1.Biological characteristics of tendon and performance evaluation of repair materials for tendon defects

OBJECTIVE: To explore the biological characteristics of tendon and performance of repair materials for tendon defects. METHODS: CNKI and Medline database were retrieved by computer for papers published between 1985 and 2009. Literature concerning biological characteristics and performance of repair materials for tendon defects were selected. Meta analysis, irrespective papers or repetitiveness studies were excluded. Totally 29 papers were included after screening and evaluating including 16 in Chinese and 13 in English. The biological characteristics of tendon and performance of repair materials were summarized.RESULTS: Nutrition for tendon mainly supplied by blood, and the tendon fatigue would be occurred with repeatability loads. Compared to other tissue-engineered materials, tendon possesses self-repair ability. The biological characteristics of tendon was closely associated with collagen fiber, which exhibited similar stress-strain curves, namely, characteristics of viscoelasticity body--hysteresis, creep and stress relaxation. Currently, main repair materials for tendon defects comprised autogenous tendons, allogeneic tendon, artificial tendon and tissue engineered tendon.CONCLUSION: Many materials can be used in tendon repair with distinct advantage and disadvantage; however, there is not an ideal repair method. With the development of tissue and gene engineering, the optimal substitute for tendon would be prepared.
2.Application of invasive and non-invasive sequential ventilation in severe chronic obstructive pulmonary disease

Twenty four patients of chronic obstructive pulmonary disease complicated with severe respiratory failure were divided into two groups randomly: sequential ventilation group and control ventilation group (n = 12 in each group). Patients in sequential group received invasive ventilation at beginning, once the pulmonary infection control window (PICW) reached the trachea cannula was extubated immediately replaced by Bi-level positive airway pressure( BiPAP) noninvasive ventilation with oral-nasal mask; while in control ventilation group the invasive ventilation was continued using pressure support ventilation (PSV) until stopping mechanical ventilation. The total ventilation time, the invasive ventilated time,the length of stay in hospital,monitoring time and the incidence rate of ventilation associated pneumonia (VAP) were evaluated in two groups. The total ventilation time [(7. 8 ±2. 5 ) d, (12 ±2. 2) d], the invasive ventilated time [(4. 9 ±2. 2)d, and (12 ± 2. 2) d] the length of hospital stay [(8. 5 ± 2. 5 ) d, (13 ± 2. 6) d] in the sequential ventilation group were significantly shorter than those in the control ventilation group (P < 0.05); the incidence rate of VAP was lower than that in the control group. The results indicates that sequential ventilation with the guidance of PICW can shorten the total ventilation time, the critical monitoring time and the length of hospital stay, it can also reduce the incidence of VAP and improve the prognosis of patients.
3.Emergency repair of soft tissue defect at thumb using miniature flaps

Objective To report the treatment outcomes and repair methods for soft tissue defects at thumb using miniature flaps. Methods Six different kinds of surgical flaps were used for the repair on different thumb injuries. They were radial lateral thumb reverse island flap, dorsal ulnar thumb revers flap, first metacarpal dorsal reverse flap, first digital dorsal island flap, neurovessel pedicled V-Y advanced island flap, free nail flap first foot digital, and so on. The emergency repair using miniature flaps were studied on 132 different cases, all 137 flaps. Results The flaps survived expect 5 flaps had partial epidermis necorsis in the 137 flaps. But the 5 flaps survived too after disinfecting and changing dressing. The appearances and functions of thumbs were satisfactory for the 1 to 2 years post-operation follow-up. Conclusion It will gain better results if choosing appropriate emergency repair methods using miniature flaps on different cases.
4.Role of β-catenin signaling pathway in the epithelial to mesenchymal transition induced by parathyroid hormone in renal tubular epithelial cells

Objective To investigate the effect of parathyroid hormone (PTH) on the epithelial to mesenchymal transition (EMT) in human renal proximal tubular epithelial cells (HK-2 cells),and determine the role of β-catenin signaling pathway.Method The expression of α-smooth muscle actin (α-SMA),E-cadherin and β-catenin in HK-2 cells was measured by real-time PCR,Western blotting and immunofluorescence technique.The signaling pathway by which PTH activated EMT in HK -2 cells was identified by using synthetic β-catenin siRNA.Results Parathyroid hormone (10-10mol/ L) increased α-SMA expression and decreased E-cadherin expression in HK-2 cells (P< 0.01,respectively).Untreated cells showed the expression of E-cadherin,whereas α-SMA staining was noticeably increased in cells treated with PTH.β-catenin activity was significantly increased after exposed to PTH.Theα-SMA expression was decreased strongly and E-cadherin expression was increased after β-catenin siRNA transfection (all P < 0.05).Conclusion PTH significantly induces epithelial to mesenchymal transition in HK-2 cells throughβ-catenin signaling pathway.
5.Endovenous laser treatment (EVLT) for varicose great saphenous veins complicated with ulcers

Objective To explore the effect of endovenous laser treatment ( EVLT) for varicose great saphe-nous veins complicated with ulcers .Methods 40 cases ( 46 limbs ) of varicose great saphenous veins complicated with ulcers underwent EVLT .The abnormal vein branches of the base of ulcers and adjacent ulcers were burned and closed.Results Except a case suffered the secondary transplantation for skin flap necrosis ,no infection,skin necro-sis,deep vein thrombosis complications occurred in other 39 cases.All the ulcers healed ,and postoperative lower limb varicose veins were disappeared .The operation time was (65 ±11.6) min.All the ulcers healed and the hospitalization time was (21 ±4.6)d.All the patients were followed up for 6-36 months,and no recurrence was found.Conclusion EVLT for varicos e great saphenous veins complicated with ulcers has the advantages of good effect ,minimal incision , and quick recovery .
6.Herbal iontophoresis and rotation manipulation management on lumbar disc herniation

Objective To observe the curative effect of herbal iontophoresis with rotation manipulation treating lumbar disc herniation.Methods 84 patients from July 2011 to July 2012 with lumber disc herniation in Electric Power hospital of Capital Medical University were randomly divided into two groups (a testing group n=42,a control group n=42).The testing group was treated with herbal iontophoresis and rotation manipulation,and the control group was treated with rotation manipulation only.The Visual Analogue Scale and Macnab standard for curative effect were observed before the treatment and 3 weeks later after the treatment.Results The total effective rate was 88.1% and 66.7% in the testing group and the control group respectively,the difference between two groups was statistically significant (chi-square=5.51,P=0.02<0.05).The VAS before and after treatment in testing group were (7.31±1.39) and (2.41±1.62) respectively,compared with (7.19± 1.38) and (3.07± 1.28) in the control group.The VAS and curative effect indexes were both improved in the two groups,but the testing group increased more apparent,and the difference between the two groups was significant (P<0.05).Conclusion The curative effect of herbal iontophoresis and rotation manipulation treating lumber disc herniation was sound.
7.Protective effect of portal vein occlusion without hepatic artery inflow control on cirrhotic rat liver

Objective In order to improve cirrhotic liver management,each aspect of the liver's complex blood flow must be understood.This study investigates the protective effect of portal vein occlusion,with hepatic artery preservation,on cirrhotic liver after ischemia and reperfusion.Methods Carbon tetrachlorideand induced cirrhotic rats and normal rats were randomly assigned into 4 groups:normal sham operation (N-SO),cirrotic sham operation (C-SO),portal triad clamping (PTC),and portal vein clamping without hepatic artery inflow control (PVC).During the occlusion,the total 3-minute blood loss from the liver surface cut was weighed.At 1,6,and 24 hours post reperfusion,the serum alapine amino transferas (ALT),the adenosine triphosphate (ATP) of liver tissue,the malonolialdehgde (MDA) of liver tissue,and the morphological changes were evaluated.Result The amount of hemorrhage between the groups ranked as follows:PTC < PVC < N-SO < C-SO (P<0.05).At 1,6,and 24 hours post reperfusion.the ALT and MDA levels of the groups ranked as follows:PTC > PVC > C-SO > N-SO (P<0.05).Additionally,each group's ATP level ranked as follows:PTC < PVC < C-SO < N-SO (P<0.05).With histopathological examination,the hepatic injuries of the PTC and PVC group were more severe than those of the C-SO group,especially in the PTC group.Conclusion Therefore,the technique of portal vein clamping and hepatic artery inflow control can reduce the ischemic reperfusion injury of the cirrhotic rats' liver.
8.Clinical significance of intercostobrachial nerve reservation in radical resection of breast cancer

Objective To study the clinical significance of intercostobrachial nerve ( ICBN) reservation in radical resection of breast cancer .Methods 42 cases with breast cancer at stageⅠ,ⅡorⅢa were randomly divided into experimental group(n=28) and control group(n=14).In the experimental group,the ICBN were preserved in the axillary lymph node dissection operation ,while in the control group ,the ICBN were resected in the operation .After the operation,both two groups were treated as breast cancer and the patients were followed up .The operation time,the number of ALND , local tumor recurrence and distant metastasis rates after operation , survival rate were compared between the two groups.Results Postoperative 14 days,3 months and 6 months,the incidence rates of the sensory disorder in the internal upper arms in the control group were 85.7%,64.3% and 50.0% respectively,those in the experimental group were 10.7%,3.5% and 0.0%.The differences were statistically significant between the two groups(P<0.05).The average operation time,average number of lymph node dissection between control group and experimental group had no significant difference [(80.0 ±12.9) min vs (88.0 ±15.6) min,(16.4 ±5.4) vs (14.8 ±6.1)](P >0.05).In the control group,the local recurrence rate,distant metastasis rate and survival rate were 0.0%,7.0%and 0.0%,those in the experimental group were 0.0%,0.0% and 0.0%.There were no statistically significant differences between the two groups .Conclusion It is feasible to preserve the ICBN in radical resection of breast cancer ,which reserves the skin sensory function of the internal upper arm and axilla ,and enhance the patients'quality of life.
9.Characterization of imaging distortion of high-field magnetic resonance imaging-guided linear accelerator (MR-Linac) and its influencing factors

Objective:To characterize the imaging distortion of the 1.5T magnetic resonance imaging-guided linear accelerator (MR-Linac) and to analyze the influence of MR-Linac and peripheral devices on the geometric distortion.Methods:Specialized MRI imaging distortion phantom and analysis software were applied. The baseline of imaging distortion within diameter spherical volume (DSV) around the center of the magnet was established. The influence of the beam generation system, mechanical system and peripheral devices on the imaging distortion was analyzed. The long-term stability of imaging distortion was tested on the MR-Linac.Results:Imaging distortion of the MR-Linac was increased with the increasing distance to the center of the magnet. Within DSV 400 mm, few test points surpassed 1 mm imaging distortion in 3D directions. However, imaging distortion surpassed 2 mm in part of region within DSV 400-500 mm, with the largest distortion over 7 mm. Imaging distortion of the MR-Linac remained unchanged within 7 months after installation. And the influence of the MR-Linac and peripheral devices on the imaging distortion was only observed in the overall largest distortion within DSV 400-500 mm.Conclusions:Cautions should be taken during the application of high-field MR-Linac in patients whose tumor location is over 20 cm from the ISO center. Imaging distortion of the MR-Linac remains stable within 7 months after installation. The influence of the MR-Linac and peripheral devices on the imaging distortion is trivial, which can be neglected in clinical practice.
10.Tissue culture of Vinca minor and determination of vincamine.

OBJECTIVETo study tissue culture of Vinca minor and determine the content of vincamine.
METHODLeaf blades, stalks, root segment of V. minor were used as explants to study the effect of 2, 4-D,6-BA,NAA on its callus induction and vincamine contents in the orthogonal design experiment. In the peak period of callus formation, vincamine content in callus of V. minor and sterile plants was determined by HPLC. The experimental data was statistically analyzed.
RESULTThe content of 6-BA and NAA had no significant effect on its callus induction. But the content of 2, 4-D had significant effect on its callus induction. Within 20,40,60 d, the content of vincamine in sterile plant was (0.015 +/- 0.003)%, (0.097 +/- 0.001)% , (0.113 +/- 0.06)%, respectively. In the peak period of callus formation, vincamine content in callus of leaf blades, stalks, root segment was (0.024 +/- 0.0025)%, (0.016 +/- 0.0015)%, (0.010 +/- 0.0015)%, respectively. To 30 days of subculture, vincamine content in callus of leaf blades, stalks, root segment was (0.041 +/- 0.002)%, (0.019 +/- 0001)%, (0.016 +/- 0.002)%, respectively.
CONCLUSIONThe optimal hormone combination for callus initiation was MS +2, 4-D 1.0 mg x L(-1) +6-BA 0.5 mg x L(-1) + NAA 0.5 mg x L(-1). In different growth periods, vincamine content in sterile plants is significantly different. From different explants in callus vincamine content is different, in which leaves callus is significantly higher than that of stems, roots produced callus organization.
