1.Lumbar Spinal Stenosis Associated with Disc Protrusion

Sixty-eight patients with a surgically proven lumbar stenosis associated with disc protrusion were studied retrospectively. According to the essential clinical feature the patients were divided into three groups: Group 1, intermittent claudication (n=21), Group 2, low back pain associated with unilateral aciatic pain (n=42), and Group 3, intermittent claudication coexisting with bilateral or unilateral leg pain (n=5). The final diagnosis prior to operation was difficult to make. Surgical decompression of the spinal canal required removal of the portion of hypertrophied superior articular faced and prolapsed disc. Fifty-eight patients were followed up from 6 months to 6 years and 3 months. In 41 patients preoperative neurogenic symptoms completely disappeared, 14 still an occasinal bachache, existed and 3 moderate symptoms persisted postoperatively.
2.TREATMENT OF THE COMPRESSION OF THE CERVICAL SPINAL CORD WITH EXTENDED HEMILAMINECTOMY

Extended hemilaminectomy was performed in the treatment of compression of the cervical spinal cord produced by ossification of the posterior longitudinal ligament, multilevel degeneration and protrusion of the intervertebral discs and simple spinal canal stenosis. The results showed that with this operation the cross sectional area of the spinal canal was enlarged by 50% of the original, and the sagittal diameter lengthened 2,5mm postoperatively, so the compression of the. spinal cord was relieved. Since the function of the spinal process, the supra-and interspinal ligaments and the structure of one side of the lamina were preserved, mechanical stability of the cervical spine was maintained.
3.STUDY OF RE-OPERATION IN LUMBAR DISC HERNIATION AND LUMBAR STENOSIS

One hundred and seven patients with lumbar disc hefniation and lumbar stenosis with poor operative results are presented in this paper. According to clinical manifestation and imaging features, appropriate treatment modality was selected for each patient. Seventy-two cases were reoperated, among which excellent and good results were obtained in 60 cases (83.3%). Thirty-five patients were treated with conservative method, 28 cases improved and 7 cases exhibited no changes.The causes of operative failure and diagnostic criteri on of lumbar disc herniaton and lumbar spinal stenosis are described in detail. The indications for re-operation were discussed.
4.Application study of cerebral blood flow circulation time by contrast-enhanced ultrasound in noninvasive intracranial pressure monitoring

Objective:To evaluate the cerebral blood flow circulation time (CCT) by contrast-enhanced ultrasound, and to explore the change rule of CCT in different degree of intracranial pressure, so as to provide a new method for non-invasive monitoring of intracranial pressure.Methods:Ten patients with hemorrhagic stroke or acute craniocerebral trauma with increased intracranial pressure were selected from Tangdu Hospital, the Air Force Military Medical University from January to December 2019. Contrast-enhanced ultrasound was performed when the invasive intracranial pressure (iICP) increased (>20 mmHg, iICP increased group) and decreased to normal (≤20 mmHg, iICP normal group), CCT was measured and analyzed. The differences of CCTs between different iICP groups were compared and the relationship between CCT and iICP was analyzed.Results:①The CCT on the lesion sides of the same patients in the iICP increased group was significantly longer than in the iICP normal group[(9.34±2.58)s vs (6.48±1.91)s, P=0.002]. ②When iICP was increased in patients with hemorrhagic stroke or acute craniocerebral trauma, the CCTs of the diseased side and the non-pathological side were not statistically significant [(9.34±2.58)s vs (9.01±3.22)s, P=0.809]. ③Pearson correlation analysis and Spearman rank correlation analysis showed that there were no correlations between patient′s breathing, heart rate, carbon dioxide partial pressure, body temperature, GCS score and CCT (all P>0.05). Age, mean arterial pressure and CCT were moderately correlated ( r=0.518, 0.463 and P=0.023, 0.046, respectively). ④Logistic regression analysis showed that CCT was an independent risk factor related to intracranial hypertension( OR=0.7, 95% CI=0.47-0.95, P=0.036). The area under ROC curve (AUC) predicted by logistic regression was 0.750(0.588~0.912). Conclusions:Contrast-enhanced ultrasound noninvasive assessment of CCT can reflect the intracranial pressure in patients with hemorrhagic stroke or acute traumatic brain injury, and CCT has a predictive value for intracranial hypertension. When the patient has limited conditions for invasive intracranial pressure monitoring, or when the invasive monitoring probe is pulled out but still needs to evaluate intracranial pressure, the change of CCT can provide an effective reference for clinical diagnosis and treatment.
5.The practice of and reflection on hospital publicity in post-epidemic era

In the wake of the COVID-19 pandemic, only through whole-hearted cooperation and joined efforts can medical institutions, governments, and social organizations cope with emergency, convert the crisis into opportunities, promote modern governance and accomplish high-quality development in the fight against the pandemic. Taking the First Affiliated Hospital of Xi’an Jiaotong University as an example, this paper analyzes the all-media communication matrix of "one network platform, 3W + N" that the hospital established during the pandemic by combining the authoritative official website with WeChat, Wesee, Weibo and that other publicity media. The study explores how to create a publicity brand that is mindful, poignant, compassionate and devoted and maintains its scientific and refined management, so as to achieve the professional goal of educating the public, shouldering the responsibility in the combat against the pandemic, and taking the lead in the research on the pandemic.
