1.The correlation of antiphospholipid antibody and factor Ⅻ deficiency in patients with retinal vein occlusion

Objective Antiphospholipid antibody and factor Ⅻ deficiency are among the coagulation disorders that have been implicated in many thrombembolic events. The aim of this study was to investigate the prevalence of antiphospholipid antibodies and factor Ⅻ deficiency in patients with retinal vein occlusion (RVO). Methods The investigation was a case control study. The periphery blood was collected from a cohort of 33 consecutive patients with RVO and 30 age- and gender-matched normal subjects. Anticardiolipin antibody (ACA) was detected by ELISA as binding index (BI) =A value/standard A value. The lupus anticoagulant antibody was examined by APTT test and the activity of factor Ⅻ was detected. This study was approved by The Human Research Ethics Committee of this hospital, and written informed consent was obtained from all the subjects before initiation of any study protocol. Results The total positive rate of APA in RVO group was 24. 24% (8/33), showing a insignificant difference in comparison with control group (6. 67%, 2/30) (P = 0. 085). The positive rate of anticardiolipin antibody in RVO group was 18.18% (6/33), presenting an obvious enhance in control group (P = 0. 025) . Three patients in RVO group disclosed positive response for IgG-anticardiolipin antibody, one patient for IgM-anticardiolipin antibody, two patients for both isotypes IgG and IgM anticardiolipin antibodies, and two patients revealed positive reaction for lupus anticoagulant antibody. The presence of lupus anticoagulant antibody among the patients with ≤ 50 years and > 50 years was similar to that in age-matched controls (P =0. 160, P =0. 206). Factor Ⅻ deficiency was found in 14 of 33 patients(42. 42%) and in 4 of 30 controls(13. 33%) (P = 0. 013). The prevalence of factor Ⅻ deficiency among the patients with ≤50 years and > 50 years was similar to that in age-matched controls (P = 0. 206, P = 0. 052) . Conclusion Our results indicate that the prevalence of ACA and factor Ⅻ deficiency in RVO patients appears to be correlated.

3.Clinical and pathological characteristics and prognostic evaluation of high-risk small renal carcinoma

Objective To study clinical and histopathological features of high-risk small renal carcinoma and provide theoretical basis for the individualized treatment.Methods This retrospective study analyzed the clinical and histological data of 21 patients of high-risk small renal cell carcinomas which were highly aggressive or advanced stage, admitted from Jan 2000 to July 2015.There were 15 males and 6 females,and the average age was (61.5 ±7.1) years (ranged 27 to 75 years).The mean diameter of tumors was 3.3cm (ranged 2.0 to 4cm).The study mainly focused on pathological character and stage, Fuhrman grading, existence of perirenal tissue or vascular invasion, venous embolus and distant metastasis.Results Of all these 21 cases, 17 cases were clear cell carcinomas, 1 was papillary carcinoma, 2 were hybrid cellular tumor ( malignant rhabdoid tumor and sarcomatoid carcinoma) and 1 was sarcomatoid carcinoma. Among the 17 clear cell carcinoma cases, 12 were Fuhrman grade Ⅱ, 5 were grade Ⅲ and one was gradeⅣ.Tumor infiltrated renal vein or its branches and renal vein embolus were found in 7 cases.The tumor infiltrated perirenal or renal sinus fat were found in 11 cases.Synchronous lung and local lymph nodes metastasis were found in 3 patients respectively.Moreover, there was 1 case with metastasis to the brain (FuhrmanⅣ).The final clinical stage were T3a N0M0 in 14,T1a N1M0 in 3 and T1a N0M1 in 4 cases. Conclusions Small renal tumors are heterogeneous in its biological behavior and the minorities are aggressive with infringement of perirenal fat or simultaneous local lymph node or distant metastasis.When the tumor is greater than 3.0 cm in diameter and with high grade in Fuhrman classification, sarcomatoid carcinoma are more likely to be highly aggressive and advanced stage.
4.Establishment and biological characteristics of a platinum-resistance nude mouse model in epithelial ovarian cancer

Objective To establish a platinum resistance nude mice model of epithelial ovarian cancer (EOC) and investigate its resistance to cisplatin (DDP) biological characteristics, so as to provide evidences for exploring chemoresistence mechanisms and screening for reversal targets in vivo micro-environment. Methods The resistance model was produced by repeating a crossover subcutaneous injection of human ovarian cancer SKOV3 cells labelled green fluorescent protein(GFP) and transplatation of tumor fragment into nude mice. Two kinds of cancer cell lines of SKOV3/DDPⅠand SKOV3/DDPⅡwere induced with acquired resistence to DDP. The chemosensitivities of EOC cells to DDP were tested and half maximal inhibitory concentration(IC50) was measured by methyl thiazolyl tetrazolium (MTT) and flow cytometry (FCS). Dynamic analysis among the concentration of DDP treatment and cell apoptosis, cell cycle phase distribution and intracellular DDP concentration. The expression of PTEN, STAT5, XIAP, BRCA1 and MDR1 were examined by real time quantitative reverser transcription PCR (qRT-PCR) in vivo. Results IC50 value of cisplatin for SKOV3/DDPⅡ were 2.83 ± 0.12 and 3.82 ± 0.19 folds than those for SKOV3/GFP by MTT and flow cytometry, separately. SKOV3/DDPⅠwere 2.20±0.16 and 3.40±0.20 folds. The apoptosis rate of SKOV3/DDPⅡ and SKOV3/DDPⅠ were decreased significantly at 29.7 and 39.6μmol/L DDP when treatment for 36 hours,which were lower than that of SKOV3/GFP cells [(57.0±1.4)%vs (37.6 ± 4.36)%vs (83.1 ± 2.71)%,P=0.024;(74.4 ± 2.3)%vs (50.5 ± 3.4)%vs (87.4 ± 4.0)%,P=0.001]. SKOV3/DDPⅠ and SKOV3/DDPⅡ was positively related with cisplatin processing time. Intracellular DDP accumulation of SKOV3/DDPⅡand SKOV3/DDPⅠwere lower than SKOV3-GFP in dynamic processes(P<0.05). Besides intracellular DDP accumulation of SKOV3/DDPⅡ also lower than SKOV3/DDPⅠin dynamic processes (P<0.05). Transplanted tumor of SKOV3/GFP appeared organelle degradation and nuclear membrane imcompleted after five times DDP injection with concentration of 4 mg/kg. SKOV3/DDPⅡand SKOV3/DDPⅠdid not generate these phenomenon untill eighth DDP injections with concentration of 4 mg/kg. STAT5 and BRCA1 of SKOV3/DDPⅡwere increased with DDP treatment at concentration of 4 mg/kg. Expression of XIAP from SKOV3/DDPⅡwas positive correlated with injection times. STAT5,XIAP and BRCA1 of SKOV3/DDPⅡwere up-regulated 3.86,28.1 and 14.6 folds than those in SKOV3/GFP cells after eighth DDP treatment, separately. While PTEN of SKOV3/DDP Ⅱ was decreased 3.77 folds. Conclusions We have successfully established platinum-resistent EOC mice model,which provides a new platform for further study on chemoresistant reversal and individualized clinical treatment. The results shown that potential mechanisms of SKOV3/DDPⅡDDP-resistance included over-expressed BRCA1 gene may be promote DNA damage repair, elevate XIAP gene to decrease cell apoptosis,up-regulated STAT5 gene and decrease PTEN gene to stimulate proliferation.
5.Study on defective interfering particles of Paramyxovirus, Tianjin strain enhanced the immune response of the mice

Objective To investigate whether the defective interfering(DI) particles of Paramyxovirus, Tianjin strain could be used as a candidate adjuvant. Methods DI particles were separated and purified. After being identified, it was equally mixed with the inactivated poliovirus (PV) vaccine. Kunming mice were administered subcutaneously with the mixture, besides we set groups of inactivated PV vaccine containing Al(OH) 3 as an adjuvant, inactivated PV vaccine of which the dose was doubled without any adjuvant, and negative control. Sera were collected in the 14th day after immunization, and the specific antibodies of PV were detected. T/B lymphocyte stimulation indexes(SI) were counted through the lymphocyte proliferation tests. The quantities of IFN-γ/IL-4 produced by the splenocytes which were stimulated again by PV antigen were detected. Results The SI and the quantity of IFN-γof the mice in the group of inactivated PV vaccine combining with DI particles of Paramyxovirus, Tianjin strain were more than other groups. Conclusion DI particles of Paramyxovirus, Tianjin strain could enhance the immune response of inactivated poliovirus, especially the cellullar immunologic response of Th1 type.

Objective To analyze the significance of the liquid-based thinprep cytologic test (TCT ) in early cervical cancer screening by comparing cervical atypical squamous cells (ASC) and the histologic diagnostic results in order to guide the clinician to correctly understand the meaning of ASC and further explore the effective method of early cervical cancer screening .Methods 163 cases of cervical ASC diagnosed by histology and TCT were retrospectively analyzed and the comparative analysis was performed . Results Among 163 cases of cervical biopsy ,50 cases were ASC of undetermined significance (ASC-US) diagnosed by TCT ,the positive cases verified by the histology accounted for 49 .3% ;13 cases were ASC of highly squamous intraepithelial lesions diag-nosed by TCT ,the positive cases verified by the histology accounted for 76 .9% .The cases of ASC-US diagnosed by cytology and cervical intraepithelial neoplasia grade Ⅱ(CIN Ⅱ) and above diagnosed by histology accounted for 12% .Conclusion ASC-H high-ly suggests the clinical possibility of high-grade intraepithelial neoplasia .ASC-US is a suggestive diagnosis rather than a clarity di-agnosis .Correctly understanding the clinical significance of ASC can discover early cervical lesions as early as possible .
8.Risk factors and intervention measures of mental disorders in ICU

Objective To investigate the risk factors of mental disorders of patients treated in intensive care unit(ICU).Methods Collected from January 2013 to December 2014 ICU stay eligible cases,and analyzed the past history (hypertension,intemperance),previous surgery,mechanical ventilation,date of ICU admission,quality of care, APACHE II score,electrolyte disorder,LVEF,sleep disturbance,PCT,oxygenation index,drug use situation (midazo-lam,opioids)and the clinical care unit the incidence of mental disorders in relationship.Results 568 patients (male 345 cases,female 223 cases),mental disorders group of 157 patients,the clinical incidence rate of 27.6% overall;568 patients were in the sex ratio of 1.55:1,mental disorders group of 157 patients(96 males,61 females),male to female ratio 1.57:1,mean age was (65.5 ±11.2)years;no mental disorders group (249 males,162 females),male to female ratio 1.30:1,mean age was (48.8 ±14.3)years.Logistic regression analysis revealed that coronary heart disease,hypotension,alcohol abuse,surgery,mechanical ventilation,ICU admission time,quality of care,APACHE II score,electrolyte imbalance,ejection fraction,sleep disorders,procalcitonin,oxygenation index,use of midazolam or opioids were among the risk factors of ICU mental disorders.Mechanical ventilation,quality of care,hypotension,alco-hol abuse,and use of midazolam were independent risk factors for ICU mental disorders (all P <0.05).Conclusion ICU inpatients with past history of hypertension and intemperance,use of mechanical ventilation and midazolam should be closely monitored and evaluated,early psycho -psychiatric screening,and take effective measures in time;high -quality care can reduce the incidence of mental disorders in ICU patients significantly.
9.Molecular characterization for neurodevelopmental disorders by chromosomal microarray analysis

Objective To assess the diagnostic yields of clinical chromosomal microarray ( CMA) testing for patients with neurodevelopmetal disorders ( NDD) , and to characterize the spectrum of pathogenic copy number variation(CNV) in NDD.Methods The study was a cross-sectional study.NDD patients from Shanghai Children′s Medical Center ( SCMC ) from April 2014 to April 2015 were recruited.DNA samples from SCMC cohort were tested on Affymetrix Cytoscan Dx microarray platform.The diagnostic yields of CMA testing were further assessed for the whole NDD cohort and each subgroup.Results A genome-wide genotype-phenotype analysis on a total of 107 NDD cases with CMA testing was conducted.Based on the SCMC clinical cohort, the overall diagnostic yield of CMA testing for NDD patients was 20.6%(22/107). Excluding one case with chromosomal aneuploid, the frequency of non-polymorphic CNVs of the rest NDD cases were 25.5%(27/106).The diagnostic yield for developmental delay/intellectual disorder(DD/ID) and autism spectrum disorder(ASD)were 26.3% (15/57) and 10.2%(4/39) respectively.DD/ID was more likely to be associated with CNV than ASD and attention-deficit/hyperactivity disorder(ADHD).Five recurrent genomic loci were significantly enriched in patients including 1q21.1-q21.2, 15q11.2-q13.1, 22q11.2, 7q11.23 and 17q11.2.Conclusion CNV is an important pathogenesis in NDD.
10. Construction and discussion of sensitive quality index of blood purification nursing

Objective:
To establish a sensitive quality index system for blood purification nursing, providing reference for the evaluation and monitoring of nursing safety quality in the blood purification room, and providing quality control targets and basis for nursing managers.
Methods:
Set up a research team to analyze the patient data, nursing bad events, draw on the data of the National Nursing Quality Control Center and other research results of nursing experts at home and abroad. The research team conducted analysis and discussion, expert meeting consultation(discussed by professional nursing experts in Beijing and Inner Mongolia), expert letters, screening and final determination of nursing sensitive quality indicators.
Results:
Finally, a set of 13 sensitive quality indicators for specialized care for blood purification was constructed, including 3 structural indicators, 10 outcome indicators and 100% effective recovery rates for the two rounds of expert questionnaires. The indicators for consultation are 0.12-0.17 and 0.10-0.14, respectively, and the expert authoritative coefficients are 0.786 and 0.807, respectively, which are statistically significant (
