1.Application of 18F-FDG PET/CT and other conventional imaging technologies in carcinoma of unknown primary site

Carcinoma of unknown primary (CUP) is a diagnostic and therapeutic challenge in oncology.The early and correct detection of the primary tumors of CUP is very important to its diagnosis, treatment, therapeutic response evaluation and prognosis prediction.This review summarizes the implications and development of the imaging technologies, especially 18F-FDG PET/CT scan, in the diagnosis, treatment and prognosis prediction of CUP patients.
2.Histopathological features of bone metastases from prostatic carcinoma after castration

Objective To understand the histologic changes of prostatic carcinoma's bone metastases and other associated observations,and to provide histopathological basis for clinical therapy. Methods Seven patients with prostatic carcinoma bone metastases,whose needle biopsy of prostate and bone metastases was positive,were treated by hormone castrative therapy.The changes of prostatic primary carcinoma,bone metastases and correlated examinations were observed,and the results were compared with those before treatment. Results The prostatic tumors of 7 patients disappeared in 2 months after treatment.The prostate specific antigen (PSA) level of 5 patients returned to normal within 2 months.Two patients had orthopedics surgery because of spinal epidural compression symptom,and their pathologic examination of bone metastases showed that the transparent cells disappeared,the number of tumor bone trabecula reduced and immunohistochemical stains of PSA was negative.Another 2 patients had bone excision because of the development of bone metastases.Transparent acinose cells,mixed with many tumor-like bone tissues,could be found in the focus,with immunohistochemical stains being positive compatible with the changes of focal bone,PSA and imageology examination showed corresponding results. Conclusions Castration therapy can make carcinoma cells of the prostate and bone metastases,partially or completely,disappear.However,part of bone metastases changes may not be consistent with those of primary carcinoma.PSA and imageology examination may show the correlated changes of bone metastases,and provide exact basis for the clinical treatment.
3.Analysis of miRNA-34c expression in cervical cancer tissue and preliminary identification of Polo-like kinase 4 as its target gene

Objective:To analyze the expression profile of miRNA-34c in cervical cancer tissue and identify its novel target gene. Methods:The expression levels of miRNA-34c were detected in 34 paired cervical cancer tissues and normal paraneoplastic tissues via quantitative reverse transcription polymerase chain reaction (qRT-PCR). Polo-like kinase 4 (PLK4) is a target gene of miRNA-34c pre-dicted in the miRNA Target Database. Luciferase vector containing the binding site of miRNA-34c to PLK4 3'UTR and miRNA-34c mimic or negative control were co-transfected into HEK293T cells, and luciferase expression was examined. The miRNA-34c mimic or negative control was transfected into SiHa cells, and the mRNA or protein expression of PLK4 was detected via qRT-PCR or Western blot, respectively. Results:MiRNA-34c expression was lower in cervical cancer tissues than in normal paraneoplastic tissues. The miR-NA-34c mimics significantly inhibited luciferase activation in the HEK293T cells (P<0.01) and significantly decreased the mRNA and protein expression levels of PLK4 in the SiHa cells (P<0.01). Conclusion:MiRNA-34c is significantly decreased in cervical cancer tis-sues. Moreover, miRNA-34c can significantly repress the mRNA and protein expression levels of PLK4 by directly targeting the 3'UTR.
4.Effect of Comprehensive Rehabilitation on Elbow Joint Dysfunction in Children with Humeral Supracondylar Fracture

Objective To explore the effects of comprehensive rehabilitation on elbow joint dysfunction in preschool children with humeral supracondylar fracture. Methods 16 cases of humeral supracondylar fracture with elbow joint dysfunction were treated with fumigation of Chinese medicine combined with manipulation and physical therapy. The range of motion (ROM) and Hospital for Special Surgery Scoring System (HSS2) were used to evaluate the effects of treatment. Results ROM of elbow joint improved significantly after treatment (P<0.001), and all the children reached "excellence" after treatment according to HSS2. Conclusion Comprehensive rehabilitation is effective on elbow joint dysfunction in preschool children with humeral supracondylar fracture.
5.Improvement of Field Liquid Transfusion Control System

Objective To improve the stability and practicability of field liquid transfusion control system.Methods SPCE062A 's high accurate AD was used to acquire the dropping speed,and a new method for weight sensor 's self proofread was applied.Results The infrared photoelectric sensor was removed,so the monitoring part could work with any appropriate sensor.Conclusion The system cost is decreased and its stability and availability are enhanced.[Chinese Medical Equipment Journal,2008,29(2):66-67]
6.Development of Nursing Station Infusion Monitoring System

7.The clinical application of video-laryngoscope in spontaneous respiration tracheal intubation

ObjectiveTo assess the safety and clinical values of video-laryngoscope in spontaneous respiration tracheal intubation for emergency patients. Methods Seventy-nine patients,who needed the endotracheal intubation,were recruited in our department between January 2010 and December 2010,and were randomly ( random number) divided into two groups according to consultative sequence.Forty patients (group A ) were operated with traditional laryngoscope and thirty-nine patients (group B ) with videolaryngoscope.The operative time and success rate of tracheal intubation,Cormack-Lehane classification,as well as adverse events,were recorded.The heart rate ( HR ),mean arterial blood pressure ( MAP),respiratory rate ( RR ),and saturation of pulse oxygen ( SpO2 ) were observed pre-operation,during operation and 2 min post-operation.Results( 1 ) The Cormack-Lehane classification in group A were significantly lower than in group B. (2) The operative time of tracheal intubation in group B was significantly less than that in group A [(35.6+12.7) svs. (58.3 ± 13.5) s; P<0.05] ; and one-time success rate of tracheal intubation in group B was higher than that in group A ( 84.6% vs.52.5% ; P <0.05).(3) Compared to group B,the HR and MAP in Group A were significantly increased at t2 and t3 ( P < 0.05 ). ( 4 ) The adverse events,including restlessness,bucking and injury,were significantly decreased in group B than those in group A ( P < 0.05 ).ConclusionsThe video-laryngoscope used in spontaneous respiration tracheal intubation,could improve Cormack-Lehane classification,short operative time,enhance one-time success rate and reduce adverse evevnts.
8.Observation on the effect of continuous infusion of dopamine and furosemide in treatment of 76 elderly patients with refractory heart failure

Objective To explore the clinical effect of continuous infusion of dopamine and furosemide on elderly patients with refractory heart failure.Methods 76 elderly patients with heart failure were selected as the observation subjects,they were randomly divided into observation group and control group,38 cases in each group.The control group was given oxygen inhalation,the expansion of vascular,cardiac and other conventional treatment.The observation group was given conventional treatment and continuous infusion of dopamine and furosemide.Clinical efficacy and cardiac function improvement were compared between the two groups.Restlts The effective rate of observation group was 92.1%,which was higher than that of the control group (73.7%) (x2 =10.29,P < 0.05).After treatment,the improvement of cardiac function between the two groups had significant difference (t =15.94,10.18,all P <0.05).Conclusion Continuous infusion of dopamine and furosemide in treatment of elderly patients with refractory heart failure could improve heart function,enhance curative effect.
9.Influence of the Altered-Cast Impression Technique on Distal-Extension Alveolar Ridge Morphology and Restorative Result

Objective To investigate the impact of the modified cast impression technique on distal-extension alve-olar ridge morphology of partially edentulous casts and the restoration outcome of removable partial dentures (PRDs). Meth-ods Fifty-four patients with mandibular free-end dentition defect were selected. Impressions were taken from each patient using both modified cast technique and conventional technique. Coronary cross-sections of the casts were scanned from dis-tal to mesial extension;the images were compared to reveal morphological differences. And the relationship between these differences and location of the cross-sections were analyzed. After the patients were evenly divided into two groups per paired design, the impressions were taken using the respective modified cast and conventional impression techniques to pre-pare RPDs. The subsequent comparisons between the two groups included frequency of adjustments during the adaptive phase, masticatory efficiency of the denture at ending of the adaptive phase and visible movement of the extension base dur-ing mastication. SPSS 16.0 software was used for statistical analysis. Results The number of pixels in the same cross-sec-tion was less in the impression taken with modified cast technique than that with conventional technique. And their differ-ence is of statistical significance (P < 0.01). The difference in the number of pixels (D) was negatively correlated with the number of cutting and grinding, and N (location of the cross-sections). In addition, there were statistically significant differ-ences in frequency of adjustments during the adaptive phase (times:1.12 ± 0.77 vs 3.41 ± 0.82), masticatory efficiency of the denture (0 vs 18.5%) and visible movement of the extension base during mastication (96.3%vs 74.1%) between the modified cast group and conventional group. Conclusion Morphology of distal-extension alveolar ridge of the casts differs signifi-cantly between the impressions obtained with modified cast technique and the impressions obtained with the conventional technique;the difference increases with greater distance to the distal surface of the terminal abutments. The restoration out-come of RPDs is significantly better with the modified cast impression technique than with the conventional impression tech-nique.
10.Dose rate distribution on cultured cell exposed to ~(125)I seed in vitro irradiation

Objective To study the dose rate distribution on cultured cell plane and establish a reference dose rate table of in vitro cell line ~(125)I seed irradiator.Methods Thermoluminescence dosimetry(TLD) was used to measure the irradiation dose rate of a single 6711 model(~(125) I) seed with apparent activity of 10.323 MBq in water at point P.Meanwhile,the theoretic value of the irradiation dose rate at point P was calculated with theoretic formula.The difference between the calculated and observed values within 10% was set as standard to analyze the accuracy of the measurement.The irradiation dose rate of a single 6711 model(~(125) I) seed was measured in 1mm-thick polystyrene + water medium at point P.The value was applied to differential or non-differential proof along with the value from water medium to study the effect of 1 mm thick polystyrene on distribution of irradiation dose in water.Finally,by simulating the(~(125) I) seed plane irradiator with nine(~(125) I) seeds,the distribution table of irradiation dose rate on the cultured cell plane was calculated with theoretical formula.Results The observed value(n= 10) of irradiation dose rate with one(~(125) I) seed in water at point P was(0.359?0.023)cGy/h and the calculated value was 0.347cGy/h,the difference was within 10%.The observed value(n=10) of irradiation dose of one(~(125) I) seed in 1mm-thick polystyrene + water medium at point P was (0.350?0.027)cGy/h,which showed no statistical difference from the observed value in water under differential and non-differential proof.The reference table on dose rate distribution for cells exposed to(~(125) I) seed irradiation in vitro was developed.Conclusions 1mm-thick polystyrene gives no significant effect on irradiation dose rate distribution from(~(125) I) seeds in water.A reference table on the dose rate distribution for cells exposed to(~(125) I) seed irradiation in vitro has been developed,which can be used to determine an optimal irradiating strategy for future work.
