The metabolic syndrome (MS) is one of the major risk factors for cardiovascular disease.The pathogenesis of MS is not fully understood.In addition to insulin resistance and central obesity,some other factors such as resting heart rate are also thought to play certain roles.The primary strategy of the intervention is still relying on lifestyle modification including healthy dietary habits.
2.Study on the distribution characteristics of Hyperthyroidism's syndrome

Objective To Analyze the distribution characteristics of Hyperthyroidism's syndrome and the relationship between symptoms and thyroid hormones. Methods The TCM syndromes of a total of 78 patients with hyperthyroidism were differentiated. The relationship between syndrome and general condition of patients (gender and age, etc.) were analyzed;correlation between symptoms and thyroid hormones were also studied. Results Of all 78 patients, patients with yin deficiency and hyperactive yang accounted for 43.6%; patients with internal disturbance of phlegm-fire accounted for 30.8 %; and patients with hypteracitivity of heart-fire and liver-fire accounted for 25.6%. The severity of all the symptoms showed positive correlation with hyroid hormones (the value of r is positive, P<0.05); and no linear relationship with TSH (P>0.05).Conclusion The syndrome of yin deficiency and hyperactive yangI was the most common syndrome type. Thyroid hormones had direct proportion with the severity of syndrome.
3.Epidemiological and clinical characteristics of 199 cases of HIV/AIDS patients

During the period of January 2006 to December 2012 at our department,retrospective analyses of epidemiological and clinical data were performed for 199 newly diagnosed HIV/AIDS inpatients and outpatients.The male-to-female ratio was 3.78∶ 1.Their age range was 20-40 years.Migrant workers accounted for a large proportion of infection.Sexual contact was a major route of transmission and it accounted for 88.4%.Most of them had non-marital sexual contact.And all infected marital sexual contacts were women.Asymptomatic patients accounted for 55.8%.Pulmonary infection was the most common clinical manifestation.There were more males than females locally.Most of them were young and middleaged.Sexual contact was a major route of transmission.Out-of-town migrant population was important for transmission.
4.Interferon α plus adefovir dipivoxil therapy for patients with HBeAg positive chronic hepatitis B nonresponding to interferon monotherapy

Objective To evaluate the efficacy and safety of interferon alpha (IFNα) combined with adefovir dipivoxil (ADV) for patients with chronic hepatitis B (CHB) non-responding to 24-week IFNα monotherapy.Methods Sixty CHB patients admitted to the First Affiliated Hospital of Xiamen University during 2009 and 2012 were enrolled in the study.All patients recieved IFNα monotherapy for 24 weeks and had no response.The patients were randomly divided into 3 groups by number table with 20 cases in each group.The experimental group was treated with IFNα combined with ADV,the control group 1 continued IFNα monotherapy,and the control group 2 shifted to ADV monotherapy.Virological,serological and biochemical responses were compared,and adverse reactions were observed.SPSS 19.0 software was used for data analysis.Results After 24 weeks of treatment,there was no statistical difference in HBV DNA loads,ALT levels and titers of HBeAg and HBsAg among three groups (F =0.985,0.717,0.985 and 0.717,P > 0.05).And no HBeAg seroconversion was observed.After 48 weeks of treatment,the experimental group had higher HBV DNA negative conversion rate,ALT normalization rate and HBeAg conversion rate than control group 1 (x2 =10.00,3.956 and 4.800,P < 0.05),but no statistically significant difference was found in HBeAg negative conversion rate (x2 =0.693,P > 0.05).There was no significant difference in HBV DNA and HBeAg negative conversion rates between experimental group and the control group 2 (x2 =1.026,1.905 and 0.156,P >0.05),but the HBeAg conversion rate in experimental group was significantly higher than that in control group 2 (x2 =4.800,P < 0.05).No HBsAg negative or serological conversion was observed,and there was no significant difference in titers of HBsAg among three groups (F =1.935,P > 0.05).No adverse reaction was observed.Conclusion For patients nonresponding to IFNα monotherapy,combination of IFNα and ADV can achieve higher ALT normalization rate,HBeAg conversion rate and HBV DNA negative conversion rate,and improve the overall efficacy of CHB antiviral therapy.
5.Evaluation of effects on diastolic function in patients with dilated cardiomyopathy by cardiac sympathetic nerve blockade with Doppler echocardiography

Objective To investigate if diastolic function could be improved by cardiac sympathetic nerve blockade in patients with dilated cardiomyopathy (DCM). Methods Sixty consecutive cases of DCM were randomed into therapeutic group and control group. Patients in therapeutic group were administered thoracic epidural blockade (TEB) (T 1-5 ) with 0.5 % lidocaine intermittent injection every two hours for four weeks, in addition to some routine medicine, while patients in control group were only administered routine treatment. Then the parameters of left ventricular diastolic function were evaluated with Doppler echocardiography before and after four weeks. Results ①All the patients in TEB group felt better within five minutes after injection of lidocaine within local epidural cavity.Their symptoms were relieved more rapidly than those of the drug therapy alone. Some patients with NYHA class Ⅳ could lie down supine in a short time. Exercise tolerance increased in patients with NYHA class Ⅱ to Ⅲ. But symptoms and signs of patients in control group were relieved very slowly, some patients even got worsen. ② Thirteen of thirty in therapeutic group got improvement on diastolic function after treatment. One of thirty in this group deteriorated. In detail, six of fifteen patients with demonstrated improvement of left ventricular diastolic filling changed from having a restrictive filling pattern to having a pseudonormal left ventricular filling pattern. Four of nine patients with a pseudonormal filling pattern changed to have a delayed filling pattern.Three of six patients, left ventricular filling pattern moved from a pseudonormal left ventricular filling pattern to a normal filling pattern. Only one patient acquired a restrictive filling pattern from a pseudonormal filling pattern. In contrast, in control group one of thirty got improvement on diastolic function after treatment. Nine of thirty in this group deteriorated. Individual changes of left ventricular filling pattern showed that only one patient improved from a delayed filling pattern to normal filling pattern, whereas seven patients had a restrictive left ventricular filling pattern developed from a pseudonormal filling pattern and another two patients achieved a pseudonormal filling pattern from delayed filling pattern. Conclusions Diastolic function in patients with DCM was improved significantly by cardiac sympathetic nerve blockade. The approach has beneficial effects on the patients of DCM.
6.Simultaneous determination of mycophenolic acid and its glucuronide in human plasma by HPLC

Objective: To establish a method for determining mycophenolic acid(MPA) and its glucuronide(MPAG) inhuman plasma. Methods: A high performance liquid chromatographic assay with diode array detection was developed, and thedetector wavelength was set at 254nm. The plasma sample purification was limited to protein precipitation with acetonitrile.The sample was separated on Hypersil ODS2 column (200 mm ?4. 6mm, 5?m), the mobile phase consisted of solution A(25% acetonitrile and 75% 0.02 mol/L KH2PO4,pH 3.0)and solution B(70% acetonitrile and 30% 0.02 mol/L K2HPO4,pH6.5)with gradient elution. The flow rate was 1. 0 ml/min. Results: Calibration curves of MPA and MPAG were linear be-tween 1. 0-5. 0 ?g/ml (r=0.994 4,n=6) and (2.5-100) ?g/ml (r= 0.999 5,n=7),respectively. The detection limit of MPAand MPAG were 0. 5?g/ml, and 1.0 ?g/ml, respectively. The mean recoveries of high, medium and low concentrations ofMPA were (95. 75?2.3l)%, (104.10?1.91) % and (98.11?4.24)%, and MPAG were (97.37?1. 43)%, (101.10?5. 41)%and (105. 44?7.59)%. Conclusion: The present study provides a reliable quantitative method for pharmacokineticstudy and monitor of MPA and MPAG.
7.The preoperative assessment of tumor size in breast ductal carcinomain situ:a comparative study between mammography and ultrasonography

Objective To compare the accuracy of mammography and ultrasonography in evaluating the tumor size of breast ductal carcinoma in situ before operation.Methods Eighty-seven patients with breast ductal carcinoma in situ confirmed by surgery pathology were retrospectively analyzed.All of the mammography and ultrasonography data were well-documented.The maximum diameter of the tumor was measured by mammography,ultrasonography and pathology,respectively.It was considered concordant if the difference between the imaging size and pathologic size was less than 0.5 cm.Pearson correlation analysis was used to determine the relation of imaging size with pathologic size.Results The range of the maximum diameter of the tumors measured by pathology,mammography and ultrasonography was 0.4-7.0 cm,0.8-6.9 cm and 0.5-4.8 cm,respectively.The mean value was (2.4±1.3)cm,(2.6±1.2)cm and (2.3±1.0)cm,respectively.The coefficient correlation between mammographic and pathologic size was 0.724,and between ultra-sonographic and pathologic size was 0.532.Conclusion Mammography is more accurate than ultrasonography in size assessment of breast ductal carcinoma in situ.
8.The genetic structure of the fibroblastic Fas in keloid

BACKGROUND:Keloid is the outcome of wound-healing process,and the result of massive accumulation of life-prolonged fibroblasts with gene mutation as well as the excessive synthesis of collagenous fibers.OBJECTIVE:To probe into the structural relations of exons 7-9 of fibroblastic Fas gene in keloid tissues.DESIGN:A self-controlled experimental study with cicatricial tissues as the subjects.SETTING:Department of Plastic Surgery of Southern Hospital of the First Military Medical University of Chinese PLA.PARTICIPANTS:This experiment was carried out at the Tropical Disease Research Institute of the First Military Medical University of Chinese PLA in 2001. All keloid and hypertrophic scar tissues were obtained from the patients who received orthopedic surgical operations at the Southern Hospital, including 15 patients with keloid and 12 patients with hypertrophic Scars. Normal skin and peripheral blood were obtained from the keloid patients as self-control. Meanwhile pathological tissues and normal skin and peripheral blood were obtained from patients with hypertrophic scars as normal control.INTERVENTION:PCR-single strand conformation polymorphism was used to find the structure of the exons 7-9 in Fas gene in15 patients with keloid.MAIN OUTCOME MEASURES:The structure of the exons 7-9 in Fas gene.RESULTS:Heterozygous loss of Fas gene exon-8 was observed in all the 15 keloid patients, and 20% of them displayed an increase in exon-9 allele band.CONCLUSION: The genetic structure of Fas gene showed no mutation in hypertrophic scars, normal skin and the peripheral blood,but mutations were detected in exons-8 ,and -9 of Fas gene in keloid. This was closely related with the disfunction of its encoded proteins.
9.Analysis of the sequence of the variant exon-8 of fibroblastic Fas gene in keloid

All keloid and hypertrophic scar tissues were obtained from the patients who received orthopedic surgical operations at the Southern Hospital, including 15 patients with keloid whose pathological areas were located respectively at the earlobe and the prothorax and 12patients with hypertrophic scars whose pathological areas being located at the instep and the elbow. At the same time, normal skin and the peripheral blood samples from the patients themselves with keloid were taken as the self-control and the skin and the peripheral blood samples from the normal people and the patients with hypertrophic scars were taken as the normal control.METHODS: PCR-SCCM technique and gene sequence analysis were used to detect the gene structure of exon-8 in the Fas gene from 15 patients.MAIN OUTCOME MEASURES:The gene structure of exon-8 in the Fas gene derived from the tissues and the peripheral blood samples of all the groups.RESULTS: ① Heterozygous loss was observed in the exon-8 of the Fas gene in all 15 keloid patients; ② Gene sequence was found to be abnormal in 11 cases out of 15 keloid patients, presenting gene mutation in 4 loci.CONCLUSION: Heterozygous loss and gene mutation was detected in the exon 8 of Fas gene of keloid, suggesting that Fas protein in keloid has functional defect that is closely associated with gene mutation.
10.Mechanism exploration on effects of cardiac sympathetic anesthesia on cardiac performance of dilated cardiomyopathy

Objective To explore the mechanism of effects of cardiac sympathetic anesthesia on left ventricular ejection fraction(LVEF) and left cardiac cavity size of patients with dilated cardiomyopathy.Method 121 consecutive patients with dilated cardiomyopathy were divided into cardiac sympathetic nerve blockade group(TEA group) and control group(c group).In TEA group,5% lidocaine was injected into thoracic epidural cavity for about 4 to 8 weeks in addition with routine therapy.In c group,only routine therapy was used.We observe the changes of LVEF and left cardiac cavity size before and after treatment in both groups. Result In TEA group,after anesthesia,LVEF was increased from(31.3± 12.8) to(47.3± 21.3),P<0.001;left ventricular end- diastolic diameter was reduced from(69.1± 7.1)to (65.1± 8.0),P<0.001;left atrial diameter was decreased from(44.0± 6.2)to(39.4± 7.2),P< 0.001. Conclusion Cardiac sympathetic anesthesia can effectively improve the ejection performance of dilated cardiomyopathy and make the dilated cardiac cavity turn to normal level.
