1.Risk factors related with the prognosis of patients with acute cerebral infarction after intravenous thrombolysis by alteplase

ObjectiveTo investigate the risk factors related with the prognosis of patients with acute cerebral infarction after intravenous thrombolysis by alteplase in 90 days.MethodsForty patients with acute cerebral infarction treated by intravenous thrombolysis with aheplase were selected and assigned into good outcome group [modified Rankin scale (mRS) 0-1 score] and poor outcome group (mRS 2-6 scores) according to mRS at 90 days.The risk factors were compared between two groups.ResultsPoor outcome group(22 cases) comparing with good outcome group (18 cases),the values of age[ (64.64±11.30) years vs.(58.94±10.47) years ],onset to therapy time(OTT) [(376.73±316.32)min vs.( 176.00±59.74) min],National Institutes of Health stroke scale (NIHSS) score before thrombolysis [ ( 16.73±8.46)scores vs. (11.22±5.20) scores],maximum of systolic blood pressure (SBPmax) in the first 24 hours after thrombolysis[ ( 186.59±24.79 )mm Hg ( 1 mm Hg =0.133 kPa) vs.( 169.00±23.04) mm Hg] and systolic blood pressure variability (SBPV)[(17.11±3.90) mm Hg vs.(13.33±4.97) mm Hg] had significant differences (P< 0.05).ConclusionThe values ofage,OTT,NIHSS score before thrombolysis,SBPmax and SBPV in the first 24 hours after thrombolysis are related with the prognosis of patients with acute cerebral infarction after intravenous thrombolysis by altcplase in 90 days.
2.OBSERVATIONS ON THE INNERVATION OF RABBIT AND RAT GASTRIC WALL BY MEANS OFHISTOCHEMICAL TECHNIQUE OF MONOAMINE FEUORESCENCE AND CHOLINESTERASE

The innervation and distribution of 5-HT cells in gastric wall of rabbit and rat were studied by means of histochemical techniques of monoamine fluorescence and cholinesterase. And the morphological relation between them was investigated using the consecutive method for demonstrating the fluorescence and ChE on the same section. At the fundic gland, 5-HT cells were in contact with both the adrenergic and cholinergic axonal terminals. There are adrenergic and cholinergic terminals to innervate the gland cells of fundic gland, partial fundic gland the dual axonal terminal are identical in their localization and morphological outline. The loyer of muscle and smooth muscles surrounding the arteriole in various stomach layers were innervated by both adrenergic and cholinergic terminals, some of them are superimposed at the samelocalization. There are a lot of nsChE nerve ending network in gastric lamina propria beneath epithelium of gastric mucous membrane, it was discussed about their sensory nature in this paper.
3.SYMPATHETIC AND PARASYMPATHETIC INNERVATION OF RAT HEART AND THEIR RELATION

Fifty two adult male rats were selected for the investigation the adrenergic and cholinergic innervation of rat heart by means of histochemical demonstration of catecholamine fluorescence and acetylcholinesterase (ACHE). Consecutive method was employed on the same section to demonstration the relation between the distribution of the sympathetic and parasympathetic nerves in various parts of rat heart, e. g. atrium, ventricular myocardium, valves, epicardium, endocardium, atrioventricular node and coronary arteries. Adrenergic and cholinergic terminals innervated all parts dually. By comparing the photographs demonstrating the fluorescence CA and AChE on the same section treated by the consecutive method, we found that the location, the density and morphology of both types of nerve terminals were more like. In other words, under light microscopy the localization of both terminals can hardly be distinguished from each other. Such kind of morphological relation may strongly support the results of interaction between sympathetic and parasympathetic nervous systems in physiological and pharmacological experiments of heart.In the cardiac ganglia there are some small intense fluorescence ceils (SIF-cells) lying besides the postganglionic cholinergic cells of the parasympathetic nervous system. Both kinds of cells were shown in close contact with each other in the same section with consecutive method. This morphological relation provided an evidence that catecholamine containing SIF-cells may control and regulate the neurotransmission of parasympathetic cholinergic neurons.
4.A combination of laparoscopy and choledochoscopy in the management of choledocholithiasis

Objective To evaluate the clinical applications and surgical methods of combined laparoscopic common bile duct (CBD) exploration with choledochoscopy. Methods From 2006 to 2009,clinical data of 42 patients with choledocholithiasis undergoing laparoscopic common bile duct exploration were retrospectively analyzed. We applied a step-by-step electric coagulating incision technique on the CBD,the step-by-step suturing technique, and the step-by-step clamping technique with alligator forceps, and soft tube irrigating technique with suctioning by selecting the proper exploration route, improving the common bile duct incision technique and calculus removing techniques. Results Procedures were successful in all the cases. There was no conversions to open surgery, no postoperative bleeding and no operative mortality. The mean operating time was 120 minutes (ranging, 90 to 150 minutes) with minimal intraoperative blood loss ( ranging, 20 to 40 ml). Ductal stone clearance was successful in 41 out of 42 patients ( 93% ). The largest number of the common bile duct stones was 16. With the diameter of stones larger than 15 mm in 18 cases in which the biggest was 30 mm. Bile leak developed in 1 patient, retained stones found in 3 patients,including intrahepatic cholelithiasis in one case. As a result, 38 out of 42 patients underwent common bile duct exploration. 35 patients were placed on T-tubes. Four patients underwent cystic duct exploration in which 3 had primary suture of the cystic duct and 1 had drainage. There was no infection and stenosis of biliary tract in the 42 followed-up cases. Conclusions Laparoscopic common bile duct exploration with stone extraction can be performed with high efficiency, minimal morbidity and without mortality. Improving the way of operation and selecting suitable exploration can result in better clinical outcomes.
5.Effect of computer-assisted postural balance training on balance disorders after cerebellar stroke

Objective To investigate the efficacy of computer-assisted postural balance training system combined with Bobath balance training on balance disorders after cerebellar stroke.Methods Forty patients with balance disorders after cerebellar stroke were randomly divided into either a combined training group or a control group (n =20 in each group).The control group was trained with Bobath balance training and routine rehabilitation treatment.On the basis of this,the combined training group used the computerassisted postural balance training system for rehabilitation treatment.The balance function,activities of daily living and motor function were evaluated with the Berg Balance Scale (BBS),modified Barthel Index,mBI)and Fugl-Meyer Assessment (FMA) before and after treatment,and the computer-assisted postural balance training system was used to conduct the postural balance ability according to the center of gravity track length,rectangle area of track,and peripheral area of track.Results There were no significant differences in various indexes before treatment between the patients of both groups.The scores of BBS,mBI and FMA after treatment were increased obviously (all P < 0.001).The the center of gravity track length,rectangle area of track,and peripheral area of track were decreased compared before treatment (P <0.001 or 0.05).In addition to the FMA score,all the indexes of the combined training group after treatment were significantly better than those of the control group (all P<0.05).Conclusions On the basis of Bobath balance training,using computer-assisted postural balance training system can obviously improve the balance disorder and extremities motion ability after cerebellar stroke,and thus improving the activities of daily living in cerebellar stroke patients.
6.Differences in the autistic severity and developmental level of children with autism spectrum disorder of different gender and age

Objective:To analyze the autistic severity and developmental level of children with autism spectrum disorder (ASD) of different gender and age.Methods:From March 2018 to February 2019, a total of 286 ASD children aged 18-96 months were enrolled.Severity of ASD was evaluated by the autism behavior checklist (ABC) and the childhood autism rating scale (CARS), while the Griffiths mental development scales-Chinese (GDS-C) was used to evaluate the children's mental development.According to gender and age, ASD severity and developmental levels of different subgroups were analyzed.SPSS 23.0 statistical software was used to analyze the data.According to the non-normal distrbution of variables, Mann-Whitney U test was used for comparison between the two groups, and Kruskal-Wallis H test was used for comparison between multiple groups and Spearman correlation analysis was used to analyze the correlation between two variables. Results:No gender difference was found in the total scores of ABC (52(40, 62), 57(36, 67), Z=0.661, P=0.509) and CARS (31(28, 35), 33(27, 36), Z=0.672, P=0.502) between the ASD boys and ASD girls.No gender difference was found in the total scores of ABC (43(33, 53), 52(34, 58), Z=0.717, P=0.473) and CARS (29(26, 32), 27(26, 30), Z=0.212, P=0.832) between the ASD boys and ASD girls when the general quotient of the GDS-C were all greater than or equal to 70 points.Also, no gender difference was found in the total ABC (55(44, 67), 59(40, 67), Z=0.067, P=0.947) and CARS (32(30, 35), 34(30, 36), Z=0.657, P=0.511) between the ASD boys and ASD girls when the general quotient of the GDS-C were all less than 70 points.The eye-hand coordination (64(52, 77), 60(43, 72), Z=2.138, P=0.033), performance (68(51, 86), 59(43, 68), Z=3.270, P=0.001), and practical reasoning (68(55, 91), 51(33, 58), Z=2.686, P=0.007) quotients of the ASD boys were significantly higher than those of the ASD girls.Compared with boys, a markedly higher proportion of developmental delay (35(80%), 124(51%), χ 2=12.083, P=0.001) was recorded for girls in relation to the performance subscale.There were significant differences in the total scores of ABC and CARS, the locomotor and eye-hand coordination quotients in GDS-C among ASD children of different ages(all P<0.05). The older the first diagnosis age, the lower the scores.The age of initial diagnosis was negatively correlated with the total scores of ABC ( r=-0.259), CARS ( r=-0.268), general quotient of GDS-C ( r=-0.127), locomotor ( r=-0.275), eye-hand coordination ( r=-0.213), performance ( r=-0.160) and practical reasoning ( r=-0.307) (all P<0.05), while positively correlated with hearing and language development quotient ( r=0.143) ( P<0.05). Conclusion:No significant gender difference was found on the severity of ASD in children aged 18 to 96 months, but the developmental level of some areas in boys was better than that of girls with ASD.ASD severity and developmental level vary depending on the age at which their ASD was diagnosed.The age of initial diagnosis is related to the severity and developmental level of ASD.
7.Topical application of leptin promotes burn wound healing in rats.

OBJECTIVETo investigate the effect of topical application of leptin in promoting burn wound healing in rats.
METHODSFour parallel second-degree burn wounds induced on the back of 18 Wistar rats were divided into leptin treatment group (treated with topical application of 400 ng/ml leptin dissolved in PBS) and control group (treated with PBS). The time of wound healing was recorded, and the wound area that was not healed was measured at 3, 7, 11, 15, and 19 days after burns. The tissue at the peripheries of the wound was sampled at 7, 14 and 21 days after burns for pathological examination with HE staining and immunohistochemistry for proliferating cell nuclear antigen (PCNA) to evaluate the proliferation of keratinocytes.
RESULTSCompared with the control group, leptin-treated wounds showed a shorter time (by 2-3 days) of wound healing, and significant differences were found between the two groups in healing at 7, 11, 15, and 19 days after burns. HE staining and immunohistochemistry revealed a faster rate of epidermis growth and a greater thickness of the cuticular layer in leptin-treated wounds at 7, 14 and 21 days after burns. PCNA positivity in the keratinocytes was stronger in leptin-treated wounds than in the control wounds at 7 and 14 days, but no such distinct difference was noted at 21 days between the two groups.
CONCLUSIONTopical application of leptin can promote re-epithelization in burn wounds to shorten the wound healing time of burns.

8.Correlation of urine phthalate metabolite levels with blood pressure in first-grade children

Objective:
To explore the correlation between blood pressure and urinary phthalandione, MMP, MEP, MnBP, MiBP, PAEs.
Methods:
Three schools were selected from Shenzhen, China for the present study. A total of 765 firstgrade students of Han ethnicity were recruited voluntarily from the selected schools during September 2016 to June 2017. They were divided into normal blood pressure (BP) group (lower than P90 group) and high BP group (BP≥P90). Linear and Logistic regression models were used to analyze the relationships between blood pressure and urine phthalate metabolite levels.
Results:
Urinary MMP and MnBP in students of high BP group were significantly higher than that of students in normal BP group(t=13.12, 3.97, P<0.05). Linear regression models showed that Z score increased when MMP and MnBP levels increased(P<0.05). Logistic regression model suggested that the risk of high BP increased with the increment of MMP level adjusting creatinine, sex, age and BMI(OR=1.47, P<0.05). There was no statistical significance in the differences after adjusting many factors including family income and education level of parents(P>0.05).
Conclusion
Urinary phthalate metabolite levels are positively associated with blood pressure in first-grade children.
9.Treatment of cerebral venous sinus thrombosis associated with intracranial hematoma: a systematic review

Objective To systemically evaluate the efficacy of all kinds of therapies of cerebral venous sinus thrombosis (CVST) associated with intracranial hematoma (ICH) and provide reference for making treatment scheme.Methods We searched English databases (Pubmed,ISI Web of science and Cochrane library databases) for publications on CVST associated with ICH and the relevant references of those articles.Retrieval time deadline was up to January 2015,and the language was set to English.All publications were searched without restrictions of publication type or published fields.Documents were screened to extract data and to analyze systemically.Results In total,17 articles fulfilled the inclusion criteria and included 31 patients.Because of the severities of reported cases were greatly different and treatments were varied,these patients were divided into 3 levels to get a better understanding of the efficacy.In 6 patients of stage Ⅰ,3 accepted anticoagulation,1 operative treatment,1 endovascular intervention and the left one used more than one methods;the prognosis was good and modified Rankin scale (mRS) score was 0 no matter which kind of therapy was performed.In 11 patients of stage Ⅱ,2 accepted anticoagulation enjoyed mRS scores of 0 and 6,3 operative treatment enjoyed mRS scores of 0,1 and 6,1 endovascular intervention enjoyed mRS scores of 0,and the left 5 used more than one method enjoyed mRS scores of 0,1,1,2 and 2.In 8 patients of stage Ⅲ,1 accepted anticoagulation enjoyed mRS scores of 2,3 operative treatment enjoyed mRS scores of 1,2 and 4,and 4 endovascular intervention enjoyed mRS scores of 0,1,1 and 1.Conclusion It seems that anticoagulation therapy alone may not be effectively enough for those patients that with severe clinical or imaging features;individualized therapy strategies with different types would be more effective.
10.Feature of semicircular canal function in obstructive sleep apnea-hypopnea syndrome.

OBJECTIVEThe aim of this study was to evaluate the effects of obstructive sleep apnea-hypopnea syndrome (OSAHS) on semicircular canal function.
METHODSBy means of a series prospective study at Department of Otolaryngology Head and Neck Surgery of our hospital, the study was performed on 77 patients suffering from OSAHS in a period from 2012 to 2014, who underwent polysomnography (PSG) and caloric test. The maximal slow-phase velocity (SPV) and unilateral weakness (UW) were used to measure the vestibular function. Severity of OSAHS was evaluated by the lowest oxygen saturation (LSaO₂) and apnea hypopnea index (AHI). The SPV after cool was signed, warm test was performed for each ear, and the sum of left ear SPV were calculated, and then, the SPV of right ear was counted as the same way. Finally, the relationships between LSaO₂, AHI, age, BMI, and SPV of caloric test were analyzed.
RESULTSCaloric vestibular tests in the 77 OSAHS patients demonstrated abnormal findings in 52 patients (67.5%) and normal vestibular functions in the remaining 25 patients (32.5%). Of the 52 patients with an abnormal test result, 16 (20.8%) patients had unilateral vestibular hyporeflexia and 36 (46.7%) patients revealed a bilateral vestibular hyporeflexia. There was no linear relationship between AHI, age, BMI with SPV of caloric test (P > 0.05). The SPV had significant difference between Lower LSaO₂group (LSaO₂< 50%) and higher LSaO₂group (LSaO₂≥ 80%) (P < 0.05). LSaO₂was lower in patients undertaken bilateral vestibular hyporeflexia.
CONCLUSIONSOSAHS patients with long-term intermittent hypoxia can disturb the vestibular organs and reduce semicircular canal function. The heavier hypoxemia will lead to the lower reflex of semicircular canal, with the heavier degree of hypoxemia, and the bilateral horizontal semicircular canal involvement may also be higher at the same time. Due to the effect of vestibule centre compensatory, OSAHS patients lack of dizziness and symptoms from balance disturbances such as typically acute vestibular damage.
