1.Analysis of the influencing factors for screening primary aldosteronism

Objective To confirm the most effectively screening conditions by comparing the ARR values of different posi‐tions after saline load test .Methods After exclusion the other secondary hypertension ,the completion of saline dressing test ,84 ca‐ses diagnosed hypertensive patients under saline load test results were divided into primary aldosteronism group (pH ,52 cases) and primary hypertension group (32 cases) .Analyzed plasma aldosterone (PAC) ,renin (PRA) values of upright and decumbent body positions ,their ARR values were calculated and the urine aldosterone was tested .Results Those whose upright position ARR rati‐os higher than 240(pg/mL)/(ng · mL -1 · h-1 ) were accounted for 80 .77% (42/52) and 43 .75% (14/32) ,respectively in the both PA group and PH group;Those whose upright position ARR ratios higher than 240(pg/mL)/(ng · mL -1 · h-1 ) and decumbent position PAC higher than 150 pg/mL were accounted for 65 .39% (34/52) and 3 .13% (1/32) ,respectively in the both PA group and ph group;The specificity of this method for PA was 96 .88% .Those whose upright position ARR ratios higher than 240(pg/mL)/(ng · mL -1 · h-1 ) and decumbent position PAC higher than 150 pg/mL and urine aldosterone higher than 10 μg/24 h ,The specificity of this method for PA was 100 .00% .Conclusion It is an effective screening method for primary aldosteronism when ARR values of upright position higher than 240(pg/mL)/(ng · mL -1 · h-1 ) ,PAC of decumbent position higher than 150 pg/mL and urine aldosterone higher than 10 μg/24 h .
2.Losartan reduces cardiomyocyte apoptosis in rats with heart failure

Objective To find the inpact(s) of Losartan on myocardial apoptosis in rats with heart failure. Methods Eight-week-old male SD rats were divided into control group (group C) , heart failure (group HF) and treatment group (group LS). Use of doxorubicin injection model, group LS at the same time in a row to the oral losartan, myocardial ultrastructure was examined by TEM. DNA-situ terminal labeling (TUNEL method) was used to detect myocardial apoptosis and myocardial BAX, BCL-2 protein expression was detected by immunohistochemistry. Results Group HF showed significant myocardial cell injury, and apoptotic bodies were found, serum CPK, CK-MB and LDH were increased. In LS group, myocardial apoptosis index was lower (P <0. 01) , myocardial cells BCL-2 expression, BAX expression reduced as compared with those in HF group. Conclusion Losartan can effectively inhibit the process of heart failure occurred in cardiomyocyte apoptosis, reverse myocardial damage and improve the prognosis of heart failure.

4.The clinicopathology analysis of three cases of carcinoma with multi-directional differentiation derived from junction of bladder and prostate

To explore the clinical ,pathological and immunohistochemical features of carcinoma with multi-directional differentiation derived from junction of bladder and prostate .We collected clinical data and tissue samples of three typical cases of carcinoma from our hospital .Routine preparation of slides and immunohistochem-ical methods combined with clinical data and pathological changes analysis were adopted .We found that all 3 ca-ses occurred in the elderly lesions involving the bladder and prostate .Dysuria was the main common symptom .All the cases had a history of chronic inflammation in urethral and prostate gland .The following up data showed that survival time in two of them were no more than 15 months.The third patient without chemotherapy who took drugs of bicalutamide,enantone and pamidronate still was alive after 28 months.The pathological changes of these cases had the common features with diversity .They all showed the structure with nests ,papillary,solid,adenoid,single or syncytial cells .The nuclear of cancer cell was enlarged with hyperchromatic feature .The mitotic figures were easily found.The metaplasia and atypical hyperplasia of prostate were all found .The immunohistochemistry results showed positive results for HCK,LCK,p53,p63,PSA,P504S and negative for vimentin and S -100.The average proliferation index of Ki-67 was 77%.

7.Balance ability and foot type characteristics during different postures of standing

BACKGROUND: Changes in the center of body weight during standing reflects the structure, function and muscles of the lower limbs and even the whole body. The relationship between standing posture control and foot type, balance ability, and muscle activation is rarely reported. OBJECTIVE: To investigate the effects of natural, lean forward and lean backward standing on foot arch, body balance and muscle activation. METHODS: Fifteen subjects without foot lesions and diseases related to foot function were selected. Three-dimensional foot scanner was used to test and analyze the foot arch in three standing postures. BTS FREEMG 300 surface electromyography tester and balance tester were used to test and analyze the maximum root mean square of muscle and two balance ability indexes (envelope area and average velocity of center of gravity) in three standing postures. RESULTS AND CONCLUSION: (1) For height index of foot arch: there was significant difference between natural standing and forward standing (P < 0.05). There was significant difference between natural standing and backward standing (P < 0.05), but there was no significant difference between forward standing and backward standing (P> 0.05). (2) The degree of muscle activation was as follows: when standing naturally, the medial peroneal muscles> erector muscles> rectus abdominis> gluteus medius> biceps femoris> external oblique femoris> anterior tibial muscle> rectus femoris muscle> gluteus maximus muscle. During forward standing: the medial of the peroneal muscle> medius gluteus> vertical ridge> long head of biceps femoris> anterior tibial muscle> rectus abdominis muscle> gluteus maximus. During backward standing: the obliquus externus femur> rectus femoris> long head of biceps femoris> erector spinae> musculus rectus abdominis> the medial of the peroneal muscle> gluteus medius> gluteus maximus. (3) Balance: there was no significant difference between natural standing and forward standing (P> 0.05); there was significant difference between natural standing and backward standing (P < 0.05), but there was no significant difference between forward standing and backward standing (P> 0.05). (4) In summary, it is easy to maintain body stability at low muscle activation intensity when the human body is standing naturally. During the body backward standing, the muscle activation intensity is highest due to instability. When the center of gravity moves forward and backward, the contralateral muscle is mainly activated. The arch of the foot rises when it leans forward and backward. The slight anterior and posterior movements of the center of gravity in the standing posture can lead to changes in muscle activation degree and balance control.

9.The diagnostic value of PLS-DA model based on seven serum cytokines for children patients with mycoplasma pneumonia

Objective To investigate the diagnostic value of the partial least square discriminant analysis (PLS-DA) model based on seven serum cytokines for children patients with mycoplasma pneumonia,the 7 cytokines includ interleukin-2 (IL-2),IL-4,IL-6,IL-8,IL-10,tumor necrosis factor alpha (TNF-α) and interferonγ(IFN-γ).Methods Serum levels of cytokines were measured by the double antibody sandwich method of ELI-SA in 140 patients with pneumoniae infection and 135 normal healthy controls,and data was analyzed with the receiver operation characteristic (ROC) curve and the PLS-DA.Results The area under the ROC curve (AUC) of serum IL-10 and IFN-γfor the diagnosis of MPP was 0.84 (95% CL:0.79 ~ 0.89).The sensitivity of IL-10 and IFN-γ for the diagnosis of MPP was 91.4% and 82.1%,with the specificity was 77.0% and 82.2%,respectively.The diagnostic accuracy of the PLS-DA model based on seven serum cytokines for children MPP and the controls was 90.0% and 88.15%,and the prediction accuracy was 86.4% and 87.4%,respectively.Conclusion Seven serum cytokines based on PLS-DA model was helpful for the diagnosis of children patients with MPP.
10.A randomized clinical trial of chemotherapy combined with oxaliplatin and cisplatin plus navelbine in the treatment of advanced non-small cell lung cancer.

BACKGROUNDCisplatin plus navelbine (NO) is a standard combination chemotherapy regimen for non-small cell lung cancer (NSCLC), but severe toxicities due to cisplatin often influence the quality of life in the patients, such as nausea and vomiting. The aims of this study are to evaluate the efficacy and toxicity of combined chemotherapy of oxaliplatin and cisplatin plus navelbine in treatment of advanced NSCLC.
METHODSA total of 168 patients were randomly divided into NO group (n=83) and NP group (n=85).
RESULTSThe overall response rate was 38.6% in NO group and 40.0% in NP group respectively. 1-year survival rate was 33.7% in NO group and 31.8% in NP group. There was no significant difference in response rate and 1-year survival between the two groups (P > 0.05). The major side effects were myelosuppression, nausea/vomiting and neurotoxicity in the two groups. Incidences of leukopenia and nausea/vomiting were significantly lower in NO group than those in NP group (P < 0.05), but neurotoxicity in NO group was more obvious than that in NP group (P < 0.001).
CONCLUSIONSThe efficacy and 1-year survival of combined chemotherapy of oxaliplatin and cisplatin plus navelbine are similar in the treatment of advanced NSCLC. However, the side effects of oxaliplatin plus navelbine are lower except for neurotoxicity.
