROUND: Previous studies have confirmed that embryonic stem cells call be induced to differentiate into insulin-producing cells, but the induction process takes a long time. Most of the processes take about one month.OBJECTIVE: Activin A, all-trans retinoic acid (ATRA), basic fibroblast growth factor (bFGF) and nicotinamide were applied in vitro in combination to observe whether mouse embryonic stem cells could be induct to differentiate into insulin-producing cell in a relatively short time.DESIGN: Cell observation experiment.SETTING: Shanghai Institute of Endocrine and Metabolic Diseases, Ruijin Hospital, Shanghai Jiao Tong University School of Medicine, Shanghai, China.MATERIALS: This study was performed at Shanghai Institute of Endocrine and Metabolic Diseases from October 2004 to February 2006. Two mice of clean grade and of 12.5-14.5 days of gestational age were provided by Shanghai SLAC Laboratory Animal Co., Ltd (Permission No. 2004A034). The protocol was performed in accordance with ethical guidelines for the use and care of animals. Mouse embryonic stem cell lines were supplied by Dr Changxian Zhang (CNRS UMR5641, France). Activin A was the product of the R & D Corporation. ATRA and nicotinamide were supplied by the Sigma Corporation, USA. BFGF was supplied by Gibco Corporaion. METHODS: Head and viscera were removed from embryos of the pregnant mouse. The remaining tissues were cut into pieces and digested with trypsin. Cell suspension was centrifuged and inoculated at 3×108L-1. The cells could be used as mouse feeder layer after 2-3 times of passage. The mouse embryonic stem cells (ESCs) were inoculated onto the feeder layer in knockout Dulbecco's modified Eagle medium (DMEM) supplemented with leukemia inhibitory factors (LIF). ESCs were passaged at 1:3-1:6 after 2-3 days of culture. Culture medium with serum was added into the culture dishes to terminate the digestion. Cell fluid was centrifuged and supernatant was discarded. The sediments were prepared into suspension and inoculated at 2.5×104 with LIF-free culture medium. After 24-48 hours, embryonic bodies (EBs) were collected and replated in 1% Matrigel-coated dishes. When began to adhere to the dishes, EBs were cultured in 10% FBS/DMEM supplemented with 100μg/L activin A for 24 hours. Then EBs were switched to 10% FBS/DMEM for 6-8 hours as an interval. After this interval. EBs differentiated were cultured in 10% FBS/DMEM with 10<-6mol/L RA for another 24 hours followed by culture in 10% FBS/DMEM supplemented with 10μg/L bFGFs for 3-5 days. Finally, EBs differentiated were cultured in DMEM/F12 supplemented with N2 supplement, B27 supplement, 1μg/L laminin, 10μg/L bFGFs, and 10mmol/L nicotinamide for 3-5 days. Dithizone (DTZ) staining, inununofluorescent staining and reverse transcription-polymerase chain reaction (RT-PCR) were applied to detect insulin expression in the differentiated cells.MAIN OUTCOME MEASURES: Induction of ESCs, DTZ staining and immunofluorescent staining as wel as RT-PCR detection.RESULTS: Mouse ESCs growing on a feeder layer formed many colonies with clear boundary and dense structure. However, there was no obvious outer limit between these ESCs. EBs began to adhere to the dishes, which were coated with matrigel, on the 2nd day. After activin A and ATRA interval induction, EBs spread, and most of the living cells were epithelial cell-like when cultured in 10% FBS/DMEM supplemented with 10μg/L bFGFs. After culturing in DMEM/F12 supplemented with N2, B27, nicotinamide, bFGFs and laminin, the cells formed small clusters. The insulin-producing cells were stained dark red with DTZ, and the cells stained with primary antibody to insulin were insulin-positive. After 2 weeks of induction of activin A, ATRA, bFGFs and nicotinamide, the insulin-producing cells expressed insulin 2, Pdxl, Nkx6.1, Nkx2.2, PP, IAPP, Glut2, Somastatin, Hnf3β and Neuro D mRNA but did not express insulin 1 mRNA.CONCLUSION: Mouse ESCs call be induced to differentiate into insulin-producing cells by activin A, ATRA, bFGFs and nicotinamide in vitro. Induction time call be shortened to 2 weeks.
