1.Security of 5-fluorouracil sustained-release reagent under orthoptic endoscope

OBJECTIVE:To explore security of interstitial chemotherapy by implanting 5-fluorouracil sustained-release reagent into interstitial tissues of oesophageal tumor tissue and tumor peripheral tissue under orthoptic endoscope. METHODS:Relative articles were retrieved from Pubmed database from January 1988 to December 2008 and CNKI database from January 2000 to December 2008. Inclusion criteria:articles which were related to 5-fluorouracil sustained-release reagent for the treatment of esophageal carcinoma were included;Exclusion criteria:Duplicated articles were excluded. A total of 29 patients with grade Ⅲ-Ⅳ esophageal carcinoma who received implantation of 5-fluorouracil sustained-release reagent under orthoptic endoscope were collected from Department of Oncology,Shenyang Red Cross Hospital from March 2001 to December 2007. There were 22 males and 7 females,aging 51-82 years and mean age of 60.2 years. 5-fluorouracil sustained-release particles were implanted into tumor interstitial tissue under orthoptic endoscope. 800 mg 5-fluorouracil sustained-release particles were gradually implanted for 3-5 particles each time. RESULTS:Implantation of 5-fluorouracil sustained-release reagent has been reported to safely and effectively treat esophageal carcinoma. Clinical results indicated complete remission(n=3) ,partial remission(n=16) ,stable pathogenetic condition(n=6) ,and no remission(n=4) . Body mass increased in 21 patients(72.4%) ,and average effectiveness level was 70.9%. Symptoms including hemorrhage,light descent of leukocyte,light malignancy,diarrhea but excluding functional disorder of liver and kidney were found. 2.Clinical application of tracheal stent implantation

Currently,tracheal stents used in clinic include silicone,dynamic and metal types.The metal stents contain Ni-Ti memory alloy,stainless steel,and coating stents,which are cylinder and network structured.These stents are more expensive than silicone stents.The internal stents,mainly metal stents,have been used recently in clinic to treat tracheal stenosis.Tracheal stenting methods are divided into bronchofibroscope and hard lens implantation.Both tracheal and esophageal stent implantations are invasive treatment.However,tracheal stent implantation is different from esophageal stent implantation.As patients with airway obstruction are always complicated by dyspnea and hypoxemia,even respiratory failure,which increases risk and difficulty of stent implantation.During tracheal stent implantation,the bronchofibroscope or stent entering the stenosis site may lead to complete tracheal obstruction-induced asphyxia.Corrosion of metal wire of stent to airway mucosa and vessels may result in hemorrhea,leading to postoperative hemoptysis.Stent dislocation is mainly caused by inappropriate selection of stent,inappropriate implantation position or severe cough.Carcinoma tissue growing along stent lumens can induce re-obstruction in stent,resulting in atelectasis relapse.Although there are many unsolved problems and the incidence of complication is 10%-20%,tracheal stent is still a safe,effective and simple therapy for tracheal stenosis.Prevention and treatment of tracheal stenosis following stent implantation and development of novel stents with better effect and fewer side effects is future study focus.
3.Image segmentation of hepatic slice by MITK and maximum entropy

This paper discusses image segmentation of liver by MITK and maximum entropy in VC++ 6.0.MITK supports an extensive set of image processing and 3D visualization algorithms,which is a very convenient tool.After filtered by Wavelet transform,good segmentation results could be obtained by maximum entropy method and MITK interactive segmentation.
4.In vitro effects of hyperbaric oxygen and DDP on proliferation of lung adenocarcinoma cell line A549/DDP

Objective To evaluate the effects of hyperbaric oxygen (HBO) and cisplatin (DDP) on inhibition of A549/DDP cells in vitro and to explore the impacts of HBO on cell cycle and apoptosis.Methods The cytotoxicity of HBO and DDP was determined by Methyl thiazolyl tetrazolium (MTF) assay; flow cytometry (FCM) was used to investigate the impacts on cell cycle and apoptosis of cell line A549/DDP.Results Hyperbaric oxygen alone significantly suppressed A549/DDP cells proliferation (P < 0.05),compared with control group of normal oxygen,and the inhibition ratio was 2.90%.DDP at concentrations ranging from 3 μg/ml to 24 μg/ml significantly inhibited the proliferation of A549/DDP cells in a dose dependent manner,that was concentrations of 3,6,12 and 24 μg/ml brought inhibition ratio of 1.87%,4.62%,11.15% and 30.45% ; Hyperbaric oxygen can enhance the inhibition effect of DDP to A549/DDP cells,when hyperbaric oxygen affiliated with DDP of concentrations above,the inhibition ratios were 4.37%,8.92%,21.88% and 48.71%,respectively,which has significant difference with the single DDP groups.FCM analyses showed that HBO arrested A549/DDP cells at the S phase,that was cell number of G0/G1 phase reduced significantly and of S phase raised significantly,compared with normal oxygen group(P <0.01),that was the percentages of cells in S phase of control group were 9.5% and HBO group were 20.9%,while the percentages of cells in G0/G1phasc were 82.2% and 66.3%,respectively.In addition,there were more apoptotic cells in the HBO group with the percentage of 7.9%,while the percentage was 2.8% in the normal oxygen group.Conclusion Hyperbaric oxygen could play together with DDP to inhibit A549/DDP cells proliferation.HBO could block the cell cycle at S period and induce apoptosis.
5.Effects of oligomeric grape seed proanthocyanidins on isoproterenol-induced cardiac remodeling in rats

The purpose of this study is to evaluate the effect of oligomeric grape seed proanthocyanidins (GSP) on isoproterenol (ISO)-induced cardiac remodeling in rats. ISO was given subcutaneously (5 mg x kg(-1), sc, 7 days) to induce cardiac remodeling in rats. Therapeutic groups were given GSP (50, 100, and 150 mg x kg(-1)) after ISO treatment. After 2 weeks intervention, heart rate (HR), left ventricular systolic pressure (LVSP), left ventricular end-diastolic pressure (LVEDP), rate of rise of left ventricular pressure (+/- dp/dt(max)) were examined. The myocardial hypertrophy index was expressed as heart weight/body weight (HW/BW) and left ventricle weight/body weight (LVW/BW), the histological changes were investigated by HE and Van Gieson stain. SOD activity and MDA content in serum, contents of hydroxyproline (Hyp) in the left ventricular tissue were assayed by xanthinoxidase method, thiobarbituric acid (TBA) method and alkaline hydrolysis method, respectively. After the onset of ISO-treatment, GSP therapy potently improved cardiac function, inhibited myocardial hypertrophy, improved cardiac pathology change, decreased the myocardial cross-section area (CSA), collagen volume fraction (CVF) and perivascular circumferential collagen area (PVCA), reduced the content of Hyp in the left ventricular tissue, inhibited the decrease of SOD activity and increase of MDA content in serum. GSP possess protective effect against ISO induced cardiac remodeling in rats, this may be related to reducing the oxidative stress and improving antioxidant capacity.
6.The correlation study between the level of serum BNP ,sFlt-1 and NO and changes of condition in preeclampsia pregnant women

Objective To research the correlation between the level of serum brain natriuretic peptide (BNP) ,soluble vascular endothelial growth factor receptor 1 (sFlt-1) and nitric oxide (NO) and the changes of condition in preeclampsia(PE) patients.Methods Totally 89 patients with PE who had prenatal examina-tion and normal delivery in the hospital from May 2014 to January 2017 were enrolled in this study as the PE group ,and 94 healthy pregnant women who underwent normal delivery during the same period were selected as the NP group.The levels of serum BNP ,sFlt-1 and NO in PE patients with different severity and different onset stages were compared and analyzed.Results The levels of serum BNP and sFlt-1 in the PE group were significantly higher than those in the NP group ,and the level of NO was significantly lower than that in the NP group ,and the differences were statistically significant (P<0.05).The levels of serum BNP and sFlt-1 in the severe PE group were significantly higher than those in the mild PE group ,and the level of NO was signifi-cantly lower than that in the mild PE group ,and the differences were statistically significant (P<0.05).The levels of serum BNP and sFlt-1 in the early and late onset PE groups were significantly higher than those in the early NP group and the late NP group ,while the NO levels were significantly lower than those in the early NP group and the late NP group ,and the differences were statistically significant (P<0.05).Conclusion On the occurrence of PE ,the levels of serum BNP and sFlt-1 in pregnant women elevated abnormally ,and the lev-el of NO decreased.The changes of serum BNP ,sFlt-1 and NO levels are closely related to the development of PE ,and can be used as a reference index to detect the changes of PE.
7.Multicenter controlled randomized clinical trial of akatinol memantine for treatment of Aizheimer's disease

Objective To evaluate the clinical efficacy and safety of akatinol memantine in the treatment of Alzheimer's disease (AD).Methods Two hundred and forty-one patients with AD were randomly assigned to receive 10 mg of donepezil daily or 20 mg of memantine daily for 24 weeks.The primary efficacy variables were the Clinician' s Interview-Based Impression of Change Plus (CIBIC-Plus),the Alzheimer Disease Assessment Scale-cognition (ADAS-cog) and the Activities of Daily Living (ADL).The secondary efficacy variables were the Neuropsychiatric Inventory (NPI) and the Mini-Mental Status Examination (MMSE).Results Two hundred and seven patients completed the study and were evaluated at week 24.Both memantine and donepezil had significant efficacies at the end point, according to the ADAS-cog, the ADL, the NPI and the MMSE.Patients receiving memantine had a similar outcome as those receiving donepezil, according to the results of all the variables changes (CIBIC-Plus: memantine 3.4±0.8vs donepezil 3.5±0.8; ADAS-cog: memantine-4.7±5.8 vs donepezil-4.6±6.5; ADL: memantine -2.4±6.7 vs donepezil-2.2±5.3 ; NP1: memantine-5.8±9.0 vs donepezil-3.1±8.5 ; MMSE:memantine 1.7±3.1 vs donepezil 1.8±2.8, all P >0.05).The adverse events were as following: donepezil group 41.88% and memanintine group 30.58%.Conclusion The memantine as a new drug for AD, has the similar efficacy as donepezil, and it is safe.
8.Voxel based morphometric study of brain structure in patients with posttraumatic stress disorder.

Voxel based morphometry (VBM) methods are used to detect the difference in brain structures between the posttraumatic stress disorder (PTSD) sufferers and the normal controls. Standard VBM method can detect the difference of the gray matter or white matter densities while the optimized VBM method can detect the difference of gray matter or white matter volumes in the whole brain. The experiments showed that for the patient group, gray matter density or volumes significantly increased in the right frontal lobe, middle frontal gyrus, vermis, left caudate and parietal lobe, compared with the normal controls. However, in the left frontal lobe and middle frontal gyrus, gray matter density significantly decreased. There is no significant difference in white matter between the two groups. These results are consistent with those of the fMRI, which not only provide the evidence for further study of the pathogeny in PTSD but also validate the efficiency of the VBM methods for detecting the difference in the whole brain structure.

9.Predictive value of bioelectrical impedance analysis-measured body fat to abnormal lipid profiles in children and adolescents: the optimal cut-off values of body fat

Objective:To assess the predictive values of bioelectrical impedance analysis(BIA)-measured body fat indices to abnormal lipid profiles, and to preliminary propose optimal cut-off values of body fat in children and adolescents.Methods:Children and adolescents, aged 6-16 years, were selected from 30 schools (8 primary schools, 21 middle schools and one 12-year education school) in Dongcheng, Tongzhou, Fangshan and Miyun districts of Beijing by adopting a stratified cluster sampling method from November 2017 to January 2018.Questionnaire survey, body mass index(BMI), body fat mass index (FMI), fat mass percentage (FMP) and four lipid profiles were conducted.Results:A total of 14 309 participants, aged (11.0±3.3) years, were enrolled in the analysis, with 49.9% boys.In boys and girls, the percentile values ( P60- P95) fitted by FMI and FMP with K-median-coefficient of variation(LMS) method were taken as the cutting points, and P75 values were selected as the cut-off points of excessive body fat for their better sensitivity, specificity, predictive value and area under curve (AUC) for identification of abnormal lipid profiles.Boys with FMI above P75 accounted for 28% of the total population, and controlling boys with FMI below P75 could prevent dyslipidemia of 8%-57%.FMI in girl population occupied about 26% of the above, and controlling FMI in girl population below this cut-off point may prevent dyslipidemia from 8%-42%.FMP observed similar results to FMI.Assessed by FMI or FMP with P75 cut-off values, adiposity performed better than BMI for recognizing abnormal lipid profiles in boys (AUC: 52.4%-69.6% vs.50.2%-67.1%, P<0.05) rather than in girls ( P>0.05). In addition, when FMI or FMP beyond P90, the specificity of each abnormal lipid profiles was around 90%. Conclusions:The recommend cut-off points for body fat may be to assess children′s adiposity, and can be applied in preventive activities.
10.Association of vitamin D nutritional status with body muscle mass in school-age children adolescents

Objective:To investigate the association between vitamin D nutritional status and the body muscle mass in children.Methods:Data were obtained from School-based Cardiovascular and Bone Health Promotion Program. In 2017, a total of 15 391 children aged 6-16 years in Beijing were selected through stratified cluster sampling in baseline survey. A follow-up investigation was conducted in 2019. The questionnaire survey and the detection of serum 25-hydroxyvitamin D [25(OH)D] level were conducted. The bioelectrical impedance analysis (BIA) apparatus was used to measure body muscle mass, and muscle mass index (MMI) was calculated. Multivariable linear models were used to analyze the association of vitamin D nutritional status with the baseline and follow-up MMI measures.Results:A total of 10 890 children aged (11.5±3.3) years(boys accounting for 49.6%) were included in the analysis. The average 25(OH)D level was (35.4±12.0) nmol/L, with an adequacy ratio of 11.1%. After multivariate linear regression adjustment for age, sex, body fat mass, smoking status, alcohol use status, dairy supplement, calcium supplement, physical activity, and pubertal development, no statistically significant association between vitamin D nutritional status and baseline MMI level was observed ( P>0.05). For the follow-up MMI, the Z-score increased by 0.008 ( P=0.058) for per 10 nmol/L increase in 25(OH)D, which were 0.002 ( P=0.815) and 0.037 ( P=0.031) higher in children with insufficient and adequate vitamin D than those with vitamin D deficiency, respectively ( P for trend =0.089). Subgroup analysis showed that in the normal BMI group, for per 10 nmol/L increase in 25 (OH) D, the MMI at baseline survey and MMI Z-score at follow-up of children with adequate vitamin D and increased by 0.019 and 0.014, respectively (both P<0.05). Conclusions:Vitamin D nutritional status was related to muscle mass in children, and children with adequate vitamin D tended to obtain higher MMI. Children and adolescents are encouraged to maintain sufficient vitamin D levels, strengthen nutrition and exercise to promote body health.
