1.The effects of pressure level in veil on upper airway in patients with obstructive sleep apnea hypopnea syndrome.

OBJECTIVE:
To observe the effects of pressure level in veil on upper airway in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) and normal adults.
METHOD:
We scanned the upper airway from roof of nasopharynx to glottis with SIMENS 16 layer spiral CT scanner. The area and the dimensions of palate, uvula, and the region after lingua and epiglottis were measured by application of image work station. Forty-six patients with OSAHS and 35 normal adults were scanned by CT at 0 kPa water column and 0.784 kPa water column pressures in face mask, respectively.
RESULT:
The area and inner diameter of OSAHS patients upper airways were less than those of normal adults at the same pressure in face mask. The area and inner diameter of upper airway at the pressure of 0.784 kPa water column were more than those at 0 kPa water column in both OSAHS patients and normal adults, and the increased extent of normal adults were more than those of OSAHS patients. We measured the minimum increased normal Cross sectional area of palate, uvula, and the region after lingua and epiglottis when the pressure in the mask increased from 0 kPa to 0.784 kPa, and we made it as the standard. The 46 OSAHS patients were sorted into 17 cases (group 1) with normal dilation and 29 cases (group 2) with less dilation in such standard. There was not significant difference between the two groups at 0 kPa pressures in area and inner diameter, but the area and inner diameter of group 1 were more than those of group 2 at 0.784 kPa pressure.
CONCLUSION
The increased pressure in face mask would lead to upper airway dilation both in OSAHS patients and normal adults, and the dilated extents of normal adults were significant more than those of OSAHS patients. The less dilation of OSAHS may be duo to anatomic constriction and some pharynx wall pathological changes.

2.Analysis on molecular mechanism of Scutellariae Barbatae Herba - Cremastrae Pseudobulbus Pleiones Pseudobulbus in the treatment of ovarian cancer based on network pharmacology and experimental verification

Objective:To study the main active components and potential mechanism of the treatment of ovarian cancer by Scutellariae Barbatae Herba - Cremastrae Pseudobulbus Pleiones Pseudobulbus based on network pharmacology and in vitro cell experiments.Methods:TCMSP and SwissTargetPrediction databases were used to retrieve and collect the active components and targets of Scutellariae Barbatae Herba - Cremastrae Pseudobulbus Pleiones Pseudobulbus, and four databases including GeneCards, OMIM, DrugBank and DisGeNET were used to search the targets related to ovarian cancer. After screening the common potential targets of Scutellariae Barbatae Herba - Cremastrae Pseudobulbus Pleiones Pseudobulbus and ovarian cancer, the STRING database was used to realize the construction of PPI network interacting with the proteins of the two common targets. DAVID database was used for GO and KEGG enrichment analysis. Cytoscape 3.9.0 software was used to construct the "active component-target-pathway" network. Finally, the key targets and pathways were validated in vitro. MTT method was used to detect the proliferation inhibition rate of human ovarian cancer SKOV3 cells with different concentrations of Quercetin and sitosterol. Human ovarian cancer SKOV3 cells were divided into control group, Quercetin group and sitosterol group according to the random number table method. After corresponding drug intervention for 48 hours, the effects of Quercetin and sitosterol on SKOV3 cell cloning ability were detected by clone formation experiment, the levels of Akt1, VEGFA, Caspase-3 mRNA in each group were detected by PCR, and the protein expressions of PI3K, Akt, p-Akt, Caspase-3, and VEGFA in cells were detected by Western blot.Results:Through screening, 29 effective chemical components in Scutellariae Barbatae Herba and 3 effective chemical components in Cremastrae Pseudobulbus Pleiones Pseudobulbus were obtained, including 2 components in total. 291 drug-related targets, 1969 potential targets of ovarian cancer were acquired. There were 137 common targets of "Scutellariae Barbatae Herba - Cremastrae Pseudobulbus Pleiones Pseudobulbus" and the drug-disease intersection of ovarian cancer. Through GO functional analysis, 930 items were obtained, involving 722 biological processes, 76 cellular components and 132 molecular functions. KEGG enrichment analysis involved 169 signaling pathways, including tumor pathway, PI3K-Akt signaling pathway, P-53 signaling pathway, IL-17 signaling pathway, etc. In vitro experiment results showed that cell proliferation inhibition rate increased and cell cloning ability decreased in Quercetin and sitosterol groups; The levels of Akt1 and VEGFA mRNA in Quercetin group and sitosterol group decreased ( P<0.01 or P<0.05), the expression of Caspase-3 mRNA and protein increased ( P<0.01), and the expressions of PI3K, p-Akt and VEGFA protein decreased ( P<0.01). Conclusion:Scutellariae Barbatae Herba - Cremastrae Pseudobulbus Pleiones Pseudobulbus inhibits the occurrence and development of ovarian cancer through multi target and multi pathway synergistic effects, and can induce SKOV3 cell apoptosis by regulating the expressions of key genes such as AKT1, VEGFA, Caspase-3, and PI3K/Akt pathway.
3.Analysis of influential factors for rivaroxaban-induced bleeding events in patients with non-valvular atrial fibril‐ lation complicated with coronary heart disease

OBJECTIVE To analyze the influential factors for rivaroxaban-induced bleeding events in patients with non- valvular atrial fibrillation (NVAF) and coronary heart disease. METHODS A total of 64 hospitalized patients with NVAF complicated with coronary heart disease who were treated with rivaroxaban and admitted to the Fuzhou University Affiliated Provincial Hospital from November 2021 to May 2023 were included in this retrospective study. The demographic data, laboratory test indexes and other general clinical data, and steady-state trough concentration of rivaroxaban were collected, and the dose- adjusted trough concentration was calculated. The occurrence of bleeding events within 6 months after discharge was recorded. The univariate analysis and binary Logistic regression analysis were adopted to determine the independent risk factors of rivaroxaban- related bleeding events. The binary Logistic regression equation was constructed to predict the probability of bleeding events. The area under the receiver operator characteristic (ROC) curve (AUC) was used to analyze the predictive value of the regression equation. RESULTS Among 64 patients, 19 patients had 24 case-times bleeding events, most of which were mild bleeding (19 case-times, 79.2%), and mainly gastrointestinal bleeding (17 case-times, 70.8%). After symptomatic treatment and adjustment of the anticoagulant regimen, most of them were improved or cured. In the univariate analysis, the proportion of patients with a history of anemia, platelet count, urea nitrogen content, steady-state trough concentration of rivaroxaban, dose-adjusted trough concentration and coagulation indexes [international normalized ratio, prothrombin time (PT), activated partial thromboplastin time] in bleeding group were significantly more or higher than those in non-bleeding group, while the albumin level was significantly lower than that in non-bleeding group (P<0.05). In binary Logistics regression analysis, high PT level (odds ratio=1.473, 95% confidence interval=1.103-1.967, P= 0.009) and high rivaroxaban dose-adjusted trough concentration (odds ratio=1.174, 95% confidence interval=1.018-1.355, P= 0.027) were independent risk factors for rivaroxaban-related bleeding events. The binary Logistic regression equation of bleeding event prediction probability (P) was LogitP=-6.975+0.387×PT level+0.161×dose-adjusted trough concentration, and the AUC of the ROC curve was 0.825 (95% confidence interval was 0.708-0.909, P<0.001). CONCLUSIONS The risk factors of rivaroxaban-related bleeding events in patients with NVAF and coronary heart disease include previous anemia history, high platelet count, high urea nitrogen content, high rivaroxaban steady-state trough concentration, high dose-adjusted trough concentration, high coagulation indexes and low albumin level. High PT level and high dose-adjusted trough concentration are independent risk factors that can be used to predict the risk of rivaroxaban-induced bleeding events. The regression equation has good predictive value.
4.Analysis of clinical characteristics of perianal Crohn's disease in a single-center.

OBJECTIVETo discuss the clinical features of perianal Crohn disease (PCD).
METHODSClinical data of 52 PCD patients who were treated at the Department of Colorectal Surgery, Affiliated Hospital of Nanjing University of Chinese Medicine from June 2011 to October 2014 were analyzed retrospectively. Montreal classification, clinical symptoms, the subtype of perianal lesions, history of perianal surgery and medical treatment, disease active indexes, serum inflammation markers and nutritional indexes were included.
RESULTSAmong 52 patients, there were 40 males and 12 females with a mean age of (27.9±9.1) years. According to Montreal classification, 51.9%(27/52) of patients had ileocolic involvement and 75.0%(39/52) had inflammatory disease behavior. Thirteen (25.0%) and 15(28.8%) patients had abdominal pain and diarrhea respectively at admission. With respect to the subtype of perianal lesions, there were 32(61.5%) cases of anal fistula and 16(30.8%) cases of perianal abscess. Thirty-nine(75.0%) and 33(63.5%) patients had a history of perianal surgery and medical treatment before admission resepectively. Laboratory findings revealed high C-reaction protein level in 63.5%(33/52), high erythrocyte sedimentation rate in 61.5%(32/52), elevated platelet in 32.7%(17/52), low body weight in 44.2%(23/52), low albumin in 26.9%(14/52) and anemia in 42.3%(22/52) of patients.
CONCLUSIONSThe clinical features of PCD patients include male predominant, early onset disease, high prevalence of ileocolic involvement and inflammation disease behavior. Features prompting underlying PCD diagnosis include characteristic gastrointestinal symptoms, requiring multiple perianal surgery, and abnormality of inflammation markers and nutritional indexes.
5.Effects of Different Processed Products of Whitmania pigra on Hemorheology and Coagulation Indexes in Acute Blood Stasis Model Rats

OBJECTIVE:To stud y the effects of different processed products of Whitmania pigra on hemorheology and coagulation indexes in acute blood stasis model rats. METHODS :SD rats were randomly divided into blank group ,model group , aspirin group ,W. pigra hang-dried product low- ,medium- and high-dose groups ,W. pigra talcum powder-ironed product low- , medium- and high-dose groups ,W. pigra wine bran-processed product low- ,medium- and high-dose groups ,with 6 rats in each group. Except for blank group ,other groups received subcutaneous injection of epinephrine hydrochloride and ice water bath for 15 d to induce acute blood stasis model. From the 8th day of modeling ,rats in aspirin group were given aspirin 0.2 g/kg intragastrically. Rats in each dose group of W. pigra processed products were given relevant medicine 0.35,1.4,3.5 g/kg intragastrically(calculated by crude drug ). Rats in blank group and model group were given constant volume of normal saline intragastrically, once a day , for consecutive 8 days. Hemorheology indexes as whole blood viscosity (high, medium and low shearrate ),plasma viscosity ,erythrocyte com deformation index ,erythrocyte aggregation index ,hematocrit, and blood coagulation indexes as prothrombin time (PT), mail:wcl19960125@163.com activated partial prothrombin time (APTT),thrombin time (TT)were determined. RESU LTS:Compared with blank group ,whole blood viscosity under different shear rates ,plasma viscosity , erythrocyte aggregation index and hematocrit of model group were increased significantly ,while erythrocyte deformation index was significantly decreased ,PT,TT and APTT were significantly shortened (P<0.01). Compared with model group ,whole blood viscosity under different shear rates ,plasma viscosity ,erythrocyte aggregation index and hematocrit of aspirin group and W. pigra hang-dried product ,talcum powder-ironed product ,wine bran-processed product high-dose groups were decreased significantly , while erythrocyte deformation index were significantly increased ,and PT (only W. pigra talcum powder-ironed products high-dose group),APTT(except for W. pigra hang-dried products high-dose group )and TT were prolonged significantly. The whole blood viscosity of W. pigra hang-dried product medium-dose group under low shear rate ,and those of W. pigra talcum powder-ironed product low-dose ,wine bran-processed product medium-dose groups under low and medium shear rates were decreased significantly. Erythrocyte deformation index of W. pigra talcum powder-ironed product medium-dose group was increased significantly ,while erythrocyte aggregation index was decreased significantly ,and PT ,TT were prolonged significantly. APTT of W. pigra hang-dried product medium-dose group was prolonged significantly. Hematocrit of W. pigra wine bran-processed product low-dose group was decreased significantly (P<0.05 or P<0.01). CONCLUSIONS : W. pigra hang-dried, talcum powder-ironed and wine bran-processed product can effectively improve hemorheology indexes and prolong blood coagulation time.
