1.Establishment of a cisplatin-resistant mouse model of 4T1 triple negative breast cancer

Objective To establish a cisplatin-resistant 4T1 mouse model of triple negative breast cancer .Meth-ods A drug resistant mice model was established with cisplatin ( DDP ) induction and in-vivo/in-vitro tumorigenic ap-proach.Its resistance characteristics were identified by MTT assay .Changes of drug resistance gene ( MDR1, BCRP, MMP7, GST-π) and protein ( P-gp, BCRP, MMP7) expression, and phosphorate-Akt and total-Akt protein expression were evaluated by real-time PCR, immunohistochemistry and western blot method , respectively.Small animal live imaging technology was applied to detect tumor growth .Results Resistance fold (RF) of cisplatin-resistant 4T1 mouse model was 12.84.The expression of MDR1, BCRP, MMP 7, GST-πmRNA and P-gp, BCRP, MMP 7 proteins in the resistant mice were higher than that in the non-resistant mice .The result of western blot showed that a statistically higher expression of p-Akt in resistant mice than that in non-resistant mice at protein levels (P<0.01).No significant difference of tumor growth rate was observed between non-resistant and resistant mice ( P>0.05 ) .Given same dose of DDP , resistant mice showed lower sensitivity than non-resistant mice significantly (P<0.01).Conclusions We have successfully established a cis-platin-resistant triple negative breast cancer model in mice , which provides a new platform for further study on chemoresis-tant reversal strategy and individualized clinical treatment of this disease .
2.Expression of p53 and E-cadherin in gastric adenocarcinoma and their relationship with prognosis

Purpose To investigate the protein expression of p53 and E-cadherin in gastric adenocarcinoma and the effects of them and the clinicopathological parameters on prognosis of patients.Methods Expression of p53,E-cadherin in 150 cases of gastric adenocarcinoma was detected by tissue microarray and immunohistochemistry.The effects of them and the clinicopathological parameters on the prognosis of patients were studied by Kaplan-Meier method to calculate and compare the cumulative survival rate and postoperative mean survival time.Multivariate analysis was also performed by COX regression.Results The positive rate of p53 and E-cadherin expression in 150 cases of gastric adenocarcinoma was 31.3% and 91.3%,respectively.74 cases were followed up, and the 1-,3-,and 5-year survival rates were 83.8%,70.3% and 63.5%,respectively.The invasion, lymph node status and pTNM stages were prognostic factors;multivariate analysis revealed that p53,age and lymph node status were independent prognostic invasion, lymph node status and pTNM stages are important prognostic factors in gastric adenocarcinoma. Nevertheless, E-cadherin and histological classification are not prognostic factors.
3.Effects of CXXC ifnger protein 5 up-regulated expression in epithelial ovarian cancer

Background and purpose:Epithelial ovarian cancer has the highest mortality rate of gynecologic cancers and overall survival rates have improved little in the last 20 to 30 years. CXXC ifnger protein 5 (CXXC5) plays an important role in AML (acute myeloid leukemia) and MDS (myelodysplasia). However, little is known about its clinical signiifcance and biological function in epithelial ovarian cancer. This study aimed to investigate the expression of the CXXC5 in ovarian cancer and the effect of the CXXC5 on ES-2 cell proliferation. Methods:①The alteration of CXXC5 in cancer genomics data of TCGA (Cancer Genome Atlas) was analyzed.②The CXXC5 protein in the tissue chips was detected containing 37 benign ovarian cyst and 173 malignant tumor samples. The relationship between the expression of the CXXC5 with the clinicopathological features of patients with ovarian cancer was analyzed by SPSS software;③The cells with the highest CXXC5 expression quantity from 5 ovarian cancer cells were selected by re-al-time quantitative PCR (qRT-PCR) and Western blot.④ES-2 cells with shRNA stable transfection were construted us-ing the strategy of lentivirus infection and analyzed cell proliferation by cell counting kit-8(CCK8). Results:①Through the TCGA database, CXXC5 ampliifcation was found in 7 of 563 cases.②The CXXC5 expression in ovarian malignant carcinoma (39.3%) was higher than that in benign ovarian cyst (13.5%, P=0.003), the histologic type was highly asso-ciated with CXXC5 (43%in serous, 22.9%in mucinous, 23.5%in endometrioid, 67%in clear cell, P=0.014) and there was a signiifcant correlation between CXXC5 and lymph node metastasis (positive vs negative, P=0.022).③The ES-2 cells with shRNA stable transfection had a growth disadvantage (P<0.05). Conclusion:The CXXC5 gene might have an advantage in proliferation of epithelial ovarian carcinoma and be expected to become the biomarker of poor prognosis.
4.Discussion of relationship between quality of life and clinical effect assessment of malignant tumor treated with traditional Chinese medicine

Traditional Chinese Medicine (TCM) therapy is effective in treating malignant tumor. TCM therapy is a multi-way, multi-layer and multi-target integrated treatment characterized by "survival with tumor" and improvement of life quality. An ideal result of the TCM therapy for cancer should be a positive response of the patient with satisfactory quality of life (QOL) and longer survival time. So it is necessary to introduce the QOL, quantification of symptoms and signs and standardization of TCM syndromes to the objective, quantitative and standardized effect assessment of TCM therapy for cancer. A criterion for evaluating the effect of TCM therapy on tumors should be established, which reflects the characteristics and advantages of the TCM therapy, and meets the requirement of the developing effect assessment of modern medicine in cancer treatment. It is suggested that the treatment effect should be the sum of quality adjusted life years (QALYs) (equal to QOL multiplied by time of survival), syndrome remission rate and tumor remission rate. But the proportion of each of the three is variable, which has close relationships with the different tumor stages and corresponding treatments.
5.Reconstruction of rabbit knee joint cartilage defect using tissues engineering method

Objective To estimate curative effect of reconstruction of rabbit knee joint cartilage defect with the homogeneitic tissue engineered cartilages.Methods The chondrocytes were isolated and collected from articular cartilages of eight New Zealand white rabbits.The tissue engineered cartilages after culturing chondrocytes and atelocollogen for two days.Cartilage defects were created in both keen joint of twenty-six rab- bits.Complexes of chodrocytes and atelocollagen was grafted into the defect of left knee joint at once as experi- mental group,and no implantation were served as control.General and histological examination were respec- tively performed in both group at four weeks and eight weeks after surgery.Results After implantation,the defects were filled with cartilaginous tissue in experiment group,while there were only tissue in control group. Histologically,defective areas were filled with chondrocytes in experiment group,but only fibroblast in control group.Conclusion The implantation of the tissue engineered cartilages contenting with chondrocytes and atelocollogen can effectively improve reconstruction of rabbit knee joint.
6.Characteristics of SA and MRSA Colonization in Orthopedic Inpatients and their Correlation with Postoperative SSI

Objective To investigate the colonization characteristics of staphylococcus aureus(SA)and methicillin-resistant staphylococcus aureus(MRSA)and their correlation with postoperative surgical site infection(SSI)in orthopedic inpatients,and to ana-lyze the related factors affecting their colonization.Methods Nasal swab and pharyngeal swab samples of 420 patients hospitalized in the Department of Orthopedics,Tongde Hospital of Zhejiang Province from January 2020 to October 2022 were collected.Polymerase chain re-action(PCR)was used to detect the positive colonization of SA and MRSA,and clinical data of patients were obtained.x2 test was used to analyze the correlation between SA positive colonization and postoperative SSI.Univariate and multivariate Logistic regression analysis were used to explore the related factors affecting SA colonization.Results Among 420 patients,19 patients were positive for SA,and the overall SA colonization positive rate was 4.5%.17 patients were positive for MRSA,and the overall MRSA colonization positive rate was 4.0%.There was no statistically significant difference in postoperative SSI between SA colonization positive and SA colonization negative patients(P>0.05).Thus,there was no significant correlation between SA colonization and the occurrence of postoperative SSI.Univari-ate Logistic regression analysis showed that age and body mass index(BMI)were influencing factors for SA colonization,while multivari-ate Logistic regression analysis showed that BMI was the risk factor for SA colonization,and the higher the BMI,the higher the colonization risk.Age was the protective factor for SA colonization,and the risk of SA colonization decreased with increasing age.Conclusion SA colonization in orthopedic inpatients does not increase the risk of postoperative SSI,and preoperative SA decolonization that increases the patient's medical cost is unnecessary.BMI is the risk factor for SA colonization in orthopedic inpatients,and age is the protective factor.SA colonization of orthopedic inpatients has regional attributes,and each region needs to make medical plans according to its regional SA colonization epidemiological characteristics.
7.The clinic effect of mini open endoscope assisted-ACDF for cervical spondylotic radiculopathy

Objective Study clinic effect of mini open endoscope assisted-ACDF(MOEA-ACDF)for cervical spondylotic radiculopathy(CSR).Methods From February 2022 to March 2023,CSR patients receiving ACDF in the hospital were included in this study,including conventional ACDF group and MOEA-ACDF group.The preoperative data,operative timeand bone graft fusionwere recorded,VAS score,JOA score,C2-7 Cobb angle and anterior column height of the surgical segment were assessed before and after surgery,and complications were recorded,including wound infection,poor wound healing,hematoma formation,cerebrospinal fluid leakage,nerve injury,and complications related internal fixation.Results There were 41 patients in the conventional ACDF group and 41 patients in the MOEA-ACDF group.Between the two groups,there was no statistically significant difference in preoperative general condition,and there was no statistically significant difference in postoperative VAS pain score,JOA score,C2-7 Cobb angle,and anterior column height.Compared with the conventional ACDF group,the MOEA-ACDF group had a slightly longer surgical time(P=0.02).Compared with pre-operation,the VAS pain scorewas reduced,JOA score,C2-7 Cobb angle,and anterior column height were improved in both groups after operation.There were no postoperative complications such as wound infection,poor wound healing,hematoma formation,cerebrospinal fluid leakage,or nerve damage in both groups.Conclusions The MOEA-ACDF technology for treating CSR can effectively alleviate clinical symptoms,restore cervical spine height and curvature,and achieve clinical effects similar to conventional ACDF surgery,having good effectiveness and safety.Although the operation time of MOEA-ACDF technology is slightly longer than conventional ACDF,it has the advantages of clear and broad surgical field,deep focus,good lighting,less bleeding,and minimal trauma,which makes it con-venient for the operator to perform cervical disc removal,nerve root canal release,posterior longitudinal ligament resection,and reduce nerve,spinal cord,dura mater,and vertebral artery injuries.It is a technology worth pro-moting and applying.
8.Best evidence audit and analysis to the medication management for patients with dysphagia

Objective:To review the clinical status based on the best evidence of drug administration in patients with dysphagia, systematically analyze the obstacle factors and promoting factors in the process of evidence transformation, and formulate reform strategies.Methods:Based on the evidence-based nursing research method and the guidance of the Ottawa Model of Research Use (OMRU), the review indicators were developed based on the best evidence. The current status of clinical practice behaviors of 223 patients and 75 nurses in the Neurology, Neurosurgery and Geriatric departments of the Affiliated Hospital of Jiangsu University were reviewed from July to December 2021.Based on the results of the review, qualitative interviews were conducted with 32 potential adopters, and content analysis was used to assess the barriers and contributing factors to the clinical translation of evidence in three aspects: evidence-based change, potential adopters and practice environment, so as to develop effective strategies.Results:Based on the 22 best evidence selected, the evidence-based team developed 25 review indicators to carry out clinical review, showing that the compliance rate of 16 indicators were less than 60%. By analyzing and summarizing the interview results of potential adopters, the main obstacles leading to the low compliance rate of nurses were analyzed as follows: evidence-based reform changed the traditional work mode, and the application of evidence was not convenient; at the level of potential adopters, nurses had poor knowledge and practice, heavy work burden, and low awareness of patients and caregivers; at the level of practice environment, there was lack of nursing norms and procedures for clinical transformation of evidence, and the channels of multi-disciplinary collaboration and communication were not smooth. The main promoting factors were the perfect supervision mechanism of evidence-based nursing projects, the evidence-based group had rich experience in evidence transformation, the management was willing to change, and the practitioners were good at innovation.Conclusions:There is still a large gap between the clinical practice and the best evidence of drug administration in patients with dysphagia. The promoting factors should be fully utilized to overcome the obstacles and implement improvements to promote the effective transformation of evidence into clinical practice.
