1.Clinical study of modified Renshen decoction plus routine western medicine on patients of chronic congestive heart failure

Objective To evaluate the effect of modified Renshen decoction plus routine western medicine on chronic congestive heart failure. Methods The selected 75 candidate patients were divided into control group with 37 patients and treatment group with 38 patients. The patients of control group received routine western medicine and treatment group received modified Renshen decoction plus routine western medicine. After 2 weeks of treatment, the effect of both group were evaluated with indictors of clinical heart function, BNP and traditional Chinese syndrome. Results There were significantly statistical difference, of total effective ratio of clinical heart function (χ2=4.343, P<0.05)between the treatment group (92.1%,35/38)and control group (73.0%, 27/37), and of total effective ratio of traditional Chinese syndrome (χ2=4.649, P<0.05) between the treatment group (94.7%, 36/38) and control group (73.0%, 27/37). The BNP levels of both group decreased significantly (P<0.05), and the significant statistical difference between two groups was also observed 1477.1 ± 316.7 pg/ml vs. 2256.9 ± 521.3 pg/ml; t=7.853, P<0.01). Conclusions The Modified Renshen decoction plus routine western medicine could improve chronic congestive heart failure patients' the heart function, reduce BNP and attenuate the symptoms.
2.Endovascular treatment of inferior vena cava syndrome

Objective To investigate the curative effect of endovascular treatment for inferior vena cava syndrome(IVCS).Methods A total of 17 cases of inferior vena cava syndrome was treated by endovascular dilatation and self-expandable metallic stent placement between June 2002 and November 2004.Routine anticoagulation therapy was given after the procedure.Results The pressure gradient across the inferior vena cava was reduced from 16.8?4.3 mmHg preoperatively to 2.6?0.6 mmHg postoperatively(t=13.280,P=0.001).The IVCS symptom scores were decreased from 4.4?1.6 preoperatively to 2.1?1.7 postoperatively(t=6.880,P=0.010).Symptoms of edema on lower limbs,scrotal or pubic dropsy,ascites,and anasarca subsided at 1~4 days after procedure.Conclusions Endovascular treatment of inferior vena cava syndrome is effective and reliable.
3.Effect of JPHSKL on the quantity of 5-Hydroxytryptamine of colonic mucosa of diarrhea-predominant irritable bowel syndrome rats

Objective To observe the effect of Jian Pi Hua Shi Ke Li(JPHSKL) on IBS‐D ,gastrointestinal peptide hormones of rats 5‐HT ,and explore the therapeutic mechanism .Methods 60 clean‐level Wistar rats were used in the experiment .The rats were randomly divided into normal group ,model group ,JPHS low ,middle ,high and Deshute group(10 rats in each group) .Used gavage senna and restraint stress to establish D‐IBS rat model .The drug contrations of JPHSKL low ,midle ,high group were 1 .5 ,2 .5 ,4 .0 g/mL ,3 mg/mol in Deshute group .Once each day ,for 2 weeks ,the other groups were nomol group without any treatment and mod‐el group which building successful .Immunohistochemical SABC method to observe 5‐HT level of colonic specimens .Results The 5‐HT level in colonic tissue of the rats in IBS‐D model were obviously higher than the nomal group(P<0 .01) ,and the 5‐HT level in colonic tissue in JPHSKL treated group were obviously lower than the mold group (P<0 .05) .Among them ,the JPHSKL high group had significant effect and there was no obviously difference with Deshute group (P>0 .05) .Conclusion JPHSKL can im‐prove gastrointestinal function of IBS‐D model rats ,effect mechanism is relating to decrease the level of 5‐HT in colonic mucosa .
4.Determination of Circulating Antigen in Neurocysticercosis Patients by Monoclonal Antibody

Objective To detect circulating antigen (Cag) for diagnosing neurocysticercosis. Method ELISA was performed with monoclonal antibody 4B6 against the cyst fluid antigen of Cyticercus cellulosae for detecting Cag in serum and/or cerebrospinal fluid (CSF) of patients with neurocysticercosis or with other diseases. Results In the group of 82 cases of neurocysticercosis, the positive rate of serum Cag was 79.2% (65/82) and the positive rate of CSF Cag was 100% (26/26). After chemotherapy for 20 cases with positive serum Cag, the titer of serum Cag in 17 cases dropped to zero(85% ). Cag could not be detected in specimens from patients with other diseases. Conclusion These results indicate that the determination of Cag, especially of the CSF Cag, is useful for the diagnosis of neurocysticercosis and the drop in serum Cag is a good parameter for the evaluation of the effectiveness of chemotherapy.
5.The evaluation value of severity and prognosis of septic shock patients based on the arterial-to-venous carbon dioxide difference

Objective To assess the value of central venous-to-arterial carbon dioxide difference [ P( cv-a) CO2 ] in evaluation of disease severity and prognosis in patients with septic shock.Methods There were 45 consecutive resuscitated septic shock patients from April 2009 to October 2010 included immediately after their admission into our ICU.The patients were divided into low P(cv-a) CO2 group and high P(cv-a) CO2 group according to a threshold of 6 mm Hg ( 1 mm Hg=0.133 kPa).All patients were treated by early goal directed therapy (EGDT).The parameters of hemodynamics,lactate clearance rate,the Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ ) score,the sequential organ failure assessment (SOFA)score,6 h rate of EGDT achievement,the ICU mortality and 28 days in-hospital mortality were recorded for all patients.Results There were 30 patients in the low P(cv-a) CO2 group,and 15 in the high P(cv-a)CO2group.There were no significant differences between low P(cv-a) CO2 and high P(cv-a) CO2 patients in age,APACHE Ⅱ score and SOFA score (all P > 0.05 ).Compared with the high P(cv-a)CO2 group,the low P(cv-a) CO2 group had higher cardiac index ( CI ) and 24 h CI,higher delivery O2 ( DO2 ) and 24 h DO2,higher central venous oxygen saturation ( ScvO2 ) [ (74 ± 9) % vs (67 ± 8) % ],lower lactate [ ( 3.4 ± 2.1 )mmol/L vs (5.7 ± 4.5 ) mmol/L] and higher △SOFA score [ (0.7 ± 1.8 ) vs ( - 0.4 ± 1.1 ) ],lower 24 h SOFA score [ (7.8 ± 2.0) vs (9.8 ± 2.0 ) ],higher 6 h rate of EGDT achievement ( 83.3% vs 53.3 % )(P < 0.05 ),however,there were no differences in 28 days mortality and ICU mortality between the two groups ( P > 0.05 ).Conclusion P(cv-a) CO2 might be an indicator for predicting the severity of patients with septic shock and evaluating tissue perfusion.
6.Immunohistochemical Stainning for Ber EP4and EMA in the Diagnosis Basal Cell Carcinoma and Squamous Cell Carcinoma of the Skin

Objective To investigate the significance of immunostainning for Ber EP4and EMA in the diagnosis of basal cell carcinoma and squamous cell carcinoma of the skin.Methods Immunohisto-chemical stainning for Ber EP4and EMA was performed on115cases of basal cell carcinoma,squamous cell carcinoma,Bowen' s disease,actinic keratosis,basosquamous cell carcinoma,seborrheic keratosis,and verruca vulgaris.Specimens were taken from neoplastic tissues as well as the surrounding skin and ap-pendages.Results Ber EP4was positively stained in all cases of basal cell carcinoma and basosquamous cell carcinoma,but negatively stained in squamous cell carcinoma,Bowen's disease,actinic keratosis,sebor-rheic keratosis and verruca vulgaris.Expression of EMA was found in most cases of squamous cell carcinoma and Bowen' s disease,and a few cases of actinic keratosis,and in none of basal cell carcinoma,basosqua-mous cell carcinoma,seborrheic keratosis and verruca vulgaris.Conclusions Routine immunohistochemical staining with both Ber EP4and EMA is helpful for distinction of skin basal cell carcinoma,squamous cell carcinoma,precancerosis and benign hyperplastic dermatoses.

Objective To investigate the relationship between CD3+T, CD4+T, CD8+T cells, Th1/Th2 cytokines IL-2, IL-4, IL-6, sIL-2R and the pathogenesis as well as disease activity in patients with cutaneous nodular vasculitis (NV). 9.Expression of a novel adjuvant TFPR1 in Pichia pastoris and its identification

Objective To express a recombinant protein TFPR1 ( the functional region of the snake venom proteins from Trimeresurus flavoviridis) in Pichia pastoris expression system. Methods The target gene was codon-optimized and synthesized according to the sequence of the conserved structural do-main of triflin and then cloned into the Pichia pastoris expression vector pPICZαA to construct the recombi-nant expression plasmid pPICZαA-TFPR1. The recombinant plasmid pPICZαA-TFPR1 was electroporated into the yeast strain X33. The transformed strains carrying expression plasmid were screened out with Zeocin and then induced by methanol to express the recombinant protein TFPR1. ELISA was performed for the screening of positive clones. SDS-PAGE and Western blot were used for further identification of the ex-pressed products. Results The recombinant plasmid pPICZαA-TFPR1 was successfully constructed. The recombinant protein TFPR1 was expressed in a secreted form at a molecular weight of 16×103. Conclusion The recombinant protein TFPR1 was successfully expressed in Pichia pastoris expression system, which laid a foundation for further researches on its biological function and application as an adjuvant.
10.Post-marketing evaluation of the effectiveness and safety of a varicella attenuated live vaccine in ;China

Objective To evaluate the effectiveness and safety of varicella attenuated live vaccine ( VarV) produced by A Co. Ltd. Methods We selected 3 provinces in China and enrolled 15 002 children aged 3-<11 in this random, multicenter study. Participants were randomly divided into two groups: the ex-perimental group and the control group. Every varicella case was collected and recorded to calculate the vac-cine efficacy. Vaccine safety was assessed by means of spontaneous report and regular follow-up visits. Re-sults During the observation period, the incidence of varicella was 0. 147% in the experimental group and 1. 155% in the control group (P<0. 001). The vaccine efficacy was 87. 27%. The adverse reaction rate af-ter vaccination was lower than the rates reported in other literatures. Conclusion The VarV produced by A Co. , Ltd. in China was effective and safe in preventing varicella.
