2.Clinical Observation of Ecthyma Treatment under Blue and Red Light Phototherapy Combined with TCM External Treatment

OBJECTIVE:To observe clinical efficacy and ADR of blue and red light phototherapy combined with TCM exter-nal treatment in the treatment of ecthyma. METHODS:64 ecthyma patients were randomly divided into experimental group and control group by number with 32 cases in each group. Both groups were provided with intravenous drip of Erigeron injection and Shenxiong glucose injection to improve limb microcirculation. The experimental group was also provided with blue and red light phototherapy (blue light 20 min+ red light 20 min) combined with TCM external treatment once a day;while control group was provided with only TCM external treatment once a day. After 1 month treatment,clinical efficacy and healing time of 2 groups were observed and compared,and ADR monitoring was conducted. RESULTS:The cure rate of experimental group was 78.13%, and the total effective rate was 96.88%,which were higher than those of control group(40.63% and 81.25%),there was statisti-cally significant difference (P<0.05). The average healing time of experimental group was (18.75 ± 5.97) d,and that of control group was(27.12 ± 6.67)d;there was statistically significant difference between 2 groups (P<0.05). No ADR was found in 2 groups during treatment. CONCLUSIONS:Blue and red light phototherapy combined with TCM external treatment in the treatment of ecthyma has the remarkable curative effect and good safety,and shorten the healing time.
3.Correlation between TCM Syndromes and Gastrointestinal Dyskinesis in Patients with Chronic Obstructive Pulmonary Diseases and Cor Pulmonale

Objective To study the correlations between the TCM syndromes of gastrointestinal dyskinesis and the disorders of gastrointestinal hormone,electrogastrogram(EGG) and blood gas in patients with chronic obstructive pulmonary diseases(COPD) and cor pulmonale.Methods The 120 cases of COPD and cor pulmonale were diagnosed with TCM syndrome differentiation,and plasma motilin(MOT),vasoactive intestinal peptide(VIP),Gastrin(GAS),EGG and blood gas were anglyzed and compared with 26 healthy subjects.Results With the aggravation of COPD and cor pulmonale,the dyskinesis of stomach and intestine was getting worse,developed from Shi-excess to Xu-deficiency.In deficiency syndrome,basic electrorhythm decreased most obviously.In phlegm-damp in lung syndrome,MOT and GAS decreased most obviously.In lung-kidney deficiency syndrome and lung-spleen-kidney deficiency syndrome,VIP increased most obviously.In lung-spleen-kidney deficiency syndrome,anoxia and carbon dioxide retention were most serious but GAS increased.Conclusion All patients of COPD and cor pulmonale have dyskinesis of stomach-intestine to a certain degree,abnormal EGG and disordered gastrointestinal hormones.Severity of ventilatory disturbance,quantity of injured organs,and condition of anoxia due to gastrointestinal blood stasis are all correlated with the development of COPD and cor pulmonale.
4.Analysis of errors of computer-assisted cervical pedicle screw installation

[Objective]To evaluate the accuracy of computer-assisted cervical pedicle screw installation and discuss the reasons of screw malposition.[Method]Totally 144 cervical pedicle screws were installed by computer-assisted navigation in 25 patients with lower cervical spinal disease.All the patients were examined by X-rays and CT after operation.Screw position and direction were measured on the sagittal and transsection image of intraoperative navigation and post-operative CT.The result was analyzed statistically.[Result]Two screws perforated the upper pedicle wall,3 deviated from the lateral pedicle wal1.There was no statistical difference in screw position or direction between navigation and CT image.Navigational images were based on the principle of rigid tissue.[Conclusion]Computer-assisted navigation can improve the accuracy of cervical pedicle screw installation.Screw malposition is responsible for the excursion phenomenon.
5.Clinical Observation of Zhenqi Fuzheng Granules in the Adjunctive Treatment of Cervical Cancer

OBJECTIVE:To observe therapeutic efficacy and safety of Zhenqi fuzheng granules in the adjunctive treatment of cervical cancer. METHODS:86 patients with cervical cancer were randomly divided into control group(43 cases)and observation group (43 cases). Control group was given intravenous dripping of Diphenhydramine hydrochloride injection 20 mg,Granisetron hydrochloride and glucose injection 5 mg,Cimetidine injection 20 mg at 0.5 h before chemotherapy. After 0.5 h of intravenous drip-ping,the patients received chemotherapy:Docetaxel injection 40 mg/m2,d1,d8,intravenous dripping;Cisplatin for injection 25 mg/m2,d1-3,intravenous dripping;21 d as a cycle,a total of 4-6 cycles. On the basis of control group,observation group was addi-tionally given Zhenqi fuzheng granules 5 g,twice 2 day. Clinical efficacies of 2 groups were observed as well as T cell subsets in-dexes [CD3+,CD4+,CD8+,natural killer cell(NK)],KPS scores,termination,terminal time,the times of chemotherapy and the oc-currence of toxic reaction before and after treatment. RESULTS:The total remission rate of observation group was significantly higher than that of control group;the proportion of termination of chemotherapy in observation group was significantly lower than control group;terminal time of chemotherapy was significantly shorter than control group;the times of chemotherapy was signifi-cantly more than control group;the incidence of gastrointestinal reaction,anemia,thrombocytopenia and leukopenia were signifi-cantly lower than control group,with statistical significance(P<0.05). There was no statistical significance in liver function dam-age,renal function injury and stomatitis between 2 groups (P>0.05). Before treatment,there was no statistical significance in T cell subsets indexes and KPS scores between 2 groups(P>0.05). After treatment,CD3+,CD4+ and NK cells in observation group were significantly higher than before treatment and control group,while CD8+ was significantly lower than before treatment and control group;KPS scores of 2 groups were significantly higher than before,and the observation group was significantly higher than the control group,with statistical significance(P<0.05). There was no statistical significance in T cell subsets indexes of con-trol group before and after treatment(P>0.05). CONCLUSIONS:On the basis of routine treatment,Zhenqi fuzheng granules can improve therapeutic efficacy,immune function and the quality of life,moreover,reduce toxic reaction in patients with cervical can-cer.
6.Expression of Aquaporins in the Inner Ear of Normal Guinea Pigs and Guinea Pigs with Endolymphatic Hydrops

Objective To study the expression of aquaporins(AQPs) in the inner ear of normal guinea pig and guinea pigs with endolymphatic hydrops, and to investigate the mechanism and significance. Methods The guinea pigs were divided into two groups randomly: model group and control group. The guinea pigs in model group were given desmopressing 4 ?g?kg-1?d-1 introperitoneally for 1 week, while the guinea pigs in control group were given saline. After one week, the expression of AQPs in the inner ears of all guinea pigs was evaluated.Results Expression of AQP0, 1, 2, 3, 5, 7 and 8 were found in the ears of normal guinea pigs. Weak expression of AQP0 was found in stria vascularis and spiral ganglion. The distribution of AQP1 consisted of cellular lining the bony labyrinth, fibrocytes lining in the endolymphatic duct and sac, cells under the basilar membrane, fibrocytes of the spiral ligament and the spiral limbus, Corti's organ, inner and outer spiral sulcus, stria vascularis, saccular and utricular wall, and spiral ganglion. AQP2 were found in stria vascularis, Corti's organ, spiral ganglion and endolymphatic sac. AQP3, 7 and 8 were distributed in a similar manner as the surrounding membranous labyrinth, including Corti's organ, inner and outer spiral sulcus, stria vascularis, fibrocytes of the spiral ligament and the spiral limbus, saccular and utricular wall, endolymphatic sac and spiral ganglion. AQP5 was found at Corti's organ, inner and outer spiral sulcus, spiral ganglion and fibrocytes in spiral ligment. The AQP2 in stria vascularis and endolymphatic sac was stronger in the inner ears with ELH than that in normal ears.Conclusion There are various AQPs in inner ears of normal guinea pigs, and their distributions are overlapping and without obvious regional specificity. Desmopressing induces the expression of AQP2 in the inner ears of guinea pigs. The results indicate that ELH may correlate to upregulation of AQP2, but the mechanism remains unclear.
7.Immunohistochemical evaluation of mutant p53 protein over-expression in non-mucinous adenocarcinoma in-situ and invasive adenocarcinoma, NOS of lung.

OBJECTIVETo study the over-expression of mutant p53 protein in non-mucinous adenocarcinoma in-situ (NMAIS) and invasive adenocarcinoma, NOS of lung.
METHODSImmunohistochemical study for p53 protein was performed on 17 cases of NMAIS and 70 cases of invasive adenocarcinoma, NOS of lung. The difference in p53 over-expression between the two tumor subtypes was analyzed.
RESULTSThe over-expression of mutant p53 protein was observed in 0 case (0%) of NMAIS and 37 cases (52.9%) of invasive adenocarcinoma, NOS of lung. The difference was of statistical significance (P = 0.000).
CONCLUSIONMutant p53 protein over-expression may play a role in the progression of NMAIS to invasive adenocarcinoma, NOS.

8.Research on sevoflurane combined with remifentanil anesthesia inabdominal surgeries

Objective To observe the anesthesia and recovery results of sevoflrane and remifentanil combined anesthesia in open or laparoscopic surgeries. Methods 60 cases of ordinary surgeries from the department of gynecology and general surgeries were included in this study with 30 cases in each group. (1)recording total sevoflurane inhalation time, muscular relaxant amount, end tidal sevoflurane concentration;(2)recording BP,HR at 10 min after induction,operation staring and ending,ventilation recovery, opening eye and extubation period;also sevoflurane concentration 5 min after stopping medicine and ventilation recovery;recording time period between surgery ending and autonomous respiration recovery , eye opening and extubation. Results No any adverse events happen in each patient.the sevoflurane inhalation time in open surgery group was (157.20 ± 47.28) min, longer than that of laparoscopic surgeries group (73.50 ± 11.23)min(P<0.05), we had seen no statistical significance in all other index observed (P>0.05). Conclusion Sevoflurane combined remifentanil anesthesia can achieve stable intra-operative maintenance and rapid postoperative recovery quality , we suggest the widespread usage of it in clinic.
9.Comprehensive Evaluation of the Diagnostic Value of the Combined Detection of IMA,NLR,hs-CRP and CK-MB for Acute Myocardial Infarction Based on ROC Curve and Logistic Regression Analysis

Objective To assess the diagnostic value of IMA,NLR,hs-CRP and CK-MB individually and the combined detec-tion for early acute myocardial infarction using ROC curve and Logistic regression.Methods To detect levels of IMA,NLR, hs-CRP,CK-MB and cTnI in serum or whole blood of AMI patients that had chest pain within 3 hours or between 3 and 6 hours,compared with 60 healthy people from Physical Examination Center.Applied Logistic regression,plotted ROC curve and calculated the area under ROC curve (AUC)to assess the diagnostic value of each index.Results The serum IMA,hs-CRP,CK-MB and cTnI or whole blood NLR levels of AMI patients with 3 hours were remarkably higher than normal con-trol,showing significant statistical difference (P<0.01)(AMI group:mean values of IMA,NLR,hs-CRP,CK-MB and cTnI were 96.04 U/L,3.77,13.39 mg/L,43.26 U/L and 0.063 ng/ml;normal control group:mean values of IMA,NLR,hs-CRP,CK-MB and cTnI were 78.10 U/L,2.02,3.12 mg/L,19.37 U/L and 0.040 ng/ml.The serum IMA,NLR,hs-CRP, CK-MB and cTnI levels of AMI patients in the group between 3~6 hours were higher than in the group within 3 hours (P<0.05).The AUC of combined detection of IMA,NLR,hs-CRP and CK-MB for early AMI was 0.98,higher than solo de-tection of IMA,NLR,hs-CRP and CK-MB,which were 0.89,0.83,0.79 and 0.85 respectively.Meanwhile,the AUC of com-bined detection for four markers also surpassed that of cTnI alone that was recognized as a classic serological marker to diag-nose AMI (AUC=0.78).Conclusion The combined detection of IMA,NLR,hs-CRP and CK-MB is superior to a single in-dex detection,which can significantly improve diagnostic efficiency for early AMI.
10.Determination of Five Pesticides in Fishpond by SPE-GC/MS

Objective To establish the solid phase extraction (SPE) with GC/MS technology for fish poi-soning cases to determine five pesticides in fishpond. Methods By three solid phase extraction column including Oasis HLB cartridge, Bond Elut C18 and SampliQ C18, the recovery rate was compared to ex-tract and purify five pesticides in fishpond. The effects of different kinds and dosages of eluents on ex-tract rate were also reviewed. Results Using Bond Elut C18 as solid phase extraction column and 3 mL benzene as eluent, the linear range of mass concentration of five pesticides in fishpond was 1-50 μg/mL, and the correlation coefficient was 0.996 2-0.999 6. The limit of detection was 3.4-26 μg/L and the re-covery was 61.49%-102.48%. The relative standard deviations was less than or equal to 3.01%. Conclu-sion With high sensitivity, good accuracy and precision, SPE -GC/MS has simple and quick operation and less solvent. It can be applied to determination of five pesticides in fishpond.
