1.Research on sevoflurane combined with remifentanil anesthesia inabdominal surgeries

Objective To observe the anesthesia and recovery results of sevoflrane and remifentanil combined anesthesia in open or laparoscopic surgeries. Methods 60 cases of ordinary surgeries from the department of gynecology and general surgeries were included in this study with 30 cases in each group. (1)recording total sevoflurane inhalation time, muscular relaxant amount, end tidal sevoflurane concentration;(2)recording BP,HR at 10 min after induction,operation staring and ending,ventilation recovery, opening eye and extubation period;also sevoflurane concentration 5 min after stopping medicine and ventilation recovery;recording time period between surgery ending and autonomous respiration recovery , eye opening and extubation. Results No any adverse events happen in each patient.the sevoflurane inhalation time in open surgery group was (157.20 ± 47.28) min, longer than that of laparoscopic surgeries group (73.50 ± 11.23)min(P<0.05), we had seen no statistical significance in all other index observed (P>0.05). Conclusion Sevoflurane combined remifentanil anesthesia can achieve stable intra-operative maintenance and rapid postoperative recovery quality , we suggest the widespread usage of it in clinic.
2.Incidence and Drug-Resistance of Helicobacter pylori from Patients with Peptic Ulcer

OBJECTIVE To investigate incidence and drug susceptibility of Helicobacter pylori(Hp) in local area.METHODS Strains of Hp were isolated and cultivated from gastric antrum mucosa of 141 peptic ulcer disease(PUD) patients diagnosed by gastroscope.The drug sensitive test of Hp strains was examined by K-B method.RESULTS The isolation rate of Hp in 141 PUD patients was 75.2%.Hp isolates showed a relative hypersensitivity to clinical commonly used drugs(ie furazolidone,amoxicillin,levofloxacin,cefuroxime and moxifloxacin).On the other hand,Hp isolates exhibited resistance to metronidazole and clarithromycin.CONCLUSIONS Hp is an important etiologic factor of PUD,and showing an increasing resistance to common clinical antibiotics.
3.Implement Continuous Quality Improvement and Improve the Satisfaction of Nursing Quality

Objective:To explore the effect of continuous quality improvement theory on patients' satisfaction and nurses'perception of working environment.Methods:Quality nursing service satisfaction survey was conducted in 450 patients,at the same time,100 nurses were investigated for the situation of working environment.Patients' satisfaction and the status of nurses' perception of their own work environment were compared before and after the implementation of continuous quality improvement theory.Results:After the implementation of continuous quality improvement theory,both patients' satisfaction and nurses' perception of working environment were significantly improved (P < 0.05).Gonclusions:Continuous quality improvement theory can improve the satisfaction of patients with high quality nursing service,and enhance the nurses' perception of their working environment.
4.Cloning and analysis of promoter of pig copper zinc superoxide dismutase gene (CuZnSOD).

Pig copper zinc superoxide dismutase (CuZnSOD) is an important antioxidant enzyme. Some studies focused on the function of CuZnSOD gene, but the transcriptional regulation of the CuZnSOD gene is not yet fully elucidated. Therefore, the aims of the study were to determine the core promoter region and to explore its mechanism of transcriptional regulation. The 853 bp DNA sequence of 5'-flanking promoter was amplified by performing PCR. A series of CuZnSOD promoter fragments with gradually truncated 5'-end were produced by nested PCR and inserted into pGL3-Basic vector. The activities of the promoters were measured by the dual-luciferase assay system after transient transfection into the NIH/3T3 cells. The results demonstrated that there were 2 potential transcription start sites in the regions from initiation codon to -87 bp and -266 bp, respectively. The region from -383 bp to +67 bp in CuZnSOD gene promoter showed higher activity than other regions, and further deletion analysis demonstrated that the region from -75 bp to -32 bp contained an essential promoter sequence for pig CuZnSOD gene transcription. In addition, several potential transcription factor binding sites were predicted with bioinformatics method. These results suggest that these transcription factor binding sites may be involved in the transcriptional regulation of CuZnSOD gene.

5.Study of Application of Coronary Endarterectomy in Coronary Artery Bypass Graft

Objective To study the application of coronary endarterectomy (CE) in coronary artery bypass graft (CABG), and to evaluate the effect of CE plus CABG on patients with chronic total occlusion (CTO) combined with diffuse distal atherosclerosis. Methods Thirty one patients, who underwent coronary endarterectomy in our institution between Oc-tober 2009 and October 2012, were reviewed retrospectively and compared the control group with 31 patients of their age, sex, LV function, and angina class. Results Among the 31 patients, 27 patients underwent one, 4 patient underwent two coronary arteries endarterectomized. Of all these 35 total endarterectomies, 13 (37%) were at left anterior descending artery, 3(9%) were at branches of the circumflex artery, and 19 (54%) involved the right coronary artery. There was no statistical dif-ference between two groups in clinical parameters including aorta cross time, cardiopulmonary bypass time, graft runoff, pul-sation index and mechanism ventilate time (P>0.05). There was no perioperative myocardial infarctions in CE group nor in control group. All patients were followed up and no recurrent angina were present in CE group. In CE group, one patient died of fungal pneumonia and heart failure half a year after operation. Conclusion In current cardiac surgical practice, coronary endarterectomy is an indispensable adjunct to CABG. The operative mortality and major morbidity were comparable or simi-lar to coronary artery bypass grafting, but its short-term and medium-term results were more favorable than to CABG.
6.Mean color vessel density with contrast-enhanced ultrasound and microvessel density: comparative study

Objective To investigate the correlation between quantitative parameters with contrast-enhanced ultrasound-power Doppler(CEUS-PD) and mierovessel density (MVD) in hepatoeellular carcinoma (HCC). Methods Thirty-four cases with HCC underwent CEUS before operation. The satisfactory images were selected through CEUS-PD and analyzed with computer to calculate mean color vessel density(MCVD). The blood flows of focal lesions were graded into 0-Ⅲ based on the numbers of color vessels with CEUS-PD. The correlation between those parameters and MVD counted with immunohistochemistry was analyzed after operation. Results In 34 cases with HCC, the MCVD of focal lesions with CEUS-PD was 0. 17±0.09, the pathologic MVD was (62.59 ±23.96)/400 × after operation. The MCVD was positively correlated to MVD( r = 0. 56, P <0.05). In 34 cases HCC,3 cases were in grade Ⅰ , 12 cases were in grade Ⅱ , 19 cases were in grade Ⅲ. There was significant differences in pathologic MVD of every grade ( F = 8.06, P <0.05). But there was no linear tendency in the grades( F = 7.57, P<0. 05). Conclusions In focal lesions of 34 HCC, MCVD calculated with CEUS-PD was positively correlated to pathologic MVD. It can be used to evaluate tumor blood perfusion exactly before operation.
7.Generation of transgenic rabbits expressing swine PID1 by testicular injection

Objective The aim of this study was to investigate the relationship between phosphotyrosine interac-tion domain containing 1 ( PID1 ) gene and variation in intramuscular fat ( IMF ) content and the possibility to generate transgenic animals by testicular injection .Methods Expression vector pIRES2-acGFP-PID1 carrying pig PID1 gene was incubated with transfection reagents and injected into the testes of male New Zealand rabbits .We examined the F1 genera-tion by fluorescence detection , PCR, Western blotting and measuring the IMF content .The F1 generation gave birth to the F2 generation.Then we examined the F2 generation through detecting the positive rate and the IMF content .Results The exogenous PID1 gene and fluorescent protein gene were expressed at different levels both in the F 1 generation and the F2 generation, and the positive rates were 35.88%and 34.33%, respectively.The IMF content was significantly elevated (P<0.05) in the transgenic positive individuals compared with the negative ones and the control group , and the PID1 protein expression was similarly higher .Conclusions The results of this study demonstrate that PID 1 gene affects intramuscular fat content significantly .Moreover, the results of our analysis provide further evidence that transgenic animals can be gener -ated by testicular injection , and the exogenous gene can be inherited steadily .
8.Study on hearing impairment at high frequency among the flight cadets.

OBJECTIVE:
To study the incidence of hearing loss at high frequency and the related influence factors among the flight cadets.
METHOD:
Using multi-stage sampling method, 312 flight cadets were randomly selected from grade 2011,2012,2013. The level of binaural hearing threshold at 4 kHz, 6 kHz, 8 kHz were measured by hearing-assistant evaluative apparatus. Whether or not have hearing loss was chosen as dependent variable. Territory, smoking, dietary habit, previous history of tinnitus, the noise exposure time, the vestibular function and the psychological quality were chosen as independent vailables. T test, ANOVA and accumulative logistic regression were performed to analyze the factors influence on hearing impairment by software SPSS 18.0.
RESULT:
The morbidity of hearing impairment among flight cadets was 18.9%. Results from single factor analysis showed that the levels of hearing thresholds at 4 kHz, 6 kHz frequency had statistically significant differences between smoking group and non-smoking group (P < 0.05). The levels of hearing thresholds at 4 kHz frequency had statistically significant differences between spicy diet group and not spicy diet group (P < 0.05). The levels of hearing thresholds at 4 kHz, 6 kHz, 8 kHz frequency had statistically significant differences among different strong noise exposure groups (P < 0.05), and that at a same frequency hearing loss increased when noise exposing increased. The levels of hearing thresholds at 4 kHz, 6 kHz frequency had statistically significant difference among different vestibular function groups (P < 0.05). Results of accumulative logistics regression showed that smoking and strong noise exposure were risk factors causing hearing impairment at 4 kHz frequency, and excellent vestibular function seemed to be a preventive factor. Smoking and strong noise exposure were also risk factors causing hearing impairment at 6 kHz frequency.
CONCLUSION
Hearing impairment appears higher morbidity among flight cadets, and it has statistical correlation with smoking, strong noise exposure and vestibular function.

9.Clinical research of the otolith abnormal migration during canalith repositioning procedures for posterior semicircular canal benign paroxysmal positional vertigo.

OBJECTIVE:
To investigate the risk factor,type and characteristic nystagmus of the otolith abnormal migration during diagnosis and treatment for posterior semicircular canal benign paroxysmal positional vertigo (PSC-BPPV). The therapy and prevention is also discussed.
METHOD:
Four hundred and seventy-nine patients with PSC-BPPV were treated by Epley's canalith repositioning procedures(CRP) from March 2009 to March 2012. We observed otolith abnormal migration complicating during diagnosis and treatment. According the type of otolith abnormal migration, the additional repositioning maneuver was performed.
RESULT:
The rate of complication was 8. 1%(39/479), with canal conversion in 5.4%(26/479) and primarily canal reentry in 2.7%(13/479). The rate of incidence of conversion to horizontal canal conversion and anterior canal were 4. 8%(23/479)and 0. 6%(3/479) respectively. All the patient was cured in follow up. The risk factors were unappropriated head movement during or after CRP, including another Dix-Hallpike were performed immediately.
CONCLUSION
To prevent the complications,the pathognostic positioning sequence and angle of head rotation are commenced during CRP. Appropriate short time postural restrictions post-treatment is necessary. Careful observation of nystagrnus variation is crucial to determine the otolith abnormal migration.

10.Application of impulse response method in bone biomechanics research

Bones are important supporting structure of human body. Impulse response method is a kind of vibration analysis technology which can be used to analyze dynamic characteristics of human bones. Along with the continuous development of signal processing and computer technology, impulse response method is applied more and more in bone biomechanics research. The basic principle of impulse response method and its progress in bone biomechanics research are briefly summarized in this paper. The article also discussed the pulse response method research direction.
