1.Efficacy of adefovir dipivoxil combined with interferon α-2b in treatment of chronic hepatitis B patients

Objective To investigate clinical efficacy of adefovir dipivoxil (ADV) combined with interferon α-2b in treatment of chronic hepatitis B.Methods During Jan 2008 to Dec 2014,ninety patients with chronic hepatitis B were divided into adefovir dipivoxil group (33 cases),interferon group (28 cases),and adefovir dipivoxil combined with interferon α-2b group (29 cases).Alanine aminotransferase (ALT) normalization rate,negative rate of HBV-DNA,and HBeAg/anti-HBe seroconversion rates were evaluated among three groups.Results After 48 weeks of treatment,HBeAg negative rate,HBV-DNA negative rate,and ALT normalization rate of combination group were significantly higher than that of interferons αt-2b group and adefovir dipivoxil group (P < 0.05).Conclusions Adefovir dipivoxil combined with interferon α-2b treatment can inhibit the hepatitis B virus,increase the negative rate of HBV-DNA and HBeAg,and reduce liver cell damage.
2.A case of infratemporal fossa abscess cause by facial acupuncture.

The infratemporal fossa abscess is an irregularly shaped cavity, situated below and media to the zygomatic arch. It consists of masseter and nervus vascularis, etc. Comparatively speaking, the low position of the infratemporal fossa has almost freed the patient from dental, nasal,ocular and aural source of pain. So facial acupuncture is more likely to cause the infection. But it is also possible that the patient get cryptogenic infection of maxillofacial cavity, the treatment of which needs further study.

3.Lipid synthesis by an acidic acid tolerant Rhodotorula glutinis.

Acetic acid, as a main by-product generated in the pretreatment process of lignocellulose hydrolysis, significantly affects cell growth and lipid synthesis of oleaginous microorganisms. Therefore, we studied the tolerance of Rhodotorula glutinis to acetic acid and its lipid synthesis from substrate containing acetic acid. In the mixed sugar medium containing 6 g/L glucose and 44 g/L xylose, and supplemented with acetic acid, the cell growth was not:inhibited when the acetic acid concentration was below 10 g/L. Compared with the control, the biomass, lipid concentration and lipid content of R. glutinis increased 21.5%, 171% and 122% respectively when acetic acid concentration was 10 g/L. Furthermore, R. glutinis could accumulate lipid with acetate as the sole carbon source. Lipid concentration and lipid yield reached 3.20 g/L and 13% respectively with the initial acetic acid concentration of 25 g/L. The lipid composition was analyzed by gas chromatograph. The main composition of lipid produced with acetic acid was palmitic acid, stearic acid, oleic acid, linoleic acid and linolenic acid, including 40.9% saturated fatty acids and 59.1% unsaturated fatty acids. The lipid composition was similar to that of plant oil, indicating that lipid from oleaginous yeast R. glutinis had potential as the feedstock of biodiesel production. These results demonstrated that a certain concentration of acetic acid need not to be removed in the detoxification process when using lignocelluloses hydrolysate to produce microbial lipid by R. glutinis.

4.Umbilical cord blood stem cell transplantation via hepatic artery in treatment of liver cirrhosis

Thirty four patients with decompensated cirrhosis received umbilical blood stem cell transplantation via hepatic artery from December 2008 to August 2009. The symptoms and biochemical testing results were observed before and after stem cell transplantation. One month after the transplantation,appetite was improved in 32 patients ( 94% ), physical capacity improved in 29 patients ( 85% ) and the abdominal distention relieved in 28 patients (82%). Abdominal B-ultrasonography showed that ascites decreased in 15 cases(60% ), and disappeared in 9 cases(36% ). Six month later the mean serum albumin levels were increased from ( 30. 9 ± 0. 6) g/L to ( 35.5 ± 2. 5 ) g/L( P > 0. 05 );alanine aminotransferase levels decreased from (57 ±7) U/L to (46 ± 10) U/L(P >0. 05);total bilirubin decreased from (47 ±5 )μmol/L to (25 ±4) μmol/L(P =0. 017);prothrombin time decreased from ( 17.9 ±0. 7) s to ( 16. 4 ±1.3) s (P > 0. 05 ). Hemophagocytic syndrome developed in one case but no severe adverse reactions occurred. The results indicate that umbilical blood stem cell transplantation may improve liver function in various degrees with safety for patients with liver cirrhosis.
5.Ion mechanism of isoproterenol on delayed afterdepolarization and triggered activity in the infarcted ventricle

Objectives This study aimed at investigating the cellular mechanism of isoproterenol (ISO) on delayed afterdepolarizations (DADs) and triggered activity (TA) of the noninfarcted myocardium in the myocardial infarcted rabbit model.Methods Rabbits with the left anterior descending coronary artery occlusion were prepared and recovered for 8 wk (healed myocardial infarction, HMI). Myocytes were isolated from regions of the noninfarcted left ventricular free wall. ISO was added to cellular surface by perfusion way. Action potentials and ion currents were recorded with whole-cell patch clamp. Results The results showed that treatment with ISO induced more DADs and TA events in HMI myocytes. Iti and ICa-L of myocytes treated with ISO were increased significantly compared with HMI cells, which contributed to DADs-related triggered arrhythmia. Conclusions The results suggested that more arrhythmia events of DADs and TA developed in myocytes with ISO treatment. The underlying mechanism was associated with the augment of Iu and calcium influxing.
6.Normal range of serum uric acid in quails of different strains and genders

Objective To explore the normal range of serum uric acid in quails.Method The data were collect-ed from male and female quails of different strains in 11 experiments, and analyzed by the way of medical reference value. Result The mean value of serum uric acid of male Difake strain quails was (221.06 ±79.59)μmol/L,and the normal range was (87.32 to 382.34)μmol/L.The mean value of serum uric acid of female Difake strain quails was (189.85 ± 68.58)μmol/L, and the normal range was ( 72.72 to 369.73 )μmol/L.The mean value of serum uric acid of male Longcheng strain quails was (217.22 ±72.91)μmol/L,and the normal range was (82.92 to 360.24)μmol/L.The mean value of serum uric acid of female Longcheng strain quails was ( 197.27 ±66.84 )μmol/L, and the normal range was (95.36 to 348.73)μmol/L.The serum uric acid value of female quails was significantly lower than that of male quails. There was no significant difference in the value of serum uric acid between female and male quails from each day-age groups.Conclusions This study has established the normal ranges for serum uric acid in quails of different strains and genders.
7.Clinical Study on Treating Mycoplasma Pneumoniae Pneumonia in Children by TCM External Treatment Combined with Dasangju Mixture

Objective To investigate the clinical efficacy of TCM external treatment combined with Dasangju Mixture in treating mycoplasma pneumoniae pneumonia (MPP) in children. Methods One hundred patients with MPP were randomly divided into control group and treatment group by parallel design, with 50 patients in each group. Patients in control group orally took azithromycin combined with other symptomatic treatment when necessary, 7 d for a course and continuously for 3 courses. Based on the treatment for control group, patients in treatment group were added with TCM external treatment (BL13 and RN17) combined with Dasangju Mixture, while azithromycin was sequentially used just for 2 courses. Other types of treatment lasted for 3 courses. The improvement time of symptoms and signs of the patients were recorded and the therapeutic effects of the two groups were evaluated. Moreover, the serum concentrations of mycoplasma pneumoniae-immune gloulin M (MP-IgM) were determined before and after treatment on the 1st and 21st days. Results The improvent time of fever, and cough and lung sign of the treatment group were more obviously shortened than the control group (P<0.05). The total effective rate was 96.0% (48/50) in observation group and 74.0% (37/50) in control group, with statistical significant difference between the two groups (P<0.05). After treatment, the negative rate of the serum concentration of MP-IgM in the treatment group was 82%, and that of the control group was 58%, with statistical significance between the two groups (P<0.05).Conclusion TCM external treatment combined with Dasangju Mixture can not only significantly improve the clinical symptoms and signs of MPP children patients, but also shorten the disease course, and the efficacy was significant. Meanwhile it can reduce the course and side effects of azithromycin, which provides reference to the administration of antibiotics in children.
8.Effect of Low-intensity Pulsed Ultrasound on the Proliferation and Differentiation of Osteoblasts Cultured on Two Piezoelectric Ceramic Porous Scaffolds

Objective To evaluate the effects of low-intensity pulsed ultrasound (LIPUS) treatment on the proliferation and differentiation of osteoblasts cultured on barium titanate/titanium alloy (BaTiO3) and zinc oxide/titanium alloy (ZnO) piezoelectric ceramic composite scaffolds.Methods The BaTiO3 and ZnO porous composite scaffolds were randomly divided into ultrasound (UBaTiO3 and UZnO) and control (CBaTiO3 and CZnO)groups,and MC3T3-E1 cells were cultured on the surface of the materials in each group.Cells in the ultrasound groups were subjected to a 20-min treatment LIPUS session daily,and the control group was sham-treated without the power source.The proliferation and differentiation rates of osteoblasts were evaluated using methyl thiazolyl tetrazolium (MTT),alkaline phosphatase (ALP),and osteocalcin (OCN) assays.Results Significant differences in the MTF values were observed between the UBaTiO3 and CBaTiO3 groups at day 4 (P < 0.05) and between the UBaTiO3 and UZnO groups at days 1,4,and 7 (P < 0.05).Compared with the CBaTiO3 group,the UBaTiO3 group showed significantly higher ALP activity and OCN levels at days 4 and 7 (P < 0.05).Further,a significant difference in the ALP and OCN values between the UBaTiO3 and UZnO groups (P < 0.05) was observed.Conclusion LIPUS promotes the proliferation and differentiation of osteoblasts on BaTiO3 and ZnO scaffolds.BaTiO3 scaffolds enable better promotion of osteogenesis under LIPUS treatment than ZnO scaffolds do.
9.Practice and effect analysis on construction of clinical medical research center of cardiovascular diseases

Objective:This article takes the practical experience of the construction of the cardiovascular disease clinical medicine research center in Nanjing First Hospital as an example to provide reference for institutions preparing to build a clinical research center.Methods:The center conducts top-level design based on clinical needs, and this study systematically elaborates the construction of the center from management mechanisms, research platforms, collaborative networks, team building and international cooperation.Results:The center has made remarkable progress in clinical research, scientific and technological innovation, transformation and promotion, standard formulation, and technology leading. On the basis of an international perspective, the level of regional diagnosis and treatment has been improved by optimizing the structure of subject talents, and focusing on the clinical needs of cardiovascular subspecialty.Conclusions:Taking the construction of the clinical medicine center as an opportunity, integrating superior resources, improving the level of disease diagnosis, treatment and service and enhancing international academic influence are effective measures to accelerate the development of health services and deepen medical reforms.
10.Risk factors and sonographic findings associated with the type of placenta accreta spectrum disorders

Objective To evaluate the risk factors and sonographic findings of pregnancies complicated by placenta increta or placenta percreta. Methods Totally, 2219 cases were retrospectively analyzed from 20 tertiary hospitals in China from January 2011 to December 2015. The data were collected based on the original case records. All cases were divided into two groups, the placenta increta (PI) group (79.1%, 1755/2219) and the placenta percreta (PP) group (20.9%, 464/2219), according to the degree of placental implantation. The risk factors and sonographic findings of placenta increta or percreta were analyzed by uni-factor and logistic regression statistic methods. Results The risk factors associated with the degree of placental implantation were age, gravida, previous abortion or miscarriage, previous cesarean sections, and placenta previa (all P<0.05), especially, previous cesarean sections (χ2=157.961) and placenta previa (χ2=91.759). Sonographic findings could be used to predict the degree of placental invasion especially the boundaries between placenta and uterine serosa, the boundary between placenta and myometrium, the disruption of the placental-uterine wall interface and loss of the normal retroplacental hypoechoic zone(all P<0.01). Conclusions Previous cesarean sections and placenta previa are the main independent risk factors associated with the degree of placenta implantation. Ultrasound could be used to make a prenatal suggestive diagnosis of placenta accreta spectrum disorders.
