1.Construction and primary application of Sh-2K~d-HBc tetramer

Objective:To prepare and test tetrameric sH-2K~d-HBc complex for the further measurement of the specific CTL response.Methods:PE labled streptavidin with 4 biotinylated binding sites can bind to 4 biotinylated monomer to form the corresponding tetramer.Mice were immunized via different methods of genetic immunization by use of the construted pcDNA3-C plasmid to get the specific CTLs.Then our prepared tetramer was applied to stain the specific CTLs by the analysis of flow cytometry.Results:We applied our prepared tetramer to stain the cells from the experimental groups and control group.The results showed the tetramer was able to discriminate the frequencies of specific CTL induced by the three immunol methods(0.24%,0.26%,0.36% vs 0.07%,P≤0.05).This demonstrated that the prepared tetramer could bind its targets specifically and efficiently.The three immunol methods induced different levels of immune responses.Compared with the traditional muscle injection,gene gun induced weaker humoral immune response and stronger cellular immune response,and hydrodynamic injection induced the strongest humoral and cellular immune responses.Conclusion:Have successfully constructed the sH-2K~d-HBc tetramer.The techniques and methods can be used for preparation of tetramers of other types of MHCⅠ molecules.
2.Proximal femoral nail anti-rotation versus locking proximal femoral plate for osteoporotic unstable intertrochanteric fractures in senile patients

BACKGROUND: Senile intertrochanteric fracture is characterized as aging, instability, more complications and high mortality, so choosing an appropriate internal fixation method is critical.OBJECTIVE: To compare the curative efficacy of proximal femoral nail anti-rotation and locking proximal femoral plate for senile osteoporotic patients with unstable intertrochanteric fractures.METHODS: Sixty elderly patients with unstable intertrochanteric fractures were enrolled, and the clinical data were collected. They were randomized into treatment and control groups (n=30 per group), and subjected to internal fixation using proximal femoral nail anti-rotation and locking proximal femoral plate, respectively. The operative indexes,functional recovery and complications were observed in the two groups RESULTS AND CONCLUSION: (1) The incision length, operation time, intraoperative blood loss and time of weight bearing in the treatment group were significantly less than those in the control group (P < 0.01). (2) Harris hip scores in the treatment group were significantly higher than those in the control group (P < 0.05). (3) The incidence of complications in the treatment group was obviously lower than that in the control group. (4) To conclude, the proximal femoral nail anti-rotation exhibits satisfactory treatment outcomes in the internal fixation of unstable intertrochanteric fractures.
3.Study on the Quality Standard of Chanhou Zhuyu Capsules

Objective To establish the quality standard for Chanhou Zhuyu Capsules.Methods Leonurus Japonicus and Ligusticum chuanxiong in Chanhou Zhuyu Capsules were identified by TLC,and the content of stachydrine hydrochloride was determined by TLC Scanning.Results The quality identification by TLC was specific.Stachydrine hydrochloride had a good linearity in the range of 4.0~12.0 ?g,r=0.999 7,and the average recovery was 99.87 %,RSD=1.67 %.Conclusion The methods of identification and content determination are simple,reliable and reproducible.They can be used effectively for the quality control of Chanhou Zhuyu Capsules.
4.Changes in Bacterial Community Structure of Drinking Water in Dongjiang River Valley,Determined by Denaturing Gradient Gel Electrophoresis Fingerprint and Its Relationships with Environment Factors

In order to study the correlation between microbial diversity and the pollution degrees of the ruraldrinking water in Dongjiang River basin. Five types of drinking water of this basin were collected,and fifteen water samples of five types of drinking water of this basin had been collected from reservoir,centralized water supply wells,wells in the vicinity of pig farms,wells nearby embankment and wells in villages. The six(physical,chemical,and biological) property indices of water samples were tested,at the same time,the DGGE analysis was done. The results of PCR-DGGE fingerprint indicated that bacterial richness of these drinking water samples were high,and different samples in fingerprint were different distinctively. The UPGMA dendrogram of sample basis on DGGE fingerprints showed the structure of different types of bacteria in drinking water in rural communities is obvious differences. And the results of CCA showed that the concentration of phosphorous has the largest relevance to the community structure of bacteria in water samples,followed by the concentration of nitrogen in the water. Ten typical bands were excised and sequenced. The sequences obtained were affiliated with Spirochaetes,Cyanobacteria,Proteobacteria,Actinobacteria,Acidobacteria.
5.Prospective cohort study on the outcomes of sacrospinous ligament fixation using conventional instruments in treating stage Ⅲ-Ⅳ pelvic organ prolapse

Objective To evaluate the medium and long term safety and efficacy of sacrospinous ligament fixation (SSLF) performed with conventional instruments in treating stage Ⅲ-Ⅳ pelvic organ prolapse (POP).Methods A prospective cohort analysis was conducted in the Peking Union Medical College Hospital,between May 2007 and June 2015,enrolling 55 women with stage Ⅲ-Ⅳ POP who intended to receive SSLF.Primary end points were objective success rates using pelvic organ prolapse quantitation system (POP-Q) and subjective satisfaction rates with questionnaires after surgery according to vaginal examination and related questionnaires for all patients who received SSLF eventually.Exploratory outcomes included perioperative parameters and complications.Results Of these 55 POP patients enrolled,52 (95%,52/55) received SSLF using conventional surgical instruments,the other 3 cases converted to ischial spinous fascia fixation due to difficulty exposing.Medium blood loss during operation was 100 ml (20-300 ml) and operative time 60 minutes (20-165 minutes).Pelvic hematoma with diameters of 5 cm and 7 cm were observed in two patients,both recovered fully with conservative methods.All patients were able to micturate spontaneously after catheter withdrawal.One patient reported right thigh pain after operation which remained till 3-month follow-up and relieved after physiotherapy.The objective success rate was 100% (52/52) at 3 months.With a medium follow-up time of 23.7 months,the objective success rate was 98% (51/52),the recurrence rate was 2% (1/52) and the satisfactory rate was 94% (49/52).De novo urinary incontinence occurred in 6% (3/52) of patients.Conclusion Most POP could be corrected with SSLF using conventional instruments which is a feasible,economic and effective procedure for Asian patients with medium compartment prolapse.
6.Axis internal fixation system for the treatment of timeworn atlantoaxial dislocation with incomplete spinal cord injury

Objective To explore effect of Axis internal fixation system while decompression and fusion for the treatment of timeworn atlantoaxial dislocation with incomplete spinal cord injury.Methods 29 cases of timeworn atlanto-axial dislocation with incomplete spinal cord injury underwent decompression fusion and Axis internal fixation with a 7~46 months (mean 20 months) flowing-up.ResultsAfter treatment, all patients got completed fusion, there was no deterioration of neurofunction, and rate of JOA scores increased was 78.6%.ConclusionAxis internal fixation system can provide immediately a rigid stability and maintain axial correction for the segment ahile decompression and fusion for the treatment of timeworn atlantoaxial deslocation with incomplete spinal cord injury.

The modern surgical treatment of cervical degenerative disc disease can be traced back to the advent of anterior cervical decompression and fusion.With the emergence of fusion-related complications,different scholars have promoted the gradual transformation of cervical degenerative disc diseases from "fusion fixation" to "non-fusion reconstruction" through in-depth fusion with materials science,engineering mechanics and other disciplines.The innovation of this treatment concept is consistent with the original intention of "structural remodeling,functional reconstruction,maximum repair and reconstruction of the morphology and function of skeletal muscle system" in orthopedic bionic treatment,which is essentially in line with the "bionic alternative therapy" in orthopedic bionic therapy.This paper focuses on the surgical treatment of cervical degenerative disc diseases,reviews the development history of artificial cervical disc replacement,analyzes the evolution from orthopedic biomimetic therapy,and explores a new direction for the design of artificial cervical disc prostheses and the treatment of cervical degenerative disc diseases in the future.

8.Percutaneous transforaminal endoscopic TESSYS technique for the treatment of bilateral lumbar disc herniation in single segment.

OBJECTIVE:
To investigate the early clinical effects of transforaminal endoscopic spine system (TESSYS) for the treatment of bilateral lumbar disc herniation in single segment.
METHODS:
The clinical data of 38 patients with single-segment bilateral lumbar disc herniation treated by TESSYS technique from February 2016 to February 2018 were retrospectively analyzed. There were 26 males and 12 females, aged from 30 to 55 years old with an average of(35.2±6.4) years, 6 cases of L₃,₄, 22 cases of L₄,₅, and 10 cases of L₅S₁1. Using the intervertebral foramen endoscope produced by Joimax GmbH, Germany, under local anesthesia, bilateral puncture to the outside of the intervertebral foramen of the diseased segment, four-stage dilatation catheter to complete the progressive enlargement of the intervertebral foramen, and the ring saw progressively enlarge the intervertebral foramen. The bilateral foramen was placed and the herniated nucleus was removed until the nerve root was completely released. Postoperatively, the patients were reviewed on regular outpatient visits and telephone follow-ups. Visual analogue scale (VAS) and Oswestry Disability Index (ODI) were compared before operation and after operation at 1, 3, 6, 12 months respectively. At the final follow-up, according to modified MacNab criteria to evaluate the clinical effect.
RESULTS:
Thirty-six patients underwent successful surgery and were followed up for more than 12 months. The ODI score and VAS score of the lower extremities pain at 1, 3, 6, 12 months after operation were obviously improved (<0.05), there was significant difference between 1, 3 months and 6, 12 months after operation(<0.05), there was no significant difference between 1 and 3 months, between 6 and 12 months after operation (>0.05). At the final follow-up, according to MacNab criteria, 14 cases got excellent results, 16 good, 4 fair, 2 poor.
CONCLUSIONS
Using TESSYS technique to remove the bilateral herniated nucleus from single segment can fully decompress for the nerve root, and can be effectively applied to patients with single-segment bilateral lumbar disc herniation.

9.Effect of early ambulation time in lower limbs muscle strength in patients with total knee arthroplasty

Objective:To investigate the effect of early ambulation time on lower limbs muscle strength and fear of falling in patients with total knee arthroplasty, so as to provide basis for promoting the postoperative rehabilitation of patients undergoing total knee arthroplasty.Methods:A total of 210 patients after unilateral total knee replacement from July 2018 to December 2019 in the Second Affiliated Hospital of Soochow University were included and assigned to experimental group 1, experimental group 2 and experimental group 3 by random digits table method, each group contained 70 cases, and the patients began ambulation at 16, 20, 24 hours after knee replacement, respectively. The peak torque (PT) of knee joints flexors and extensor as well as hamstrings quadriceps ratio (H/Q) were compared among three groups at 1 week, 1 month and 3 months after knee replacement. The fall efficiency was evaluted by Modified Fall Efficacy Scale (MFES), the fear of falling rate was conducted by single item method.Results:After 1 month of knee replacement, the PT of knee joints flexor and extensor and H/Q were (18.73±5.49) N·m, (37.56±7.76) N·m, (48.08±9.19)% and (18.44±5.27) N·m, (37.04±7.07) N·m, (47.49±9.30)% in the experimental group 1 and experimental group 2, which were higher than those in the experimental group 3(16.38±2.85) N·m, (33.75±6.75) N·m, (43.48±7.17)%, the differences were statically significant ( t values were 2.316-3.057, P<0.05). After 1 week of knee replacement, the fear of falling rate were 72.3%(47/65) and MFES scores were (3.14±0.58) points in the experimental group 1, 53.7%(36/67), (3.81±0.65) points and 50.8%(32/63), (3.87±0.74) points in the experimental group 2 and experimental group 3, the fear of falling rate significantly increased and MFES scores significantly decreased in the experimental group 1 compared to the experimental group 2 and experimental group 3, the differences were statistically significant ( χ2 values were 5.780, 4.878, t values were 6.221, 6.129, P<0.05). Conclusions:Twenty hours after knee replacement is the best time for ambulation, which can shortern the time for lower limbs muscle strength recovery and reduce the risk of fear of falling.
10.Experimental study on the prevention of bone loss by deferoxamine in type 1 diabetic mice with osteoporosis

Objective To observe the effect of a hypoxia mimicking agent deferoxamine (DFO) on the mineral density,volume,architecture,strength,and metabolism of the bones in type 1 diabetic mice withosteoporosis.Methods Type 1 diabetic mice model was established by intraperitoneal injections of streptozotocin.The mice were divided into control (normal mice),diabetes mellitus,and DFO groups.Micro-CT was used to analyze the bone mineral density,volume,architecture,and strength of the trabecule in the distal part of femurs.Three point bending test was carried out to evaluate the bone strength.Hematoxylin and eosin (HE) staining was performed to observe the alteration in the number of osteoblasts.Real-time PCR was used to detect the mRNA expressions of Runt-related gene 2 (Runx-2),osteoclacin,and tartrate resistant acid phosphatase (TRAP) in tibias.Western blot was used to detect the protein expressions of Hypoxia-inducible factor-1α(HIF-1α) and vascular endothelial growth factor (VEGF) in tibias.Results There was a decrease in mineral density,volume,strength of bones as well as deteriorated trabecular microarchitecture in diabetic mice as compared to control mice,which were partially improved by DFO treatment.Moreover,DFO treatment increased the number of osteoblasts and mRNA expression levels of Runx-2,osteoclacin,TRAP,as well as protein expression levels of HIF-1 α and VEGF(P<0.05).Conclusion Bone loss could be partially prevented by DFO treatment in type 1 diabetic osteoporosis mice,which might be ascribed to increased bone formation via stimulating hypoxia inducible factor singnaling pathway.
