1.Imaging features of solitary fibrous tumors in the head and neck

Objective To explore the imaging features of solitary fibrous tumor (SFT)in the head and neck.Methods The CT and MR findings of 10 patients with SFT in the head and neck confirmed by hsitopathology were analyzed retrospectively.All patients underwent CT (including contrast-enhanced dual-phase CT),and MR imaging was obtained in 2 patients.Results All 10 lesions were found as solitary masses ranging in maximum diameter from 20 to 1 1 5 mm (average 35.5 mm),with the location in the orbit (2 cases),nasal cavity and nasal sinuses (4 cases),parotid gland (1 case),supraclavicular region (1 case),subcutaneous tissue of the maxillofacial (1 case)and occipital region (1 case).The lesions showed elliptic shape in 7 cases and lobulated configuration in 3 cases.The majority (8/10 )of lesions demonstrated well-defined margin.The lesions on CT imaging were isodensity or low density, homogeneous density in 4 cases and inhomogeneous density in 6 cases,cystic changes in 2 cases,calcification in 1 case.9 lesions showed strongly homogeneous (n=3)or inhomogeneous (n=6)enhancement,1 lesion showed mildly homogeneous enhancement on contrast-enhanced CT imaging.On dual-phase enhanced CT,8 patients exhibited a rapidly enhancing and slow washout pattern,1 patient exhibited a rapidly enhancing and rapid washout pattern,and 1 patient exhibited delayed enhancement.The lesions showed homogeneous isointense relative to gray matter on T1 WI in 1 patient,mixed isointense and hypointense in 1 patient.On T2 WI,the lesion showed isointense signal in 1 patient,and heterogeneous mixed isointense and hyperintense signal in another.2 lesions demonstrated markedly homogeneous and inhomogeneous enhancement respectively.Conclusion Solitary fibrous tumors shall be considered in cases of inhomogeneous hypervascular tumors in the head and neck.Compared with the gray matter,isointense or mildly hyperintense signal on T2 WI, marked enhancement on contrast-enhanced CT,and a rapidly enhancing and slow washout pattern on dual-phase enhanced CT may be valuable imaging features of SFT.
3.Idenfication of microRNAs profiles in nasopharyngeal carcinoma.

OBJECTIVE:
To filtrate and prove the different microRNAs (miRs) profiles in nasopharyngeal carcinoma.
METHOD:
Screening the different expressions of miRs between nasopharyngeal carcinoma and the inflammatory tissues by the application of expression profiling of chip high-throughput and large-scale microarray analysis. Then we used RT-QPCR technology to prove the accuracy of screening results.
RESULT:
There were significant expression differences of miRs between nasopharyngeal carcinoma and the control tissues, 144 human miRs had 2 or more fold the difference ratio. Compared with the inflammatory tissues, we have found that miRs-34b, miRs-449b and miRs-7-1 significantly low expressed in nasopharyngeal carcinoma, yet miRs-125b, miRs-184, miRs-196b, miRs-205 and miRs-24-1 expressed high. The results were consistent with the microarray analysis.
CONCLUSION
The difference expressed miRs might be closely related to the process of nasopharyngeal carcinoma, and the research on miRs profiles maybe provide a powerful target basis for early diagnosis and therapy of nasopharyngeal carcinoma.

4.Time-effect observation of stimulating effect on acupoints after micro-invasion embedding in healthy subjects.

OBJECTIVETo in vivo dynamically observe the time-effect characteristic of local stimulating effect on acupoints after micro-invasion embedding, which could provide references for the interval period of micro-invasion embedding.
METHODSWith magnetic resonance imaging (MRI) technique, the local T2WI fat-suppression images and T2 Mapping 8-echo sequence images were collected at multiple time points from 8 healthy subjects who received embedding at left Sanyinjiao (SP 6).After the 8-echo sequence images were transformed into T2 Mapping images by using software FuncTool, the T2 average value of embedding area was measured, and the changes of local signal strength of T2WI fat-suppression images and T2 average value along with time after embedding at left Sanyinjiao (SP 6)were analyzed.
RESULTSCompared before embedding, the signal strength of local T2WI fat-suppression images and the T2 average value began to increase 8 h after being embedded(<0.01); the signal strength of T2WI fat-suppression images and the T2 average value were significantly increased 3 d and 7 d after being embedded(all<0.01);the signal strength of local T2WI fat-suppression images and the T2 average value 14 d after being embedded were lower than those at previous 2 time points, but higher than those before embedding(both<0.01); 21 d, 28 d and 35 d after embedding, the signal strength of local T2WI fat-suppression images and the T2 average value were similar to those before treatment (all>0.05).
CONCLUSIONSAfter micro-invasion embedding at Sanyinjiao (SP 6), the stimulation effect period on acupoint is approximately 21 days.When applying micro-invasion embedding under similar condition at acupoints which has similar structure as Sanyinjiao (SP 6), the interval period of embedding could consider 21 days as a reference.
5.Practice of applying multimedia technology in doctor-patient communication before diagnosis and treatment

The existing doctor-patient communication pattern often falls prey to insufficient informed consent and even medical disputes. In the patient centered perspective, Zhejiang Provincial People′s Hospital explored a new communication mode centering on patients. Based on diagnosis-related groups catalogues and high-frequency surgeries catalogues of the departments, multimedia technology was called into play to produce dubbed PPTs and videos that were easy to understand, standardized and homogeneous, which were embedded into medical records system. Following observation of the PPT or video, patients could directly sign an informed consent on the computer. This practice not only deepens patient′s understanding and achieves homogeneous level of the communication, but also elevates doctor′s work efficiency, contributing to building a harmonious doctor-patient relationship.
6.Innovative practice of " two hearts and four wings" in the construction of safe hospital

The construction of safe hospital is the foundation of high-quality development of the hospital, and innovation provides power for the construction of safe hospital from the perspective of high-quality development. Taking Zhejiang Provincial People′s Hospital as an example, the authors introduced the innovation construction path of safe hospital in detail, and put forward the construction strategy of safe hospital with " two hearts" (patient-centered, employee-centered)and " four wings" (multimedia doctor-patient communication, Wulin aunt medical studio, integrated operation safety inspection, third-party medical liability insurance). Through the combination of basic safety management and innovative practice, the hospital vigorously promoted the culture of " two hearts" , and established an efficient collaborative information management system, so as to form replicable and promotable practical experience and promote the development of safe hospital.
7.Mechanism of local time-sensitive stimulation effect of catgut and polyglycolide-co-lactide thread embedding at"Zusanli"acupoint area of rats

BACKGROUND:The researchers noted that upon embedding clinical-grade catgut and polyglycolide-co-lactide threads in the normal human"Zusanli"(ST 36)acupoint,the local area displayed temporal and inflammatory stimulatory effects,resulting in thread differentiation.However,the underlying mechanism behind thread involvement remains to be studied. OBJECTIVE:To investigate the expression levels of calcitonin gene-related peptide,5-hydroxytryptamine,leukotriene B4,and bradykinin at point"Zusanli"(ST 36)in rats after embedding catgut and polyglycolide-co-lactide respectively at different time points. METHODS:110 male SD rats were divided into a blank group(10 rats),a catgut embedding group(50 rats),and a polyglycolide-co-lactide embedding group(50 rats)according to the random number table method.In the blank group,no thread was embedded.In catgut embedding group and the polyglycolide-co-lactide embedding group,the thread was embedded in the left side of the ST36 acupoint once.Tissue was collected from the left side of the ST36 acupoint area 8 hours,3,7,14,and 21 days after embedding.The expression levels of calcitonin gene-related peptide and 5-hydroxytryptamine were detected by immunohistochemistry,and the contents of leukotriene B4 and bradykinin were detected by ELISA. RESULTS AND CONCLUSION:(1)Compared with the blank group,the expression of calcitonin gene-related peptide,5-hydroxytryptamine,bradykinin,and leukotriene B4 was significantly increased in the 8 hours,3,7,14,and 21 days of the catgut embedding group(P<0.05);calcitonin gene-related peptide expression was significantly increased in 8 hours,3,7,and 14 days in the polyglycolide-co-lactide embedding group(P<0.05);the expression of bradykinin was significantly increased in 8 hours,3,and 7 days(P<0.05);the expression of leukotriene B4 was significantly increased at 8 hours,3,7,14,and 21 days(P<0.05).(2)Compared with the polyglycolide-co-lactide embedding group,the expression of calcitonin gene-related peptide was increased at 7,14,21 days after thread embedding(P<0.05),and the expression of 5-hydroxytryptamine was increased at 8 hours,3,7,14 and 21 days after thread embedding(P<0.05);contents of leukotriene B4 and bradykinin in tissues were increased at 8 hours,3,14 and 21 days after embedding(P<0.05)in the catgut embedding group.(3)The results show that calcitonin gene-related peptide,5-hydroxytryptamine,leukotriene B4,and bradykinin in the acupoint region alter after catgut embedding in the ST36 of rats,as well as the alteration of calcitonin gene-related peptide,leukotriene B4,and bradykinin is found in the acupoint region after polyglycolide-co-lactide embedding in rats,which may be one of the mechanisms involved in the local time sensitive stimulus effects caused by embedding threads at acupoints.Moreover,there is a discernible difference between the two thread types.
