1.Content Determination of the 3 Constituents in Jiangzhiyin by HPLC Simultaneous

OBJECTIVE:To establish a method for simultaneous determination of the effective constituents rhein,emodin and chrysophanol in jiangzhiyin by HPLC.METHODS:SHIMADU VP-ODS column was used as analytical column,methanol-0.2%phosphoric acid(80∶20)as mobile phase,the flow rate was set at1ml/min,the detection wavelength at225nm,column temperature at room temperature.RESULTS:The linear ranges of detected rhein,emodin and chrysophanol in jiangzhiyin were0.015~0.15?g(r=0.9996),0.014~0.14?g(r=0.9998)and0.0085~0.085?g(r=0.9993)respectively;The average recoveries were99.26%(RSD=2.00%),98.72%(RSD=1.80%)and99.76%(RSD=2.3%)respectively.CONCLUSION:The determination results are stable and accurate,and the method is feasible and reliable,which can be used for the quality control of jiangzhiyin.
2.Exploration of Rational Use of Injection

OBJECTIVE:To discuss rational use of injection in order to provide reference for rational use of injection in the clinic. METHODS:Drug use was analyzed in respect of route of administration,selection of solvent,compatibility of drugs, speed of intravenous administration,the concentration of mixture,etc. RESULTS&CONCLUSION:There are some mistakes in the application of injections,but some faults and adverse drug reactions (ADRs) can be warned early. By strengthening pharmaceutical monitoring,related faults and ADRs can be reduced to the minimum.
3.Hormone therapy for tuberculous meningitis patient with history of hormone allergy

Objective The aim is to provide drug regimen for patients who have hormone allergy history but need hor-mone therapy.Methods Pharmacists analyzed the sensitization mechanism of methylprednone in combination with insturction and literature,assisted clinicians to formulate oral hormone therapy for tuberculosis meningitis with history of menthylprednisolone and verified the effectiveness and safety of the drug regimen.Results Methylprednisolone injection contains trace amount of milk protein and should not be used in patients with definite allergy to cow's milk.After changing to oral hormone therapy,the patient did not appear allergic again,and the condition was stable.Conclusion Gucocorticoids can also induce allergic reactions,so patients'previous allergic history should be paid attention in the process of medical history collection.Meanwhile,pharmacists can also give full play to their pharmaceutical expertise to assist clinicians in formulating individualized drug administration plans to ensure the effectiveness and safety of patients.
4.Analysis of adverse events related to linezolid in tuberculosis treatment based on the US FAERS database

Objective To analyze the adverse events(AEs)associated with linezolid in tuberculosis treatment reported in the FDA Adverse Event Reporting System(FAERS)database,assess its safety profile,and provide scientific evidence for ra-tional clinical use.Methods Reports of adverse events related to linezolid in tuberculosis treatment from Q1 2016 to Q1 2024 were extracted.The data were cleaned and pre-processed,and signal detection was performed using the Reporting Odds Ratio(ROR)and Information Component(IC)methods.Results A total of 1,135 linezolid-related AEs were identified,with the most common types being hospitalization(18.46%)and death(10.99%).The Results indicated that linezolid AEs primarily involved disorders of the blood and lymphatic system,as well as liver and biliary system diseases.Common adverse reactions in-cluded anemia,peripheral neuropathy,and optic neuropathy.Additionally,some adverse reaction signals not mentioned in the prescribing information were identified,such as polyneuropathy(ROR=75.25,IC025=5.8),toxic optic neuropathy(ROR=776.95,IC025=8.86),and hearing loss(ROR=14.66,IC025=3.26).Conclusion This study suggests that linezolid may be associated with some serious adverse reactions in tuberculosis treatment.It is recommended that patients on long-term linezolid therapy undergo regular neurological,ophthalmological,and auditory assessments to provide a more adequate basis for rational clinical use.
5.The influencing factors of particle formation during the configuration of rifampicin for injection were investigated

Objective To investigate the change of infusion particles and PH value of rifampicin for injection under dif-ferent configuration and application conditions,and to provide the best configuration method and the best infusion quality control strategy for rifampicin for injection.Methods The particle number and PH value of rifampin for injection were investigated and measured under different configuration and application conditions,and the factors affecting the formation of particles were ana-lyzed.Results The stable pH value of Rifampicin for injection was 7.2-8.1,and the number of particles was the least in the light filtering and 5%glucose 500 mL,and the number of particles was the least in 1~4 h after the configuration.Conclusion The optimal infusion quality control strategy is to filter rifampicin for injection by blocking light,500 mL of solute 5%glucose,1~4 h after configuration,and use of disposable precision filter infusion set.
6.Clinical analysis of pulmonary tuberculosis complicated with Nocardia farcinica

Objective To analyze the clinical characteristics of pulmonary tuberculosis patients compli-cated with infection of Nocardia farcinica aiming to improve the diagnosis and treatment the disease.Methods The clinical data of 22 cases of pulmonary tuberculosis complicated with Nocardia farcinica infection admitted to Guang-zhou Chest Hospital from June 2020 to December 2023 were collected and the clinical manifestations,imaging,laboratory tests,treatment process,and disease outcomes were retrospectively analyzed.Results Among the 22 patients,there were 13 males and 9 females,aged 20~86 years,with a median age of 52 years.Common clinical manifestations included cough(22/22),sputum(21/22),and underlying diseases(13/22).One case was positive for cerebrospinal fluid culture,and 21 cases were positive for sputum culture.The culture period was 5~26 days,with a median culture period of 18 days.The imaging manifestations were mainly plaques,plaques and cavities,and the lesions were spread in both lungs(17/22)and cavities(11/22).After anti-tuberculosis treatment,the absorption of lung lesions in some patients was poor,and the absorption of the lesions was improved after anti-nocardia treat-ment.8 cases were cured,13 cases were improved and 1 case died.Conclusion The clinical symptoms of pa-tients with pulmonary tuberculosis complicated with Nocardia farcinica were atypical,the culture period of Nocardia was long,and the imaging manifestations were similar to pulmonary tuberculosis,which is prone to misdiagnosis and missed diagnosis.For patients with poor response to anti-tuberculosis treatment and slow lesion absorption,the possi-bility of a concurrent Nocardia infection should be considered.
