1.Spinal tuberculosis with focus-healed kyphosis and compression of spinal cord in thoracolumbar region

[Objective]To explore the treatment method of Spinal tuberculosis with focus-healed kyphosis and compression of spinal cord in thoracolumbar region.[Method]Seventeen cases were treated surgerically by single stage procedure,including posterior fusion,posterior instrumentation,decompression of spinal canal and interbody autografting.[Result]All patients were followed up from 9 months to 7 years postoperatively.The solid fusion was showed radiologically in 2.8 months at posterior autografting and in 3.3 months at interbody autografting on the average.The anteriolateral decompression of spinal canal was performed on 6 cases,completely recovered in 1 case,partially recovered in 3 cases and bad result in 2 cases.[Conclusion]Spinal tuberculosis with focus-healed kyphosis and compression of spinal cord in thoracolumbar region may show little spinal symptom clinically.The purpose of treatment is to provide rigid fixation and good stability of spine.
2.Application of invasive and non-invasive sequential ventilation in severe chronic obstructive pulmonary disease

Twenty four patients of chronic obstructive pulmonary disease complicated with severe respiratory failure were divided into two groups randomly: sequential ventilation group and control ventilation group (n = 12 in each group). Patients in sequential group received invasive ventilation at beginning, once the pulmonary infection control window (PICW) reached the trachea cannula was extubated immediately replaced by Bi-level positive airway pressure( BiPAP) noninvasive ventilation with oral-nasal mask; while in control ventilation group the invasive ventilation was continued using pressure support ventilation (PSV) until stopping mechanical ventilation. The total ventilation time, the invasive ventilated time,the length of stay in hospital,monitoring time and the incidence rate of ventilation associated pneumonia (VAP) were evaluated in two groups. The total ventilation time [(7. 8 ±2. 5 ) d, (12 ±2. 2) d], the invasive ventilated time [(4. 9 ±2. 2)d, and (12 ± 2. 2) d] the length of hospital stay [(8. 5 ± 2. 5 ) d, (13 ± 2. 6) d] in the sequential ventilation group were significantly shorter than those in the control ventilation group (P < 0.05); the incidence rate of VAP was lower than that in the control group. The results indicates that sequential ventilation with the guidance of PICW can shorten the total ventilation time, the critical monitoring time and the length of hospital stay, it can also reduce the incidence of VAP and improve the prognosis of patients.
3.Biological characteristics of tendon and performance evaluation of repair materials for tendon defects

OBJECTIVE: To explore the biological characteristics of tendon and performance of repair materials for tendon defects. METHODS: CNKI and Medline database were retrieved by computer for papers published between 1985 and 2009. Literature concerning biological characteristics and performance of repair materials for tendon defects were selected. Meta analysis, irrespective papers or repetitiveness studies were excluded. Totally 29 papers were included after screening and evaluating including 16 in Chinese and 13 in English. The biological characteristics of tendon and performance of repair materials were summarized.RESULTS: Nutrition for tendon mainly supplied by blood, and the tendon fatigue would be occurred with repeatability loads. Compared to other tissue-engineered materials, tendon possesses self-repair ability. The biological characteristics of tendon was closely associated with collagen fiber, which exhibited similar stress-strain curves, namely, characteristics of viscoelasticity body--hysteresis, creep and stress relaxation. Currently, main repair materials for tendon defects comprised autogenous tendons, allogeneic tendon, artificial tendon and tissue engineered tendon.CONCLUSION: Many materials can be used in tendon repair with distinct advantage and disadvantage; however, there is not an ideal repair method. With the development of tissue and gene engineering, the optimal substitute for tendon would be prepared.

5.Clinical research of Wanyao-Tingli manipulation in the treatment of instability of lumbosacral joint causedto low back pain and activity

Objective To observe the effect of Wanyao-Tingli manipulation in the treatment of instability of lumbosacral jioint causedto low back pain and activity. Methods From March 2009 to May 2012, 130 patients with Instability of Lumbosacral Joint were recruited and randomly divided into atreatment group(63 cases)and acontrol group(67 cases). The treatment group was given Wanyao-Tingli Manipulation, and the control group was received Votalin Futalin Cream. The therapeutic effect was observed in both groups after continuous treatment for 7 days, different time record of low back pain score and activity was observed after treatment at 1,3,5,7 days of metrics. Results After1, 2, 3, 4 times treatment, the VAS score of and treatment group VAS score, the efficacy of lumbar flexion activity of treatment group were significantly better than the control group (t=71.619, 57.166, 56.329, 56.632;72.579, 55.330, 38.418, 25.889, P<0.01);After 1, 2 times treatment, the treatment group lumbar extension activity and flexor activity of treatment group comparison with the control group significantly improved compare with the control group(t=26.372, 22.092, 13.418, 16.373, P<0.01). Conclusion Wanyao-Tingli Manipulation can significantly relieve improve low back pain and improve activity in the treatreatment ofing instability of lumbosacral joint with immediate significant effect.
6.Constitution Factors for Metabolic Syndrome: A Case-control Study

Objective To investigate the constitution factors for metabolic syndrome(MS). Methods 271 MS cases and 271 non-MS controls were selected. They were assessed according to the diagnosis standard of constitution of TCM. Results Multiple logistic regression analysis revealed that phlegm-dampness constitution and dampness-heat constitution were dangerous factors for MS, normal constitution was protective factor for MS (P<0.05). Conclusion Research on constitution play a role in investigating pathogenesis of MS.
7.Survey of mental health and social support of medical staff in general hospitals

Objective To probe into the social support, coping styles, and mental health status of medical staff in a tertiary hospital in Beijing, for identifying potential mental problems. Methods Such means as the Social Support Rating Sheet (SSRS), Simple Coping Style Questionnaire (SCSQ), Symptom Checklist (SCL-90), and demographic characteristics questionnaire were called into play, to study 2460 medical staff in five tertiary hospitals in Beijing. Results Differences were identified among medical staff of different demographic characteristics, in terms of positive coping style and support dimensions. Analysis of the survey data indicates that the total SCL scores, total symptom problem index, positive symptom pain level, total average score of the factors, are positively related to passive coping style, as P<0. 01; and are negatively related to subjective/objective support, support utilization, and total support scores, as P<0. 01. Conclusions Hospital administration is expected to implement focused intervention to such medical staff as those under 40 years old, women, widows or divorcees, those with education lower than university degree, those with junior academic title or lower, those without titles, those in emergency departments and emergency intensive care units, doctors and nurses, frequent night-shift workers, those of excessively low self evaluation, and medical staff with chronic diseases. The purpose of this practice is to alleviate their mental stress.

Grassroots organizations of the party function as the center to advocate voluntary services.The hospital volunteer service campaign is considered part of the party's work at the grassroots level and as an indicator of its branch appraisal.volunteer ambassadors are employed to serve as role model for volunteer services.Party branch guides volunteer service teams to carry out voluntary services.And students,retiree party members actively participate in the campaigr.Outstanding ones are awarded and the management is improved.All these measures enable these organizations to play a spearhead role,encouraging volunteers and party members to promote each other,promoting healthy development of the campaign,and making volunteer services sustainable.
9.Correlation between Plasma-soluble Urokinase Plasminogen Activator Receptor(suPAR) Levels and Disease Severity in Psoriasis Patients

Objective To investigate the correlation between plasma-soluble urokinase plasminogen activator receptor (suPAR) levels and disease severity in psoriasis patients.Method 60 psoriasis patients and 60 healthy controls were enrolled from Jan.2013 to Dec.2015 in the hospital.The plasma suPAR of all objects were measured by ELISA.Kruskal-Wallis and Mann-Whitney U test were used to compared plasma suPAR in the difference groups.Correlation between clinical data and plasma suPAR were analyzed by Spearmans's rho method.Result The plasma suPAR of psoriasis patients (3.92± 1.74 ng/ml) were higher than controls (3.03 ± 1.08 ng/ml,Z=13.05,P=0.009).The plasma suPAR of mild patients (PASI< 10) were lower than moderate patients (10≤ PASI≤ 20,3.90 ± 1.67 ng/ml,Z =8.00,P =0.035) and severity patients (PASI>20,4.55 ± 1.88 ng/ml,Z=48.5,P=0.031).Positive correlation were found between plasma suPAR and psoriasis area and severity dndex (PASI) score (r=0.264,P=0.041).The plasma suPAR of the patients with disease duration>10years (n=35,4.43 ± 1.98 ng/ml) were higher than the patients with disease duration<10 years (n=25,3.41 ± 0.69 ng/ ml,Z=-2.064,P=0.035).Conclusion There was a positive correlation between the plasma suPAR and psoriasis disease severity.The Plasma suPAR can be the biomarker of psoriasis disease severity.It facilitate the clinical diagnosis of psoriasis.
10.Expressions of programmed death 1 and programmed death ligand 1 in rat model of acute liver failure

Objective To study the expressions of programmed death 1 (PD-1) and programmed death ligand 1 (PD-L1) in liver injury of acute liver failure (ALF) in rats and the role of PD-1/ PD-L1 in liver inflammatory injury. Methods SD rats were divided into two groups: 6 in normal group and 30 in ALF model group. The ALF models in rats were induced by D-galactosamine (D-Gal). The sera and hepatic tissue samples were collected at 12, 24, 48, 72 and 120 hours after D-Gal injection. Expressions of PD-1 mRNA and PD-L1 mRNA in hepatic tissue samples were detected by reverse transcription-polymerase chain reaction (RT-PCR). Comparison of measurement data between groups was done by t test. Correlation test was performed using Pearson linear correlation analysis. Results The levels of alanine animotransferase (ALT) and aspartate animotransferase (AST) at 12 h of D-Gal injection were (217. 3±33. 7) U/L and (397. 2± 101.3) U/L, respectively,which were both significantly higher than those in normal group [(30. 5 ±3. 1) U/L and (78. 6±4.2) U/L, respectively; t=-8. 921 and -6. 121, respectively; both P<0.01] and peaked at 48 h.The expression of PD-1 mRNA in model group at 12 h (0. 385±0. 074) was significantly higher than that in normal group (0. 097±0.009) (t= -7. 725, P<0.01) , and peaked at 48 h (0. 927±0. 132),then decreased obviously at 72 h. The expression of PD-L1 mRNA in the liver tissue of normal rats was very little, while that in model group was increased gradually over time, then peaked at 48 h (0. 593±0. 105; t =- 10. 076, P<0. 01). The expressions of PD-1 and PD-L1 were positively correlated with ALT level (r=0. 807 and 0. 792, respectively; both P<0. 01). Conclusion The expressions of PD-1/PD-L1 may play an important role in liver inflammatory injury in rat model of acute liver failure.
